1.Construction of a community-family management model for older adults with mild cognitive impairment

ObjectiveTo develop a community-family management model for older adults with mild cognitive impairment (MCI) and to formulate detailed application specifications, and to fully leverage the initiative of communities and families under limited resource conditions, for achieving community-based early detection and early intervention for older adults with MCI. MethodsA systematic literature review was conducted to identify pertinent publications. Corpus-based research methodologies were employed to extract, refine, integrate and synthesize management elements, thereby establishing the specific content and service processes for each stage of the management model. Utilizing the 5W2H analytical framework, essential elements such as management stakeholders, target populations, content and methods for each stage were delineated. The model and its application guidelines were finalized through expert consultation and demonstration. ResultsAn expert evaluation of the management model yielded mean scores of 4.84, 4.32 and 4.84 for acceptability, feasibility and systematicity, respectively. By integrating the identified core elements with expert ratings and feedback, the final iteration of the community-family management model for older adults with MCI was formulated. This model comprised of five stages: screening and identification, comprehensive assessment, intervention planning, monitoring and referral pathways to ensure implementation, and enhanced support for communities, family members and caregivers. Additionally, it included 18 specific application guidelines. ConclusionThe proposed management model may theoretically help delay cognitive decline, improve cognitive function and potentially promote reversal from MCI to normal cognition. It may also enhance the awareness and coping capacity of older adults and their families, strengthen community healthcare professionals' ability to early identify and manage MCI.
2.Analysis of the chemical constituents of Maxing Shigan decoction by UPLC-Q-TOF/MS

Objective To analyze chemical constituents of compound Maxing Shigan decoction by ultra-high perfor-mance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-QTOF/MS). Methods The separation was performed on a UPLC BEH C18 column (2.1 mm×100 mm, 2.5 µm),with a gradient elution applying 0.1% aqueous formic acid solution and 0.1% formic acid acetonitrile as a mobile phase. The column temperature was 40 °C. The flow rate was 0.4 ml/min and the analysis time was 15 min. Mass spectrometry (MS) data were collected in both positive and negative ESI ion modes. Results Through UPLC-QTOF/MS analysis and reference validation, a total of 59 chemical components in Maxing Shigan decoction were identified. Conclusion An ultra-high performance liquid chromatography quadrupole time-of-flight mass spectrometry (UPLC-QTOF/MS) method was established to identify the chemical components of Maxing Shigan decoction. This method is simple, efficient, sensitive and accurate, and provides a basis for the elucidation of the pharmacodynamic material basis and mechanism of Maxing Shigan decoction. It can provide data reference for the optimization of the compatibility of traditional Chinese medicine in the treatment of COVID-19.
3.Synthesis of asiatic acid derivatives and determination of their anti-tumor activities

Twelve derivatives of asiatic acid were synthesized through acylation, alkylation, oxidative dehydrogenation and other reactions using asiatic acid from usoxane-type pentacyclic triterpenoids as the parent compound. Their structures were confirmed by 1H NMR and 13C NMR, and determined to be novel compounds never reported in literature. Through the MTT method, high-expression human cancer cells (A549 and SGC-7901) were selected for a preliminary in vitro anti-tumor activity study on these compounds. Among them, the IC50 of compound I1 were 11.39 and 9.08 μmol/L respectively, and those of compound I2 were 12.64 and 9.15 μmol/L respectively, which were close to those of sorafenib, a common drug for clinical use. The experimental results show that the synthesized asiatic acid derivatives have certain anti-proliferative effects on the two types of human cancer cells, A549 and SGC-7901, significantly higher than those of asiatic acid. Compounds I1 and I2 show quite strong anti-proliferative effects on human cancer cells A549 and SGC-7901.
4.Efficacy and safety of obinutuzumab combined with short-course dexamethasone for relapsed immune thrombocytopenia after treatment with rituximab

