1.Prenatal ultrasound graded management of Sylvian fissure for diagnosing fetal lissencephaly

Objective To observe the value of prenatal ultrasound graded management of Sylvian fissure for diagnosing fetal lissencephaly.Methods Totally 39 fetuses with MRI diagnosed lissencephaly who underwent prenatal ultrasound examination were retrospectively enrolled and divided into non-graded management group(n=20)and graded management group(n=19)according to prenatal ultrasound examination before or after the application of prenatal ultrasound graded management of Sylvian fissure(i.e.prenatal ultrasound routine screening for fetal Sylvian fissure morphology).The diagnosis of prenatal ultrasound were compared and analyzed between groups,and the diagnostic value of graded management was evaluated.Results Among 20 fetuses in non-graded management group,prenatal ultrasound showed lissencephaly in 4 fetuses,but only other structural abnormalities in 16 fetuses.Then the latter were re-evaluated based on prenatal ultrasound graded management of Sylvian fissure,among which 4 fetuses could not be evaluated since not standard ultrasonic section,2 fetuses with severe hydrocephalus and Sylvian fissure could not be seen,while Sylvian fissure morphology did not match the corresponding gestational week in 4 fetuses,and type Ⅰ(no platform type)and type Ⅴ(Z-shaped)were noticed in 5 and 1 fetus,respectively.In graded management group,prenatal ultrasound indicated 15 fetuses with lissencephaly,including Sylvian fissure morphology did not match the corresponding gestational week in 6 fetuses,type Ⅰ(no platform type),type Ⅲ(linear type)and type Ⅴ(Z-shaped)were detected in 7,1 and 1 fetus,respectively,while no clear diagnosis was obtained in 4 fetuses.Prenatal ultrasound detection rate of fetal lissencephaly in graded management group(15/19,78.95%)was significantly higher than that in non-graded management group(4/20,20.00%)(P<0.01).Conclusion Based on graded management of Sylvian fissure could improve the efficiency of prenatal ultrasound for diagnosing fetal lissencephaly.
2.Analysis of project funding in the direction of geriatric medicine of the National Natural Science Foundation of China from 2000 to 2020

Objective:To examine the funding landscape of the National Natural Science Foundation of China(NSFC)concerning research focused on the elderly as a specific demographic group.It seeks to analyze prevailing research hotspots and to offer insightful research ideas for scholars in this field.Methods:The study employed a retrospective research design.It involved retrieving data on approved projects related to the elderly from the NSFC's official database, covering the period from 2000 to 2020.The data set included variables such as funding year, project title, project category, supporting institution, and funding amount(application codes: H1901, H1902, H1903).Statistical analysis was conducted using Excel software, while visual analysis was performed using CiteSpace software.Results:Between 2000 and 2020, a total of 710 projects focused on geriatrics received funding, amounting to RMB 361, 032, 300, 000.This period exhibited a fluctuating upward trend in both the number of projects and the total funding amount.The primary funding categories were face-to-face projects and youth science fund projects, which together accounted for 44.4%(315/710), 34.4%(244/710)respectively.The distribution of project-relying units demonstrated significant regional imbalance, with Beijing, Zhejiang, Jiangsu, and Shanghai being the predominant areas.Notably, Nanjing Medical University, Peking University, and Shanghai Jiao Tong University ranked as the top three institutions, with funding percentages of 5.9%(42/710), 5.4%(38/710), and 4.5%(32/710), respectively.The research themes encompassed nucleoplasmic interactions, brain networks, and bioinformatics, among others.Conclusions:The NSFC has consistently provided funding for in-depth research in the critical field of geriatrics.The primary research directions focus on aspects such as the mechanisms of aging and the pathogenesis of neurological diseases associated with aging.With the application of innovative technologies and methodologies, significant breakthroughs and achievements are anticipated in geriatrics, contributing to the improvement of health outcomes and the enhancement of the quality of life for the elderly.
3.Survey on pharmacovigilance functions and operational models in 66 tertiary medical institutions in China

