Objective:To explore the effect of case-based learning (CBL) based on WeChat platform in orthopedics practice teaching.Methods:A total of 101 orthopedic interns in The First Affiliated Hospital of Navy Medical University Hongkou Campus were randomly divided into two groups, with 50 interns in experimental group and 51 interns in control group. The experimental group conducted CBL based on WeChat platform for teaching, and the control group adopted traditional teaching method for teaching. At the end of the internship, the teaching effect on the interns was assessed by the "department graduation examination" and "questionnaire survey". The examination included basic theoretical knowledge and case analysis. Questionnaire survey included active preparation before class, teaching methods, class participation and discussion, interest in class and teaching effect. SPSS 17.0 was used for statistical analysis.Results:The total scores of the examinations, theoretical test and case analysis of the interns in the experimental group [(83.58±5.68) points, (49.98±3.71) points, (33.60±2.28) points, respectively] were better than those of the interns in the control group [(80.28±6.04) points, (48.20±4.12) points, (32.08±2.06) points, respectively], with statistical significance ( P < 0.05). The results of the questionnaire survey showed that the interns in the experimental group were significantly more satisfied with the teaching than those in the control group in such aspects as active preparation before class, teaching methods, class participation and discussion, class interest, class effect and satisfaction, and the differences in active preparation, participation and interest were statistically different ( P < 0.05). Conclusion:In the process of orthopedics practice teaching, the teaching method of CBL based on WeChat platform can mobilize the enthusiasm and participation of students, and can improve the clinical teaching effect of orthopedics, which is worthy of promotion and application in practice teaching.

Objective To observe curative effects of inflatable thoracolumbar fixation vest on nonspecific low-back pain among naval afloat personnel during prolonged deployment at sea.Methods The 179 patients with nonspecific low-back pain from the 3 naval vessels of the flotilla were randomly divided into 3 groups in accordance with the sequence of medical date:i.e.the pharmacotherapy and medium frequency pulse physiotherapy group (the PMFPP group) (or the control group),the traditional lumbar girdle + PMFPP group (the TLG + PMFPP group) and the inflatable thoracolumbar fixation vest + PMFPP group (ITLFV + PMFPP group).The patients in the 3 groups all received PMFPP treatment,but the patients of the 2 other groups were respectively supplemented with traditional lumbar girdle and inflatable thoracolumbar fixation vest.VSA scores before and after treatment and post-treatment pain duration were recorded for the evaluation of clinical outcomes between the 3 groups.Results Total effective rates of the ITLFV + PMFPP group,the TLG + PMFPP group and PMFPP group were respectively 96.83％,94.74％ and 98.31％.No statistical significance could be noted,when comparisons were made between the 3 groups (P ＞ 0.05).However,post-treatment pain duration for the ITLFV + PMFPP group was significantly shortened as compared with those of the other 2 groups (P ＞ 0.05).Conclusions During prolonged deployment at sea,the inflatable thoracolumbar fixation vest could quickly alleviate symptoms and shorten pain duration in the treatment of nonspecific low-back pain.

Objective To develop an all-in-one multifunctional thoracolumbar fixation rescue vest for land and water rescue of thoracolumbar injury and investigate its application effect.Methods According to the thoracolumbar physiological curve,type Ⅰ thoracolumbar fixation rescue vest with keel and inflatable airbag and type Ⅱ thoracolumbar fixation rescue vest with additional lifesaving balloon and inflatable cylinder were developed using thermoplastic polyurethane (TPU)-nylon composites and polycarbonate (PC) as the main materials.Clinical application of type Ⅰ thoracolumbar fixation rescue vest in 532 cases of thoracolumbar injury was discussed.Type Ⅱ thoracolumbar fixation rescue vest were tested at marine rescue and related parameters measured were automatic inflation time,buoyancy force,floating time,floating condition and victims' face orientation.Clinical outcome was quantified by the MacNab standard,and VAS for pain was recorded.Results According to the MacNab standard,excellent outcome was achieved for all cases.VAS improved from 8.2 points to 2 points after the bracing was applied,showing 100％ improvements.The brace into the water showed automatic inflation time of 3-5 seconds and maximum buoyancy force of 100 kg,and ensured a 90 kg dummy of floating over 96 hours.At marine rescue,the wounded in braced condition showed face upward with 24-hour buoyancy loss ≤5％ and freedom of movement.Conclusions The multifunctional thoracolumbar fixation rescue vest provides dual immobilization and ensures marine rescue for its automatic inflation device.This invention provides the feasibility to remove,transport and evacuate the thoracolumbar fracture patients in cabin.

