1.Efficacy of direct-acting antiviral agents combined regimens for hepatitis C virus with different genotypes in Dehong Prefecture, Yunnan Province from 2022 to 2024

ObjectiveTo investigate the therapeutic effects of direct-acting antiviral agents (DAAs) combined regimens for hepatitis C virus (HCV) patients in Dehong Prefecture, Yunnan Province from 2022 to 2024, to analyze the characteristics of treatment failure patients, so as to provide a basis for discovering more effective treatment regimens in the future. MethodsData on HCV prevention and treatment in Dehong Prefecture was extracted from the China Disease Control and Prevention Information System. A total of 617 patients with HCV antiviral therapy were included, and the differences in variable characteristics among patients with different genotypes were analyzed using comparative statistical tests, including basic socio-demographic characteristics, biochemical testing indicators, and information on previous treatment and current treatment. In addition, the cure rate of HCV patients with diverse characteristics was compared, and the potential causes of treatment failure were explored simultaneously. ResultsThe cure rate of HCV was 96.8%, and statistically significant differences were observed in aspartate transaminase (AST) and alanine transaminase (ALT) levels, previous antiviral therapy history and initial treatment regimens among patients with different HCV genotypes (all P<0.05). Among the multi-type combination regimens, the cure rate of sofosbuvir (SOF)-containing regimens was 97.00%, that of velpatasvir (VEL)-containing regimens was 95.45%, and the cure rate of other treatment regimens, including the regimens with ribavirin (RIB) intervention, was 93.10%. Among the patients with treatment failure, 45.00% had genotype 3, 40.00% had abnormal abdominal ultrasound results, and all presented with elevated baseline AST test levels. ConclusionThe clinical treatment of HCV patients should consider the differences in genotype and biochemical test results. DAAs combined regimens for HCV have achieved a high cure rate in Dehong Prefecture and are applicable to HCV patients with diverse clinical characteristics, providing research evidence for wider application.
2.Characteristics of HIV-infected persons and their spouses who became HIV-positive in HIV discordant couples in Dehong Prefecture, Yunnan Province

ObjectiveTo determine the characteristics, viral load and immunological status of HIV-infected persons and their spouses who became HIV-positive, and the reasons for HIV seroconversion in 55 HIV discordant couples in Dehong Dai and Jingpo Autonomous Prefecture (Dehong Prefecture), Yunan Province. MethodsData on the 55 couples meeting the criteria of having a previously positive spouse were retrieved from the AIDS Integrated Prevention and Control Data Information System of the China Disease Control and Prevention Information System during 2015-2021. General socio-demographic information, age at diagnosis, exposure history, CD4+T lymphocyte count, and antiviral treatment were collected. Descriptive analysis and chi-square test were used to compare the distribution of pre-HIV-positive spouses and their HIV seroconverted spouses. ResultsA total of 55 spouses from HIV discordant couples had HIV seroconversion. Of them, 72.7% (40/55) of pre-HIV-positive spouses were husbands. The most recent CD4+T lymphocyte count in the pre-HIV-positive spouses was (328.31±246.27) cells·μL-1 at the time of diagnosis of their seroconverted spouses, of which 36.3% (20/55) had a CD4+T lymphocyte count of less than 200 cells·μL-1. Furthermore, of those pre-HIV-positive spouses with low CD4+T lymphocyte count, 45.0% (9/20) had an undetectable viral load, 15.0% (3/20) <400 copies·mL-1, and 25.0%(5/20) ≥400 copies·mL-1. Additionally, 16.4% (9/55) of the pre-HIV-positive spouses did not have a viral load test. The main reasons for HIV seroconversion among HIV-negative spouses in the discordant couples were poor condom use, poor compliance with antiviral therapy, and treatment discontinuation. ConclusionThe follow-up management of HIV discordant couples should be strengthened in Dehong Prefecture, especially the monitoring of viral load levels and immunological status of pre-HIV-positive spouses, to improve their compliance with antiviral therapy and reduce treatment discontinuation, which would effectively prevent and control HIV transmission between spouses.
3.Hepatitis C virus co-infection and influencing factors among newly reported HIV concordant couples in Dehong Prefecture of Yunnan Province from 2016 to 2019

