Objective:To investigate the correlation between the expression level of glypican-6 (GPC6) in gliomas and the prognosis of patients, as well as the effect of GPC6 on the proliferation of glioma cells in vitro. Methods:The transcriptome sequencing (RNA-seq) data of GPC6 gene and clinical data of glioma patients were downloaded from The Cancer Genome Atlas (TCGA) database. A total of 667 patients were enrolled, and the median FPKM value of GPC6 RNA-seq was used to distinguish the low and high expression of GPC6 gene. Kaplan-Meier method was used to analyze the overall survival (OS) of GPC6 high and low expression groups in 664 patients with survival data, and log-rank test was used for inter group comparison. The postoperative tumor tissue chips were retrospectively collected from glioma patients who underwent surgery from February 2008 to October 2011, immunohistochemistry (IHC) was used to detect the expression of GPC6 protein in 147 samples, and semi-quantitative scoring ≥ 4 points or not was used to distinguish the high and low expression of GPC6 protein based on the proportion of positive cells and staining intensity. The distributions of patients with high and low expression of GPC6 protein in tumor tissues were compared between different clinical pathological features in 147 patients, and Kaplan-Meier method was used to analyze the overall survival and disease-free survival (DFS) of patients in the GPC6 protein high and low expression groups; the risk factors that affect poor OS and DFS in patients were analyzed using univariate and multivariate Cox proportional hazards models. The human glioma U251 cells were infected with lentivirus packaged with GPC6 small interfering RNA and lentivirus packaged with unrelated control sequences, namely ShGPC6 group and ShCtrl group, respectively; green fluorescent cells were counted daily and the cell proliferation fold was calculated for 5 days, and the cell proliferation curve was plotted. Results:In TCGA database, the median FPKM value of 667 glioma patients was 225.66. The median FPKM value of GPC6 gene in 515 World Health Organization (WHO) classification low-grade glioma patients was lower than that in 152 high-grade glioma patients (201.10 vs. 347.92), and the difference was statistically significant ( Z = 5.36, P < 0.001); the OS of patients in GPC6 low expression group (332 cases) was better than that in GPC6 high expression group (332 cases), and the difference was statistically significant ( P < 0.001). IHC staining of the tissue chip showed that GPC6 protein was mainly expressed in the cytoplasm and nucleus of glioma cells; high expression of GPC6 protein accounted for 31.3% (46/147) of patients. Among patients who died, relapsed, had high grading, had survival time ≤ 84 months, and had positive epidermal growth factor receptor, the proportion of patients with GPC6 protein high expression was relatively higher, and the differences were statistically significant (all P < 0.05). There were no statistically significant differences in the proportions of GPC6 protein high expression patients among patients with different genders, age ≤ 43 years old, Ki-67 positivity, and programmed death receptor ligand 1 positivity (all P > 0.05); compared to the GPC6 low expression group (101 cases), the GPC6 protein high expression group (46 cases) had worse overall survival (median OS time: 57.0 months vs. not reached) and DFS (median DFS time: 33.0 months vs. not reached), and the differences were statistically significant (both P < 0.001). Multivariate Cox regression analysis showed that high expression of GPC6 protein (high expression vs. low expression, HR = 1.86, 95% CI: 1.04-3.31, P = 0.036) was the independent risk factor for poor OS in glioma patients, but the expression level of GPC6 protein was not the independent influencing factor for DFS (high expression vs. low expression, HR = 1.55, 95% CI: 0.95-2.53, P = 0.077). After infecting U251 cells with recombinant lentivirus for 3 days, reverse transcription polymerase chain reaction detection showed that the relative expression level of GPC6 mRNA in the ShGPC6 group was lower than that in the ShCtrl group, and the difference was statistically significant ( P < 0.001), the knockdown efficiency of GPC6 reached 79.32%; under the fluorescence microscope, the number and intensity of fluorescence positive U251 cells in the ShCtrl group increased day by day after infection with recombinant lentivirus, while there was no significant change in the ShGPC6 group; starting from the second day, the proliferation rate of U251 cells in the ShCtrl group was higher than that in the ShGPC6 group, and the differences were statistically significant (all P < 0.01). Conclusions:High GPC6 expression level may associate with the high risk of death in glioma patients, and high GPC6 expression may promote the proliferation of glioma cells.

