This paper introduces a real-world cohort research model for community-acquired pneumonia (CAP) based on the Jiangsu Traditional Chinese Medicine (TCM) Dominant Diseases Diagnosis and Treatment Data Platform. Firstly, data cleaning is performed by standardizing diagnosis, symptoms, treatment and imaging, intelligently extracting unstructured information, and cleaning and constructing a standardized database. Secondly, for cohort establishment, CAP patients across the province are screened in accordance with CAP diagnostic criteria to build a high-quality disease-specific cohort. Lastly, in terms of protocol design, the characteristics of TCM research and the CAP disease profile are considered to determine appropriate inclusion and exclusion criteria, estimate sample size, define interventions, outcomes and economic evaluations, providing a reference for real-world TCM research on CAP.

4.The clinical value of introducing DSA angiography after extra corporeal membrane oxygenation

Objective To discuss the accuracy and effectiveness of digital subtraction angiography(DSA)in determining the cause of limb ischemia after extra corporeal membrane oxygenation(ECMO.Methods The clinical data of 3 child patients,who developed 4 times of acute limb ischemia during perioperative period of ECMO at the Affiliated Children's Hospital of Zhengzhou University of China from July to October of 2023,were retrospectively analyzed.In all the child patients,emergency angiography was carried out to quickly identify the cause,then,appropriate treatment plan was adopted to open the blood vessels of the right lower limb.Results After the child patients entered the operating room and received DSA examination,the causes of the limb ischemia were quickly identified.After treatment,the blood supply to the lower limbs was restored.Except for one child who experienced irreversible necrosis of the distal limb due to repeated ischemia-reperfusion injury and required amputation,the other two child patients recovered well.Conclusion It is of great significance to perform DSA examination as soon as possible when the child patients develop limb ischemic manifestations after ECMO so as to quickly identify the cause,promptly restore blood supply to ischemic limbs and increase limb preservation rate,besides,DSA examination can also be used as a preventive measure for child patients after ECMO.
5.Semiconductor laser combined with sclerotherapy for the treatment of venous malformations in child patients

Objective To explore the safety and efficacy of semiconductor laser combined with sclerotherapy in the treatment of venous malformations(VM)in child patients.Methods The clinical data of 68 child patients with VM,who were admitted to the Affiliated Children's Hospital of Zhengzhou University of China from April 2023 to April 2024,were retrospectively analyzed.The child patients were divided into semiconductor laser combined with sclerotherapy group(laser+sclerotherapy group)and simple sclerotherapy group(sclerotherapy group),with 34 child patients in each group.The reduction rate of lesion size and the incidence of complications were compared between the two groups.Results The total effective rate and significant effective rate in the laser+sclerotherapy group were 100% and 79.41%(27/34)respectively,which were higher than 82.35%(28/34)and 29.41%(10/34)respectively in the sclerotherapy group.The incidences of blister and pigmentation complications in the laser+sclerotherapy group were 11.76%(4/34)and 14.71%(5/34)respectively,which were lower than 41.18%(14/34)and 38.24%(13/34)respectively in the sclerotherapy group,and the differences between the two groups were statistically significant(all P<0.05).Conclusion In treating child patients with VM,the combination use of semiconductor laser and sclerotherapy has higher clinical safety and effectiveness.
6.Application of intracavitary semiconductor laser therapy in improving facial appearance for child patients with facial venous malformations after receiving sclerotherapy