ObjectiveTo determine the characteristics and influencing factors of hepatitis C virus (HCV) co-infection among HIV concordant couples in Dehong Prefecture. MethodsUsing the data of newly reported HIV concordant couples in Dehong Prefecture from 2016 to 2019, we collected the demographic characteristics, exposure history, and HCV infection to determine the characteristics of HCV co-infection among HIV concordant couples. ResultsAmong the 160 HIV concordant couples included in the study, 46 (28.8%) males and 14 (8.8%) females were co-infected with HCV. The prevalence of HIV-HCV co-infection was higher among the male spouses who were diagnosed less than 40, Jingpo ethnic, Burmese, illiteracy, farmers, and intravenous drug users. In contrast, the prevalence of HIV-HCV co-infection was higher among the female spouses who were diagnosed less than 40, Jingpo ethnic, and Burmese. Logistic regression analysis among male spouses showed that the Jingpo ethnic and intravenous drug users had higher risk of HCV co-infection. ConclusionHIV concordant couples in Dehong Prefecture have high prevalence of HIV-HCV co-infection. Effective intervention strategies should be developed based on ethnic-specific factors and exposure characteristics among male and female spouses of HIV concordant couples.
4.Clinical implications of first CD4+T cell counts among newly reported HIV-infected Burmese in Dehong Prefecture of Yunnan Province in 2019 and 2020

ObjectiveTo investigate the demographic characteristics, high-risk behavior and first CD4+T cell(CD4 cell) counts among newly reported HIV-infected Burmese in Dehong Dai Jingpo Autonomous Prefecture (Dehong Prefecture) from 2019 to 2020. MethodsA cross-sectional survey was conducted among newly reported HIV-infected Burmese cases in Dehong Prefecture from 2019 to 2020 to collect related information and measure their first CD4 cell counts. ResultsAmong 1 228 HIV-infected Burmese cases, 846 (68.9%) were males and 382 (31.1%) were females; 445 (36.2%) were from Kachin, and 210(17.1%) were from Shan. Of them, 960 cases (78.2%) had participated in the first CD4 cell counts testing, and 405 cases (42.2%) had a first CD4 cell counts <350 cells·μL-1. In the multivariable logistic regression analysis, age ≥30 years (adjusted OR=1.44, 95%CI: 1.07‒1.92) and history of illicit drug use in Myanmar (adjusted OR=0.41, 95%CI: 0.21‒0.79) were independently associated with first CD4 cell counts <350 cells·μL-1. ConclusionThe first CD4 cell counts are generally low among newly reported HIV-infected Burmese in Dehong Prefecture, especially those aged ≥ 30 years and with a history of intravenous drug use in Myanmar. Therefore, early detection of HIV infection should be strengthened for timely intervention and treatment management among Burmese.
5.Application research of ISBAR communication mode based on nursing priorities in nursing rounds

