Objective To assess the clinical value of a novel surgical technique—Tubeless subxiphoid uniportal video-assisted thoracoscopic surgery with percutaneous suspension technique via balance-shaped sternal elevation device in the resection of anterior mediastinal masses. Methods Patients who underwent tubeless subxiphoid uniportal video-assisted thoracoscopic surgery via balance-shaped sternal elevation device in anterior mediastinal masses process at the Department of Thoracic Surgery, West China Hospital, Sichuan University from March to April 2025 were included, and their clinical data were analyzed. Results A total of 4 patients were included, with 2 males and 2 females, aged 58-75 years. The diameter of the tumor was 2.5-3.0 cm. The operation time was 60.0-150.0 min, intraoperative blood loss was 5-10 mL, pain score on the 3rd day after surgery was 0 points, and postoperative hospital stay was 2-3 days. All patients achieved complete resection of the masses and thymus without perioperative complications. Conclusion The tubeless subxiphoid uniportal video-assisted thoracoscopic surgery with percutaneous suspension technique via balance-shaped sternal elevation device technique optimizes surgical visualization and instrument maneuverability while avoiding complications related to conventional anesthesia and tubing, thereby markedly enhancing the minimally invasive profile of anterior mediastinal masses resections. In addition to maintaining procedural safety, this approach effectively reduces postoperative pain and accelerates patient recovery, highlighting its potential for widespread clinical adoption.

Objective:To analyze the incidence and progression of overactive bladder(OAB)symp-toms following radical prostatectomy for prostate cancer patients and to identify related risk factors.Methods:A retrospective study was conducted on 263 local stage prostate cancer patients who underwent radical prostatectomy at Peking University Third Hospital from January 2013 to May 2017.Clinical base-line information,comprehensive imaging features,perioperative parameters,preoperative urinary control status,pathological diagnosis,and the incidence of OAB within one year postoperatively were collected and analyzed.In the imaging features,two parameters were defined:Bladder wall thickness(BWT)and bladder mucosal smoothness(BMS),which were used to predict the occurrence of OAB.Patients were evaluated based on their clinical baseline characteristics,including age,body mass index(BMI),co-morbidities,and prostate-specific antigen(PSA)levels.The imaging characteristics were assessed using preoperative MRI,focusing on BWT and BMS.Perioperative parameters included operative time,blood loss,and length of hospital stay.The OAB symptoms were assessed using the overactive bladder symptom score(OABSS)and the international prostate symptom score(IPSS).These scores were correlated with the postoperative incidence of OAB.Results:Among the 263 patients who underwent radical prostatecto-my,52(19.8％)exhibited OAB within one year postoperatively.Of the 40 patients with preoperative OAB symptoms,17(42.5％)showed remission postoperatively,while 23(57.5％)had persistent symptoms.Additionally,29 patients developed new-onset OAB,accounting for 55.77％of all postopera-tive OAB cases.Univariate analysis indicated that BWT,BMS,OABSS,and IPSS score were all associ-ated with the occurrence of postoperative OAB.Further multivariate analysis identified BMS as an inde-pendent risk factor for long-term OAB(P＜0.001).Conclusion:Long-term postoperative overactive bladder is a common complication following radical prostatectomy.The findings suggest that preoperative MRI measurements of bladder wall thickness and bladder mucosal smoothness during bladder filling phase can predict the risk of OAB occurrence postoperatively.Identifying these risk factors preoperatively can help in counseling patients about potential complications and in developing strategies to mitigate the risk of developing OAB after surgery.Early detection and management of these parameters might improve the quality of life for patients undergoing radical prostatectomy.

Objective To evaluate carotid sinus block (CAS) in preventing the low hemodynamic state in the perioperative period of carotid artery stenting (CAS).Method A prospective single-center randomized controlled study was conducted on 172 patients with internal carotid artery stenosis receiving CAS from May 2015 to May 2018.Patients were randomly divided into group A treated with local anesthesia of the carotid artery sinus of the trouble side under the guidance of ultrasound preoperatively and group B without local block anesthesia.The carotid artery stenosis rate,the maximum intraoperative reduction of blood pressure and heart rate,the use of atropine and vasopressor were monitored and compared between the two groups.Results There were no significant differences between the two groups in preoperative general data,degree of stenosis,plaque nature,degree of residual stenosis (P ＞ 0.05).The maximum reduction of intraoperative blood pressure and heart rate in group A was significantly lower than that in group B,and the number and dose of intraoperative atropine were smaller than those in group B (P ＜ 0.05).The number of postoperative patients with low hemodynamic status in group A and the time period of vasopressor use were significantly less than those in group B (P ＜ 0.05).Conclusion Preoperative carotid sinus block anesthesia is safe and effective in preventing low hemodynamic status in perioperative period of CAS.

Objective To evaluate the efficacy and experience of Q-switched laser with small light spot in the treatment of melasma.Methods Forty-six patients with melasma were emolled in study.Subjects received total of forty-six patients treatment with 1064 nm Q switched laser at one month intervals.Light spot of 3-4 nm were used.Results Forty-six patients completed the trial.The physician evaluation showed that 50％ of patients achieved 90％ to 100％ clearance,and 89.1％ patients achieved 60％ to 90％ clearance.Side effects was minimal and all the patients tolerated the treatment well.Conclusions Small light-spot and low-energy 1064 nm Q Switched laser is substantially effective and highly safe for the treatment of melasma.

