1.Textual Research on Key Information and Modern Clinical Application of Classical Famous Formula Liumotang

Liumotang comes from the Yuan dynasty's Effective Prescription Handed Down for Generations of Physicians. It is composed of six medicinal materials: Arecae Semen, Aquilariae Lignum Resinatum, Aucklandiae Radix, Linderae Radix, Rhei Radix et Rhizoma, and Aurantii Fructus. It is a classical formula for treating abdominal pain due to Qi stagnation and constipation accompanied by heat. This study systematically collated the records of Liumotang in ancient medical books and modern clinical literature and conducted in-depth analysis and textual research on its formula source, main diseases, composition, dosage, medical books, container capacity, processing, preparation method, usage, drug basis, formula meaning, and other key information, so as to provide a powerful reference for the development and clinical application of compound preparations of the classical formula Liumotang. The results show that Liumotang was first seen in Effective Prescription Handed Down for Generations of Physicians, and many medical books of the past dynasties have imitated this. In terms of drug basis, the dried and mature seeds of the palm plant Areca catechu, resin-containing wood of the Daphneaceae plant Aquilaria sinensis, the dried roots of the Asteraceae plant woody Aucklandia lappa, the dried tuber root of the Lauraceae plant Lindera aggregata, the dried roots and rhizomes of the knotweed plant, R. palmatum, R.tangutikum, and R. officinale, and the dried and unripe fruits of the citrus genus C. aurantium and its cultivated varieties from the family Rutaceae were selected. In terms of dosage, through the textual research on bowls in the Ming and Qing dynasties, combined with the conversion of medicines and bowl capacity in the Qing dynasty, it was estimated that the dosage of each drug in the Yuan dynasty was 10.86 g. In the Ming and Qing dynasties, the dosage of drugs was mostly equal, but the dosage of drugs was somewhat different. In terms of processing, preparation method, and usage, in the medical books of the past dynasties, the processing of drugs has slightly changed, but raw drugs are used in all preparations. The preparation method and usage did not change much during the Yuan, Ming, and Qing dynasties, except for certain differences in dosage. In terms of syndrome, Liumotang was first used to treat abdominal pain due to Qi stagnation and constipation accompanied by heat. Medical books of the past dynasties often omit the symptoms of heat. In modern clinical practice, Liumotang is mainly used in the digestive system and urinary system diseases and is mostly used to treat constipation-predominant irritable bowel syndrome, biliary reflux gastritis, functional constipation, slow transit constipation, and other diseases, with no adverse reactions found yet. The above results provide a reliable scientific basis for the development and clinical treatment of Liumotang compound preparations.
2.Application of high-flow nasal oxygen therapy combined with non-invasive positive pressure ventilation in premature infants after weaning from the ventilator

Objective To explore the application value of high-flow nasal oxygen therapy combined with non-invasive positive pressure ventilation in very low birth weight infants after weaning from invasive mechanical ventilation.Methods From February 2022 to May 2024,80 cases of very low birth weight infants who were weaned from invasive mechanical ventilation in Second Affiliated Hospital of Wenzhou Medical University were selected and divided into single ventilation group and combined oxygen therapy group according to the random number table method,with 40 cases in each group.Single ventilation group was intervened with non-invasive positive pressure ventilation,while combined oxygen therapy group was intervened with high-flow nasal oxygen therapy combined with non-invasive positive pressure ventilation.The disappearance time of clinical symptoms,the score of neonatal behavioral neurological assessment(NBNA),vitamin D and blood gas indicators of two groups of premature infants were compared.And the degree of broncho pulmonary dysplasia(BPD)in premature infants was evaluated.Results The disappearance time of clinical symptoms,non-invasive positive pressure ventilation time and total oxygen requirement time of preterm infants in combined oxygen therapy group were significantly shorter than those in single ventilation group(P<0.05).After 14 days of intervention,the NBNA scores,vitamin D,oxygenation index and blood oxygen saturation of premature infants in both groups were significantly higher than those before intervention in this group(P<0.05).The NBNA score,vitamin D,oxygenation index and blood oxygen saturation of preterm infants in combined oxygen therapy group were significantly higher than those in single ventilation group(P<0.05).The degree of BPD in combined oxygen therapy group was significantly milder than that in single ventilation group(x2=8.571,P=0.003).Conclusion After weaning from invasive mechanical ventilation in very low birth weight infants,high-flow nasal oxygen therapy combined with non-invasive positive pressure ventilation intervention can help reduce inflammatory responses and brain damage,improve blood gas indicators,and lower the degree of BPD.