Objective:To study the clinical value of the ISBAR communication model based on nursing priorities in nursing ward rounds.Methods:Using a quasi-experiment method, 104 patients who received secondary and tertiary care from April to June 2020 were set as the control group, and 122 patients who received secondary and tertiary care from July to September 2020 were set up as the experimental group, with non-randomized method. The experimental group used the ISBAR communication model based on nursing priorities for nursing ward rounds, and the control group used the traditional method for nursing ward rounds. The abilities of bedside nurses collecting the patients′condition, the bedside nurses′ condition observation and the nurses′ critical thinking were compared between the two groups, and the patients' nursing quality and satisfaction were also compared.Results:The scores of the bedside nurses′ knowledge of the patients′ condition were 81.14±3.09 in the test group and 75.90±2.84 in the control group, with statistically significant difference ( t value was -12.88, P<0.01). The scores of the bedside nurses′ ability in observing patients′ condition were 140.50 (137.00, 144.50) in the experimental group and 135.50 (128.75, 139.00) in the control group, with statistically significant difference ( Z value was -3.399, P<0.01). The scores of the Critical Thinking Disposition Inventory Chinese-version (CTDI-CV) were 309.82±36.33 in the experimental group and 286.86±33.54 in the control group, with statistically significant difference ( t value was -2.177, P<0.05). The scores of patient care quality evaluation were 62.50 (60.00, 64.00) in the experimental group and 59.00 (55.00, 61.50) in the control group, with statistically significant difference ( Z value was -6.47, P<0.01). The patient discharge satisfaction score of the experimental group was 53.00 (48.75, 55.00), and that of the control group was 51.00 (44.00, 54.00), with statistically significant difference ( Z value was -3.56, P<0.01). Conclusions:The application of ISBAR communication model based on nursing priorities in nursing ward rounds had a significant promotion effect on the improvement of nurses' abilities of collecting the patients′ condition, observing patients′ condition, and critical thinking, and can effectively improve nursing care quality and patients′ satisfaction.
6.Research on the rules of Traditional Chinese Medicine prescriptions of depression based on Ancient and Modern Medical Records Cloud Platform

Objective:To explore the relationship between drug evidence and core prescription for depression.Methods:We retrieved literature of TCM for depression from CNKI, VIP and Wangfang databases to November 2019, 30th as well as there cords from Ancient and Modern Medical Records Cloud Platform (V 1.5). The Excel 2010 was used to establish the standardized database of medical records. After the standardization of medicines, Ancient and Modern Medical Records Cloud Platform (V1.5) statistics methodswere used for association rules analysis, complex networks, and analysis of drugs’ frequency, medical characteristics, core prescription drugs.Results:A total of 632 effective prescriptions were included, involving a total of 527 drugs. The results of frequency of herbs showed that 23 kinds of high-frequency herbs were obtained. Bupleuri Radix was the most frequently used medicine. Most herbs are warm or flat, with pungent, sweet and bitter in taste, belonging to the lung, liver, heart and spleen meridians. A total of 25 drug-pair association and 13 TCM association were obtained by association rule analysis. Conclusions:TCM treatment for depression is mainly based on soothing the liver and regulating qi, clearing the heart and calming the nerves. Bupleuri Radix, Curcumae Radix, Paeoniae Radix Alba, Chuanxiong Rhizoma, Ziziphi Spinosae Semen are the basic prescriptions. Angelicae Sinensis Radix, Ophiopogonis Radix, Albiziae Cortex, Polygalae Radix, Poria are used as reference.
7.Impact of continuous nursing based on WeChat Public Platform and Sojump on fluid exchange in patients with peritoneal dialysis

