ObjectiveTo investigate the mechanism by which Simiao Wan improve intestinal uric acid excretion in hyperuricemic (HUA) rats by inhibiting ferroptosis via the solute carrier family 7 member 11/glutathione peroxidase 4 (SLC7A11/GPX4) signaling pathway. MethodsForty-eight male SD rats were randomly assigned to a normal group, a model group, a low-dose Simiao Wan group (282.6 mg·kg-1), a high-dose Simiao Wan group (565.2 mg·kg-1), an etoricoxib group (5.4 mg·kg-1), and a ferroptosis inhibitor (Fer-1) group (2 mg·kg-1). Except for the normal group, rats in all other groups were intraperitoneally injected with potassium oxonate (100 mg·kg-1) combined with intragastric administration of hypoxanthine (500 mg·kg-1) to establish the HUA rat model. After 21 days, rats in each administration group received the corresponding dose of drug by gavage, rats in the Fer-1 group received intraperitoneal injection of Fer-1, and rats in the normal and model groups were given an equal volume of normal saline by gavage. After sampling, serum levels of uric acid (SUA), creatinine (Cr), and blood urea nitrogen (BUN) were measured using biochemical assays. The content of ferrous ion (Fe2+), reduced glutathione (GSH), and malondialdehyde (MDA), as well as superoxide dismutase (SOD) activity in ileal tissues were determined. Hematoxylin and eosin (HE) staining was performed to observe the histopathological changes in the ileum, and transmission electron microscopy was used to observe mitochondrial alterations in the ileal tissue. Immunohistochemistry (IHC) and Western blot were used to detect the protein expression levels of GPX4, ferritin heavy chain 1 (FTH1), acyl-CoA synthetase long-chain family member 4 (ACSL4), SLC7A11, and ATP-binding cassette transporter G2 (ABCG2) in ileal tissue. Real-time quantitative polymerase chain reaction (Real-time PCR) was used to determine the mRNA expression of GPX4, FTH1, ACSL4, SLC7A11, and ABCG2 in the ileal tissue. ResultsCompared with the normal group, the model group exhibited significantly elevated serum UA, Cr, and BUN levels (P<0.01), significantly increased content of MDA and Fe2+ in ileal tissue, and significantly decreased GSH content and SOD activity (P<0.01). HE staining showed extensive shedding of the ileal villous epithelium, disrupted and disorganized villous structure, and incomplete local tissue architecture. Transmission electron microscopy revealed reduced mitochondrial volume, decreased or even vanished cristae, indicating ultrastructural features characteristic of ferroptosis. Molecular and immunohistochemical results demonstrated that the protein and mRNA expression levels of GPX4, FTH1, SLC7A11, and ABCG2 were significantly downregulated, whereas those of ACSL4 were significantly upregulated (P<0.05, P<0.01). Compared with the model group, after intervention with Simiao Wan, the serum UA, Cr, and BUN levels significantly decreased in all dose groups (P<0.05,P<0.01), the MDA and Fe2+ content in ileal tissues were significantly reduced (P<0.01), and the GSH content and SOD activity significantly increased (P<0.01). HE staining showed that the shedding of ileal villous epithelium was markedly alleviated, the villous structure became relatively intact and regularly arranged, and local tissue damage was obviously improved. Transmission electron microscopy showed attenuated mitochondrial damage, improvement in mitochondrial shrinkage, and partial restoration of mitochondrial cristae. Molecular and immunohistochemical results showed that Simiao Wan upregulated GPX4, FTH1, SLC7A11, and ABCG2, and downregulated ACSL4 in a dose-dependent manner. In the high‑dose group, all indices improved significantly (P<0.05,P<0.01), whereas in the low‑dose group, only improving trends were observed for FTH1, SLC7A11 mRNA, and ABCG2 protein. ConclusionSimiao Wan may inhibit ferroptosis of ileal epithelial cells by regulating the SLC7A11/GPX4 signaling pathway, and upregulate the expression of the uric acid transporter ABCG2, thereby promoting intestinal uric acid excretion and improving HUA.

