Recent studies have shown that the physiological homeostasis of oral mucosal cells is regulated by the circadian clock.Dis-ruption or dysfunction of the circadian clock is closely associated with the development of oral squamous cell carcinoma(OSCC).Research based on the circadian clock offers a novel perspective on the pathogenesis and therapeutic strategies for OSCC.However, there is current-ly limited research on this topic, and people generally have insufficient understanding and recognition of the circadian clock.Given the complexity and challenges of circadian clock which is the fourth dimension of medical research,we organize relevant experts based on summarizing the current research results of circadian clock in the pathogenesis and precision diagnosis and treatment of OSCC,combining the scientific principles of the circadian clock's role and their long-term research experience, then summarizes and recommends the con-sensus opinions for the research of circadian clock in the pathogenesis mechanism and precision diagnosis and treatment of human OSCC,with the hope of providing guidance for the basic research and clinical application of circadian clock or circadian rhythm in the pathogene-sis mechanism and precision diagnosis and treatment of oral and maxillofacial squamous cell carcinoma.

Objective To compare the differences in the average transaction costs per-referral patients under different models of Integrated Delivery Systems(IDS).Methods Using a typical case sampling method,it selected referred patients from three IDS models:the county medical alliance in D City(Qinghai Province),the urban medical consortium in J District(Zhengzhou City,Henan Province),and the health management coalition in N County(Shandong Province).Structured questionnaires collected demographics, average transaction costs per-referral and cost perceptions.t-tests and ANOVA assessed cost differences;generalized linear regression identified influencing factors. Results Among 915 patients, the average transaction costs per-referral were 1 035.05 yuan(county alliance),195.31 yuan(urban consortium),and 700.97 yuan(health management coalition),with statistically significant differences(P<0.05).The urban consortium exhibited lower time costs and specialized input costs.Key influencing factors included older age(county alliance),education level, employment status, and referral travel time(urban consortium),as well as urban-rural disparities(health management coalition).Patients'cost perceptions significantly differed across models(P<0.05).Conclusion The urban medical consortium demonstrated the lowest patient the average transaction costs, highlighting its institutional advantage in minimizing financial burdens.

Objective This study aimed to examine the risk factors and prognostic factors of brucellar spondylitis for early prevention and treatment of the disease, and improving the outcomes of patients. Methods A retrospective case-control study was conducted on the patients with brucellosis who were hospitalized in the Third Hospital of Hebei Medical University from June 2020 to June 2022.Patients were assigned to brucellosis without spondylitis group or brucellar spondylitis group according to the presence of spondylitis. The patients in brucellar spondylitis group were followed for 1 year. Then they were stratified into a subgroup of good or poor prognosis according to clinical outcomes. The dataset for the demographic and clinical variables of patients were analyzed using SPSS 26.0 software. Results A total of 300 patients with brucellosis were enrolled, including 113 cases of brucellosis without spondylitis and 187 cases of brucellar spondylitis.Multivariate analysis showed that age, time from onset to diagnosis, low back pain, and erythrocyte sedimentation rate were independent risk factors for brucellar spondylitis(P<0.05).Of the 154 cases of brucellar spondylitis with known outcomes,121 cases showed good prognosis and 33 cases had poor prognosis.COX regression analysis showed that the time from onset to diagnosis, diabetes mellitus, paravertebral abscess, and neurological impairment were independent risk factors for poor prognosis in patients with brucellar spondylitis(P<0.05).Conclusions Old age, prolonged time from onset to diagnosis, low back pain, and increased erythrocyte sedimentation rate are independent risk factors for spondylitis in patients with brucellosis.Prolonged time from onset to diagnosis, diabetes mellitus, paravertebral abscess, and neurological impairment are independent risk factors for poor prognosis of brucellar spondylitis.

Objective To explore the transvaginal three-dimensional ultrasound characteristics in women with abnormal uterine cavity and compare the diagnostic value with hysteroscopy. Methods A total of 102 women who underwent transvaginal three-dimensional ultrasound and hysteroscopy in the Department of Ultrasound of Lu'an People's Hospital for various reasons were selected as the research subjects.Hysteroscopy was the gold standard. The three-dimensional ultrasound characteristics of patients with different uterine cavity lesions were statistically analyzed.A 2×2 4-grid table was drawn to calculate the diagnostic value of transvaginal three-dimensional ultrasound for various types of uterine cavity lesions. Results There were 22 cases of endometrial polyps,9 cases of intrauterine adhesions, and 5 cases of submucosal fibroids. The accuracy rates of diagnosing endometrial polyps, intrauterine adhesions and submucosal fibroids through transvaginal three-dimensional ultrasound were 90.91%,88.89%and 80.00%,respectively.The transvaginal three-dimensional ultrasound of patients with endometrial polyps mainly showed deformation of the uterine cavity line, high echoic masses, clear differentiation between the edge of the echogenic mass and the endometrium, almost no deformation of the endometrial basal line, and scattered distribution of blood flow on CDFI ultrasound.Intrauterine adhesions mainly manifested as uneven endometrial thickness and low echogenicity by transvaginal three-dimensional ultrasound, and fluid accumulation may found in the uterine cavity of some patients.Transvaginal three-dimensional ultrasound showed that submucosal fibroids were mainly hypoechoic, elliptical or irregular in shape, with CDFI revealing abundant blood flow signals. Conclusion When women with abnormal uterine cavity are examined with transvaginal three-dimensional ultrasound, patients with different types of lesions have different typical ultrasound characteristics, and transvaginal three-dimensional ultrasound diagnosis has a high diagnostic accuracy with respect to hysteroscopy as the gold standard, exhibiting its advantages of non-invasive and rapid diagnosis, worthy of clinical promotion.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
