1.Characteristics of the Five Tones of Parkinson's Disease Patients Based on the Theory of "Five-Viscera Phonology":A Case-Control Study

ObjectiveTo study the characteristics of the five tones of Parkinson's disease (PD) patients based on the theory of "five-viscera phonology", and provide references for the syndrome differentiation and treatment of PD. MethodsA total of 272 cases of PD patients were collected as the PD group, and 240 individuals, including patient family members and hospital staff, were recruited as the control group. The 25-tone analyzer was used to collect the five-tone characteristics of both PD patients and control group participants. The participants were then stratified into three age groups, 41~55, 56~70, and 71~85 years old, and categorized by gender (male and female) for analysis. The frequency and composition ratio of the five tones were analyzed for both groups across the different age ranges and genders. Additionally, the average voice frequency of each participant was calculated to compare differences between groups, stratified by age range and gender. ResultsIn the 41~55 and 56~70 age groups, male participants in the PD group and the control group exhibited the highest frequency of Yu (羽) tone, with the PD group showing a significantly higher composition ratio of Yu tone compared to the control group (P<0.05); for males in the 56~70 age group, the composition ratios of Shang (商) and Zhi (徵) tone in the PD group were lower than those in the control group (P<0.05). For males in the 71~85 age group, both the PD group and the control group had the highest frequency of Yu tone, but there was no statistically significant difference in the composition ratios of the five tones between groups (P>0.05). For female participants in the PD group across all age groups, Yu tone was the most frequent, whereas for the control group, Jue (角) tone was the most frequent in all age groups, and the composition ratio of Yu tone in the PD group was significantly higher than that in the control group across all age groups (P<0.05); in the 56~70 age group, the composition ratio of Jue tone was lower in the PD group compared to that in the control group (P<0.05). Regarding voice frequency, males in the PD group aged 41~55 and 56~70 had higher voice frequency than those in the control group of the same age range, and similarly, females in the PD group aged 56~70 and 71~85 had higher voice frequency than their counterparts in the control group (P<0.05). ConclusionPD patients have a voice with a higher frequency and an increased proportion of Yu tone in their five-tone distribution. According to the theory of five-viscera phonology, PD patients may have disease mechanism of kidney essence deficiency.
2.Effects of Shaoyao decoction on intestinal microecology in rats with damp-heat diarrhea

OBJECTIVE To explore the impact of Shaoyao decoction on the intestinal microecology of rats with damp-heat diarrhea. METHODS Male SD rats were randomly divided into physiological group, model group, and Shaoyao decoction low-, medium- and high-dose groups (single dose of 5.62, 28.10 and 56.20 g/kg, by raw drugs), with 7 rats in each group. Except for the physiological group, the remaining groups were all fed with high-sugar and high-fat diet, raised in a high-temperature and high- humidity environment, and administered white liquor and Escherichia coli suspension by gavage to establish a rat model of damp- heat diarrhea. On the second day after modeling, rats in each dosage group of Shaoyao decoction were administered the corresponding solution by gavage, once in the morning and once in the evening, for 3 consecutive days. After the last administration, fecal metagenomic sequencing analysis and biological information analysis were performed to detect the activities of intestinal cytochrome P450 (CYP) enzymes and intestinal flora metabolic enzymes. RESULTS There was no significant difference in the α diversity indexes of intestinal flora among the groups (P>0.05). The main phyla of rats in each group included Firmicutes, Actinobacteriota and Verrucomicrobiota, and the core dominant genera included Dubosiella, Lactobacillus and Romboutsia; low- , medium- and high-dose of Shaoyao decoction could reverse the decrease in the relative abundance of Firmicutes, Romboutsia, Bifidobacterium and Monoglobus, as well as the increase in the relative abundance of Proteobacteria, Verrucomicrobiota, Escherichia-Shigella and Corynebacterium (P<0.05). Rats in each dosage group of Shaoyao decoction showed higher relative abundance of intestinal flora involved in carbon metabolism and cofactor biosynthesis pathways. Compared with the model group, the elimination rates of aminophenol and midazolam, as well as the concentration (except for the low-dose group) and metabolic rate of p-nitrophenol in rats of each dose group of Shaoyao decoction were significantly increased (P<0.05). CONCLUSIONS Shaoyao decoction can correct intestinal flora disorder in rats with damp-heat diarrhea, promote the recovery of intestinal CYP metabolic enzymes and intestinal flora metabolic enzymes, and thereby comprehensively improve intestinal microecology. 3.Efficacy of the far lateral Key-hole technique in the treatment of central cervical disc herniation.