OBJECTIVE To provide practical basis and policy recommendations for improving the pharmacovigilance (PV) system construction in medical institutions across China. METHODS A questionnaire survey was conducted using a mixed sampling strategy of “online random sampling+offline supplementary sampling” to distribute questionnaires among pharmaceutical professionals in tertiary medical institutions nationwide. The questionnaire covered aspects such as the construction of PV systems, job position settings, information system support, operational practices, and multi-stakeholder collaboration. The data were analyzed using descriptive methods and SPSS 20.0 statistical software. RESULTS A total of 70 valid questionnaires were collected from 66 medical institutions, primarily Class A tertiary hospitals. The survey found that 90.00% had designated PV personnel and 74.29% routinely conducted PV activities. However, there were notable disparities in resource allocation and information system capacity, with less than 50% of the institutions conducting post-marketing drug re-evaluation. PV activities were primarily focused on the collection and reporting of adverse drug reactions, with limited capabilities in signal detection and risk assessment. CONCLUSIONS Among the surveyed tertiary hospitals, PV systems have begun to take shape. However, challenges persist in terms of system establishment, resource allocation, risk assessment, and inter-organizational coordination. Policy efforts should focus on strengthening regulatory frameworks, improving information sharing mechanisms, enhancing professional training, and strengthening collaboration between hospitals and market authorization holders to ensure the effective implementation of PV in medical institutions.
4.Research Progress on the Clinical Efficacy and Safety of Telitacicept in the Treatment of IgA Nephropathy

Immunoglobulin A nephropathy (IgAN) is the most common primary glomerular disease worldwide, accounting for 20%-40% of primary glomerulonephritis cases. Approximately 40% of patients progress to end-stage renal disease within 20 years. Traditional treatments primarily involve renin-angiotensin system inhibitors and glucocorticoids, but some patients show limited response and face infection risks. In recent years, the dual-target biologic agent Telitacicept, which targets B cell-activating factor/B lymphocyte stimulator(BAFF/BLyS) and a proliferation-inducing ligand (APRIL), has demonstrated significant potentialin the treatment of IgAN. By simultaneously neutralizing BAFF/BLyS and APRIL, Telitacicept suppresses abnormal B-cell activation and the production of pathogenic IgA1, significantly reducing urinary protein levels and preserving renal function. This article systematically reviews its mechanism of action, clinical efficacy, and safety, aiming to provide insights for the clinical management of IgAN.
5.Analysis of clinical trials and progress of registered osteoporosis drugs in China

Objective:The aim of this study was to analyze the registration status of osteoporosis drug clinical trials in China, providing a reference for the research and development of therapeutic drugs for osteoporosis.Methods:Using the Drug Clinical Trial Registration and Information Publication Platform on the website of the State Drug Administration as the search platform, we retrieved the information on the public announcement of drug clinical trials with osteoporosis as an indication, and the search period was limited from 2013 to February 2025.Researchers extracted the key information and performed statistics.Results:A total of 182 registered osteoporosis drug clinical trial projects were collected.The main target indications were predominantly osteoporosis in postmenopausal women, and there were relatively few trials targeting the elderly subjects.56.0%(102/182)of the registered trials were bioequivalence studies, and 21.4%(39/182)were phase I clinical studies.Conclusions:The number of registered trials and the research and development process of domestic osteoporosis drug clinical trials in China have shown a stable trend in general, and the types of trials are mainly concentrated in bioequivalence studies and early clinical studies.In order to further meet the medication needs of domestic patients, China still needs to increase the research and development efforts of innovative osteoporosis drugs.
6.Role of the Brock, Mayo, and PKUPH combined model in risk stratification of solitary pulmonary nodules in a health check-up cohort

