1.Efficacy and safety of eltrombopag in the treatment of primary immune thrombocytopenia: real-world data from a single medical center

Objective:This study aimed at investigating the efficacy and safety of eltrombopag in the treatment of adult primary immune thrombocytopenia (ITP) and evaluated the factors influencing its efficacy and side effects.Methods:A total of 198 patients with adult ITP who were admitted to Tianjin Medical University General Hospital between January 2018 and March 2022 were retrospectively analyzed. The efficacy of each starting dose of eltrombopag was evaluated, and adverse events were analyzed. The factors influencing efficacy were investigated, including sex, age, adult ITP type, platelet antibodies, and combined drug treatments.Results:Of the 198 patients, 70 males and 128 females with a median age of 45 years (18-88 years) were included; 130 (65.7%) had newly diagnosed adult ITP, 25 (12.6%) had persistent adult ITP, and 43 (21.7%) had chronic adult ITP. The bleeding event scores at baseline were assessed; 84.3% had scores of<4 and 15.7% had scores of ≥4. The eltrombopag response rate (initial response) at 6 weeks was 78.8% (complete response [CR]: 49.0%; CR1: 14.6%; CR2: 15.2%). The median response time to eltrombopag was 7 (7, 14) days. The initial response rates to 25, 50, and 75 mg eltrombopag were 74.1%, 85.9%, and 60.0%, respectively ( P=0.031). The initial response rate to the 50 mg dose was significantly higher than that of the 25-mg and 75-mg doses. Two patients received 100 mg as the starting dose, and their initial response was 0. Regarding dose adjustment, 70.7% of the patients remained on the starting dose, 8.6% underwent dose adjustment to 50 mg, and 6.1% underwent dose adjustment to 75 mg. Another two patients underwent dose adjustment to 100 mg. After dose adjustment, the persistent response rates were 83.6%, 85.3%, and 85.7% for the 25-, 50-, and 75-mg doses, respectively, with no significant difference. After dose adjustment, the sustained efficacy rate for the 100-mg dose (4 patients) was 100.0%. After 6 weeks of treatment with eltrombopag, the overall bleeding score of patients with ITP decreased. The number of patients with a score of ≥4 decreased to 0, the number of patients with a score of<4 decreased, and there was no significant change in the number of patients with a score of 1-2. The most common adverse event associated with eltrombopag was impaired liver function (7.7%). No thrombosis events or other adverse events were observed. ITP type and number of megakaryocytes significantly affected the initial response to eltrombopag. The initial response rates to eltrombopag for newly diagnosed adult ITP, persistent adult ITP, and chronic adult ITP were 85.3%, 56.0%, and 76.2%, respectively ( P=0.003). For megakaryocytes, the initial response rates were 61.8%, 87.1%, and 84.3% ( P=0.009) for the decreased, normal, and increased megakaryocyte groups, respectively. Conclusion:Eltrombopag, as a second-line or higher treatment for adult ITP, has a rapid onset of action and good safety. The initial response rate is significantly higher with a dose of 50 mg than with a dose of 25 mg. Patients with newly diagnosed ITP and those with normal or increased megakaryocyte numbers have a higher initial response rate to eltrombopag.
2.Clinical remission and transmural healing of ustekinumab in patients with Crohn's disease

