1.Research progress on the interaction between epithelial cells and macrophages in lung diseases

The interaction between epithelial cells and macrophages plays a crucial role in the progression of lung diseases,including immune de-fense,inflammation regulation,tissue repair,and regeneration.However,an imbalance in this inter-action can contribute to the development of vari-ous lung diseases.In idiopathic pulmonary fibrosis,chronic obstructive pulmonary disease,and lung cancer,macrophages can promote disease progres-sion by secreting cytokines that act on epithelial cells.Simultaneously,activated epithelial cells re-lease cytokines to recruit and activate more macro-phages in a positive feedback loop.Thus,compre-hensive studies of the interactions between epithe-lial cells and macrophages in various lung diseases have the potential to identify new therapeutic tar-gets and develop innovative strategies for disease management.This review aims to summarize the current knowledge regarding the interaction be-tween epithelial cells and macrophages in idiopath-ic pulmonary fibrosis,chronic obstructive pulmo-nary disease,and lung cancer,with the goal of in-spiring new research directions for the treatment of lung diseases.

This paper summarizes the origin, concept and characteristics of digital therapeutics and the application form, effect, challenge and opportunity of symptom management in cancer patients, in order to provide reference for the scientific and efficient application of Digital therapeutics to cancer patients.
3.The postoperative nursing care of a child with Uhl's anomaly undergoing complex heart surgery

To summarize the postoperative nursing experience for a child with Uhl's anomaly undergoing complex heart surgery.Nursing key points include implementing goal-oriented liquid negative balance management to promote the recovery of cardiac function;carrying out pressure load management and closely monitoring the heart rate and rhythm to maintain the stability of the circulatory system;closely monitoring the central venous pressure in the upper chamber and paying attention to the changes of the pulmonary artery pressure in real time;adopting a refined integrated management to avoid an abnormal rise in the pulmonary artery pressure;withdrawing invasive mechanical ventilation as early as possible and carrying out the non-invasive ventilator deconditioning training in an orderly manner;the synergistic management of bleeding and anticoagulation;the adoption of various measures to actively prevent complications;the prevention and control of infections in an all-round and multi-angle manner;the formulation of individualized cardiac rehabilitation training and nutritional intervention programs.The child was transferred from the pediatric intensive care unit back to the pediatric ward on postoperative day 9,transferred back to the pediatric intensive care unit on postoperative day 11 for 13 d for right atrial thrombus,stabilized and transferred back to the pediatric ward,and discharged after 10 d for recovery.The prognosis showed good recovery after 1 month and 3 months.
4.Study on nursing safety in treating sub-health conditions with laying moxibustion based on Delphi method

Objective To investigate the clinical effectiveness of a nursing safety program for laying moxibustion treatment in sub-health population based on the Delphi method.Methods 160 sub-health patients who sought treatment between April 2022 and August 2024 in Hangzhou Red Cross Hospital were selected and randomly divided into observation group(n=80)and control group(n=80).Both groups received laying moxibustion rehabilitation therapy.The control group received routine nursing care,while the observation group received nursing intervention based on the laying moxibustion nursing safety program developed through the Delphi method.Results Through Delphi method questionnaire survey of 15 experts,three main factors affecting laying moxibustion safety were identified and 11 specific nursing points were established.The incidence of adverse events in observation group was significantly lower than that in control group(P<0.05),and nursing satisfaction was significantly higher than that in control group(P<0.05).Conclusion The laying moxibustion nursing safety program constructed based on the Delphi method can effectively enhance the clinical safety of laying moxibustion treatment.
5.Consistency of T2 Mapping and 3D-WATSc Sequences in Quantitative Measurement of Knee Cartilage Volume