Objective:To investigate the role of the Brock, Mayo, and PKUPH combined model in risk stratification of solitary pulmonary nodules (SPNs) in health check-up population.Methods:An ambispective cohort study was conducted on 668 eligible SPNs cases from the health management center in Chongqing General Hospital from June 2018 to June 2019. The exposure condition was prospectively followed or historically retrospected, and the clinical outcomes were prospectively followed. SPNs were classified into benign and malignant groups. Descriptive statistics and univariate analysis were performed to assess the differences in risk characteristics between two groups. Receiver operating characteristic (ROC) curve, clinical decision curve, and Hosmer-Lemeshow goodness-of-fit test were used to evaluate and compare the predictive performance, clinical utility, and calibration of the combined model versus individual Brock, Mayo, and PKUPH models.Results:Among the 668 SPNs cases, 82 (12.28%) were diagnosed as malignant. Age, sex, smoking history, extrapulmonary tumor history, diameter, upper lobe, clear border and spicule sign in the malignant group were significantly different from those in the benign group (all P<0.05). The combined model demonstrated superior predictive performance, clinical utility, and calibration compared to the best-performing individual Brock model [Area under the curve (AUC): 0.88 (95% CI: 0.84-0.92) vs 0.86 (95% CI: 0.82-0.91)]. Besides, multi-grade risk stratification enabled by the combined model was better than binary classification, with the malignant rate of the four risk levels were 0.60%, 4.62%, 14.58% and 56.07%, respectively. Conclusion:The combined model addresses the limitations of individual models in SPNs risk stratification for health examination populations, improving predictive performance, clinical utility, and calibration, while proposing a superior multi-grade risk stratification system.
7.miR-146a chitosan nanoparticles preparation and protective effects on acute liver cell injury induced by LPS

This study aims to investigate the characteristics of chitosan-loaded miR-146a nanoparti-cles and explore their protective effect against acute liver cell injury induced by LPS in vitro.Galac-tosylated arginine chitosan(GCA)nanoparticles were successfully synthesized,prepared and used to further prepare GCA-miR-146a nanoparticles with different mass ratios by ion cross-linking method.The binding efficiency of miR-146a by GCA nanoparticles was detected by gel retardation experiment.Particle size,morphology and potential were observed and detected by transmission e-lectron microscopy and Zetasizer NanoZS90 respectively.The cytotoxicity of GCA nanoparticles on HepG2 cells was detected by CCK-8 kit.Cellular uptake was assayed by fluorescence microscopy to select the optimum ratio for the further study.The LPS induced acute liver cell injury were evalua-ted by real-time fluorescence quantitative PCR.The results showed that the mass ratio of GCA-miR-146a was 5∶1.Encapsulation efficiency and drug loading efficiency of the nanoparticles reached 97.11%and 20.08%,respectively.The 100 nm nanoparticles with 1.15 mV surface charge showed uniform morphology.The GCA-miR-146a nanoparticles had no cytotoxicity and had high transfection efficiency on HepG2 cells.The study demonstrated that GCA-miR-146 ananoparticles could significantly increase the expression of miR-146a in HepG2 cells and reduce the mRNA ex-pression levels of IL-6 and TNF-α in vitro.The prepared GCA nanoparticles loaded with miR-146a had high loading efficiency and could protect LPS-induced acute liver cell injury in vitro.

9.Acceptance of generative artificial intelligence among maternal and infant care personnel and its influencing factors

Objective:To assess the current level of acceptance of generative artificial intelligence (GAI) among maternal and infant care providers and to explore the factors influencing their acceptance.Methods:A convenience sampling method was used to survey 164 maternal and infant care providers from Beijing Hospital and China-Japan Friendship Hospital between March and May 2024. The survey instruments included a general information questionnaire, GAI Acceptance among Maternal and Infant Care Providers Scale, and Self-assessment Scales for Digital Technology Awareness and Self-assessment Scales for Digital Competence. Multiple linear regression analysis was conducted to identify influencing factors.Results:The total acceptance score of GAI among 164 participants was [181 (158, 200) ]. Multiple linear regression analysis showed that interest in GAI, digital technology awareness, and digital competence were significant influencing factors for GAI acceptance, with statistically significant differences ( P<0.05) . Conclusions:The level of GAI acceptance among maternal and infant care providers was relatively high. It is recommended to strengthen the promotion and training on the application and precautions of GAI, improve their digital competence and professional literacy, while fostering critical and independent thinking, to enhance the integration of GAI into maternal and infant care services.
