1.Analysis of hyperopic reserve status and associated factors among preschool children in Jiading District, Shanghai

Objective:
To investigate the current status of hyperopic reserve among preschool children and to determine the associations with genetic and behavioral factors, so as to provide evidence for early intervention strategies and the development of working protocols for myopia prevention with a forward shift of the prevention threshold.
Methods:
A total of 924 preschool children (3-6 years of age) from 6 kindergartens in Jiading District of Shanghai were enrolled by stratified random sampling from March to June 2024. Cycloplegic refraction using 1% atropine eye drops was performed to obtain accurate spherical equivalent (SE) values. Questionnaires were used to collect the genetic history and daily behavioral habits of the children. Refractive status was classified into three categories based on the SE:sufficient hyperopic reserve, relatively insufficient hyperopic reserve, and pre myopia.
Results:
The overall detection rate of insufficient hyperopic reserve among preschool children was 82.25%, consisting of 72.29% for relatively insufficient hyperopic reserve and 9.96% for pre myopia. Multivariate Logistic regression analysis showed that parental bilateral myopia was independently and positively associated with relatively insufficient hyperopic reserve ( OR =2.13) and pre myopia ( OR =4.35) compared to children with sufficient hyperopic reserve; electronic screen exposure time ≥2 h/d on school days was an independent and positive risk factor for pre myopia ( OR =2.83); all of the above associations were statistically significant (all P <0.05). No statistically significant association was noted between outdoor activity and hyperopic reserve (all P >0.05).
Conclusions
The insufficient hyperopic reserve detection rate is relatively high among preschool children in Jiading District, Shanghai. Early management of electronic device use with regular screening and monitoring for preschool children with a family history of myopia should be strengthened to shift the prevention threshold earlier and prevent childhood myopia.
2.Simultaneous determination of lacosamide and perampanel concentration in human plasma by LC-MS/MS

OBJECTIVE To establish a method for simultaneous determination of two third-generation anti-epileptic medicines such as lacosamide and perampanel in human plasma and apply this method in clinical practice. METHODS Using clozapine as internal standard, the concentrations of lacosamide and perampanel of plasma samples in 10 epileptic patients were determined by LC-MS/MS after protein precipitation with acetonitrile and dilution with acetonitrile-water (20∶80,V/V), and the plasma minimum concentrations were obtained by dilution of multiple. The determination was performed on Welch Ultimate XB-C18 column, with mobile phase A consisted of 10 mmol/L ammonium formate and mobile phase B consisted of methanol-acetonitrile-isopropanol (0.2% formic acid) mixed solution (7∶1.5∶1.5, V/V/V) for gradient elution at the flow rate of 0.4 mL/min. The column temperature was set at 40 ℃ , and the sample size was 5 μL. The electrospray ion source and multi-reaction monitoring mode were used for positive iron scanning. The ion pair used for quantitative analysis of lacosamide, perampanel and internal standard were m/z 251.2→ 144.1, m/z 350.2→219.2 and m/z 327.2→270.0, respectively. RESULTS The linear ranges of lacosamide and perampanel were 0.001 25-0.125 μg/mL(r>0.99), 0.037 5-3.75 ng/mL (r>0.99); the limits of quantification were 0.001 25 μg/mL and 0.037 5 ng/mL, respectively. The precision and accuracy within and between batches, extraction recovery rate, matrix effect, and stability all met relevant requirements. The minimum concentrations of lacosamide in No. 1-5 patients were 5.3-12.2 μg/mL, and the minimum concentrations of perampanel in No.6-10 patients were 208-510 ng/mL, respectively. CONCLUSIONS The established method is simple, rapid and suitable for the therapeutic drug monitoring of lacosamide and perampanel.
3.Treatment of Helicobacter pylori induced chronic atrophic gastritis with traditional Chinese medicine combined standard triple therapy and its mechanisms

