1.Chlorhexidine and Fondaparinux-Induced Kounis Syndrome: a Case Report

Kounis syndrome is an acute coronary syndrome triggered by an allergic reaction, which is clinically rare and frequently subject to misdiagnosis or missed diagnosis. This article presents a case report of a 70-year-old male patient who developed a rash, pruritus, and chest pain following colon polyp resection. Coronary angiography revealed occlusion of the left anterior descending artery, and blood flow was restored after stent implantation. However, the patient experienced recurrent symptoms accompanied by loss of consciousness. Drug skin tests confirmed positive reactions to chlorhexidine and fondaparinux sodium, leading to a diagnosis of type Ⅱ Kounis syndrome. By avoiding allergenic drugs and combining antihistamines with symptomatic treatment to correct myocardial ischemia, the patient′s clinical symptoms significantly improved, and he eventually recovered and was discharged from the hospital. This case underscores the importance of maintaining vigilance for this syndrome in patients with allergies accompanied by chest pain and promptly identifying and avoiding allergens.
2.Soft and hard tissue changes of hyperdivergent class Ⅱ patients before and after orthodontic extraction treatment

Objective:To investigate the hard and soft tissue changing trend and contributing factors of skeletal class Ⅱ hyperdivergent patients before and after orthodontic camouflage treatment by analyzing the cephalogram and the three dimensional(3D)facial scan data.Methods:Eighteen skeletal class Ⅱhyperdivergent adult female patients who finished camouflage orthodontic treatment were selected.Skele-tal and dental measurements were carried out with the cephalometric analysis before and after the treat-ment.3D facial data before and after orthodontic treatment were acquired and the anatomical landmarks were set after the repositioning and superimposition process.Hard tissue measurement included 17 mea-surement indicators(sella-nasion-subspinale angle,sella-nasion-supramental angle,subspinale-nasion-supramental angle,facial angle,angle of convexity,Frankfort horizontal plane-mandibular plane angle(FH-MP),Y axis angle,sella-nasion plane-mandibular plane angle(MP-SN),pogonion-nasion-supra-mental distance,upper incisor-nasion-subspinale distance,upper incisor to sella-nasion,lower incisor-nasion-supramental distance,lower incisor-nasion-supramental angle,upper incisor to lower incisor,up-per incisor to sella-nasion,lower incisor-mandibular plane angle,and Z angle),and the changes before and after treatment were measured for 11 of them.Twenty soft tissue landmarks(left/right cheekbone,left/right chelion,left/right crista philtra,soft tissue gnathion,left/right gonion,glabella,labrale infe-rius,labrale superius,soft tissue menton,left/right mid-mandibular border,soft tissue pogonion,stomi-on superius,sublabial,subnasale,and supralabial)and 9 soft tissue indicators(lower lip height,facial convexity,lower vermilion height,mandibular contour,nasolabial angle,philtral length,philtral width,upper lip height,and upper vermilion height)were measured and recorded for treatment changes.Linear-regression analysis and correlation analysis were carried out for analyzing the relationship between hard and soft tissue changes before and after the treatment.Results:Significant differences were noticed for 18 out of the 20 cephalometric measurements and facial measurements before and after the treatment(P<0.05),which mainly represented the sagittal retraction of lip area after the treatment.Significant vertical displacements were revealed for soft tissue menton after treatment[(1.88±2.61)mm,P<0.05].Significant sagittal displacements were revealed for left/right cheilion[(-2.95±1.9)mm,(-2.90±1.92)mm],labrale inferius[(-4.94±1.95)mm],labrale superius[(-3.25±1.44)mm],sublabial[(-3.10±3.5)mm],and subnasale[(-1.23±1.06)mm]after treatment(P<0.05).An average of 4.10°±2.57° increasement was noticed for Z angle after treatment.High correla-tion(r>0.7)was noticed for the displacement of menton after treatment with FH-MP,with the rate of-0.183:1,and MP-SN,with the rate of-0.157:1.Moderate correlations(0.7≥r>0.4)were no-ticed for the other measurements with correlations(P<0.05).Conclusion:A certain extent of facial improvements could be achieved with orthodontic camouflage treatment for skeletal class Ⅱ hyperdiver-gent patients,which were mostly represented by the improvement of sagittal relationship of nose,lips,and chin.Certain correlations were noticed for the hard and soft tissue changes.
3.Low-frequency electrical stimulation relieves pain and promotes gastrointestinal function recovery after gynecological laparotomy