Objective:To treat the Crohn's disease(CD)patients with ustekinumab(UST),to eva-luate their clinical and endoscopic remission,and to evaluate their transmural response(TR)and trans-mural healing(TH)condition using intestinal ultrasonography(IUS).Methods:Retrospective analysis was made on patients diagnosed with CD in Peking University People's Hospital from January 2020 to Au-gust 2022,who were treated with UST for remission induction and maintenance therapy.All the patients were evaluated on both week 8 and week 16/20 after treatment,including clinical,biochemical indica-tors,colonoscopy and IUS examination.Results:A total of 13 patients were enrolled in this study,inclu-ding 11 males and 2 females.The minimum age was 23 years,the maximum age was 73 years and the mean age was 36.92 years.All the patients were in the active stage of disease before treatment,and the average Best Crohn's disease activity index(Best CDAI)score was 270.12±105.55.In week 8,the Best CDAI score of the patients decreased from 270.12±105.55 to 133.16±48.66(t=4.977,P<0.001).Eight patients achieved clinical remission while 5 patients remained in the active stage.Nine patients underwent colonoscopy evaluation.The average simple endoscopic score for Crohn's disease(SES-CD)score decreased from 10.71±7.14 before treatment to 6.00±7.81(t=2.483,P=0.048)in week 16/20.Four patients achieved endoscopic remission while 5 patients did not.In week 8,5 pa-tients achieved TR,2 patients achieved TH,the other 6 patients did not get TR or TH.In week 16/20,6 patients achieved TR,3 patients achieved TH while the other 4 patients did not get TR or TH.There was no significant statistical difference in the TR effect of UST between small intestine and colon lesions(Fisher test,P>0.999).The rate of UST transmural response in the patients who had had previous bio-logical agent therapy was lower than those with no previous biological agent therapy,but there was no sig-nificant statistical difference(Fisher test,P=0.491).Conclusion:After treatment of UST,the clinical and endoscopic conditions of the CD patients had been improved,and some patients could achieve clini-cal remission and endoscopic remission.UST had good TR and TH effects on CD.TR might appear in week 8,and the TR effect increased in week 16/20.There was no significant statistical difference in the TR effect between small intestine and colon lesions.TR effect of UST was better in the patients who had no previous biological agent therapy than those who had had other biological agents,but the result had no significant statistical difference.

The real-time PCR method was used for the detection and genotyping of HRSV. Basic demographic information was collected and used for the epidemiological analysis.Results:A total of 9 278 respiratory specimens of influenza-like cases were collected and detected, with a total positive rate of 4.77% (443/9 278) for HRSV. In 2021 (8.48%, 167/1 970), the positive rate of HRSV was significantly higher than in 2019 (3.35%, 52/1 552), 2022 (1.80%, 39/2 169), and 2023 (4.49%, 133/2 960), and the difference was statistically significant ( χ 2=102.395, P<0.001). The prevalence of HRSV was mainly in summer and early autumn (September), and there was an abnormal increase in the positive rate of HRSV in winter 2022. The highest positive rate of HRSV was in children under five years old (9.84%, 330/335). The typing results showed that in 2022, the prevalence of HRSV-A was predominant (71.79%, 28/39), and in 2023, HRSV-A and HRSV-B subtypes coexisted. Conclusions:The prevalence of HRSV in Shenzhen from 2019 to 2023 has obvious seasonality, mainly in summer and early autumn. Children under five years old are the main population of HRSV infections.

6.Proteomic Study on the Effect of Wenshen Tongdu Formula (温肾通督方) on Spleen B Cells in Spinal Cord Injury Model Mice

ObjectiveTo explore the effects and possible mechanism of Wenshen Tongdu Formula (温肾通督方, WTF) on spinal cord injury. MethodsThirty-six C57BL/6 female mice were randomly divided into sham operation group, model group and WTF group, with 12 mice in each group. The spinal cord injury model was established in the model group and the WTF group using the modified Allen's method, while in the sham operation group the spinal cord was only exposed. Since the 1st day after surgery, 50 g/(kg·d) of WTF solution was given to the WTF group by gavage, while 20 ml/(kg·d) of normal saline was given to the sham operation and model group by gavage, all for 14 days. Before surgery and on the 1st, 7th, and 14th days after surgery, the motor function of the mice was evaluated using the inclined plane test and hind limb motor function score (by BMS). On the 3rd day after surgery, the nerve electrophy-siology was detected through electromyography and motor evoked potential; the spleen length was measured, and B cells in the spleen were sorted by magnetic beads; the differential expression of proteins were detected through proteomics technology; and the protein expression of mitochondrial outer membrane transport porin 20 (Tom20) and downstream cleaved caspase-3 in spleen B cells were measured using Western blotting. On the 14th day after surgery, MRI was used to observe the recovery of the spinal cord. ResultsCompared to those in the sham operation group at the same time, the BMS scores and subscores and the inclined plane test angle in the model group were reduced on the 1st, 7th and 14th days after surgery; the peak value of electromyogram and motor evoked potential were reduced, and the spleen length was shortened, while the expression of Tom20 and cleaved caspase-3 increased in splenic B cells increased (P<0.05). Compared to those in the model group at the same time, the BMS subscores on the 14th day and the angle of the inclined plane test on the 7th and 14th days after surgery increased in the WTF group; the peak value of electromyography and motor evoked potential, as well as the length of spleen increased, and the expression of Tom20 and cleaved caspase-3 decreased (P<0.05). The proteomics results showed that there were 100 differential proteins in the WTF group versus the model group, of which 37 were up-regulated and 63 were down-regulated. GO enrichment analysis showed that differential proteins mainly played their roles in oxygen binding, exogenous apoptosis negative feedback, zinc ion response, and oxygen transport. KEGG enrichment analysis showed that differential proteins were mainly concentrated in metabolic pathways, Huntington's disease, oxidative phosphorylation and other pathways. Subcellular localization showed that differential proteins were associated with mitochondria. Magnetic resonance imaging on the 14th day after surgery showed that the spinal cord structure of the mice in the sham operation group was intact, and the segments were clear, with normal spinal cord signal; the low signal area in the spinal cord injury area increased in the model group, and the spinal cord became significantly thinner; the injured segment had obvious depression in the WTF group, but the structure was more complete than that in the model group. ConclusionWTF may promote spinal cord injury repair by regulating immune function, and its mechanism may be related to inhibiting pyroptosis of spleen B cells.
7.Lipid nanoparticle delivery of siRNA targeting Cyp2e1 gene attenuates subacute alcoholic liver injury in mice.