OBJECTIVES:
In the early stage of central cervical disc herniation, clinical symptoms may be mild. However, as the spinal cord becomes compressed by herniated nucleus pulposus tissue, progressive edema and degeneration may occur, resulting in more severe clinical manifestations, including limb weakness, bladder and bowel dysfunction, spastic paraplegia of the lower extremities, and even respiratory difficulty. The spinal endoscopic Key-hole technique is widely applied in treating radiculopathic cervical spondylosis and has demonstrated good clinical outcomes. This study aims to analyze and summarize the technical points and therapeutic efficacy of the far lateral Key-hole technique in the treatment of central cervical disc herniation, providing reference for clinical application.
METHODS:
Eight patients with central cervical disc herniation treated with the far lateral Key-hole technique were included as the experimental group. Another 8 patients who underwent single-level anterior cervical diskectomy and fusion (ACDF) during the same period were selected as the control group. Data collected included gender, age, intraoperative blood loss, incision length, and length of hospitalization. Pain severity was evaluated using the Visual Analogue Scale (VAS); cervical function was assessed using the Japanese Orthopaedic Association (JOA) score; and cervical disability was measured by the neck disability index (NDI). Radiological outcomes were assessed using disc height index (DHI), cervical Cobb angle, and operative segment Cobb angle.
RESULTS:
Compared with the control group, the experimental group had significantly less intraoperative blood loss, shorter incision length, and shorter hospital stay (all P<0.05), while there was no significant difference in operation time (P>0.05). Postoperative VAS and NDI scores in both groups were significantly lower than preoperative values, and JOA scores significantly improved (P<0.05). No significant differences were noted between the two groups preoperatively (P>0.05). Postoperatively, the experimental group showed significantly lower VAS and NDI scores and higher JOA scores than the control group (P<0.05). There was no statistical significance in DHI before and after surgery in the experimental group (P>0.05), while the DHI increased significantly postoperatively in the control group (P<0.05). Postoperative DHI in the experimental group was significantly lower than in the control group (P<0.05). No significant differences were observed in the cervical Cobb angle either within or between groups (all P>0.05). The operative segment Cobb angle increased significantly after surgery in the control group (P<0.05), while no other operative segment angle changes were statistically significant (all P>0.05).
CONCLUSIONS
The far lateral Key-hole technique offers advantages in treating central cervical disc herniation, including reduced intraoperative bleeding, smaller incision length, shorter hospitalization, and improved postoperative pain relief. Additionally, the technique does not affect cervical physiological curvature or stability in the short term, making it suitable for clinical application.

4.Role of the Brock, Mayo, and PKUPH combined model in risk stratification of solitary pulmonary nodules in a health check-up cohort

Objective:To investigate the role of the Brock, Mayo, and PKUPH combined model in risk stratification of solitary pulmonary nodules (SPNs) in health check-up population.Methods:An ambispective cohort study was conducted on 668 eligible SPNs cases from the health management center in Chongqing General Hospital from June 2018 to June 2019. The exposure condition was prospectively followed or historically retrospected, and the clinical outcomes were prospectively followed. SPNs were classified into benign and malignant groups. Descriptive statistics and univariate analysis were performed to assess the differences in risk characteristics between two groups. Receiver operating characteristic (ROC) curve, clinical decision curve, and Hosmer-Lemeshow goodness-of-fit test were used to evaluate and compare the predictive performance, clinical utility, and calibration of the combined model versus individual Brock, Mayo, and PKUPH models.Results:Among the 668 SPNs cases, 82 (12.28%) were diagnosed as malignant. Age, sex, smoking history, extrapulmonary tumor history, diameter, upper lobe, clear border and spicule sign in the malignant group were significantly different from those in the benign group (all P<0.05). The combined model demonstrated superior predictive performance, clinical utility, and calibration compared to the best-performing individual Brock model [Area under the curve (AUC): 0.88 (95% CI: 0.84-0.92) vs 0.86 (95% CI: 0.82-0.91)]. Besides, multi-grade risk stratification enabled by the combined model was better than binary classification, with the malignant rate of the four risk levels were 0.60%, 4.62%, 14.58% and 56.07%, respectively. Conclusion:The combined model addresses the limitations of individual models in SPNs risk stratification for health examination populations, improving predictive performance, clinical utility, and calibration, while proposing a superior multi-grade risk stratification system.
5.Quantifying Oxaliplatin-Related Hepatic Sinusoidal Injury Based on 3D CT Image Segmentation Technology

