1.Impact of hyperglycemia on liver fibrosis and end-stage liver disease in patients with comorbidities of chronic hepatitis B and steatotic liver disease

ObjectiveTo analyze the clinicopathological features of patients with comorbidities of chronic hepatitis B (CHB) and steatotic liver disease (SLD), to investigate the impact of hyperglycemia on the risk of liver fibrosis and end-stage liver diseases (ESLD), and to provide a basis for the clinical management of such population. MethodsA total of 668 adult patients with CHB-SLD confirmed by liver biopsy in The Fifth Medical Center of Chinese PLA General Hospital from January 2011 to December 2019 were enrolled, and a retrospective cohort was established with the time of liver biopsy as the baseline and the onset of ESLD as the endpoint. All patients were followed up to March 31, 2024. Propensity score matching (PSM) was performed at a ratio of 1∶4 to balance baseline features between groups, resulting in 82 patients in the hyperglycemia group and 281 in the non-hyperglycemia group. The two groups were compared in terms of metabolic profiles, laboratory markers, and histopathological features. The Mann-Whitney U test was used for comparison of non-normally distributed quantitative data between two groups. The chi-square test or Fisher exact test was used for comparison of categorical data between two groups. A multivariate Logistic regression analysis was used to investigate the influencing factors for advanced fibrosis (AF), and the Kaplan-Meier survival analysis and the Cox proportional-hazards regression model were used to determine the influencing factors for the development of ESLD. ResultsCompared with the non-hyperglycemia group, the hyperglycemia group had a significantly higher number of factors for metabolic disorders, a significantly higher degree of hepatic steatosis, and a significantly higher detection rate of AF (all P<0.05). The multivariate Logistic regression analysis showed that hyperglycemia was a risk factor for AF (odds ratio = 1.753, 95% confidence interval [CI]: 1.017 — 3.023, P=0.043). The survival analysis showed that hyperglycemia increased the risk of ESLD (χ2=4.340, P=0.037). The multivariate Cox regression analysis confirmed that hyperglycemia was a significant metabolic risk factor for ESLD in patients with AF (adjusted hazard ratio=3.208, 95%CI: 1.201 — 8.568, P=0.020). ConclusionHyperglycemia can increase the risk of AF and ESLD in CHB-SLD patients. Clinical monitoring and active management should be strengthened for patients who have already developed AF and hyperglycemia.
2.Research progress on the role of inflammation in post-stroke depression and the protective effect of Chinese medicine preparations

Inflammation plays a crucial role in the occurrence and development of post-stroke depres-sion(PSD),yet the specific molecular mechanisms are still not fully understood.In the field of drug research,traditional Chinese medicine preparations have attracted increasing attention for improving PSD symptoms due to their advantages of multiple targets,many links,and few side effects.At the same time,they have great re-search value in the prevention and treatment of PSD.This article reviews the mechanism of inflammatory re-action,such as the balance regulation of pro-inflammatory/anti-inflammatory factors,inflammatory markers,gene polymorphism of inflammatory factors in PSD,and the related research progress of Chinese medicine preparations in regulating inflammatory reaction to prevent and treat PSD,with a view to providing reference for the selection of clinical Chinese medicine preparations and the prevention and treatment of PSD.
3.Downregulation of Neuralized1 in the Hippocampal CA1 Through Reducing CPEB3 Ubiquitination Mediates Synaptic Plasticity Impairment and Cognitive Deficits in Neuropathic Pain.

Neuropathic pain is frequently comorbidity with cognitive deficits. Neuralized1 (Neurl1)-mediated ubiquitination of CPEB3 in the hippocampus is critical in learning and memory. However, the role of Neurl1 in the cognitive impairment in neuropathic pain remains elusive. Herein, we found that lumbar 5 spinal nerve ligation (SNL) in male rat-induced neuropathic pain was followed by learning and memory deficits and LTP impairment in the hippocampus. The Neurl1 expression in the hippocampal CA1 was decreased after SNL. And this decrease paralleled the reduction of ubiquitinated-CPEB3 level and reduced production of GluA1 and GluA2. Overexpression of Neurl1 in the CA1 rescued cognitive deficits and LTP impairment, and reversed the reduction of ubiquitinated-CPEB3 level and the decrease of GluA1 and GluA2 production following SNL. Specific knockdown of Neurl1 or CPEB3 in bilateral hippocampal CA1 in naïve rats resulted in cognitive deficits and impairment of synaptic plasticity. The rescued cognitive function and synaptic plasticity by the treatment of overexpression of Neurl1 before SNL were counteracted by the knockdown of CPEB3 in the CA1. Collectively, the above results suggest that the downregulation of Neurl1 through reducing CPEB3 ubiquitination and, in turn, repressing GluA1 and GluA2 production and mediating synaptic plasticity impairment in hippocampal CA1 leads to the genesis of cognitive deficits in neuropathic pain.