Objective:To explore the effect of continuing care based on the WeChat public platform questionnaire star on the fluid replacement operation of peritoneal dialysis patients.Methods:Convenience sampling method was used to select 210 patients undergoing peritoneal dialysis in the Panyu Central Hospital, Guangzhou from January to June 2019. They were divided into a experimental group and a control group by random number table method, each with 105 cases. The patients of the control group were given routine continuous care for peritoneal dialysis, such as outpatient follow-up, telephone follow-up, home visit, etc., and oral education. On this basis, in the experimental group, the patients first joined the WeChat public platform, implementing network-based health education and questionnaire stars for real-time testing, and multi-dimensional communication with patients. The knowledge of the peritoneal dialysis fluid replacement operation, the fluid replacement operation evaluation score, the catheter outlet replacement drug evaluation score, the fluid replacement operation self-management score and compliance were compared before and after intervention in the two groups. And the incidence of complications related to the peritoneal dialysis fluid exchange operation of the two groups were compared before and after intervention.Results:The experimental group′s knowledge of peritoneal dialysis fluid exchange operation, examination score of fluid exchange operation, and abnormal situation management score of fluid exchange operation management were 55.88±8.97, 92(89, 96), 12(11, 12) after three-month intervention, and higher than the control group [51.88±10.24, 91 (86, 94), 12 (9, 12)], with statistically significant difference ( t value was 3.00, Z value was -2.483, -2.308, P<0.05). After six-month intervention, the relevant knowledge of peritoneal dialysis fluid replacement operation, fluid replacement operation evaluation score, catheter outlet replacement drug evaluation score and fluid replacement operation management score (routine fluid replacement operation, abnormal situation handling) of the experimental group were 57.52±9.41, 93 (90,96), 90 (87,94), 18 (17,18), 12 (11,12), which were higher than the control group [53.61±10.29, 90 (86,94), 90 (87,92), 17 (15,17), 12 (9,12)], with statistically significant difference ( t value was 2.87, Z value was from -7.181 to -2.664, P<0.01). The difference between the groups was statistically significant ( χ2 value was 4.21-7.517, all P<0.05); and the difference between the intervention time points was statistically significant ( χ2 value was 11.805-164.95, all P<0.05). After six-month intervention, the complications of catheter infection, peritonitis, and poor drainage were 9 (8.91%), 5 (4.95%), and 5 (4.95%) in the experimental group, which were significantly lower than those in the control group [21 (20.39%), 14 (13.59%), 15 (14.56%)], with statistically significant differences ( χ2 value was 5.36, 4.51, 5.33, both P<0.05). Conclusions:The continuous care of the WeChat public platform questionnaire star has a significant promotion effect on the improvement of the knowledge, operation ability, compliance and self-management behavior of the peritoneal dialysis patients, and can effectively reduce the complications caused by the peritoneal dialysis operation.
8.Prevalence and related factors of CD4 +T lymphocytes immune recovery among adult HIV/AIDS on antiretroviral therapy in Dehong Dai and Jingpo Autonomous Prefecture

Objective:To analyze the longitudinal characteristics of CD4 +T lymphocytes (CD4) among the adult HIV/AIDS on antiretroviral therapy (ART) and the related factors. Methods:A retrospective cohort of adult HIV/AIDS starting ART in Dehong Dai and Jingpo Autonomous Prefecture (Dehong) in 2007-2016 was followed up to December 31, 2018. Group-based trajectory models were utilized to identify CD4 subgroups based on immune recovery (whether and when CD4 reached the average level of >500 cells/μl). The demographics and information at ART baseline were described, and the related factors were analyzed with polytomous logistic regression. The SAS 9.4 software was used for statistical analysis.Results:A total of 7 605 adults with HIV/AIDS were included, of which the median ( P 25, P 75) age at ART were 36 (30,43) years old, 61.0% were male, 42.5% were Han nationality, and 60.8% with the education of primary school or below. The follow-up duration M ( P 25, P 75) was 6.1 (4.1,8.1) years. HIV/AIDS in Dehong showed four CD4 trajectory subgroups from low to high: below the average level, primary recovery to a normal level, full recovery to a moderate level, and normal steady level, accounting for 34.4%, 39.8%, 20.6%, and 5.2%, respectively. When compared with corresponding control groups, age <35 years at ART, female, education of middle school or above, sexual transmission, no opportunistic infection, CD4 ≥200 cells/μl, baseline regimen with tenofovir (TDF) and time from HIV diagnosis to ART <1 year were the related factors facilitating the higher CD4 subgroups. Conclusions:The various CD4 immune recoveries of HIV/AIDS were changing patterns after ART. Starting ART with a high CD4 level was beneficial to CD4 recovery to normal level during the follow-up period. Early initiation of ART and exceptional attention to CD4 immune recovery should be encouraged after the ART.
9.Effect of multidisciplinary collaborations in control of peritoneal dialysis patients with hypertension