Purpose To investigate the value of oxaliplatin-induced sinusoidal injury based on 3D CT image segmentation technology,and to analyze its variation.Materials and Methods A total of 24 patients with uneven hypoperfusion of liver parenchyma in the portal vein phase of enhanced CT after receiving oxaliplatin regimen chemotherapy from November 2019 to July 2021 in the Second Affiliated Hospital,Jiangxi Medical College,Nanchang University were analyzed,retrospectively.The imaging data of patients with epigastric CT enhancement before chemotherapy,early chemotherapy(1-3 cycles)and late chemotherapy(≥4 cycles)were collected.Two doctors used 3D Slicer software to segment and measure the liver volume and spleen volume at each stage on the CT image,as well as the hepatic hypoperfusion volume(HHV)of the liver in the portal vein phase after chemotherapy,and calculated the HHV ratio(HHVR),liver volume hepatomegaly rate and spleen volume splenomegaly rate,and performed statistical analysis.Results The ICC for HHV,liver volume and spleen volume measured by two doctors were 0.997(P=0.000),0.905(P=0.000)and 0.998(P=0.000),respectively.Reddish-brown could be used to represent normal liver parenchyma,and blue could be used to render the HHV area to achieve HHV visualization.There was a statistically significant increase in HHVR in the late stage of chemotherapy compared with the early stage of chemotherapy[(74.35±30.56)%vs.(57.15±30.53)%;t=3.226,P=0.004].In the early and late stages of chemotherapy,the liver volume and spleen volume increased statistically significantly compared with those before chemotherapy(all P<0.05),and the spleen volume splenomegaly rate value in the late stage of chemotherapy increased statistically compared with the early stage of chemotherapy[25.75(7.40,70.66)%vs.17.86(4.44,63.59)%;Z=2.229,P=0.026].The Pearson correlation coefficient between liver volume hepatomegaly rate and HHVR was 0.377(P=0.008),and the Spearman correlation coefficient between spleen volume splenomegaly rate and HHVR was 0.296(P=0.041).Conclusion The application of 3D CT image segmentation technology to measure the HHV caused by oxaliplatin has good consistency,and can realize the visual display of HHV.With the increase of chemotherapy cycle,the extent of oxaliplatin-induced sinusoidal injury shows an increasing trend,and is positively correlated with hepatomegaly and splenomegaly.
6.Impacts of LncRNA XIST on proliferation,apoptosis and invasion of tongue squamous cell carcinoma cells by regulating miR-337/ADAM28

Objective:To investigate the mechanism of LncRNA XIST on proliferation,apoptosis and invasion of tongue squamous cell carcinoma(TSCC)cells by regulating miR-337/ADAM28.Methods:qRT-PCR was used to detect the expression of XIST,miR-337,ADAM28 mRNA in TSCC tissues and cells.The expression of ADAM28,CyclinD1,MMP-9,MMP-14,Caspase-3 and Bax proteins was detected by Western blot.MTT assay was applied to detect cell activity.Transwell method was applied to detect the invasion ability of cells.Cell apoptosis was detected by flow cytometry.The double luciferase reporter gene experiment was applied to verify the targeting relationship between miR-337 and LncRNA XIST,miR-337 and ADAM28.Results:Compared with the adja-cent tissues,the expression of XIST and ADAM28 mRNA in TSCC increased,the expression of miR-337 decreased(P<0.05).Compared with human normal oral mucosa cell HOK,the expression of XIST and ADAM28 in human TSCC cell lines increased,the expression of miR-337 decreased(P<0.05).Silencing XIST reduced the expression of CyclinD1,MMP-9 and MMP-14,up-regula-ted the expression of Caspase-3 and Bax proteins,restrained the proliferation and invasion of CAL27 cells,and induced apoptosis.Inhibition of miR-337 expression reversed the silencing XIST on the proliferation and invasion of CAL27 cells,as well as the promo-ting effect on apoptosis.The double luciferase reporter gene experiment showed that XIST targeted down-regulation of miR-337,and miR-337 targeted negative regulation of ADAM28 expression.Conclusion:LncRNA XIST down-regulates miR-337 expression,up-regulates the expression of ADAM28,induces the proliferation and invasion of TSCC cells,and inhibits cell apoptosis.
7.The correlation between abnormal metabolic indexes and the severity of coronary artery lesions in patients with acute coronary syndrome