4.Role of the iNOS/IRS1/AKT/GSK-3β signaling pathway in chronic intermittent hypoxia-induced insulin resistance

Objective :
To pathological changes and inducible nitric oxide synthase(iNOS), phosphorylated insulin receptor substrate 1 serine 307(p-IRS1ser 307), phosphorylated protein kinase B serine 473(p-AKTser 473), glycogen synthase kinase-3β(GSK-3β), and gluconeogenic synthase(GS) proteins were observed in the liver of rats under the condition of chronic intermittent hypoxia-replicated oxygen in control. And to explore the role of iNOS/IRS1/AKT/GSK-3β signaling pathway in chronic intermittent hypoxia-induced insulin resistance.
Methods :
Forty SD rats were randomly divided into a control group(NC group) and an experimental group(CIH group), with 20 rats in each group. The NC group was placed in a normoxic environment for 12 weeks, while the CIH group was first subjected to intermittent hypoxia for 8 weeks, and then resumed normoxic rearing until the 12th week. Fasting blood glucose(FBG) and fasting insulin(FINS) were measured at baseline, week 8 and week 12, and liver tissues were taken for pathology and measurement of iNOS, p-IRS1ser 307, p-AKTser 473, GSK3β and GS levels, to compare the differences between groups.
Results:
t baseline, there was no significant difference in liver pathology between the two groups, and the observed indexes were not statistically significant(P>0.05); at 8 weeks, compared with the NC group, liver pathology in the CIH group showed significant disorganization of hepatic blood sinusoids and hepatocyte cords, obvious hepatocyte edema, smaller nuclei, increased lymphocyte infiltration, and a small number of fat vacuoles, significantly higher levels of FBG, FINS, insulin resistance index(HOMA-IR), iNOS mRNA, p-IRS1ser 307 protein, GSK-3β protein levels, and decreased p-AKTser 473 protein and GS protein levels, all of which were statistically significant(allP<0.05). IRS1ser 307 protein, GSK-3β protein levels were increased, p-AKTser 473 protein and GS protein levels were decreased, and the differences were statistically significant(allP<0.05); at 12 weeks, no lymphocyte infiltration was seen in the CIH group compared with that of the NC group and fat vacuoles significantly increased, and there was no improvement in the other pathological damage that had already occurred, and the levels of p-AKTser 473 protein significantly increased. AKTser 473 protein level significantly increased, p-IRS1ser 307 protein and GS protein levels were significantly reduced, all of which were statistically significant(allP<0.05), and the rest of the observational indexes were not statistically significant. Pearson′s correlation analysis showed that HOMA-IR of CIH group was significantly positively correlated with the levels of iNOS mRNA, p-IRS1ser 307 protein, and GSK-3β protein at 8 weeks(r=0.874, 0.817,0.872;allP<0.05), and significantly negatively correlated with the levels of p-AKTser 473 protein and GS protein(r=-0.886,-0.879;allP<0.05).
Conclusion
Chronic intermittent hypoxia can lead to hepatic pathological damage that cannot be reversed even by reoxygenation interventions and may mediate the development of insulin resistance by upregulating the IRS1/AKT/GSK-3β signaling pathway through the upregulation of iNOS mRNA expression.

Intravascular large B-cell lymphoma (IVLBCL), a rare subtype of non-Hodgkin lymphoma, is classified as an independent subtype of extranodal diffuse large B-cell lymphoma (DLBCL) in the 2008 World Health Organization (WHO) Classification (Turner et al., 2010). The 5th edition of the World Health Organization (WHO 2022) classification of hematolymphoid tumors retains this subtype (Alaggio et al., 2022). IVLBCL, which is characterized by neoplastic lymphocyte proliferation within the lumen of small blood vessels, tends to invade organs, such as the nervous system, skin, bone marrow (BM), and lung (D'Angelo et al., 2019; Satoh et al., 2019; Vásquez et al., 2019; Fukami et al., 2020).