Objective To observe the effect of low-frequency electrical stimulation on the rehabilitation of pa-tients after gynecological abdominal surgery.Methods Sixty-three patients who underwent open surgery in gy-necology department of Taixing Clinical College of Bengbu Medical College from June 2021 to July 2022 were selected.The patients were randomly divided into control group(31 cases)and a low-frequency electrical stimulation group(32 cases).The low-frequency electrical stimulation group was subjected to stimulation with-in the patient′s tolerable range once a day for 30 minutes each time,and the intensity of each stimulation was adjusted based on clinical situation.The control group selected the same acupoints and pasted electrodes,connected to the treatment device but no electrical stimulation.The electrode strip was removed after 30 minutes,then re-cord the postoperative Visual Analog Scale(VAS)score as well as the time from the end of the surgery to the first discharge and defecation.Results The VAS score at 48 hours after surgery showed a low degree of pain in the low-frequency electrical stimulation group(3.6±1.2)compared to that in control group(4.5±1.4);After 72 hours of surgery,the VAS score was lower in the low-frequency electrical stimulation group(1.7±0.9)compared to the control group(3.3±1.4),indicating a lower degree of pain.The first exhaust time(26.9±6.7)h vs.(35.5±13.0)h was shorter in the low-frequency electrical stimulation group;The first bowel movement time(49.0±5.4)h vs.(64.4±13.8)h was shorter in the low-frequency electrical stimula-tion group compared to the control group.Conclusions Low frequency electro-physiological stimulation can alleviate post-operative pain and shorten exhaust and defecation time in patients undergoing gynecological open surgery.
4.The Influence of Shoutai Wan (寿胎丸) on Lactic Acid Content at the Maternal-Fetal Interface and Expression of Immune-Related Factors in Recurrent Miscarriage Model Mice

ObjectiveTo explore the possible mechanisms of Shoutai Wan (寿胎丸) in treating recurrent miscarriage (RSA) from the perspective of immune tolerance under the acidic microenvironment at the maternal-fetal interface. MethodsFemale CBA/J mice were randomly divided into normal group, model group, progesterone group, and Shoutai Wan group, with 15 mice in each group. The mice in the normal group and model group were given 0.2 ml distilled water by gavage each day, the Shoutai Wan group given Shoutai Wan decoction 0.15 g/(10 g·d) by gavage, the progesterone group given progesterone tablets 0.44 mg/(10 g·d) by gavage. After gavage for 14 days, the mice were cohabited. Female CBA/J mice in the normal group were mated with male BALB/c mice at a ratio of 2∶1, and female CBA/J mice in the other groups were mated with male DBA/2 mice at a ratio of 2∶1 to establish the RSA mouse model. Vaginal smears were taken from the female mice the next morning, and the appearance of a large number of spermatozoa and the presence of a vaginal plug were considered as the first day of pregnancy. After the appearance of the plug, the mice were continued to be administered according to the previous method until the 10th day of pregnancy. On the 10th day of pregnancy, maternal-fetal interface tissues were collected from each group of mice, and lactate dehydrogenase colorimetric method was used to detect lactate (LA) content; qPCR method and Western blot method were used to detect the expression of immune-related factors interleukin-4 (IL-4), interferon-gamma (IFN-γ), transforming growth factor beta 1 (TGF-β1), and forkhead box protein 3 (Foxp3) mRNA and protein; flow cytometry was used to detect the numbers of helper T lymphocyte 1 (Th1), helper T lymphocyte 2 (Th2), regulatory T cell (Treg), classical macrophage (M1), and alternative macrophage (M2). The bivariate Pearson test was used to analyze the correlation between LA content and the numbers of Th1, Th2, Treg, M1, and M2 cells, as well as the correlation between LA content and the expression of IL-4, IFN-γ, TGF-β1, Foxp3 protein, and mRNA. ResultsOn the 10th day of pregnancy, compared with the normal group, the LA content decreased in the model group, and the expression of IL-4, TGF-β1, Foxp3 protein and mRNA in the maternal-fetal interface tissues decreased, while the expression of IFN-γ protein and mRNA increased. The numbers of Th1 and M1 cells increased, while the numbers of Th2, Treg, and M2 cells decreased (P<0.05 or P<0.01). Compared with the model group, the LA content increased in the Shoutai Wan group and progesterone group. The expression of IL-4, TGF-β1, Foxp3 protein and mRNA in the maternal-fetal interface tissues increased, while the expression of IFN-γ protein and mRNA decreased. The numbers of Th1 and M1 cells decreased, while the numbers of Th2, Treg, and M2 cells increased (P<0.05 or P<0.01). The LA content was positively correlated with the numbers of Th2, Treg, and M2 cells, and the expression of IL-4, TGF-β1, Foxp3 protein, and mRNA (P<0.05 or P<0.01); the LA content was negatively correlated with the numbers of Th1, M1 cells, and the expression of IFN-γ protein and mRNA (P<0.05 or P<0.01). ConclusionShoutai Wan may improve immune tolerance by regulating the expression of immune-related factors in the acidic microenvironment at the maternal-fetal interface of RSA model mice, thereby exerting its role in preventing miscarriage.

