Objective To explore the association between knee torsion and multiple bony abnormali-ties of the lower limb in patients with recurrent patellar dislocation(RPD).Methods The preoperative imaging data of RPD patients treated in our institution between May 2020 and October 2024 were col-lected retrospectively.A total of 238 patients were included in this study,with 83.6%being females.All patients underwent standard hip-knee-ankle CT scans,and lower limb bony structural parameters were reconstructed and measured using Mimics 20.0,focusing on variables such as knee torsion,femo-ral anteversion(FAA),femoral distal torsion angle(DFTA),supratrochlear spur,tibial tuberosity to trochlear groove distance(TT-TG),Caton-Deschamps index,and tibial torsion.Bony abnormalities were categorized based on previously established risk thresholds.Moreover,chi-square tests were em-ployed to compare the composition ratio differences between knee torsion and multiple lower limb osse-ous abnormalities.Results Among all affected knees,the proportion of excessive knee torsion was 33.6%,while in cases with osseous abnormalities,the value was over 40%.Compared with cases of low knee torsion,patients with excessive knee torsion showed significantly higher proportions of FAA(41.3%vs.27.2%,P=0.028),excessive DFTA(60%vs.32.9%,P<0.001),supratrochlear spur(55%vs.38.6%,P=0.016),trochlear dysplasia(93.8%vs.73.4%,P<0.001),excessive TT-TG(68.8%vs.43.7%,P<0.001),and patella alta(50%vs.22.2%,P<0.001).Additionally,95%of knees with excessive torsion exhibited two or more bony risk factors,whereas 55%had four or more bony structural abnormalities,which was significantly higher than those of low knee torsion(55%vs.21.5%,P<0.001).Conclusion In RPD patients,excessive knee torsion is associated with multiple low-er limb bony abnormalities.Moreover,patients with high knee torsion(>12°)are significantly more likely to have multiple bony abnormalities than those with low torsion.Therefore,in surgical decision-making for such patients,lower limb bony abnormalities should be assessed comprehensively.

Background and purpose:Diffuse large B-cell lymphoma(DLBCL)is subclassified into germinal center B-cell-like(GCB)and non-GCB subtypes,which differ in prognosis and treatment response.However,current distinction still relies on invasive pathological assays.This study developed radiomics and deep-learning models based on multiparametric magnetic resonance imaging(MRI)to non-invasively differentiate the two subtypes preoperatively,thereby reducing dependence on histopathological examination.Methods:This study retrospectively included patients with pathologically confirmed DLBCL diagnosed at Huashan Hospital,Fudan University,and other institutions between March 2013 and December 2024.Using multiparametric MRI data,we developed DLBCL-subtype classification models that combined 4 radiomics-based machine-learning algorithms:support vector machine(SVM),logistic regression(LR),Gaussian process(GP)and Naive Bayes(NB),with 3 deep-learning architectures[densely-connected convolutional networks 121(DenseNet121),residual network 101(ResNet101)and EfficientNet-b5].Additionally,two radiologists with different experience levels independently classified DLBCL on MRI in a blinded fashion.Model and radiologist performance were quantified using the area under the receiver operating characteristic curve(AUC),accuracy(ACC),and F1-score to evaluate their ability to distinguish GCB from non-GCB subtypes.This study was approved by the Ethics Committee of Huashan Hospital of Fudan University(No.KY2024-663),and all patients signed informed consents.Results:A total of 173 patients were enrolled(55 with GCB subtype and 118 with non-GCB subtype).Radiomics and deep learning methods effectively distinguished DLBCL subtypes.Among these,the GP radiomics model(based on T1-CE+T2-FLAIR+ADC sequences)and DenseNet121 deep learning model(based on T1-CE+T2-FLAIR+ADC sequences)demonstrated optimal performance.Both achieved excellent results on the internal validation set(GP:AUC=0.900,ACC=0.896,F1=0.840;DenseNet121:AUC=0.846,ACC=0.854,F1=0.774)and maintained robustness on the external validation set.Furthermore,the classification efficacy of the optimal AI model surpassed that of experienced radiologists(highest physician AUC=0.678).Conclusion:Radiomics and deep-learning models based on multiparametric MRI features can effectively differentiate GCB from non-GCB subtypes of DLBCL.Among them,GP and DenseNet121 exhibit outstanding performance,especially when integrating multi-sequence feature sets for classifying DLBCL subtypes on complex imaging data.