Objective To explore the application effect of the multidisciplinary collaborations in control of peritoneal dialysis patients with hypertension. Methods A total of 220 cases of peritoneal dialysis patients divided into experimental group and control group (each group had 110 cases) according to the random number table. In which, the control group received routine capacity control and health education with a total of 105 patients finished the study. The experimental group received of multidisciplinary collaborations on the basis of routine capacity control and health education with a total of 107 patients finished the study. Observe changes with knowledge of drugs, medication compliance, self-management behavior and blood pressure of patients before and after the intervention respectively. Results In experimental group,the scores of drug knowledge, medication compliance and self-management behaviors were (0.93 ± 0.49), 0.00 (0.00, 0.25), (2.69 ± 0.25) points before the intervention, 6 months after the intervention were (1.17 ± 0.54), 0.25 (0.00, 0.50), (2.86 ± 0.15) points, the difference between the groups was statistically significant (t=38.60, Z=4.34, t= 2.45, P < 0.01 or 0.05). In control group,the scores of drug knowledge, medication compliance and self-management behaviors were (0.87 ± 0.45), 0.00 (0.00, 0.25), (2.64 ± 0.27) points before the intervention, 6 months after the intervention were (0.89 ± 0.43), 0.00 (0.00, 0.38), (2.73 ± 0.27) points, there was no significant difference between drug knowledge and medication compliance (t=0.44, Z=1.83, P > 0.05), there were statistically significant differences in self-management behavior (t=6.23, P<0.01);there was no difference between the statistical significance between the 2 groups before intervention (t=1.02, Z=1.46, t=1.32, P > 0.05); there was significant difference between the 2 groups after intervention (t=4.11, Z=4.03, t=4.34, P<0.01). Patients in the experimental group with the systolic and diastolic blood pressure were (147.11 ± 14.31), (90.16 ± 13.02) mmHg (1 mmHg=0.133 kPa) respectively, before the intervention; 6 months after the intervention were (139.39 ± 17.05), (83.76 ± 12.52) mmHg respectively, the difference was statistically significant (t=3.59, 2.92, P<0.01). The control group before intervention were (149.56 ± 18.11), (93.56 ± 15.09) mmHg respectively, 6 months after the intervention were (145.14±20.50), (88.14±10.88) mmHg respectively, the difference was statistically significant (t=2.02, 2.72, P<0.05 or 0.01);there was no significant difference between the two groups before intervention (t=1.09, 1.82, P>0.05);6 months after the intervention there was significant difference between the 2 groups (t=2.22, 2.72, P < 0.05 or 0.01). Conclusions Multidisciplinary collaborations have a significant role in patients with peritoneal dialysis, especially in blood pressure control, medication compliance and self-management behavior.
10.Research of the clinical effect and immune regulatory mechanism of red blue light combined with Niuhuang-Shangqing capsules for the moderate-severe acne

Objective To investigate the the clinical effect and immune regulatory mechanism of red-blue light combined with Niuhuang Shangqing capsules for the moderate-severe acne.Methods A total of 180 patients with moderate to severe acne treated in People's Hospital of Nanhai District in Foshan City from June 2014 to June 2016 were randomly divided into 2 groups with each group 90 patients. The control group was treated with red and blue light, and observation group was treated with the combination of red and blue light andNiuhuang-Shangqingcapsules. The levels of SP(Substance P), IL-1 and IL-6 in peripheral blood were observed and analyzed. The recurrence rates and adverse events were observed.Results After treatment, the SP (657.4 ± 36.6 pg/mlvs.799.9 ± 60.2 pg/ml,t=19.188), IL-1(61.8 ± 24.7pg/L vs.92.1 ± 23.5 pg/L,t=8.431), IL-6 (38.7 ± 10.3pg/mlvs.66.7 ± 14.1pg/ml, t=12.421) of the observation group were significantly better than those of the control group (allPs<0.01). The total effect rate of the observation group were 91.11% (82/90), which were significantly higher than 70.00% (63/90) of control group (χ2=12.804,P<0.01). Conclusions The Red-blue light combined withNiuhuang-Shangqing capsules for treating moderate-severe acne, has significant effect and can reduce the serum levels of SP, IL-1 and IL-6, with fewer adverse events and lower recurrence rates. They can be used as a safe and effective treatment of acne.