Objective:To investigate the efficacy and safety of obinutuzumab combined with short-course dexamethasone in patients with relapsed immune thrombocytopenia (ITP) who had previously been treated with rituximab.Methods:A retrospective case series study was conducted. A total of 8 patients with relapsed ITP after treatment with rituximab who received obinutuzumab combined with short-course dexamethasone between January 2023 and January 2024 in the First Affiliated Hospital of Harbin Medical University were collected. The clinical characteristics, changes in platelet counts, changes in peripheral blood B-lymphocyte counts, treatment outcome and treatment-related adverse events were analyzed.Results:There were 1 male and 7 females in 8 patients with relapsed ITP after treatment with rituximab. The median age [ M ( Q1, Q3)] of the 8 enrolled patients was 52.5 (40.5, 56.0) years. The median relapsed times was 2.0 (2.0, 2.5) times and the median course of disease was 16.0 (13.0, 18.5) months. The platelet count increased from 8.73 (5.79, 11.65)×10 9/L pre-treatment to 180.00 (83.40, 255.00)×10 9/L post-treatment, and the difference was statistically significant ( Z = -2.37, P = 0.018); conversely, peripheral blood B-lymphocyte count decreased from 322.59 (148.29, 403.07) × 10 9/L pre-treatment to 1.23 (0.57, 1.76) ×10 9/L post-treatment, and the difference was statistically significant ( Z = -2.52, P = 0.012). After obinutuzumab and short-course dexamethasone treatment, 6 patients achieved complete remission, 1 case showed response, and 1 case had no response. No severe adverse events were observed during treatment and follow-up in all patients. Conclusions:Obinutuzumab combined with short-course dexamethasone appears to be effective in treating relapsed ITP patients after treatment with rituximab, and its safety is good.
5.Correlation between gut microbiota and diffuse large B-cell lymphoma based on Mendelian randomization

Objective To investigate the causal relationship between gut microbiota and diffuse large B-cell lymphoma(DLBCL).Methods Employed a two-sample bidirectional Mendelian randomization approach,using summary statistics from genome-wide association studies(GWAS)to extract single nucleotide polymor-phisms(SNPs)associated with exposure and outcome as instrumental variables.Exposure instrumental varia-bles(P<1×10-5)and outcome instrumental variables(P<5×10-8)were selected based on the P-values of SNPs.Three different Mendelian randomization methods were used to analyze the relationship between gut microbiota and DLBCL,with sensitivity analyses including heterogeneity,pleiotropy,and leave-one-out tests.Results Bilophila(OR=2.043,95%CI:1.279-3.264),Coprobacter(OR=1.371,95%CI:1.035-1.816),Eubacterium eligens group(OR=1.996,95%CI:1.291-3.087)increased the risk of DLBCL.Alistipes(OR=0.588,95%CI:0.359-0.963),Eubacterium eligens group(OR=1.996,95%CI:1.291-3.087),Slackia(OR=0.688,95%CI:0.479-0.988)reduced the risk of DLBCL.Reverse Mendelian randomization a-nalysis failed to reveal any evidence of a causal relationship between DLBCL and the six gut microbiota.Con-clusion There is a causal association between gut microbiota and DLBCL.
6.The mechanism and practical path of red doctor spirit in nurturing medical students’ professional identity

The red doctor spirit is a precious spiritual wealth formed in the great practice of the Chinese Communist Party leading the Chinese revolution and creating and developing people’s health undertakings. It is also an important cultural resource for nurturing medical students’ professional identity. The red doctor spirit provides cognitive resources and spiritual guidance for the formation of medical students’ professional identity, prompting medical students to form emotional identity, value identity, and behavioral identity with the profession of doctor, there by enabling medical students to more clearly understand their own missions and development goals, enhance their confidence in engaging in medical and health work, and strengthen their beliefs in working hard for the construction of a healthy China. In practice, to further enhance the leading power of the red doctor spirit in the formation of medical students’ professional identity, it is essential to deepen classroom teaching of the red doctor spirit to provide theoretical guidance; to strengthen the clinical practice of the red doctor spirit to provide practical guidance; and to foster a campus cultural atmosphere infused with the red doctor spirit , enabling medical students to naturally develop a sense of identification with the medical profession through immersive cultural influence.
7.Artificial intelligence warning model for urosepsis after upper urinary tract stone surgery:based on clinical multimodal data