Objective To compare the clinical effects of ROI-C self-locking fusion cage and common cage+titanium plate fixation on treatment of multi-segment anterior decompression and fusion for cervical spondylotic myelopathy.Methods From March 2010 to September 2013,92 patients with multi-segment cervical spondylotic myelopathy were treated with anterior decompression and fusion.52 patients were treated with common cage plus titanium plate fixation,while 40 patients were treated with ROI-C self-locking fusion cage.Clinic data including clinical preoperative condition,operation time,intraoperative blood loss,intervertebral space height,Cobb angle of cervical spine,bone graft fusion rate,complications,and JOA score and VAS score were compared.Results Follow-up visit was conducted for all cases,with the time of 12-48 months (22 months on the average).The neurologic symptoms of patients in two groups were relieved obviously after postoperative 1 week,including chest and abdomen constriction feeling,finger pain,finger and lower limb activities.Intervertebral space height of cage +titanium plate group and ROI-C fusion cage group increased to 79.06±6.67 mm and 78.80±6.85 mm respectively after postoperative 3 months from preoperative 47.15±6.96 mm and 46.95±7.14 mm;the Cobb angle increased to 9.29°±12.90° and 8.57°±13.00° respectively after postoperative 3 months from preoperative 4.27°±11.15° and 2.80°±10.81°.In ROI-C group,the operation time,intraoperative blood loss and postoperative complications were significantly lower than those in cage+titanium plate group.The differences of two groups have statistical significance.In the last follow-up visit,JOA score of ROI-C group increased to 15.15±0.91 from preoperative 9.32±1.74,with the improvement rate of 75.82％±13.28％;JOA score of cage + titanium plate group increased to 15.29± 1.07 from preoperative 9.11 ± 1.23,with the improvement rate of 77.91％± 14.14％.The differences of two groups in the improvement rate have no statistical significance.Conclusion Curative effects of ROI-C self-locking fusion cage and common cage+titanium plate fixation on treatment of muhi-segment anterior decompression and fusion for cervical spondylotic myelopathy are similar,while ROI-C self-locking fusion cage has such advantages as short operation time,less blood loss,little injury and low complication incidence.

Objective The purpose of this investigation was to study the effects of the Sildenafil citrate on mice hearing.Methods Seven-week-old adult male Kunming mice were used.The mice were randomly divided into four groups with 10 mice in each group.Sildenafil groups were orally administered daily with sildenafil [0.1 mg/(kg · d),1 mg/(kg · d),10 mg/(kg · d)] and control group was orally administered with normal saline.Then mice were tested for auditory brainstem response(ABR) to observe the changes of ABR's thresholds at before administration and 1,5,10,15,20 day afterwards.The mice basilar membrane samples were studied by immunofluorescent labeling.High performance liquid chromatography was used for determination the concentration of sidenafil in endolymph of mice cochlea.Statistical analysis was performed using SPSS 13.0.Results After 30 min following administration,the Sildenafil in endolymph of mice cochlear could be assayed by high performance liquid chromatography,and it was doserelated.Sildenafil increased the hearing thresholds with the time of administration.Hearing thresholds increased significantly in the sidenafil group at 20 d compared to the control group (P ＜ 0.05).After administered high dose of Sildenafil,on the 20th day,the ABR thresholds average threshold was (60.0 ± 10.0) dBnHL,and the control group was (14.5 ± 6.0) dBnHL.Hair cells damages in the base ring of cochlea could be observed in experimental group in a concentration-dependent manner.Conclusion Sidenafil can pass through blood-labyrinth barrier to the inner ear,and doses of sildenafil administration can induce hearing impairment in mice.