Objective:To treat the chronic non-atrophic gastritis patients induced by Helicobacter pylori with Qingweizhitong Weiwan combined with standard triple therapy,and to detect the differential expression of related immflammation genes with PCR array,and to clarify its mechanism.Methods: Ten patients with chronic non-atrophic gastritis complicated with Helicobacter pylori infection were used as treatment group and 10 health people were used as health control group. The patients in treatment group were treated with Qingweizhitong Weiwan combined with standard triple therapy for 14 d. The blood samples of the subjects in treatment group and health control group were collected before and after treatment,and QIAGEN human antibacterial response PCR array was performed to test the total RNA inperipheral blood and to analyze the differential expressions of 84 inflammation-related genes.Results:The differential expressions of 20 inflammation-related genes were found.Compared with health control group,the expression levels of 20 genes in treatment group before treatment were up-regulated (Fold-change>2);after treatment,the expression levels of 20 genes were down-regulated,and 11 of them were similar to the level in health control group (Fold-change< 2).More specifically,part of 20 genes was related to NLRP3 inflammasome.Compared with health control group,the gene expression levels of CASP1,IL1B,NLRP3,and PYCARD in treatment group before treatment were up-regulated (P <0.05).Compared with before treatment,the expression levels of CASP1,IL1B,NLRP3,and PYCARD in treatment group after treatment were down-regulated (P <0.05).Conclusion:The mechanism of Qingweizhitong Weiwan combined with standard triple therapy in the treatment of chronic non-atrophic gastritis patients induced by Helicobacter pylori may be related to inhibiting the expressions of NLRP3 inflammasome-related genes and interfering the antimicrobial innate immune response.
4.Effect of the collaboration care model on posttraumatic growth and resilience in patients with breast cancer

Objective To evaluate the effect of the collaboration care model on posttraumatic growth and resilience in patients with breast cancer.Methods 70 patients who were diagnosed with breast cancer in department of Breast Surgery of the First Affiliated Hospital of Dalian Medical University were assigned into two groups by random number table.32 patients who completed the research in the experimental group received collaboration care while 31 patients who completed the research in the control group received routine care both for there months.The patients were investigated with the Posttraumatic Growth Inventory (PTGI) and Connor-Davidson Resilience Scale (CD-RISC) before and after intervention.Results After the intervention,there were significant differences in scores of posttraumatic growth [(63.83±11.87) scores vs.(60.33±10.09) scores,t=2.384] and resilience [(66.52±15.23) scores vs.(61.33±10.09) scores,t=2.472] between the two groups (P<0.05).And also there were significant differences of posttraumatic growth scores before and after intervention in the experimental group [(56.24±10.01) scores vs.(63.83±11.87) scores,t=-3.988,P< 0.01] and the control group [(56.68±10.32) scores vs.(60.33±10.09) scores,t=-2.426,P<0.05].Differences of resilience also existed in the experimental group [(61.28±14.40) scores vs.(66.52±15.25) scores,t=-3.225,P<0.01] and the control group [(63.97±17.66) scores vs.(61.33±10.09) scores,t=-2.370,P<0.05].Conclusions Collaboration care model can significantly improve the level of patients' posttraumatic growth and resilience.
5.Comparison among the mental profile of earthquake student, teacher and the masses survivors

Objective To investigate the prevalence and severity degree of post traumatic stress disorder(PTSD), anxiety and depression, and to explore the different mental profile in different groups involved with an earthquake. Methods 26 days after 8.0 grade earthquake in Wenchuan, psychological rescue team of State Administration of Traditional Chinese Medicine went to some communities in Deyang and Mianzhu area of Sichuan province. In this period, a total of 119 survivors, including students(42), teacher(40) and masses(37), were investigated through impact of event scale (IES), self-rating anxiety scale (SAS) and self-rating depression scale(SDS). Results①The detection rate of score over 19 in IES in 3 groups (students, teacher and masses) were 69%, 80% and 91.9%, respectively. There was significant difference among these 3 groups (F=5.611, P=0.005<0.01) in respect of severity degree of PTSD. ②The values of severity degree of anxiety in 3 groups were significant higher than normal value (P<0.001, all). There was also significant difference among these 3 groups (F=3.376, P=0.038<0.05) in respect of severity degree of anxiety, with the masses group being significant higher than student and teacher group (P=0.029, P=0.022, respectively). ③The values of severity degree of depression in these 3 groups were significant higher than normal value (P<0.05, all). But there was no significant difference (F= 0.670, P=0.514) among these 3 groups. ConclusionThe results suggested the importance to assess PTSD, anxiety and depression in post-disaster area, especially PTSD and anxiety evaluations in the masses group.