Objective To construct and validate a prediction model for urosepsis in patients after upper urinary tract stone surgery using various machine learning algorithms.Methods A total of 7 464 upper urinary tract stone patients who underwent surgery at the Sixth Affiliated Hospital of Guangxi Medical University from Jun.2018 to Jun.2023 were enrolled and randomly assigned to training(5 224 cases)or validation sets(2 240 cases)at a ratio of 7∶3.Among them,622(8.33%)cases developed urosepsis postoperatively.Six machine learning algorithms,including extreme gradient boosting(XGBoost),logistic regression,light gradient boosting machine(LightGBM),random forest(RF),adaptive boosting(AdaBoost),and gradient boosting decision tree(GBDT),were used to construct prediction models for postoperative urosepsis.The model's predictive ability and clinical benefits were evaluated using receiver operating characteristic(ROC)curves,Shapley additive explanation(SHAP)analysis,calibration curves,and decision curve analysis(DCA).Results The clinical features included body mass index(BMI),number of surgeries,heart rate,Barthel index,venous thrombo embolism(VTE)risk assessment,gender,American Society of Anesthesiologists(ASA)grade,urinary nitrite,and urinary leukocyte in the models.In the training set,the XGBoost,LightGBM,and RF models performed excellently,with area under curve(AUC)values of ROC curves reaching 1.00.In the validation set,the logistic regression model performed the best,with an AUC value of ROC curve of 0.76,showing good predictive stability and calibration.The AdaBoost and GBDT models followed with AUC values of 0.74 and 0.75,respectively,while the AUC values of the LightGBM,XGBoost,and RF models were 0.71,0.70,and 0.68.In terms of model interpretability,SHAP analysis showed the contribution of variables in a descending order as:heart rate,urinary leukocytes,gender,BMI,Barthel index,VTE risk assessment,urinary nitrite,number of surgeries,and ASA grade.Conclusion A logistic regression model for early risk prediction of postoperative urosepsis in upper urinary tract stone patients has been successfully constructed.This model has good predictive performance and calibration,and can effectively assist clinical diagnosis.
8.Potential pharmacodynamic substances and mechanism of Zhizi Houpu decoction in treating insomnia based on UHPLC-QTOF-MS and network pharmacology

Objective To establish an ultra-high performance liquid chromatography with quadrupole time-of-flight mass spectrometry(UPLC-QTOF-MS)method for identifying the active ingredients of Zhizi Houpu decoction,and to predict its material basis and mechanism of anti-insomnia effects using network pharmacology.Methods The chemical components of Zhizi Houpu decoction were identified according to UHPLC-QTOF-MS ion fragmentation rule combined with compound library and literature review.Components with oral bioavailability(OB)≥30%and drug-like properties(DL)≥0.18 were selected to construct the Zhizi Houpu decoction-active ingredients-target network.The STRING database and the Database for Annotation,Visualization,and Integrated Discovery(DAVID)were used to screen key target proteins.Subsequently,enrichment analysis was performed for biological processes,molecular functions,cellular components,and signaling pathways.Results A total of 107 chemical components were identified,including 35 compounds related to insomnia and 11 anti-insomnia targets such as AKT serine/threonine kinase 1,tumor protein 53,and prostaglandin-endoperoxide synthase 2.Gene Ontology functional enrichment analysis indicated that Zhizi Houpo decoction may exert its effects through cellular components such as the presynaptic membrane,dendrite,and plasma membrane,by participating in biological processes like chemical synaptic transmission,G protein signaling pathway,and neuron apoptosis,and by regulating molecular functions such as G protein-coupled serotonin receptor activity.Kyoto Encyclopedia of Genes and Genomes pathway enrichment analysis revealed that its anti-insomnia effects were mainly concentrated in pathways such as neuroactive ligand-receptor interaction,calcium signaling pathway,serotonergic synapse,and cyclic adenosine monophosphate signaling pathway.Conclusion The anti-insomnia effect of Zhizi Houpo decoction is the result of a synergistic action involving multiple components,multiple targets,and multiple pathways.Its material basis and mechanism of action are primarily related to regulating biological processes such as neurotransmitter metabolism,synaptic function,neuronal apoptosis,and inflammatory response.
9.Study on KAP of drug safety behavior among residents in Hezhou city