Objective:To study the current status of longitudinal extrauterine growth restriction (EUGR) in extremely preterm infants (EPIs) and to develop a prediction model based on clinical data from multiple NICUs.Methods:From January 2017 to December 2018, EPIs admitted to 32 NICUs in North China were retrospectively studied. Their general conditions, nutritional support, complications during hospitalization and weight changes were reviewed. Weight loss between birth and discharge > 1SD was defined as longitudinal EUGR. The EPIs were assigned into longitudinal EUGR group and non-EUGR group and their nutritional support and weight changes were compared. The EPIs were randomly assigned into the training dataset and the validation dataset with a ratio of 7∶3. Univariate Cox regression analysis and multiple regression analysis were used in the training dataset to select the independent predictive factors. The best-fitting Nomogram model predicting longitudinal EUGR was established based on Akaike Information Criterion. The model was evaluated for discrimination efficacy, calibration and clinical decision curve analysis.Results:A total of 436 EPIs were included in this study, with a mean gestational age of (26.9±0.9) weeks and a birth weight of (989±171) g. The incidence of longitudinal EUGR was 82.3%(359/436). Seven variables (birth weight Z-score, weight loss, weight growth velocity, the proportion of breast milk ≥75% within 3 d before discharge, invasive mechanical ventilation ≥7 d, maternal antenatal corticosteroids use and bronchopulmonary dysplasia) were selected to establish the prediction model. The area under the receiver operating characteristic curve of the training dataset and the validation dataset were 0.870 (95% CI 0.820-0.920) and 0.879 (95% CI 0.815-0.942), suggesting good discrimination efficacy. The calibration curve indicated a good fit of the model ( P>0.05). The decision curve analysis showed positive net benefits at all thresholds. Conclusions:Currently, EPIs have a high incidence of longitudinal EUGR. The prediction model is helpful for early identification and intervention for EPIs with higher risks of longitudinal EUGR. It is necessary to expand the sample size and conduct prospective studies to optimize and validate the prediction model in the future.
6.Protective effect of extracorporeal cardiac shock wave combined with ultrasound microbubble post-conditioning on myocardial ischemia-reperfusion injury in rats

Objective:To investigate the myocardial protective effect of extracorporeal cardiac shock wave therapy (CSWT) combined with sulfur hexafluoride microbubble post-conditioning on myocardial ischemia-reperfusion injury (MI/RI) in rats, and to provide theoretical support for clinical treatment of MI/RI.Methods:A total of 32 male SD rats were randomly divided into 4 groups: sham operation group (Sham group), MI/RI group (IR group), CSWT group (IR+ SW group), and CSWT combined with sulfur hexafluoride microbubble treatment group (IR+ SW+ MB group), with 8 rats in each group. Therapeutic intervention was performed in IR+ SW group and IR+ SW+ MB group on the 1st, 3rd and 5th day after modeling. The left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) of the rats were measured by echocardiography before and after treatment. On the 7th day, myocardial fibrosis was assessed by Masson staining, and cardiomyocyte apoptosis was observed by TUNEL staining. The myocardial apoptotic proteins Bcl-2, BAX, Cleaved-Caspase-3 and Cleaved-Caspase-9 in the infarct boundary area were detected by Western blot. The differences of the above indexes among different groups were compared.Results:①There was no significant change in heart rhythm and heart rate among the groups before and after treatment, and there was no significant difference in heart rate ( P>0.05). ②The echocardiographic results after treatment showed that, compared with IR group, LVEDD and LVESD in IR+ SW group and IR+ SW+ MB group decreased in turn, while LVEF and LVFS increased in turn with significant differences between each two groups (all P<0.05). ③Compared with IR group, the degrees of myocardial fibrosis and apoptosis in IR+ SW group and IR+ SW+ MB group were alleviated in turn, and the relief in IR+ SW+ MB group was the most obvious. Quantitative analysis showed that compared with IR group, the proportions of cardiomyocyte apoptosis in IR+ SW group and IR+ SW+ MB group decreased in turn, and there were significant differences between each two groups (all P<0.05). ④The results of Western blot detection showed that compared with IR group, the levels of Bcl-2 in IR+ SW group and IR+ SW+ MB group increased in turn, while the levels of BAX and the activation level of Caspase-3 and Caspase-9 protein decreased in turn. These differences were all statistically significant between each two groups (all P<0.05) except for the activation level of Caspase-3 protein between IR+ SW group and IR+ SW+ MB group ( P>0.05). Conclusions:CSWT combined with sulfur hexafluoride microbubble therapy can improve left ventricular remodeling and left ventricular systolic function by inhibiting cardiomyocyte apoptosis.
7.Reliability and validity of assessment tools of Brief ICF Core Sets for Arthroplasty of Knee Osteoarthritis in Peri-operative Period