Turning to critical illness is a common stage of various diseases and injuries before death. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor′s professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Therefore, there is an urgent need to develop a standardized, high-quality visualization telemedicine round system .Therefore, experts have been organized to search domestic and foreign literature on telemedicine round for critically ill patients and to form this consensus based on clinical experiences so as to further improve the level of critical care treatments in regions.
4.Exploration of the path of cultural construction in red cross-named medical institutions:a case study of a Shanghai hospital

Objective Against the backdrop of the Medical Humanistic Care Enhancement Action Plan(2024-2027),this study aims to explore the pathways through which medical institutions can promote cultural construction by leveraging Red Cross initiatives.Methods Taking Hospital P in Shanghai as a case study,participatory observation,document analysis,and in-depth interviews were conducted.Based on Schein's three-level model of organizational culture,the hospital's Red Cross prac-tices were analyzed.Results The study summarizes the implementation process,outcomes,and challenges of Red Cross initia-tives at Hospital P.Reflections and recommendations are provided.Conclusion It is recommended to strengthen communication to deepen recognition of Red Cross culture,enhance teamwork to foster the development of Red Cross teams,and promote volun-teer services to implement Red Cross practices,thereby comprehensively elevating the hospital's cultural construction.
5.Mechanism of nitidine chloride against Talaromyces marneffei,determined through network pharmacology

This study was aimed at exploring the mechanism underlying the effects of nitidine chloride against Talaromyces marnef-fei through network pharmacology analysis.We collected NC and TM action targets from various databases;constructed a protein-protein interaction(PPI)network by using common drug and disease targets;and performed KEGG pathway and GO enrichment analy-ses.In vitro cellular experiments were conducted to test the antibacterial ability of NC at various concentrations,qPCR was used to de-tect the mRNA expression of genes in the target pathway,and WB was used to examine the expression of proteins associated with tar-get signaling pathways in cells.We identified 153 target genes for NC and 2 095 target genes for TM,among which 23 targets over-lapped.By integrating the PPI network with KEGG enrichment analysis,we selected key target genes in the MAPK signaling pathway,such as FLT1,FLT3,CD38,and PRF1.The CFU results indicated that NC had favorable antibacterial capability.Moreover,qPCR demonstrated that NC downregulated the mRNA expression of FLT1,FLT3,and RPS6KA3,and upregulated the mRNA expression of MAP3K8.WB findings indicated that NC downregulated the expression of RSK2,VEGF,and FLT3 proteins,and upregulated the ex-pression of MAP3K8 protein.NC may exert its anti-TM effects by downregulating the expression of RSK2,VEGF,and FLT3 proteins,thereby inhibiting MAPK pathway activation.The potential targets and signaling pathways underlying NC's anti-TM action may pro-vide new insights to guide the clinical application of NC.
6.Liuwei-Dihuang decoction modulates ovarian function in POF mice via the Notch signaling pathway

AIM:To explore the therapeutic effects of Liuwei-Dihuang decoction(LD)on ovarian function in pre-mature ovarian failure(POF)mice,and to uncover the role of the Notch signaling pathway in its mechanism.METHODS:A mouse model of POF was constructed through intraperitoneal injection of cyclophosphamide/busulfan.Subsequently,the mice were randomly divided into six groups(n=12 per group):control,POF model,low-dose LD(LD-L),medium-dose LD(LD-M),high-dose LD(LD-H),and positive drug(0.7 mg/kg diethylstilbestrol)groups.The ovarian histopathologi-cal alterations were assessed by hematoxylin-eosin(HE)staining.Serum levels of follicle-stimulating hormone(FSH),anti-Müllerian hormone(AMH),and estradiol(E2)were quantified via ELISA assays.Immunohistochemistry(IHC)was conducted to compare the expression level of cell proliferation marker Ki-67 in mouse ovarian tissues among the groups.The expression of proteins involved in the Notch pathway components[Notch1,Notch2,Jagged1(JAG-1),and hairy and enhancer of split 1(Hes1)]was determined via IHC and Western blot,while the corresponding mRNA levels of Notch1,Notch2,JAG-1,JAG-2,Hes1,and Hes5 were quantified through the qPCR analysis.RESULTS:Ovarian weight was significantly higher,and the number of atretic follicles was significantly lower in the LD-H and positive drug groups com-pared to the model group(P<0.05).However,serum FSH levels were decreased(P<0.01),whereas those of E2 and AMH were elevated in both treatment groups compared with the model group(P<0.01).In the LD-H group,the expres-sion of key molecules of the Notch pathway components was increased at the protein and mRNA level compared with the model group(P<0.01).CONCLUSION:LD ameliorates ovarian dysfunction in POF mice via activating the Notch sig-naling pathway.
