1.Hemolysis rates of three red blood cell components at the end of storage: a 5-year retrospective study

Objective: To evaluate the suitability of the existing hemolysis rate standards for locally processed red blood cell components by retrospectively analyzing 5-year hemolysis rate data at the end of storage. Methods: A total of 720 blood samples of three types of red blood cell components from our blood station from January 2019 to December 2023 were collected. Parameters included hemoglobin concentration (Hb), hematocrit (Hct), and free hemoglobin concentration (fHb). Hemolysis rate were taken as the control standard of 0.8% in accordance with the national standard. The hemolysis rates were compared against the national standard threshold of 0.8% (GB18469-2012), and annual trends of the detection parameters were observed. Results: The hemolysis rates (x-+s,%) of leukocyte-depleted whole blood at the end of storage were (0.038±0.023 8) in 2019, (0.049±0.039 5) in 2020, (0.043±0.040 7) in 2021, (0.049±0.030 7) in 2022, and (0.058±0.054 8) in 2023, respectively; The hemolysis rates (x-+s" />,%) of leukocyte-depleted suspended red blood cells at the end of storage were (0.093±0.050 2) in 2019, (0.086±0.049 5) in 2020, (0.123±0.072 3) in 2021, (0.122±0.052 1) in 2022, and (0.106±0.058 6) in 2023, respectively; The hemolysis rates (x-+s,%) of washed red blood cells at the end of storage were (0.127±0.038 2) in 2019, (0.150±0.066 5) in 2020, (0.121±0.052 2) in 2021, (0.124±0.038 9) in 2022, and (0.128±0.044 3) in 2023, respectively. Conclusion: Hemolysis rates at the end of blood storage of three red blood cell components were significantly lower than the limits specified in Quality Requirements for Whole Blood and Components (GB18469-2012), as well as standards from the EU, AABB and the United States. The results demonstrate excellent product quality control. A regional internal control standard of <0.2% is proposed for hemolysis rates at the end of storage.
2.Efficacy of selective laser trabeculoplasty in the treatment of primary open angle glaucoma

AIM: To assess the effectiveness of selective laser trabeculoplasty(SLT)in the treatment of primary open angle glaucoma(POAG).METHODS:Totally 24 patients(33 eyes)with POAG who had poor control of intraocular pressure(IOP)from June 2022 to December 2023 were included in this retrospective study, and all of them were treated with SLT. Furthermore, the IOP, mean deviation(MD), mean sensitivity(MS), and the thickness and vascular density(VD)of optic disc nerve layer were compared after treatment. RESULTS: There were significant reducion in IOP at 1, 3, 6, 12 mo after treatment(all P<0.001). There were no statistically significant differences in MD and MS before treatment and at 6 and 12 mo after treatment(all P>0.05), and there were no statistical significant differences in the thickness and VD of optic disc(all P>0.05).CONCLUSION: SLT can significantly reduce IOP in POAG patients in the short term, without damaging the visual field and optic disc nerves, while the effectiveness of SLT decreases over time.
3.Local abaloparatide administration promotes in situ alveolar bone augmentation via FAK-mediated periosteal osteogenesis.

Insufficient alveolar bone thickness increases the risk of periodontal dehiscence and fenestration, especially in orthodontic tooth movement. Abaloparatide (ABL), a synthetic analog of human PTHrP (1-34) and a clinical medication for treating osteoporosis, has recently demonstrated its potential in enhancing craniofacial bone formation. Herein, we show that intraoral submucosal injection of ABL, when combined with mechanical force, promotes in situ alveolar bone thickening. The newly formed bone is primarily located outside the original compact bone, implying its origin from the periosteum. RNA sequencing of the alveolar bone tissue revealed that the focal adhesion (FA) pathway potentially mediates this bioprocess. Local injection of ABL alone enhances cell proliferation, collagen synthesis, and phosphorylation of focal adhesion kinase (FAK) in the alveolar periosteum; when ABL is combined with mechanical force, the FAK expression is upregulated, in line with the accomplishment of the ossification. In vitro, ABL enhances proliferation, migration, and FAK phosphorylation in periosteal stem cells. Furthermore, the pro-osteogenic effects of ABL on alveolar bone are entirely blocked when FAK activity is inhibited by a specific inhibitor. In summary, abaloparatide combined with mechanical force promotes alveolar bone formation via FAK-mediated periosteal osteogenesis. Thus, we have introduced a promising therapeutic approach for drug-induced in situ alveolar bone augmentation, which may prevent or repair the detrimental periodontal dehiscence, holding significant potential in dentistry.