Purpose To explore the consistency between T2 mapping and three-dimensional water-selective cartilage(3D-WATSc)sequence measurement of knee cartilage volume.Materials and Methods A total of 30 healthy volunteers were recruited and underwent 3D-WATSc and T2 mapping sequence scans of the left knee joint in the Affiliated Lihuili Hospital of Ningbo University from May to December 2022.A total of 20 subjects'T2 mapping sequences were randomly selected and eight groups of raw images with different TE(13,26,39,52,65,78,91,104 ms)were analyzed.The quality of the raw images was rated by two observers,and the consistency of two observers'scores were subjected by Kappa test.The TE value with the best image quality was determined via Welch analysis of variance and Games-Howell text post hoc test.Based on T2 mapping and 3D-WATSc sequences,semi-automatic segmentation measurements were performed on the cartilage volume of femoral condyle,tibial plateau and patellar posterior margin in 30 subjects.Intra group correlation coefficient(ICC)and Bland-Altman plot were used to test the consistency of cartilage volume between two observers under T2 mapping sequence,as well as between T2 mapping and 3D-WATSc sequences.Results The consistency of the scores of the two observers was moderate except for poor consistency at TE 104 ms(Kappa=0.318)and no statistical difference at TE 78 ms(P=0.052),with Kappa values greater than 0.40 and P<0.05,further analysis showed that the cartilage display quality score was optimal at TE 26 ms(M=2.90).Comparing TE 26 ms with the scores of the other seven groups in Games Howell text,except for TE 26 ms and TE 13 ms(M=2.55),TE 26 ms and TE 39 ms(M=2.62),which had no statistically significant difference in scores(P>0.05),there were significant differences in scores between TE 26 ms and the other TE groups(all P<0.05).On the T2 mapping(TR 2 000 ms,TE 26 ms)sequence,the ICC values of the cartilage volumes of each part segmented by two observers were all greater than 0.6,P<0.05,with the consistency of femoral condyle cartilage volume being the best(ICC=0.877,P<0.01).The consistency of the cartilage volumes of each part measured by 3D-WATSc and T2 mapping sequences was good(ICC>0.75,P<0.01),with the consistency of tibial plateau cartilage volume being the best(ICC=0.857,P<0.01).Conclusion The T2 mapping original grayscale image(TR 2 000 ms,TE 26 ms)can clearly display the morphology of knee joint cartilage,the consistency between T2 mapping and 3D-WATSc sequences in quantifying knee cartilage volume is good,and the T2 mapping single sequence provides a reliable scanning scheme for one-stop quantitative analysis of knee joint cartilage morphology and function.
6.Expert consensus on perinatal care management of infants with congenital heart disease

Objective To explore the expert consensus on perinatal care management of infants with congenital heart disease(hereinafter referred to as"Consensus")in order to promote the standardization of integrated nursing.Methods The literature was systematically searched and several discussions were organized within the group to compile the first draft of the Consensus.From January to March 2024,20 experts in the clinical nursing,nursing management,clinical medicine and other fields of congenital heart disease were solicited through 2 rounds of Delphi,and 8 experts were invited to conduct a validation to revise the items to form the final Consensus.Results The recovery rates of the 2 rounds of questionnaires were 100%;the experts'authority coefficient was 0.89;the Kendall's W were 0.172,0.211,with statistical significance(P<0.05).The Consensus included 8 first-level subjects,namely prenatal examination and consultation,postpartum screening,standardized referral,preoperative nursing,intraoperative nursing,postoperative nursing,other disease screening,health education and discharge follow-up.Conclusion The Consensus is scientific and rigorous,and it can provide a reference basis for clinical nursing staff to carry out the care and management of newborns with congenital heart disease.