Objective To investigate the reliability and validity of the assessment tools of Brief ICF Core Sets for Arthroplasty of Knee Osteoarthritis in Perioperative Period(ICSAKOPP). Methods From May,2022 to April,2023,320 patients undergoing knee arthroplasty were selected in Peking University Third Hospital,China-Japan Friendship Hospital,Peking University First Hospital and Chinese PLA General Hospital.Trained assessors used Brief ICSAKOPP to evaluate all enrolled patients before arthroplasty,three days(±one day)after arthroplasty,three weeks(±one week)after arthroplasty,and three months(±one month)after ar-throplasty.Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores were recorded at the same time.Five professionals were asked to score all the items of Brief ICSAKOPP,and the content validity index(CVI)was caculated. Results A total of 64 cases were dropped down.CVI of all the items of the Brief ICSAKOPP were above 0.8,with a av-erage CVI of the scale of 0.938.The Cronbach's α coefficient of the Brief ICSAKOPP was 0.813.There was a moderate correlation(r=0.681,P<0.001)between the overall Brief ICSAKOPP and WOMAC scores,as well as body functional dimension score(r=0.668,P<0.001)and activities and participation dimension score(r=0.657,P<0.001). Conclusion Brief ICSAKOPP is good in content validity,internal consistency reliability and criterion validity.
8.Effect of periplaneta Americana extract CⅡ-3 on cell adhesion molecule level after transcatheter arterial embolization for hepatocellular carcinoma and its protective effect on liver injury

Objective To explore the effect of periplaneta Americana extract CⅡ-3 on cell adhesion molecules level in para-cancerous liver tissue after transcatheter arterial embolization(TAE)for hepatocellular carcinoma(HCC)and its protective effect on liver injury.Methods Two tumor-loaded rabbits of Southeast University and 50 New Zealand white rabbits were used for this study.Laparotomy tumor granule injection method was adopted to establish rabbit VX 2 liver cancer models.Imaging examination was used to confirm the success of modeling and to assess the tumor implantation status.Successfully modeled 45 experimental rabbits were randomly divided into the following 3 groups with 15 rabbits in each group:control group(receiving no any treatment),TAE group(receiving embolization of the tumor-feeding artery with iodized oil),and C Ⅱ-3 group(CⅡ-3 of 16 mg/kg dosage being administered through the ear marginal vein for 3 consecutive days before TAE).Chemical colorimetry was used to detect the preoperative and postoperative serum ALT and AST of each group.Immunohistochemistry was used to detect the expression levels of PPAR-α and NF-κB in para-cancerous liver tissue.The levels of intercellular adhesion molecule-1(ICAM-1)and vascular cell adhesion molecule-1(VCAM-1)were measured by ELISA.The mRNA expression levels of PPAR-α,NF-κB,ICAM-1 and VCAM-1 in para-cancerous liver tissue were detected by RT-PCR.Results In the control group,TAE group and C Ⅱ-3 group,the preoperative ALT levels were(34.49±1.33)U/L,(34.01±1.21)U/Land(34.06±1.23)U/L respectively,and the preoperative AST levels were(38.44±1.41)U/L,(38.45±1.47)U/L and(38.42±1.38)U/L respectively;the postoperative ALT levels were(35.61±1.56)U/L,(118.67±3.39)U/L and(87.24±3.67)U/L respectively,and the postoperative AST levels were(41.03±1.83)U/L,(220.60±3.00)U/L and(104.46±2.66)U/L respectively.Compared with the control group,in the TAE group and C Ⅱ-3 group the AST levels were apparently elevated,but the levels of ALT and AST in the C Ⅱ-3 group were obviously lower than those in the TAE group,and the differences were statistically significant(all P<0.05).In control group,TAE group and CⅡ-3 group,the ICAM-1 levels were(1.74±0.13)ng/L,(3.43±0.19)ng/L and(1.81±0.13)ng/L respectively;the VCAM-1 levels were(1.66±0.05)ng/L,(2.18±0.06)ng/L and(1.67±0.06)ng/L respectively;the number of experimental rabbits with positive PPAR-α were 4,3,and 10 respectively;the number of experimental rabbits with positive NF-κB were 2,11 and 3 respectively.In TAE group the positive rate of NF-κB,and the levels of NF-κB mRNA,ICAM-1,VCAM-1 protein and mRNA levels were significantly increased;the levels of PPAR-α protein and mRNA in the C Ⅱ-3 group were prominently increased when compared with the control group and TAE group,and the differences were statistically significant(all P<0.01).Conclusion Through antagonizing the expression of NF-κB and reducing ICAM-1 and VCAM-1 levels,C Ⅱ-3 alleviate the inflammatory response,thus reducing the liver injury after TAE.
9.Microwire penetration combined with ozone intervention for fallopian tube obstructive infertility

