OBJECTIVE:
To screen the optimal regimen of acupuncture and moxibustion for obstructive sleep apnea hypopnea syndrome (OSAHS), so as to provide the evidences for clinical decision-making.
METHODS:
From 7 databases in Chinese and English i.e. the Full-Text Database of China Journal Network (CNKI), Wanfang Data Knowledge Service Platform (Wanfang), VIP Information Chinese Journal Service Platform (VIP), Chinese Biomedical Literature Database (SinoMed), PubMed, Web of Science (WOS) and Cochrane Library, randomized controlled trial (RCT) articals of OSAHS treated with acupuncture and moxibustion were searched. The quality of evidence was evaluated with the modified Jadad scale, the evaluation index was established and the optimal regimen of acupuncture and moxibustion for OSAHS was screened by multi-index decision analysis.
RESULTS:
A total of 10 RCTs were included, and the filiform needling therapy was optimal in treatment of OSAHS. The acupoints included Lianquan (CV23), Danzhong (CV17), Zhongwan (CV12), and bilateral Kongzui (LU6), Pishu (BL20), Fenglong (ST40), Zusanli (ST36), Yinlingquan (SP9) and Zhaohai (KI6). Zusanli (ST36) received the reinforcing method, Pishu (BL20) and Fenglong (ST40) were stimulated with the reducing technique, and the rest acupoints with the uniform reinforcing-reducing. Each acupoint was manually manipulated once every 10 min during the needle retention for 30 min. Acupuncture was delivered once a day, 5 times a week and for consecutive 4 weeks. Among the included literature, the severity of disease was not reported in detail, the filiform needling was the dominant intervention, the local acupoints such as Lianquan (CV23) and Panglianquan (Extra) were mainly selected. The apnea-hypopnea index and the minimum oxygen saturation were taken as the evaluation indexes, and the effect was evaluated in reference to the generally accepted standards. The attention to safety evaluation was insufficient, the report on methodology was not adequate and the quality was low.
CONCLUSION
Filiform needling is the dominant therapy of acupuncture and moxibustion for OSAHS, and the local acupoints are considered specially. But the quality of clinical research should be improved.

In October 2024, a 36-year-old female patient with botulinum toxin type A intoxication for 15d was admitted to the Department of Neurology of Northern Jiangsu People’s Hospital. Although type A botulinum antitoxin (BAT) therapy remained effective, the patient developed a serum sickness-like reaction (SSLR) on day 4 of treatment after receiving eight consecutive desensitizing intramuscular injections of BAT. After stopping the injection of the drug and giving intravenous dexamethasone, the patient’s symptoms improved. On the 6th day after stopping the injection of botulinum antitoxin type A, the patient was followed up in the outpatient clinic and the skin symptoms had almost disappeared. This article analyzed the patient’s medical records and explored the association between BAT and SSLR, suggesting that medical personnel should be alert to the risk of adverse reactions when applying antitoxin therapy, and that they should identify and intervene in a timely manner in order to ensure the safety of the medication and therapeutic efficacy of the patients.

Ferroptosis is an iron-dependent cell death modality triggered by the accumulation of lipid peroxides and is closely linked to tumor pathogenesis. Ferritinophagy refers to the selective autophagic degradation of ferritin, releasing intracellular stored iron to maintain iron metabolic homeostasis. This process is also significantly associated with tumor-targeted interventions. This review systematically elucidates the mechanisms of ferroptosis and ferritinophagy, their roles in tumor progression, and the therapeutic potential of pharmacologically induced ferritinophagy in anticancer strategies.