Objective To observe the influence of different acceleration factors(AF)on compressed sensing susceptibility weighted imaging(CS-SWI)for cerebral medullary veins of healthy people,and to screen the best AF.Methods Forty healthy volunteers were prospectively enrolled.Axial brain SWI images were obtained with CS technique under different AF(AF0,CS2,CS4,CS6,CS8 and CS10),and the phase value(PV)and standard deviation(SD)of bilateral septal vein(SV),internal cerebral vein(ICV),thalamus vein(TV),basal vein(BV)and dentate nucleus vein(DNV)were measured.Taken PV and SD of parietal white matter as controls,the signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of venous images were calculated.Then the original images were reconstructed with minimum intensity projection,and the subjective quality scoring of reconstructed images were performed using a 5-point scale.SNR,CNR,PV and quality score were compared among images under different AF,and the best AF,i.e.with the best performances for displaying and quantitatively analyzing cerebral medullary veins in healthy people was obtained.Results Compared with those acquired with AF0,SNR and CNR of all cerebral medullary veins acquired with CS6,CS8 and CS10 were significantly different(all adjusted P＜0.05).Meanwhile,significant differences of PV in bilateral SV and right TV were found among CS6,CS8 and CS10,also in bilateral ICV,left TV and bilateral BV between CS8 and CS10(all adjusted P＜0.05).Conclusion Excessive AF might decrease image quality of CS-SWI for cerebral medullary veins.CS4 was the best AF for displaying and quantitatively analyzing cerebral medullary veins in healthy people.

Objective To explore the impact of δ-catenin expression level on resting-state brain function in breast cancer patients.Methods Totally 104 female breast cancer patients were prospectively enrolled and divided into δ-catenin high expression group(DH group,n=51)and δ-catenin low expression group(DL group,n=53)according to δ-catenin expression level,while 36 female healthy volunteers were selected as controls(control group).Neuropsychological tests were performed,and resting-state functional MRI(rs-fMRI)were acquired,then parameters of brain function,including amplitude of low frequency fluctuation(ALFF),fractional ALFF(fALFF),regional homogeneity(ReHo)and functional connectivity strength(FCS)of brain regions with differences among groups were obtained.Spearman correlation analysis was used to evaluate the correlations of function parameters of brain regions with general data and neuropsychological test scores.Results Significant differences of fALFF,ReHo and FCS values were found among 3 groups(familywise error rate[FWE]correction,all P＜0.05).fALFF value of left inferior temporal gyrus in DH and DL groups were both higher than that in control group(FWE correction,both P＜0.05),ReHo value of right inferior temporal gyrus in DH group,as well as of right middle temporal gyrus,right inferior temporal gyrus and right fusiform gyrus in DL group were all lower than that in control group(FWE correction,all P＜0.05),FSC value of left lenticular nucleus,left putamen,left fusiform gyrus,left calcarine fissure surrounding cortex and left inferior temporal gyrus in DH group were all higher than that in DL group(FWE correction,all P＜0.05),FSC value of left lenticular nucleus,left putamen,left fusiform gyrus and left calcarine fissure surrounding cortex in DH group were all higher than that in control group(FWE correction,all P＜0.05),while FSC value of left lingual gyrus,left lenticular nucleus and left putamen were both higher than that in control group(FWE correction,both P＜0.05).In brain regions with different fMRI indexes between DH group and DL group,FSC values were lowly positively correlated with CogPCA results(r=0.313,P＜0.05).In brain regions with different fMRI indexes between DH group and control group,fALFF value were lowly positively correlated with trail making test A(TMT A)and trail making test B(TMT B)(r=0.301,0.310,both P＜0.05),ReHo values were lowly negatively correlated with TMT B(r=-0.307,P＜0.05),FCS values were weakly/lowly positively correlated with TMT A and TMT B(r=0.282,0.309,both P＜0.05)and lowly negatively correlated with results of digital symbol substitution test(DSST)(r=-0.363,P＜0.05).In brain regions with different fMRI indexes between DL group and control group,fALFF values were weakly/lowly negatively correlated with results of mini mental state examination(MMSE),Montreal cognitive assessment(MoCA),auditory verbal learning test(AVLT)short-term memory and DSST(r=-0.399,-0.362,-0.344,-0.288,all P＜0.05).Conclusion The expression level of δ-catenin had certain impact on brain function of breast cancer patients,resulted in asymmetry changes of brain network in bilateral hemispheres,as well as memory loss through affecting left inferior temporal gyrus,left lenticular nucleus,left putamen and left fusiform gyrus.