Objective To investigate the knowledge,attitude,and behavior(Knowledge-Attitude/belief-Practice,KAP)of residents in Hezhou City regarding the safety risks of using traditional Chinese medicine(TCM).Methods A stratified cluster random sampling method was used to select residents from different areas of Hezhou City.A questionnaire based on the KAP model was designed for the survey.Descriptive analysis,univariate analysis,and multiple linear regression analysis were conducted to explore the factors influencing the KAP of TCM safety behavior.Results Among the 2 000 questionnaires distribu-ted,all were conducted online.Of these,98 were incomplete,resulting in a total of 1 902 valid responses,with a valid response rate of 95.10%.In the survey of 2 000 residents of Hezhou City,the average knowledge score was(54.34±2.50),the average behavior score was(64.32±2.44),and the average attitude score was(62.44±3.07).The differences in knowledge scores among residents with different places of residence,educational levels,medical insurance status,monthly incomes,and other de-mographic characteristics were statistically significant(P<0.05).The differences in behavior scores among residents with differ-ent medical insurance status and occupations were statistically significant(P<0.05).The differences in attitude scores among residents with different monthly incomes and medical insurance status were also statistically significant(P<0.05).Results of multiple linear regression analysis indicated that place of residence,medical insurance(self-paid),educational level(junior high school and below),and monthly income(below 2 000 yuan,2 001-4 000 yuan)were influencing factors for the knowledge score on safe use of proprietary Chinese medicines(P<0.05).Medical insurance(self-paid)and occupation(enterprise work-er,freelancer,student)were influencing factors for the behavioral risk score regarding safe use of proprietary Chinese medicines(P<0.05).Medical insurance(self-paid)and monthly income(below 2 000 yuan)were influencing factors for the attitudinal risk score regarding safe use of proprietary Chinese medicines(P<0.05).Conclusion The overall KAP level regarding the safety of using traditional Chinese medicine among residents in Hezhou City is relatively low,and medication safety is closely re-lated to place of residence,medical insurance,monthly income,occupation,etc.
10.Efficacy of combined magnetic-electrical stimulation,intelligent exercise prescription,and novel matrix radiofrequency therapy in the treatment of pelvic organ prolapse

Objective To investigate the therapeutic effects of combined magnetic and electrical stimulation with an"intelligent exercise prescription"and novel matrix radiofrequency therapy in patients with pelvic organ prolapse(POP).Methods A total of 158 patients with POP who received treatment at the Gynecological Pelvic Floor Rehabilitation Center of the Eighth Affiliated Hospital of Southern Medical University between October 2022 and July 2025 were retrospectively enrolled and divided into an observation group(n=64)and a control group(n=94)based on their treatment plans.The control group underwent magnetic and electrical stimulation combined with an"intelligent exercise prescription"regimen.Specifically,patients received 10 sessions of electrical stimulation,5 sessions of magnetic stimulation,and performed 15~20 minutes of daily home exercise training guided by the"intelligent exercise prescription."The observation group received,in addition to the aforementioned treatments,four sessions of novel matrix radiofrequency therapy.Changes in the muscle strength grades of type Ⅰ and type Ⅱ pelvic floor muscles,Glazer surface electromyography(EMG)values,and POP-Q staging were compared between the two groups before and after treatment.Results After treatment,both groups demonstrated significant improvements in type Ⅰ and type Ⅱ muscle fiber strength compared to baseline(all P<0.05),with the observation group showing greater improvement in type Ⅰ muscle fiber strength than the control group(P<0.05).The muscle potential values of the observation group during rapid contraction,tense contraction,and endurance contraction stages were markedly increased compared to pre-treatment levels.Moreover,the muscle potential values during the pre-resting stage were significantly reduced after treatment(P<0.05).In the observation group,POP-Q grades of the anterior vaginal wall,uterus,and posterior vaginal wall were all significantly lower post-treatment than pre-treatment(all P<0.05).However,no statistically significant differences were observed between the observation group and the control group in these parameters(P>0.05).Both groups exhibited relatively high compliance rates(both≥75.0%),with no significant difference between them(P>0.05).The treatment cost for the observation group was significantly higher than that for the control group(P<0.05).Conclusions The combination of magneto-electrical stimulation,an"intelligent exercise prescription,"and novel matrix radiofrequency therapy can significantly improve pelvic floor muscle strength and muscle potential values in the short term,compared to pre-treatment levels.This integrated approach also effectively alleviates the prolapse of the anterior vaginal wall,uterus,and posterior vaginal wall.Furthermore,the combination of magnetic and electrical stimulation,"intelligent exercise prescription,"and matrix radiofrequency therapy demonstrates superior efficacy in enhancing type Ⅰ pelvic floor muscle fiber strength when compared to the combination of magnetic and electrical stimulation with"intelligent exercise prescription"alone.However,this treatment protocol entails a relatively high economic burden,and its clinical application should be carefully evaluated in consideration of patients'functional needs and financial conditions.