4.Role of macrophages in fungal keratitis

Fungal keratitis is a serious blinding eye disease. The development of fungal infections depends primarily on the interaction of fungal virulence with host immune defense factors. The cornea is considered an immune-privileged organ, and resident macrophages are the main immune cells that respond to the heterogeneity exhibited by the microenvironment with their polarization. In the early stage of infection, macrophages polarize towards M1, which promotes inflammation and facilitates fungal clearance but produces a cellular storm that exacerbates immune damage; in the late stage of infection, macrophages polarize towards M2, which suppresses the inflammatory response and facilitates tissue repair, but may be immunosuppressed or even immune escape to the detriment of pathogen clearance. The balance between pro-inflammatory and anti-inflammatory responses is key to maintaining the functional integrity of the cornea. Current antifungal drug therapy is limited, so it is particularly important to find a therapeutic target for the inflammatory response triggered by the immune response in addition to antifungal therapy. In this review, the functional and phenotypic characterization of macrophage subsets associated with fungal keratitis was reviewed, more in-depth research is needed to explore the specific mechanisms by which macrophage polarization and their impact on fungal keratitis. Targeted regulation of macrophage differentiation based on their phenotype and function could be an effective approach to treat and manage fungal keratitis in the future.
5.Clinical efficacy of adjunctive perampanel in focal epilepsy patients≥12 years old

Objective:To evaluate the clinical efficacy, tolerability and safety of adjunctive perampanel in focal epilepsy patients≥12 years old.Methods:One hundred and nineteen focal epilepsy patients≥12 years old accepted adjunctive perampanel in Department of Neurology, First Affiliated Hospital of Guangxi Medical University from July 2020 to December 2022 were chosen. At 1-3 months, 4-6 months, 7-9 months and 10-12 months after adjunctive perampanel, seizure frequency changes every 28 d, medication retention rate and adverse reactions were recorded to evaluate the clinical efficacy (a reduction in seizure frequency≥50% from baseline was defined as overall valid treatment), tolerability and safety of adjunctive perampanel. According to efficacy results after adjunctive perampanel of 4-6 months (short-term) and 10-12 months (long-term), these patients were divided into valid group and invalid group; and the influencing factors for short-term and long-term efficacy were analyzed.Results:At 1-3, 4-6, 7-9, 10-12 months after adjunctive perampanel, reduction in seizure frequency every 28 d was 66.7% (24.3%, 97.2%), 77.5% (48.6%, 100%), 94.6% (50%, 100%), 100% (70.9%, 100%), enjoying overall valid rate of 60.2% (59/98), 75.0% (7/76), 78.9% (45/57), 86.5% (32/37). The retention rate at 3, 6, 9 and 12 months after adjunctive perampanel was 85.2% (98/115), 67.9% (76/112), 54.3% (57/105), 41.1% (37/90). Adverse reactions were reported in 33 patents (27.7%), mainly with dizziness and secondly with mental symptoms. After short-term and long-term adjunctive perampanel, no significant difference was noted in gender, initial age of adjunctive perampanel, course of disease, etiology, EEG results, imaging results, number and type of combined anti-seizure drugs, or maximum dose of pirampanel between the valid group and invalid group ( P>0.05). Conclusion:Perampanel has good efficacy, tolerability and safety in adolescents and adults≥12 years old with focal epilepsy; no clear influencing factors for pirampanel valid treatment is found so far.
6.Analysis of influencing factors on clinical efficacy of neutropenia with febrile neutropenia in tumor pa-tients