Objective To explore the influencing factors of coronary artery lesion severity in patients with acute coronary syndrome (ACS). Methods Clinical data of ACS patients admitted to Zhongshan Hospital, Fudan University from December 2017 to December 2019 were consecutively collected. The modified Gensini score was used to assess the severity of coronary artery lesions. Univariate and multivariate linear regression analyses were performed to identify independent factors associated with coronary artery lesion severity. Results A total of 1 689 ACS patients were included, with an average age of (64.04±11.45) years; 1 353 (80.11%) were male, and the mean modified Gensini score was (8.12±4.03). Multivariate linear regression analysis revealed that sex (β=0.97, P=0.001), age (β=0.03, P=0.021), estimated glomerular filtration rate (eGFR; β=-0.03, P<0.001), low-density lipoprotein cholesterol (LDL-C; β=0.58, P<0.001), apolipoprotein A1 (Apo A1; β=-1.28, P=0.012), lipoprotein(a) [Lp(a); β=0.001, P=0.033], and glycated hemoglobin A1C (HbA1C; β=0.45, P<0.001) were independent influencing factors of the modified Gensini score. Conclusions Metabolic indicators, including Apo A1, LDL-C, HbA1C, and Lp(a), may serve as risk factors for coronary artery lesion severity in ACS patients, with Apo A1 demonstrating the strongest impact.

9.Research Progress on the Clinical Efficacy and Safety of Telitacicept in the Treatment of IgA Nephropathy

Immunoglobulin A nephropathy (IgAN) is the most common primary glomerular disease worldwide, accounting for 20%-40% of primary glomerulonephritis cases. Approximately 40% of patients progress to end-stage renal disease within 20 years. Traditional treatments primarily involve renin-angiotensin system inhibitors and glucocorticoids, but some patients show limited response and face infection risks. In recent years, the dual-target biologic agent Telitacicept, which targets B cell-activating factor/B lymphocyte stimulator(BAFF/BLyS) and a proliferation-inducing ligand (APRIL), has demonstrated significant potentialin the treatment of IgAN. By simultaneously neutralizing BAFF/BLyS and APRIL, Telitacicept suppresses abnormal B-cell activation and the production of pathogenic IgA1, significantly reducing urinary protein levels and preserving renal function. This article systematically reviews its mechanism of action, clinical efficacy, and safety, aiming to provide insights for the clinical management of IgAN.
10.Advances in Animal Modeling in the Study of Bone-Vascular Axis Comorbidities

Osteoporosis and cardiovascular calcification,two major age-related chronic diseases that China is confronting today,pose serious threats to public health.Previous studies have reported overlapping connections in the pathological processes and molecular mechanisms of these two diseases,particularly concerning inflammation,oxidative stress,and dysregulation of mineral metabolism,and that these two diseases tend to share common pathogenic factors.However,research exploring the comorbidity mechanisms of the two diseases remains limited in both depth and scope,largely due to the lack of widely accepted comorbidity animal models.Herein,we analyzed the latest research findings on the comorbidity mechanisms of vascular calcification and osteoporosis,focusing on summarizing the animal disease models currently in extensive use and the relevant evaluation criteria.We aim to provide new references for comorbidity research models and offer scientific evidence for future studies on pathological mechanisms and the development of new therapeutic strategies.