Idiopathic pulmonary fibrosis (IPF) is a progressive disease lacking effective therapy. Metformin, an antidiabetic medication, has shown promising therapeutic properties in preclinical fibrosis models; however, its precise cellular targets and associated mechanisms in fibrosis resolution remain incompletely defined. Most research on metformin's effects has focused on mesenchymal and inflammatory responses with limited attention to epithelial cells. In this study, we utilized Sftpc lineage-traced and Fgfr2b conditional knockout mice, along with BMP2/PPARγ and AMPK inhibitors, to explore metformin's impact on alveolar epithelial cells in a bleomycin-induced pulmonary fibrosis model and cell culture. We found that metformin increased the proliferation and differentiation of alveolar type 2 (AT2) cells, particularly the recently identified injury-activated alveolar progenitors (IAAPs)-a subpopulation characterized by low SFTPC expression but enriched for PD-L1. Single-cell RNA sequencing revealed a reduction in apoptosis among mature AT2 cells. Interestingly, metformin's therapeutic effects were not significantly affected by BMP2 or PPARγ inhibition, which blocked the lipogenic differentiation of myofibroblasts. However, Fgfr2b deletion in Sftpc lineage cells significantly impaired metformin's ability to promote fibrosis resolution, a process linked to AMPK signaling. In conclusion, metformin alleviates fibrosis by directly activating AT2 cells, especially the IAAPs, through a mechanism that involves AMPK and FGFR2b signaling, but is largely independent of BMP2/PPARγ pathways.
7.Identification of potential biomarkers of proliferative diabetic retinopathy based on proteomics and transcriptomics data

Objective To identify potential biomarkers for proliferative diabetic retinopathy(PDR)using proteomics and transcriptomics data.Methods In this study,the proteomics dataset(PXD046630)and two transcriptomics datasets(GSE60436 and GSE102485)were derived from the aqueous humor samples and fibrovascular membranes of PDR patients,respectively.Differentially expressed genes(DEGs)were identified via R software,specifically the limma and edgeR pack-ages.The shared DEGs between PXD046630 and GSE60436 were analyzed via protein-protein interaction(PPI),Gene On-tology(GO)enrichment,and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses.The key DEGs were validated in GSE102485 via receiver operating characteristic(ROC)curve analysis.A quantitative polymerase chain reaction(qPCR)assay was used to confirm the mRNA of these candidate biomarkers in human retinal microvascular endothelial cells(HRMECs)cultured in high glucose and low oxygen conditions.Results A total of 59 shared DEGs and 26 hub genes were identified from the PXD046630 and GSE60436 datasets.KEGG analysis revealed that six pathways,inclu-ding extracellular matrix-receptor interaction,proteoglycans in cancer,and complement and coagulation cascades,were enriched in 12 key DEGs.Fibronectin 1(FN1),tissue inhibitor of metalloproteinase 3(TIMP3),complement factor H(CFH),decorin(DCN),and lipoprotein receptor-related protein-2(LRP2)were identified as potential biomarkers on the basis of their AUC values being greater than 0.900(CI≥95%).The mRNA expression levels of FN1,CFH,and LRP2 were significantly increased in HRMECs cultured in high glucose and low oxygen conditions.Conclusion FN1,CFH,and LRP2 are potential biomarkers for PDR,and further studies are needed to explore their roles and therapeutic potential in PDR.
8.NLUS-VQA: construction and evaluation of a visual question answering model for neonatal lung ultrasound diagnosis

Objective:To develop and evaluate a medical visual question answering (VQA) model for neonatal lung ultrasound (LUS) images to enhance intelligent auxiliary diagnosis of neonatal pulmonary diseases.Methods:Using data from neonates admitted to Beijing Obstetrics and Gynecology Hospital, Capital Medical University (January 2023 to December 2024), an image-question-answer dataset comprising 251 LUS images was constructed [43 pneumonia (17.1%), 42 neonatal respiratory distress syndrome (16.7%), 83 transient tachypnea (33.1%), and 83 normal (33.1%) images] with a four-tier medical question-answer framework. Building upon the Qwen2.5-VL-7B base model and integrating LoRA fine-tuning with chain-of-thought prompting, we developed the NLUS-VQA model to enhance visual-language semantic alignment and enable stepwise clinical reasoning, achieving efficient small-sample adaptation. Model performance was comprehensively assessed through natural language generation metrics (BLEU-4, ROUGE-1/2/L), qualitative evaluation of characteristic recognition, and clinical consistency analysis.Results:(1) Quantitative evaluation demonstrated that NLUS-VQA achieved scores of 22.38 (BLEU-4), 48.26 (ROUGE-1), 22.40 (ROUGE-2), and 37.20 (ROUGE-L), representing significant improvements over baseline models. (2) Qualitatively, the model exhibited strong performance in identifying lung consolidation, coalescent B-lines, and snowflake signs, with its chain-of-thought strategy enhancing clinical interpretability and answer accuracy. (3) Clinically, NLUS-VQA achieved a Cohen's Kappa coefficient of 0.78 and diagnostic accuracy of 80.8% (21/26), indicating substantial agreement with clinical experts.Conclusion:The NLUS-VQA model demonstrates robust interpretability in recognizing key sonographic patterns (e.g. lung consolidation, confluent B-lines, and snowflake signs), providing a scalable framework for small-sample medical image analysis, though diagnostic performance on complex conditions remains limited by dataset scale and minority class representation.
9.Analysis of the High-risk Factors and Maternal Fetal Outcomes of Intrahepatic Cholestasis of Pregnancy in Twin Pregnancies