Objective To investigate the real-world factors influencing the clinical outcome of Febrile neu-tropenia(FN)in oncology patients.Methods We conducted a retrospective analysis of clinical data from 130 FN patients admitted to our hospital between January 2020 and December 2022.The patients were categorized into three groups based on their clinical efficacy:cured group,effective group,and ineffective group.A comparison was made among the three groups regarding general data and laboratory examination results.Univariate and ordered multicategorical logistic regression analyses were performed to identify factors affecting the clinical efficacy of FN.Results The overall effective rate of FN in our hospital was 86.15%.Univariate analysis revealed statistically significant differences among the three patient groups regarding the duration of granulomatous defects,Physical Status Score(PS Score),procalcitonin(PCT)levels,and timing of administration(P<0.05).Ordinal multicat-egorical logistic regression analysis demonstrated that patients with PS scores<2,granulomatous defects lasting less than 7 days,and PCT levels below 0.5 ng/mL exhibited better clinical treatment outcomes.Conclusion In the management of FN,it is crucial to prioritize patients with high PS scores and elevated PCT levels while optimizing drug utilization to enhance clinical efficacy.Timely intervention should be implemented to address granulopathy and improve overall clinical outcomes.
7.Genetic analysis of a child with autosomal recessive primary microcephaly due to variant of ASPM gene and a literature review

Objective:To explore the clinical and genetic characteristics of a child with autosomal recessive primary microcephaly (MCPH).Methods:A case study has been carried out on a boy who had presented at the Inner Mongolia Maternity and Child Health Care Hospital for microcephaly and mental deficiency in September 2022. Prenatal ultrasound images were retrospectively analyzed, and whole exome sequencing and Sanger sequencing were carried out for his family. A literature review was also carried out using keywords such as " ASPM gene", "microcephaly", "prenatal diagnosis", "primary microcephaly", " ASPM", "MCPH5", "MCPH", "autosomal recessive microcephaly", and "prenatal diagnosis on ultrasonography" on the PubMed database, Wanfang Data and China National Knowledge until September 2023. This study was approved by Medical Ethics Committee of the Inner Mongolia Maternity and Child Health Care Hospital (Ethics No. 2021-093-1). Results:The proband had shown progressive reduction in biparietal diameter (BPD) and head circumference (HC) during the fetal period. He was found to harbor compound heterozygous variants of the ASPM gene, which included a paternally derived c. 8044C>T (p.R2682X) and a maternally derived c.8652dup (p.A2885Sfs*35). Both variants were classified as pathogenic (PVS1+ PM2_Supporting+ PP4; PVS1+ PM2_Supporting+ PM3) based on the guidelines from the American College of Medical Genetics and Genomics (ACMG). For other fetuses in his family, prenatal ultrasound and genetic testing were all normal. Literature research has identified 11 relevant articles, which included 14 MCPH cases. All of the MCPH5 cases had shown various degrees of reduced BPD/HC on fetal imaging (100%, 15/15). Developmental delay, intellectual disability, and attention deficits were noted in all survived cases, with one case having seizures (12.5%, 1/8). Their genotypes had included homozygotes (46.2%, 6/13) and compound heterozygotes (53.8%, 7/13) for nonsense variants (45%, 9/20) and frameshifting variants (55%, 11/20). Conclusion:The compound heterozygous variants c. 8044C>T (p.R2682X) and c. 8652dup (p.A2885Sfs*35) of the ASPM gene probably underlay the reduced BPD and HC in this proband with MCPH.
8.Status quo of nursing clinical support system in tertiary hospitals in Shenzhen City and its influencing factors analysis