Objective To investigate the distribution and antimicrobial resistance profiles of common pathogens isolated from cerebrospinal fluid(CSF)in CHINET program from 2015 to 2021.Methods The bacterial strains isolated from CSF were identified in accordance with clinical microbiology practice standards.Antimicrobial susceptibility test was conducted using Kirby-Bauer method and automated systems per the unified CHINET protocol.Results A total of 14 014 bacterial strains were isolated from CSF samples from 2015 to 2021,including the strains isolated from inpatients(95.3%)and from outpatient and emergency care patients(4.7%).Overall,19.6%of the isolates were from children and 80.4%were from adults.Gram-positive and Gram-negative bacteria accounted for 68.0%and 32.0%,respectively.Coagulase negative Staphylococcus accounted for 73.0%of the total Gram-positive bacterial isolates.The prevalence of MRSA was 38.2%in children and 45.6%in adults.The prevalence of MRCNS was 67.6%in adults and 69.5%in children.A small number of vancomycin-resistant Enterococcus faecium(2.2%)and linezolid-resistant Enterococcus faecalis(3.1%)were isolated from adult patients.The resistance rates of Escherichia coli and Klebsiella pneumoniae to ceftriaxone were 52.2%and 76.4%in children,70.5%and 63.5%in adults.The prevalence of carbapenem-resistant E.coli and K.pneumoniae(CRKP)was 1.3%and 47.7%in children,6.4%and 47.9%in adults.The prevalence of carbapenem-resistant Acinetobacter baumannii(CRAB)and Pseudomonas aeruginosa(CRPA)was 74.0%and 37.1%in children,81.7%and 39.9%in adults.Conclusions The data derived from antimicrobial resistance surveillance are crucial for clinicians to make evidence-based decisions regarding antibiotic therapy.Attention should be paid to the Gram-negative bacteria,especially CRKP and CRAB in central nervous system(CNS)infections.Ongoing antimicrobial resistance surveillance is helpful for optimizing antibiotic use in CNS infections.

Objective To investigate the antimicrobial resistance of clinical isolates from elderly patients(≥65 years)in major medical institutions across China.Methods Bacterial strains were isolated from elderly patients in 52 hospitals participating in the CHINET Antimicrobial Resistance Surveillance Program during the period from 2015 to 2021.Antimicrobial susceptibility test was carried out by disk diffusion method and automated systems according to the same CHINET protocol.The data were interpreted in accordance with the breakpoints recommended by the Clinical and Laboratory Standards Institute(CLSI)in 2021.Results A total of 514 715 nonduplicate clinical isolates were collected from elderly patients in 52 hospitals from January 1,2015 to December 31,2021.The number of isolates accounted for 34.3%of the total number of clinical isolates from all patients.Overall,21.8%of the 514 715 strains were gram-positive bacteria,and 78.2%were gram-negative bacteria.Majority(90.9%)of the strains were isolated from inpatients.About 42.9%of the strains were isolated from respiratory specimens,and 22.9%were isolated from urine.More than half(60.7%)of the strains were isolated from male patients,and 39.3%isolated from females.About 51.1%of the strains were isolated from patients aged 65-<75 years.The prevalence of methicillin-resistant strains(MRSA)was 38.8%in 32 190 strains of Staphylococcus aureus.No vancomycin-or linezolid-resistant strains were found.The resistance rate of E.faecalis to most antibiotics was significantly lower than that of Enterococcus faecium,but a few vancomycin-resistant strains(0.2%,1.5%)and linezolid-resistant strains(3.4%,0.3%)were found in E.faecalis and E.faecium.The prevalence of penicillin-susceptible S.pneumoniae(PSSP),penicillin-intermediate S.pneumoniae(PISP),and penicillin-resistant S.pneumoniae(PRSP)was 94.3%,4.0%,and 1.7%in nonmeningitis S.pneumoniae isolates.The resistance rates of Klebsiella spp.(Klebsiella pneumoniae 93.2%)to imipenem and meropenem were 20.9%and 22.3%,respectively.Other Enterobacterales species were highly sensitive to carbapenem antibiotics.Only 1.7%-7.8%of other Enterobacterales strains were resistant to carbapenems.The resistance rates of Acinetobacter spp.(Acinetobacter baumannii 90.6%)to imipenem and meropenem were 68.4%and 70.6%respectively,while 28.5%and 24.3%of P.aeruginosa strains were resistant to imipenem and meropenem,respectively.Conclusions The number of clinical isolates from elderly patients is increasing year by year,especially in the 65-<75 age group.Respiratory tract isolates were more prevalent in male elderly patients,and urinary tract isolates were more prevalent in female elderly patients.Klebsiella isolates were increasingly resistant to multiple antimicrobial agents,especially carbapenems.Antimicrobial resistance surveillance is helpful for accurate empirical antimicrobial therapy in elderly patients.
9.Clinical characteristics of liver injury induced by different types of drugs and Logistic analysis of the influencing factors on the prognosis