Objective To discuss the application value of intracavitary semiconductor laser therapy in improving facial appearance for child patients with facial venous malformations(VM)after receiving sclerotherapy.Methods The clinical data of 12 child patients with facial VM after receiving sclerotherapy,for whom the improvement of their facial appearance was difficult and who were admitted to the Affiliated Children's Hospital of Zhengzhou University of China from February 2023 to February 2024,were retrospectively analyzed.Intracavitary semiconductor laser treatment was adopted in all the 12 child patients.All the child patients were followed up to check the degree of appearance improvement and lesion volume reduction,and the curative effect was evaluated.Results Three months after intracavitary semiconductor laser treatment,obvious improvement of facial appearance was obtained in all the 12 child patients.The mean postoperative PAC-QOL psychological discomfort score was(10.4±3.9)points,when compared with the preoperative(23.7±4.3)points the difference was statistically significant(P<0.05).MRI examination showed that after treatment no obvious improvement of the lesion was seen in 0 case,moderate improvement in one case,significant improvement in 4 cases,and cure in 7 cases.Conclusion Intracavitary semiconductor laser therapy has significant therapeutic effect in improving facial appearance for child patients with facial VM after receiving sclerotherapy,for whom the improvement of their facial appearance is difficult,therefore,this therapy has high clinical application value.
7.Evaluation-criteria-based development of a lean management system for a clinical blood transfusion

Objective This study aims to examine the effect of a fine-grained management system for hospital clinical blood transfusion based on evaluation criteria.Methods The data related to clinical blood transfusion before and after the system were collected and analyzed retrospectively in terms of 8 dimensions:transfusion approval process,transfusion indication evalua-tion,time interval from blood collection to transfusion,and transfusion medical record writing.Results After the system was put into operation,the rationality rate of blood transfusion approval process increased from 90.30%to 99.52%,the rationality rate of blood transfusion indication evaluation increased from 84.22%to 90.17%,the rationality rate of informed consent filling before blood transfusion increased from 87.62%to 94.72%,and the rationality rate of post-transfusion efficacy evaluation increased from 82.28%to 89.93%.The time interval from blood collection to transfusion was significantly shortened,with statistical sig-nificance(P<0.05).Conclusion The lean management system for clinical blood transfusion can significantly enhance the safe-ty and efficacy of blood transfusion,and improve the quality of healthcare services and the management level of hospitals.
8.A study on the practical effect of critical value management based on the goal and key result approach

Objective To explore the effectiveness of the OKR method in improving the efficiency of the whole process of critical value management,and to provide new ideas for the implementation of medical quality improvement in other medical insti-tutions.Methods A hospital in Tianjin was selected as the study object,which used the OKR method to reform the management of critical value since January 2024.The core goal of"improving the effectiveness of critical value management"was set,and the goal was broken down into quantitative key results such as"the rate of receiving the critical value system is over 95%"and"the rate of completing the standardised writing of medical records is up to 85%".Results After the reform,several quantitative key results,such as the completion rate of critical care medical record writing,the rate of standardised medical record writing,and the rate of overtime acceptance,were all better than before.Conclusion Through the OKR method to unify the whole hospital's strategic objectives,and the dynamic adjustment of the program based on data review,the hospital's critical value management efficiency has been significantly improved,effectively guaranteeing the safety of patients,and providing new perspectives and methods for the management of critical value in other medical institutions.
9.Study on the Correlation between Asprosin,Angiopoietin-like Protein 3 and Peripheral Vascular Disease in Type 2 Diabetes Mellitus

Objective To investigate the relationship between asprosin(Asp)and angiopoietin-like protein 3(ANGPTL3)levels and Diabetic Peripheral Vascular diseases in type 2diabetes mellitus Disease,DPVD)to evaluate their predictive value for DPVD.Methods A total of 141 patients with type 2diabetes who met the inclusion criteria were included and divided into non-DPVD group(n=71)and DPVD group(n=70),and healthy subjects in the same period were selected as normal control group(n=37).General data and expression levels of Asp and ANGPTL3 in the three groups were compared.Spearman correlation analysis was used to explore the correlation between Asp and ANGPTL3 levels and clinical indicators,and Logistic regression was used to analyze the influencing factors of DPVD.The predictive value of serum Asp and ANGPTL3 levels to DPVD was analyzed by receiver operating characteristic curve(ROC).Results Compared with non-DPVD group,the levels of asprosin and angiopoietin-like protein 3 in DPVD group were higher(all P<0.05).Spearman correlation analysis showed that Asp level was positively correlated with age,SBP,FBG,LDL-C and ANGPTL3(all P<0.05).ANGPTL3 level was positively correlated with age,SBP,DBP,FBG,TC,TG,LDL-C and Asp(all P<0.05).Age,course of disease,HbA1c,TG,LDL-C,FCP,Asp and ANGPTL3 were the influencing factors of DPVD(all P<0.05),while course of disease,TG,Asp and ANGPTL3 were the independent influencing factors of DPVD(all P<0.05).ROC curve results showed that Asp and ANGPTL3 levels alone predicted DPVD AUC of 0.679 and 0.706,corresponding sensitivity of 88.7%and 78.9%,specificity of 47.1%and 55.7%,respectively.The combined prediction of Asp and ANGPTL3 levels for DPVD was 0.777 AUC,80.3%sensitivity and 64.3%specificity.Conclusion Asp and ANGPTL3have certain predictive value for DPVD,and their combined predictive value is higher.
10.Transcriptome sequencing analysis of the mechanism by which cold water swimming regulates inflammatory response in rats