Objective To observe the efficacy of microwire penetration combined with ozone intervention for fallopian tube obstructive infertility.Methods Data of 149 patients with fallopian tube obstructive infertility were retrospectively analyzed.Patients in group A(n=44)underwent conventional fallopian tube recanalization(FTR),in group B(n=51)underwent microwire penetration FTR,while those in group C(n=54)received microwire penetration FTR combined with ozone.The recalculation rate 1 month after treatment and natural pregnancy rate within 1 year after treatment were compared among groups.Results The recalculation rate 1 month after treatment of group A,B,C was 63.38%(45/71),80.22%(73/91)and 92.78%(90/97),respectively,increased in order among groups(all P<0.05).The natural pregnancy rate within 1 year after treatment of group A,B,C was 20.45%(9/44),27.45%(14/51)and 48.15%(26/54),respectively,of group C was higher than of group A and B(both P<0.05),not significantly different between group A and group B(P=0.427).Conclusion Microwire penetration combined with ozone intervention had better efficacy than conventional FTR and microwire penetration FTR alone for treating fallopian tube obstructive infertility.
10.Characteristics of whole genome analysis of severe acute respiratory syndrome coronavirus 2 during different epidemic periods in Wuxi City

Objective:To understand the genome sequence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and spike protein variations during different epidemic periods in Wuxi City.Methods:Nucleic acid was extracted from the nasopharyngeal swab samples of six local cases of coronavirus disease 2019 (COVID-19) (from January to February, 2020) and 13 imported cases of COVID-19 (from March to September, 2021) in Wuxi City, and the whole genome was amplified to construct the sequencing library. The second-generation sequencer was used for sequencing. The CLC Genomics Workbench (21 version) software was used to analyze the offline data with NC_045512.2 as the reference strain, and MEGA 7.0 software was used to construct the phylogenetic tree.Identification of type was conducted by Nextstrain typing method and phylogenetic assignment of named global outbreak lineages (Pangolin) typing method.Results:There were five subtypes in Nextstrain and seven subtypes in Pangolin of the nineteen patients with COVID-19. Compared with NC_045512.2, the median nucleotide mutation sites were 29 (range 0 to 42) and amino acid mutation sites were 20 (range 0 to 34). The six local and 13 imported cases had no common nucleotide mutation sites and were in different evolutionary branches. The sequences of the six local cases were highly homologous with the reference strain sequences (NC_045512.2) at the early stage of the pandemic, and the evolutionary distance was close to that of the reference strain. The 13 imported cases were obviously divided into three evolutionary branches (Alpha, Beta, Delta variant).The four Beta variants shared eight amino acid mutation sites in spike protein, and the two Alpha variants shared eight amino acid mutation sites in spike protein, and the seven Delta variants shared five amino acid mutation sites in spike protein.Conclusions:New mutations of SARS-CoV-2 are constantly emerging during the epidemic. The increase of the nucleotide sites number may result in the change of spike protein amino acid.