OBJECTIVES:
To investigate the effect and mechanism of lipid nanoparticle (LNP) delivery of small interfering RNA (siRNA) targeting Cyp2e1 gene on subacute alcoholic liver injury in mice.
METHODS:
siRNA targeting Cyp2e1 gene was encapsulated in LNP (si-Cyp2e1 LNP) by microfluidic technique and the resulting LNPs were characterized. The optimal dose of si-Cyp2e1 LNP administration was screened. Forty female C57BL/6N mice were randomly divided into blank control group, model control group, si-Cyp2e1 LNP group, LNP control group and metadoxine group. The subacute alcoholic liver injury mouse model was induced by ethanol feeding for 10 d plus ethanol gavage for the last 3 d. Serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities, and the superoxide dismutase (SOD) activity as well as malondialdehyde, reactive oxygen species, glutathione, triacylglycerol, total cholesterol contents in liver tissue were measured in each group, and liver index was calculated. The expression of genes related to oxidative stress, lipid synthesis and inflammation in each group of mice were measured by realtime RT-PCR.
RESULTS:
Compared with the model control group, the levels of liver index, serum ALT, AST activities, malondialdehyde, reactive oxygen species, triacylglycerol, total cholesterol contents in liver tissue decreased, but the SOD activity as well as glutathione increased in the si-Cyp2e1 LNP group (all P<0.01). Hematoxylin-eosin staining result showed disorganized hepatocytes with sparse cytoplasm and a large number of fat vacuoles and necrosis in the model control group, while the si-Cyp2e1 LNP group had uniformly sized and arranged hepatocytes with normal liver tissue morphology and structure. Oil red O staining result showed si-Cyp2e1 LNP group had lower fat content of the liver compared to the model control group (P<0.01), and no fat droplets accumulated. Anti-F4/80 monoclonal antibody fluorescence immunohistochemistry showed that the si-Cyp2e1 LNP group had lower cumulative optical density values compared to the model control group (P<0.01) and no significant inflammatory reaction. Compared with the model control group, the expression of catalytic genes P47phox, P67phox and Gp91phox were reduced (all P<0.01), while the expression of the antioxidant enzyme genes Sod1, Gsh-rd and Gsh-px were increased (all P<0.01). The mRNA expression of the lipid metabolism genes Pgc-1α and Cpt1 were increased (all P<0.01) and the lipid synthesis-related genes Srebp1c, Acc and Fasn were decreased (all P<0.01); the expression of liver inflammation-related genes Tgf-β, Tnf-α and Il-6 were decreased (all P<0.01).
CONCLUSIONS
The si-Cyp2e1 LNP may attenuate subacute alcoholic liver injury in mice mainly by reducing reactive oxygen levels, increasing antioxidant activity, blocking oxidative stress pathways and reducing ethanol-induced steatosis and inflammation.

8.Amelioration of ethanol-induced oxidative stress and alcoholic liver disease by in vivo RNAi targeting Cyp2e1.