Objective To observe the therapeutic effect of probiotics combined with montelukast in children with Mycoplasma pneumoniae pneumonia(MPP)complicated with myocardial injury.Methods A total of 102 children with MPP and myocardial injury were selected as the study objects,and were randomly divided into control group and observation group,with 51 cases in each group.The control group was treated with montelukast,and the observation group was treated with probiotics based on the control group.The clinical efficacy,symptom resolution time,hospital stay,myocardial injury index,inflammatory cytokines,intestinal flora level and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 92.16％,which was significantly higher than 76.47％of the control group(P＜0.05).The disappearance time of symptoms of fever,cough,shortness of breath,chest tightness and chest pain and hospital stay in the observation group were significantly shorter than those in the control group(P＜0.05).After treat-ment,the levels of cardiac troponin T(cTnT),B-type natriuretic peptide(BNP),myoglobin(Mb)and cardiac fatty acid binding protein(H-FABP)in the observation group were significantly lower than those in the control group(P＜0.05);the levels of interleukin-1 receptor 1(IL-1R1),gamma-interferon(γ-IFN)and high mobility group protein 1(HMGB1)in the observation group were significantly lower than those inthe control group(P＜0.05);the number of Bifidobacteria,Lactobacillus aci-dophilus,Enterococcus faecalis and Bacillus cereus in the observation group were significantly higher than the control group,while the number of Escherichia coli and Salmonella were significantly lower than those in the control group(P＜0.05).There was no significant difference in the incidence of ad-verse reactions between the two groups(P＞0.05).Conclusion Probiotics combined with monte-lukast have good efficacy in MPP children with myocardial injury,which can shorten the time of symp-tom resolution and hospital stay,improve the indexes of myocardial injury in children,inhibit the lev-el of inflammatory cytokines,adjust the structure of intestinal flora disorder,and have good safety.

Objective To explore the major risk factors of post transplantation diabetes mellitus (PTDM) and analyze the correlation between tacrolimus and PTDM after kidney transplantation. Methods Clinical data of 123 kidney transplant recipients were collected. All recipients were divided into the PTDM group (n=19) and non-PTDM group (n=104). Clinical data of all recipients in two groups were analyzed. The risk factors of PTDM were analyzed by binary logistic regression. Twenty-four mice were evenly divided into the control group (normal saline), low-dose tacrolimus (0.1 mg/kg), medium-dose tacrolimus (0.75 mg/kg) and high-dose tacrolimus groups (1.5 mg/kg). The solutions were given twice a day. The effect of tacrolimus on blood glucose metabolism in mice was evaluated. Results The incidence of PTDM was 15.4% (19/123) within 1 year after kidney transplantation. Age≥48 years old and the trough concentration of tacrolimus≥9 ng/mL within 3 months after kidney transplantation were the risk factors for PTDM after kidney transplantation (both P < 0.05). The fasting blood glucose levels of mice after administration in the low-, medium- and high-dose tacrolimus groups were significantly lower than those before administration (all P < 0.05) in a dose-independent manner (P=0.750). In the low-, medium- and high-dose tacrolimus groups, the postprandial blood glucose levels of mice after administration were significantly higher than those before administration (all P < 0.05) in a dose-dependent manner (P=0.012). Conclusions Tacrolimus is intimately correlated with the incidence of PTDM after kidney transplantation. Age≥48 years old and the trough concentration of tacrolimus ≥9 ng/mL within 3 months after kidney transplantation are the independent risk factors of PTDM. Tacrolimus affects the postprandial blood glucose levels of mice in a dose-dependent manner.

Coronaviruses (CoVs) are a group of common viruses that can infect humans and pose a great threat to global public health. Mounting evidence has shown that seven zoonotic CoVs can infect human through cross-species transmission. These continuously occurring yet unpredictable events of CoVs repeatedly crossing species barriers have attracted special attention to CoVs and caused panic worldwide. It is generally believed that the spike (S) protein is the key factor determining the cross-species transmission and the invasion potential of CoVs. This review focused on the new-found coronaviruses with potential cross-species transmission capabilities, and summarized and analyzed the research progress in S protein-mediated viral invasion as well as the potential mechanisms, aiming to provide reference for developing effective prevention and control strategies against potential cross-species transmission of CoVs in the future.