Objective To investigate the evaluation of the nursing staff of the tertiary hospitals in Shenzhen City on the status quo of the nursing clinical support system,and to analyze its influencing factors so as to provide reference and basis for perfecting the nursing clinical support system.Methods The nursing staffs in 16 hospitals of 8 districts of Shenzhen City from December 2022 to January 2023 were selected as the survey subjects,and the general data questionnaire and the nursing clinical support system questionnaire were used for conducting the survey.Results A total of 572 questionnaires were collected,and 520 questionnaires were valid,with an effective recovery rate of 90.9%.The scores of each dimension in the nursing clinical sup-port system scale were(1.87±0.81)points for equipment and appliance support,(1.07±0.62)points for aux-iliary staff support,(1.91±0.80)points for the logistics departments support,(0.88±0.67)points for the auxiliary departments support.The results of univariate analysis showed that there were statistical differences in the equipment and appliance support scores among the nurses with different ages,different professional ti-tles and different education levels(P<0.01);the scores of 4 dimensions had statistical differences among the nursing staffs with different departments(P<0.01).All factors had statistically significant differences in the dimension of auxiliary department support(P<0.05).Conclusion The popularity degree of nursing clinical support system in tertiary hospitals in Shenzhen City is high,and equipment and appliance show the character-istics of advancement and diversity.The hospital managers should strengthen the force of nursing clinical sup-port system and reduce the nursing staff to engage in non-nursing work.
9.Research progress on the relationship between type 2 diabetic retinopathy and vitamin D

With the change of lifestyle,the obese population is gradually increasing,and the prevalence of type 2 diabetes mellitus(T2DM)is increasing year by year,while the age of onset is declining year by year.Diabetic retinopathy(DR),as one of the common complications of T2DM,has a serious impact on the quality of life of patients.At present,there is no way to predict and completely stop the progression of DR.Studies have shown that vitamin D supplementation can effectively alleviate DR in vitamin D deficiency patients,which is expected to become a new idea and method to predict and treat DR.This article-reviews the role of vitamin D in the occurrence and development of DR,as well as its therapeutic effect and mechanism on DR,in order to provide a basis for the application of vitamin D supplementation in the prevention and treatment of DR.
10.Visualization analysis of the application status of wearable smart devices in the medical field at home and abroad

Objective:To evaluate the application status of wearable smart devices in domestic and foreign medical fields through visual analysis.Method:With “smart wearable device” “wearable smart device” “Smart wearable device” and “wearable smart devices” as search terms, literatures on the application status of wearable intelligent devices in the medical field at home and abroad in the core collection of CNKI, Wanfang, VIP, PubMed and Web of Science databases were retrieved from the establishment of the databases to April 30, 2023. The software CiteSpace was used for bibliometric analysis, including publication trend, institutional cooperation network, publication journal distribution analysis, keyword co-occurrence, clustering and emergence analysis, as well as publication journal and literature co-citation analysis.Results:A total of 121 valid Chinese literatures and 289 English literatures were obtained. Domestic and foreign research began in 2013 and 2004, respectively, and the overall number of published papers showed an upward trend. The top 3 institutions of publication in China were Chinese Academy of Sciences (13 articles), Union Hospital Affiliated to Huazhong University of Science and Technology (11 articles), and West China Nursing College of Sichuan University (10 articles). The top 3 institutions in the number of foreign publications were Harvard University (13 articles), the University of California (14 articles), and the University of London (5 articles). The top 3 Chinese journals in terms of publication volume were Nursing Research, Journal of Nursing Science and Chinese Journal of Nursing, with 38 publications, accounted for 31.4% of the total publications. The top 3 foreign journals in terms of publication volume are Sensors, IEEE Access and Applied Sciences-Basel, with a total of 39 publications, accounted for 13.5% of the total publications. A total of 8 Chinese keyword clusters and 10 English keyword clusters were formed. The top 5 categories were “senior citizens” “artificial intelligence” “mobile medical” “extended care” “nursing” and “wearable electronics” “sensor” “deep learning” “mobile health” and “big data”; there were 9 emergent words in Chinese and 10 emergent words in English. The top 5 were “wearable” “internet of things” “review literature” “big data” “smart watch” and “care” “activity recognition” “wearable sensors” “system” and “accelerometer”. The journal with the highest total citation frequency was Sensors, and the journal with the highest total citation center was Biosens. Bioelectron.. There were 2 literatures with a total citation frequency of 11-20 times, and 10 literatures with a frequency of 5-10 times. Conclusions:The research popularity of wearable smart devices in the medical field is generally increasing, and the quantity and quality of English papers are higher than those of Chinese. At present, mobile health is mainly used in the fields of physical signs monitoring, pain assessment, rehabilitation and exercise. Mobile health is a common research hotspot at home and abroad.