Intra-aortic balloon pump(IABP),as the most commonly used percutaneous mechanical circulatory assist device,is routinely implanted through the femoral artery pathway. However, when implantation through the femoral artery pathway is difficult or contraindicated, other pathways including the brachial artery pathway, axillary artery pathway, etc.can be considered.IABP assisted interventional treatment for complex high-risk and indicated patients(CHIP)can reduce the risk of intraoperative complications, stabilize hemodynamics, increase the complete revascularization rate of CHIP,and improve long-term prognosis. The reverse guidewire technique can improve the success rate of percutaneous coronary intervention(PCI)for chronic total occlusion(CTO)of coronary arteries. In clinical practice, the reverse pathway is often chosen in addition to the forward pathway, such as the contralateral radial and brachial artery pathway, femoral artery pathway, etc. This article reports a case of bilateral femoral artery occlusion,in which IABP was implanted through the left brachial artery pathway, and then with the assistance of IABP,the right coronary artery CTO lesion was successfully opened through the right radial artery in the forward direction and the right brachial artery in the reverse direction, with the aim of providing reference for clinical PCI treatment for such special cases.

Objective:To explore the value of ultrasound imaging characteristics combined with clinical indicators in assessing the prognosis of patients with pancreatic ductal adenocarcinoma (PDAC).Methods:A retrospective analysis was conducted for patients who underwent pancreatic contrast-enhanced ultrasound (CEUS) from September 2017 to October 2023 at Peking Union Medical College Hospital and were diagnosed with PDAC based on pathological findings. Various parameters were recorded, including CA19-9 levels, tumor size, location, morphologic features, echogenicity, presence of internal cystic components, dilatation of the main pancreatic duct, peripheral vascular invasion, CEUS characteristics, presence or absence of liver metastasis, and treatment methods. In April 2024, patient survival information was obtained through telephone follow-up or review of medical records. Based on the results of the cox regression model analysis, a nomogram model of the risk of death was developed. The receiver operating characteristic (ROC) curves were applied to evaluate the predictive efficacy of the model. The calibration curves were plotted to evaluate the accuracy of the model, and clinical decision curves were used to evaluate the clinical benefit of the model. Results:This study included a total of 207 patients with PDAC. As of April 2024, 71 patients were alive and 136 died, with a median survival time of 14 months (95% CI: 12 -17). Multivariate analysis confirmed that the elevated CA19-9 ( HR=1.689, 95% CI: 1.102-2.588), tumor size >4 cm ( HR=1.641, 95% CI: 1.159-2.322), taller-than-wide shapes ( HR=1.450, 95% CI: 1.019-2.065), incomplete hypo-enhancement ( HR=1.618, 95% CI: 1.100-2.380), and liver metastasis ( HR=1.687, 95% CI: 1.175-2.423) were independent risk factors for survival in patients with PDAC. A nomogram model was further constructed for 6-month, 12-month and 3-year survival of patients with PDAC. The areas under the ROC curve were 0.679, 0.705 and 0.815, respectively. The calibration curves suggested that the model was more accurate, and the clinical decision curves showed that the model had a better clinical benefit. Conclusion:The combined use of ultrasound imaging characteristics and clinical indicators could effectively predict the prognosis of PDAC patients. Specifically, tumor size >4 cm, taller-than-wide shapes, incomplete hypo-enhancement, elevated CA19-9, and the presence of liver metastasis are correlated with poorer survival outcomes. The nomogram model constructed on the basis of these factors can be used to assess the survival of patients with PDAC.

A mounting body of research suggests that circRNAs significantly contribute to the develop-ment of myocardial fibrosis. The microarray results of human circular RNA expression profile indicated that circHERC4_041 expression increased in the myocardium of patients with heart failure,RT-qPCR a-nalysis confirmed that the myocardial expression level of circHERC4_041 in individuals with heart failure were considerably elevated compared to that in healthy organ donors.Fluorescence in situ hybridization(FISH)confirmed that circHERC4_041 was abundant in the cytoplasm of human cardiomyocyte AC16.Overexpression of circHERC4_041 in mouse myocardial fibroblasts(mCFs)mediated by adenovirus in-hibited the expression of fibrosis-related proteins in mCFs.Experiments involving cell proliferation, wound healing, and Transwell assays demonstrated that overexpression of circHERC4_041 suppressed the growth and mobility of mCFs(P<0.001).Sequence analysis results suggested that circHERC4_041 con-tains potential ribosome entry sequence(IRES)and open reading frame(ORF).Western blot confirmed that circHERC4_041 could translate the 516 amino acid HERC4-516aa protein, which was mainly located in the cytoplasm of the cell.Cell functional experiments confirmed that circHERC4_041 inhibited the fi-brotic phenotype of mCFs by specifically translating HERC4-516aa(P<0.05).The specific interaction between HERC4-516aa and transglutaminase 2(TGM2)was confirmed by IP-MS screening and Co-IP i-dentification.Further results found that the degradation of TGM2 was promoted through proteasome path-way.The overexpression of TGM2 in mCFs facilitated by adenoviral vectors could counteract the suppres-sive effects of HERC4-516aa on the fibrotic phenotype of mCFs.Therefore, this study confirmed that the HERC4-516aa protein translated by circHERC4_041 can specifically bind to TGM2 to inhibit the fibrotic phenotype of myocardial fibroblasts.