Objective:To investigate the risk factors and their predictive efficacy for early postoperative infection in elderly patients with intertrochanteric femur fracture. Methods:A retrospective cohort study was conducted to analyze the clinical data of 286 elderly patients with intertrochanteric femur fracture admitted to Zhengzhou Orthopedic Hospital between August 2021 and August 2024, including 154 males and 132 females, aged 60-80 years [(72.5±5.8)years]. Fracture involved the left side in 148 patients and the right side in 138 patients. Internal fixation was performed on 214 patients and joint replacement on 72. Based on the occurrence of infection within two weeks postoperatively, the patients were divided into infection group ( n=25) and non-infection group ( n=261). Data were collected from the two groups, including basic information [gender, age, body mass index (BMI), cause of injury, fracture side], admission data (fasting blood glucose, diastolic blood pressure, systolic blood pressure), preoperative data [American Society of Anesthesiologists (ASA) classification, AO classification, serum C-reactive protein (CRP), serum albumin (Alb), serum CRP/Alb ratio, time from injury to surgery], and treatment-related information (surgical type, duration of surgery, intraoperative blood loss, quality of intraoperative reduction, postoperative antibiotic use). Univariate analysis and multivariate Logistic stepwise regression analysis were used to identify independent risk factors for early postoperative infection in elderly patients with intertrochanteric femur fracture. The receiver operating characteristic (ROC) curve was plotted and the area under the curve (AUC) was calculated to evaluate the predictive efficacy of each factor. Results:Univariate analysis showed significant differences between the two groups in fasting blood glucose on admission, preoperative serum CRP, preoperative serum Alb, preoperative serum CRP/Alb ratio, and duration of surgery ( P<0.01). There were no significant differences between the two groups in the remaining variables ( P>0.05). Multivariate Logistic stepwise regression analysis indicated that fasting blood glucose on admission ( OR=2.65, 95% CI 1.32, 5.32, P<0.01), preoperative serum CRP ( OR=1.10, 95% CI 1.04, 1.18, P<0.01), preoperative serum Alb ( OR=0.79, 95% CI 0.70, 0.90, P<0.01), preoperative serum CRP/Alb ( OR=143.78, 95% CI 4.46, 46.77, P<0.01), and duration of surgery ( OR=1.07, 95% CI 1.02, 1.11, P<0.01) were significantly associated with early postoperative infection in elderly patients with intertrochanteric femur fracture. ROC curve analysis showed that the sensitivity and specificity of preoperative serum CRP/Alb in predicting early postoperative infection in elderly patients with intertrochanteric femur fracture were 88.00% and 88.10%, and that the AUC of preoperative serum CRP/Alb prediction was 0.92, significantly greater than the AUC predicted separately by fasting blood glucose at admission, preoperative serum CRP, preoperative serum Alb and duration of surgery (0.76, 0.75, 0.77, 0.76, respectively). The optimal cut-off value for the preoperative serum CRP/Alb ratio was 1.78. Conclusions:Fasting blood glucose on admission, preoperative serum CRP, Alb, CRP/Alb ratio, and duration of surgery are independent risk factors for early postoperative infection in elderly patients with intertrochanteric femur fracture. These factors all possess certain predictive value for early postoperative infection, but the preoperative serum CRP/Alb ratio demonstrates the best predictive efficacy.

Clinical characteristics, lung response rates to induction chemotherapy, treatment outcomes, prognostic factors and sever chemotherapy toxicities at different stages were analyzed. Survival analysis was performed by Kaplan-Meier method. Univariate prognostic analysis was conducted by Log-Rank test. Results:Of the 27 patients, there were 17 males and 10 females, with the age of 21 (15, 33) months. During the follow-up of 31 (12, 45) months for 17 continuous complete remission patients, 4 cases disease progression (2 cases death) and 6 cases relapse were observed. The 2-year event free survival (EFS) and overall survival (OS) rate was (58±11)% and (89±7)%, respectively. All the 27 patients had response to block 1-3 induction chemotherapy (cisplatin+doxorubicin), with 14 cases (52%) achieving complete response and 13 cases (48%) achieving partial response of lung metastatic lesions, the 2-year EFS rate was (81±12)% and (34±14)%, respectively ( χ 2=6.76, P=0.009), the 2-year OS rate was 100% and (79±13)%, respectively ( χ2=2.12, P=0.145). Patients with caudate lobe tumors or ≥10 pulmonary metastatic nodules had significantly lower EFS rates ( χ2=5.36, 7.84, P=0.021, 0.005, respectively). The incidence of grade 3/4 neutropenia after block 1-3 induction chemotherapy, CD (carboplatin+doxorubicin), and VI (vincristine+irinotecan) consolidation chemotherapy was 90% (73/81), 75% (58/77), and 31% (11/35), respectively. The incidence of grade 3/4 thrombocytopenia was 77% (62/81), 69% (53/77), and 14% (5/35), respectively. The incidence of grade 3/4 infections was 64% (52/81), 25% (19/77), and 20% (7/35), respectively. The differences between the groups were statistically significant ( χ2=43.51, 42.69, 33.00, all P<0.001). Two patients (10%) of the 20 evaluable patients for ototoxicity occurred grade 3 and higher hearing impairment, with 1 patient requiring a hearing aid. Conclusions:The modified SIOPEL-4 regimen shows good preliminary efficacy and safety in treating pediatric HB with lung metastases. The prognosis for patients with residual lesions in the lungs after induction chemotherapy needs to be improved. Attention should be given to the ototoxicity induced by high-dose cisplatin chemotherapy.

Chinese Journal of Ocular Fundus Diseases 2025;41(6):483-488
Uveal melanoma (UM) is a common type of adult ocular malignancy, and its metastasis potential and prognosis are closely related to tumor stage, histopathological features and genetic molecular markers. Currently, eyeball-preserving radiation therapy, including adhesive radiotherapy and proton beam radiotherapy, has become the preferred method for UM treatment. Despite this, overall survival is low, with about 50% of patients eventually developing distant metastases. Recently, remarkable progress has been made in the field of treatment for metastatic uveal melanoma, especially in the development of new technologies and new drugs. These advances reduce the risk of tumor metastasis and spread while improving the cure rate for patients. Tibenfox is a major breakthrough in the treatment of UM. Through these explorations, it is hoped that in the future, early diagnosis of the disease can guide prognosis assessment and implement personalized treatment strategies aimed at protecting patient vision, controlling tumor metastasis, and improving survival.