Alcoholic liver disease (ALD) results from continuous and heavy alcohol consumption. The current treatment strategy for ALD is based on alcohol withdrawal coupled with antioxidant drug intervention, which is a long process with poor efficacy and low patient compliance. Alcohol-induced CYP2E1 upregulation has been demonstrated as a key regulator of ALD, but CYP2E1 knockdown in humans was impractical, and pharmacological inhibition of CYP2E1 by a clinically relevant approach for treating ALD was not shown. In this study, we developed a RNAi therapeutics delivered by lipid nanoparticle, and treated mice fed on Lieber-DeCarli ethanol liquid diet weekly for up to 12 weeks. This RNAi-based inhibition of Cyp2e1 expression reduced reactive oxygen species and oxidative stress in mouse livers, and contributed to improved ALD symptoms in mice. The liver fat accumulation, hepatocyte inflammation, and fibrosis were reduced in ALD models. Therefore, this study suggested the feasibility of RNAi targeting to CYP2E1 as a potential therapeutic tool to the development of ALD.
9.Effect of Jingui Shenqiwan on Diabetic Osteoporosis in Mice via AGEs/RANKL/NF-κB Pathway Based on Theory of "Kidneys Governing Bones"

ObjectiveTo investigate the effect of Jingui Shenqiwan on diabetic osteoporosis (DOP) in mice by regulating the advanced glycation end products (AGEs)/receptor activator of nuclear factor-κB ligand (RANKL)/nuclear factor-κB (NF-κB) signaling pathway based on the theory of "kidneys governing bones". MethodForty 6-week-old male and female skeletal-muscle-specific, dominant negative insulin-like growth factor-1 receptor (MKR) mice were selected and fed on a high-fat diet for eight weeks to establish the DOP model. The model mice were randomly divided into a model group, low- and high-dose Jingui Shenqiwan group (1.3, 2.6 g·kg-1), and an alendronate sodium group (0.01 g·kg-1), with 10 mice in each group. Additionally, 10 FVB/N mice of the same age were assigned to the normal group. The corresponding drugs were administered orally to each group once a day for four weeks. After the administration period, fasting blood glucose (FBG) measurement and oral glucose tolerance test (OGTT) were conducted. Kidney function and kidney index were measured. Renal tissue pathological changes were observed through hematoxylin-eosin (HE) and Masson staining. Immunohistochemistry was performed to assess the protein expression levels of AGEs, phosphorylated NF-κB (p-NF-κB), and RANKL in renal tissues. Western blot analysis was conducted to measure the expression of proteins related to the AGEs/RANKL/NF-κB signaling pathway, osteoprotegerin (OPG), and Runt-related transcription factor 2 (RUNX2) proteins in femoral bone tissues. ResultCompared with the normal group, mice in the model group exhibited significantly increased FBG (P<0.01), trabecular bone degeneration, abnormal bone morphological parameters, significantly increased area under the curve (AUC) of OGTT (P<0.01), enlarged kidney volume, significantly increased kidney function indicators and kidney index (P<0.01), disrupted renal glomeruli and renal tubule structures, significantly increased expression of AGEs, RANKL, and p-NF-κB/NF-κB in renal tissues (P<0.05), and significantly decreased expression of OPG and RUNX2 in femoral bone tissues (P<0.01). Compared with the model group, mice in the Jingui Shenqiwan groups showed a significant decrease in OGTT AUC (P<0.01). Histopathological analysis revealed alleviated structural lesions in renal glomeruli and renal tubules. Furthermore, the expression of AGEs, RANKL, and p-NF-κB/NF-κB in renal tissues was significantly reduced (P<0.05, P<0.01), and the expression of RUNX2 and OPG in femoral bone tissues was significantly increased (P<0.05, P<0.01). ConclusionJingui Shenqiwan can improve kidney function and downregulate the AGEs/RANKL/NF-κB signaling pathway to inhibit inflammatory reactions, thereby alleviating the symptoms of DOP in mice, demonstrating a therapeutic effect on DOP from the perspective of the kidney.
10.Research and Application Analysis of Risk Prediction Models for Prostate Cancer from a Data-driven Perspective

Purpose/Significance The paper retrospectively analyzes the research progress on risk prediction models for prostate cancer,and provides references for the construction of prostate cancer intelligent diagnosis and treatment system.Method/Process Through literature mining and analysis,the significance of molecular,imaging,individual,population and other omics level evaluation indexes in the diagnosis of prostate cancer is discussed,and the key characteristic variables and clinical application value of different cal-culation models are compared.Result/Conclusion The existing models have advantages of easy calculation and strong feasibility,but they also have limitations such as limited prediction accuracy and insufficient generalization ability.The integration of multi-omics data and artificial intelligence models is of great significance for medical informatics research and smart medical system construction of prostate cancer.