Objective:To investigate the clinical manifestations, serological and cerebrospinal fluid test results for syphilis, imaging features, and prognoses of cerebral syphilitic gumma. Methods:The clinical data of 1 patient with cerebral syphilitic gumma admitted to Department of Neurology, Second Affiliated Hospital of Soochow University in March 2023 were retrospectively analyzed. Papers about cerebral syphilitic gumma were searched from journals in Journal Citation Reports Q1 from 2000 to 2019, journals from 2020 to 2024 in PubMed, WOS, Embase, and Scopus databases, and journals from 2000 to 2024 in Wanfang Database, CNKI, and VIP database; the clinical data of 54 patients with cerebral syphilitic gumma reported in above databases and 1 patient in our hospital were collected for pooled analysis. Results:The main clinical manifestations of 55 cerebral syphilitic gumma patients included headache (32, 58.2%), lateral limb/facial weakness (25, 45.5%), nausea and vomiting (14, 25.5%), dizziness (11, 20.0%), sensory disturbances (10, 18.2%), blurred vision (7, 12.7%), seizure (5, 9.1%)), hearing loss (5, 9.1%), tinnitus (5, 9.1%), memory loss (3, 5.5%), aphasia (3, 5.5%), dysarthria (2, 3.6%), drop attack (2, 3.6%), weakness in opening eyes (2, 3.6%), unresponsiveness (1, 1.8%), Argyll-Robertson pupil (1, 1.8%), tabes dorsalis gait (1, 1.8%), and fever (1, 1.8%). In 51 patients who reported complete serologic test results, 45 patients (88.2%) were positive for non-specific antibodies to syphilis, and all patients were positive for specific antibodies to syphilis. In 34 patients underwent cerebrospinal fluid examination, 25 (73.5%) were positive for non-specific antibodies to syphilis, and 32 (94.1%) were positive for specific antibodies to syphilis. Isolated intracranial lesion (43, 78.2%) was mostly common in imaging test, and the frequently involved cranial sites were, orderly, the frontal lobe (14, 25.5%), parietal lobe (14, 25.5%), temporal lobe (5, 9.1%), frontotemporal lobe (3, 5.5%), frontoparietal lobe (2, 3.6%), parieto-occipital lobe (2, 3.6%), nucleus pulposus (1, 1.8%), clivus (1/55, 1.8%), and cerebral peduncle of the midbrain (1, 1.8%). Thirty patients (54.5%) were misdiagnosed as having other intracranial space-occupied diseases, orderly, glioma (11, 36.7%), metastatic tumors (5, 16.7%), meningiomas (4, 13.3%), other unexplained intracranial space-occupying (4, 13.3%), brain abscess (3, 10.0%), cavernous hemangioma (1, 3.3%), intracranial lymphoma (1, 3.3%), auditory nerve and pituitary tumors (1, 3.3%). Of the 42 patients who reported prognosis after anti-syphilitic treatments, 41 had varying degrees of improvement, and one died of brain herniation. Conclusion:Because of atypical clinical manifestations and lack of clear diagnostic criteria, cerebral syphilitic gumma is often misdiagnosed as intracranial tumors; cerebral syphilitic gumma should be considered in patients with positive non-specific antibodies to syphilis/specific antibodies to syphilis in serum and cerebrospinal fluid having neurological symptoms and intracranial space-occupied foci; timely diagnosed and treated patients can prognosed well.

4.Expression of VEGF induced by PES1 via enhancing hypoxia inducible factor-1αexpression in epithelial ovarian cancer cells
Chongqing Medicine 2013;(27):3211-3213
Objective To investigate the expression of PES1 in ovarian cancer and its relationship with the vascular endothelial growth factor(VEGF) expression .Methods The expression of PES1 in ovarian cancer tissues and corresponding pericancerous tis-sues was detected with Western blot .Results The expression of PES1 in ovarian cancer tissues was obviously higher than that in the pericancerous tissues .The secretion amounts of VEGF in cell culture fluid of CAOV-3 and ES-2 with transfection of FLAG-PES1 were elevated from (178 .0 ± 11 .8) pg/mL and (309 .5 ± 18 .5) pg/mL to (375 .0 ± 18 .3) pg/mL and (633 .2 ± 25 .7)pg/mL respectively (P<0 .01) ,the secretion amounts of VEGF were also increased(P<0 .01) .The relative VEGF mRNA levels were also raised by 1 .8 times and 2 times respectively (P< 0 .01);Western blot simultaneously showed that the over expression of PES1 could increase the expression of HIF-1αin these two kinds of cells .The luciferase report gene transcriptional activation activity de-tection reveald that PES1 had no direct effect on the VEGF transcription .After cotransfecting HIF-1αSiRNA and FLAG-PES1 to CAOV-3 and ES-2 cells ,Western blot demonstrated that HIF-1αSiRNA could obviously inhibit the increase of HIF-1αexpression induced by PES1 ,at the same time the secretion amounts of VEGF and mRNA level were also supressed .Conclusion The expres-sion of PES1 is obviously up-regulated in ovarian cancer tissues .