Objective To analyze the clinical characteristics of drug-induced liver injury(DILI)caused by different drug categories,and to investigate factors influencing DILI prognosis.Methods Medical records of DILI patients diagnosed in our hospital from January 1,2019 to December 31,2023 were retrospectively collected.Clinical characteristics of DILI induced by different drug categories were analyzed.Multivariate Logistic regression was employed to identify prognostic factors of DILI.Results A total of 347 DILI patients were included.The predominant DILI pattern was hepatocellular injury(67.15%),followed by cholestatic(20.46%)and mixed types(12.39%).The top three causative drug categories were traditional Chinese medicine(25.34%),antineoplastic drugs(24.15%)and anti-infective agents(21.26%).Regarding prognosis outcomes,7 cases(2.02%)achieved complete recovery,269 cases(77.52%)showed improvement,and 71 cases(20.46%)demonstrated no improvement.Significant differences were observed among prognosis groups in age,gender,alanine aminotransferase level,aspartate transaminase level,alkaline phosphatase levels,DILI patterns,number of combined hepatoprotective drugs used,and causative drug categories(P<0.05).Multivariate analysis revealed that cholestatic type[OR=0.237,95%CI(0.083,0.673),P=0.007]and combination therapy with>2 hepatoprotective drugs[OR=0.551,95%CI(0.295,0.998),P=0.018]were independent risk factors for prognosis.Conclusion The number of hepatoprotective drugs used in combination and DILI patterns may serve as potential indicators for prognosis evaluation.Particular attention should be paid to the hepatotoxicity of traditional Chinese medicine,antineoplastic agents,and anti-infective drugs.Rational drug selection is essential for improving DILI prevention and treatment.
10.Analysis of infection status and genetic evolution of B2L and F1L genes in epidemic orf virus isolates from primary goat and sheep-producing areas in Anhui Province from 2021 to 2023

This study was aimed at investigating the infection status of orf virus(ORFV)and the genetic evolution characteristics of epidemic ORFV isolates from Anhui province.A total of 303 clinical samples collected from major meat sheep breeding cities in An-hui during 2021-2023 were subjected to ORFV detection with fluorescence quantitative PCR(qPCR).The full-length B2L and F1L genes of ORFV in the positive samples were amplified through conventional PCR and sequenced.Genetic evolution analysis of the B2L and F1L genes was conducted after sequencing.The qPCR results indicated a total ORFV positivity rate in the clinical samples of 48.8%(148/303).Multiple sequence comparisons indicated that the B2L genes of 56 sample isolates shared 96.7%-100.0%DNA and 97.4%-100.0%amino acid sequence identity.Moreover,the F2L genes of 56 sample isolates shared 95.1%-100.0%DNA and 95.0%-100.0%amino acid sequence identity.The genetic evolution tree constructed with the B2L gene DNA sequences indicated sample iso-lates and 21 reference strains located in subgroup 1,and 26 sheep-derived sample isolates and 17 reference strains located in sub-group 2.Among them,the goat-derived sample isolate FY-TYA was located in the same sub-branch as the human-derived reference strain Gansu,whereas the goat-derived sample isolate FY-XQC was located in the same sub-branch as the reference strains China Vaccine and OV-HLJ-04.The genetic evolution tree constructed with the F1L gene DNA sequences showed,the goat sample isolates FY-XQA and FY-XQC were located in the same sub-branch as the sheep-derived reference strain Xinjiang.ORFV infection was rela-tively widespread in the major meat sheep breeding areas of Anhui province,and the DNA and amino acid sequences of the B2L and F1L genes of current circulating ORFV isolates showed different degrees of genetic variation,among which F1L gene had a high de-gree of variation.Furthermore,some goat-derived sample isolates were closely related to human,vaccine,and sheep-derived refer-ence strains.These results may serve as a reference for the prevention and control of ORFV infection in Anhui province.