Objective:To analyze the characteristics,high-risk factors,and maternal fetal outcomes of intrahe-patic cholestasis of pregnancy(ICP)in twin pregnancies,and provide a basis for clinical intervention.Methods:A retrospective analysis was conducted on 791 cases twin pregnant women who gave birth ≥24 weeks of gestation at Changsha Maternal and Child Health Hospital affiliated to Hunan Normal University from January 1,2021 to No-vember 30,2024.According to whether the pregnant women were complicated with ICP,all included twin pregnant women were divided into two groups,with 732 cases in the non ICP group and 59 cases in the ICP group.The differences in clinical characteristics,high-risk factors,and maternal fetal outcomes between two groups were compared,and Logistic regression analysis was used to identify the influencing factors of ICP in twin pregnancies.Results:①Among the women with twin live births who gave birth at ≥24 weeks of gestation,the incidence of ICP was 7.46%(59/791),and the proportion of mildly elevated total bile acid(TBA)was 83.05%(49/59);The inci-dence of ICP was highest at 34-36+6 weeks of pregnancy,accounting for 52.54%(31/59),and the incidence be-fore full-term was 77.97%(46/59);The treatment rate of TBA lowering drugs was 37.29%(22/59),and the inef-fective rate of drug treatment was 68.18%(15/22).②Univariate analysis found that the proportion of preeclamp-sia(PE)and previous cesarean section history in the ICP group was higher than that in the non ICP group,and the difference was statistically significant(P<0.05).③ Logistic regression analysis suggests that,PE(OR 3.104,95%CI 1.763-5.466)and a history of previous cesarean section(OR 2.252,95%CI 1.039-4.878)were an inde-pendent risk factor for ICP in twins(P<0.05).④The gestational age at delivery and birth weight of newborns in the ICP group were lower than those in the non ICP group.The premature birth rate,cesarean section rate,and neonatal transfer rate were higher than those in the non ICP group,and the differences were statistically signifi-cant(P<0.05).Conclusions:PE and previous cesarean section history are high-risk factors for the occurrence of ICP in twin pregnancies.A detailed understanding of past medical history and active prevention of PE are bene-ficial for preventing the occurrence of ICP in twin pregnancies.
10.Attentional processing of verbal emotional information in college students with subclinical depression

Objective:To examine the attentional processing of verbal emotional information in college students with subclinical depression.Methods:Fifty college students were recruited(24 with subclinical depression group and 26 in the control group)were recruited from a university in Liaoning Province to investigate the atten-tional bias of subclinically depressed college students towards verbal emotional information(experiment 1).In addi-tion,48 college students(20 with subclinical depression and 28 in the control group)were recruited to investigate the attention disengagement of subclinically depressed college students from verbal emotional information(experi-ment 2).In experiment 1,two groups passively viewed words and recorded the eye movement data of emotional words.In experiment 2,two groups were asked to watch the emotional word-neutral word pair.When color borders were presented around neutral words,the attention disengagement latency that was the time taken by the eye saccade from emotional words to neutral words was recorded.Results:The results of experiment 1 showed that the total fix-ation duration of negative words in the subclinical depression group was longer than that in the control group(P<0.01).The results of experiment 2 showed that attention disengagement latency of negative words in the subclinical depression group was longer than that in the control group(P<0.05).Conclusion:College students with subclini-cal depression exhibit attentional bias towards negative verbal information and difficulties in attentional disengage-ment.