BACKGROUND:When exercising in a cold environment,the body's inflammatory response is affected by both low temperature and exercise intervention,and its impact and mechanism remain to be explored.OBJECTIVE:To explore the effects and mechanisms of cold water swimming on inflammatory response of rats based on transcriptome sequencing technology.METHODS:40 male SD rats were randomly divided into room temperature control group,room temperature swimming group,cold water control group,and cold water swimming group,with 10 rats in each group.The room temperature control group had no intervention and was free to eat.The room temperature swimming group received swimming at 30 min/time,6 times/week,for 5 weeks;the water temperature was(28±2)℃,and the water depth was 35 cm.In the cold water control group,the rats were placed in a water tank with a depth of 3 cm;the water temperature was(18±2)℃,and they were free to move.The cold water swimming group received swimming at 30 min/time,6 times/week,for 5 weeks;the water temperature was(18±2)℃,and the water depth was 35 cm.Enzyme-linked immunosorbent assay was used to detect the levels of serum interleukin-6,tumor necrosis factor-α,and high-sensitivity C-reactive protein.Based on the transcriptome sequencing results,differentially expressed genes were screened to draw Venn diagrams and heat maps,and Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analysis were performed.The protein-protein interaction network was used to screen core genes.RT-qPCR was used to detect the mRNA expression of IRF7,OAS2,and OASL in rat spleen tissue.RESULTS AND CONCLUSION:(1)The ELISA results showed that compared with the room temperature control group,the levels of various inflammatory indicators in the room temperature swimming group and the cold water swimming group were significantly increased(P<0.05),and there was no significant difference in the cold water control group.Compared with the room temperature swimming group,there was no significant difference in the expression of inflammatory indicators in the cold water swimming group.Compared with the cold water control group,the expressions of interleukin-6 and tumor necrosis factor-α in the cold water swimming group showed an upward trend,and high-sensitivity C-reactive protein increased significantly(P<0.05).(2)Transcriptome analysis:Venn diagram showed that there were 39 differentially expressed genes affected by the dual factors of temperature and exercise intervention.Cluster heat map analysis results showed that the overall gene expression trends of the room temperature swimming group and the cold water swimming group were similar,and the cold water control group showed an opposite trend.Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analysis results showed that differentially expressed genes were enriched in the immune system,locomotion,nucleic acid-binding transcription factor activity,NOD-like receptor signaling pathways and other pathways.The number of genes enriched in the NOD-like receptor signaling pathway was relatively large,and the q value was small,which may be a key pathway.The protein-protein interaction network screened out IRF7,OAS2,OASL,IFIT2,IFIT3 and other core genes.(3)RT-qPCR verification results showed that compared with the room temperature control group,the expressions of IRF7,OAS2 and OASL were significantly increased in the room temperature swimming group and the cold water swimming group(P<0.01),and there was no significant difference in the cold water control group.Compared with the cold water control group,the expression of each gene was significantly increased in the cold water swimming group(P<0.01).(4)It is concluded that cold water swimming can promote inflammatory response,and its mechanism may be regulated through the NOD-like receptor signaling pathway.