Objective To investigate the chemokine 12 (CXCL12) and chemokine receptor 4 (CXCR4) expressions in hypopharyngeal carcinoma and its place in the disease development,invasion and metastasis of significance.Methods Immunohistochemistry was used to detect the expressions of CXCL12 and CXCR4 in 35 cases of hypopharyngeal cancer tissues and in 28 cases of tumor-adjacent non-tumor tissues.Results The expressions of CXCL12 and CXCR4 in the hypopharynx carcinomas were significantly higher (P ＜ 0.05).Both expressed in hypopharyngeal carcinomas was significantly positively correlated (P ＜ 0.01).Both hypopharynx cancer in lymph node metastasis group were significantly higher than the expression of cervical lymph node metastasis group,the difference was significant (P ＜ 0.05).Conclusions CXCL12 and CXCR4 are involved in hypopharynx cancer development,invasion and metastasis,and there is a positive feedback regulation mechanism between two factors.Moreover,CXCL12 and CXCR4 have synergistic effect in development,invasion and metastasis of hypopharynx cancers.

Objective To investigate the feasibility of contrast-enhanced CT with low tube voltage using iterative model reconstruction (IMR) technique.Methods Sixty patients were randomly assigned into 2 groups (group A and group B, 30 each) according to random number table.All patients underwent contrast-enhanced hepatic CT.Group A was scanned with 100 kV at arterial phase(AP) and 120 kV at portal vein phase (PVP), while group B was scanned with 120 kV at AP and 100 kV at PVP.All protocols were performed at the same tube current of 250 mAs.Raw data were reconstructed with IMR for AP images in group A and PVP images in group B;and reconstructed with FBP for AP images in group B and PVP images in group A.Images of 4 different groups were obtained: A1(AP,100 kV,IMR) , B1(AP,120 kV, FBP), A2(PVP, 120 kV,FBP) and B2(PVP, 100 kV, IMR).Subjective evaluation indexes for image quality including low-contrast detectability, lesion edge sharpness, image distortion and diagnostic confidence.Objective evaluation indexes included CT attenuation of hepatic parenchyma, image noise, SNR and CNR, which were assessed and compared between groups A1 and B1, groups A2 and B2.Effective radiation doses were calculated.Results Effective dose in group A1 was reduced 35.1％ compared toB1 (t=ll.05, P＜0.01), while a reduction of 37.7％ in group B2 compared to A2 (t=11.64,P ＜ 0.01).Subjective image quality score of low-contrast detectability and lesion edge sharpness were significantly higher in group A1 compared to B1 (Z =6.391, 3.200, P ＜ 0.01), as well as in group B2 compared to A2 (Z =6.559, 3.409, P ＜ 0.01).No differences were found in image distortion and diagnostic confidence between groups A1 and B1, groups A2 and B2, respectively (P ＞ 0.05).Significantly lower image noise and higher SNR/CNR were found in group A1 compared to group B1 (t =12.889, 15.458, 1.325, P ＜ 0.01) , as well as in group B2 compared to group A2(t =15.163, 15.308, 3.136, P ＜0.01).Conclusions Significant radiation dose reduction and image quality improvement in contrast-enhanced hepatic CT can be reached by using low tube voltage protocol combining with IMR technique.

Objective To observe the characteristics of bone development and body development in 3-7 years old children with cerebral palsy. Methods 50 girls and 50 boys aged 3-7 years with cerebral palsy were included. Their height, weight, length of right and left upper extremities, and both hands bone age were measured by trained professionals. Results Their bone ages of both hands were 1 year younger than actual age (P<0.05), and there was no significant difference between right and left hand bone age (P>0.05). Their height growth was in line with the normal children, while the weight growth was a little different. Both sides of upper limbs growed synchronously, and slowed down with the time. There was no significant difference in both sides (P>0.05). Conclusion The bone development of children with cerebral palsy lags behind the life age. Their height growth is close to normal children. The development shows no no significant lateral dominance.

Objective To study the characteristics of symptom distress and its inlfuencing factors in patients with primary liver cancer after transcatheter arterial chemoembolization (TACE). Methods A total of 108 patients with primary liver cancer receiving TACE in Department of Hepatobiliary Surgery, the Third Afifliated Hospital of Sun Yat-sen University from January 2012 to January 2013 were voluntarily enrolled in this study on the 3rd day after operation by convenience sampling method. There were 94 males and 14 females with age ranging from 26 to 72 years old and median age of 51 years old. The informed consents of all patients were obtained and the ethics committee approval was received. Cross-sectional study and questionnaire survey were used in the research. The questionnaire survey included questionnaire of general information, M.D. Anderson Symptom Inventory (MDASI), Social Support Measuring Questionnaire (SSS) and Medical Coping Modes Questionnaire (MCMQ). Multiple linear regression analysis was used to identify factors inlfuencing the symptom distress. Results The mean score of MDASI symptom subscale of patients during therapy was 3.0±1.6 with a mild degree of symptom distress. The score of MDASI life distress subscale of patients was 3.4±1.7, also with a mild degree of symptom distress. Work was most influenced by the symptoms, followed by emotion, joy of life, common activity, relation with others and walks. Fever, discomfort of stomach, abdominal distention, icteric skins and weight loss were common symptoms in sequence which distressed patients. The total score of social support was 42.2±6.7 which was at the middle level, and the sore of use of social support was 7.9±2.2. The score of 3 dimensions facing, avoiding, surrender were 2.5±0.5, 2.4±0.5, 2.0±0.6 respectively. Facing and avoiding were the common coping modes. Patients who had college educational degree or above, no job after being ill, low level of subjective support, surrender coping mode were more distressed by symptoms than the other patients (RR=0.320, 0.254, 0.569, 0.217;P<0.05). Conclusions Patients with primary liver cancer have mild symptom distress after TACE, and patients with college educational degree or above, no job after being ill, low level of subjective support, surrender coping mode are more distressed by symptoms than the other patients.

Objective To observe the effect of changes in CD34+ cell level on various degrees of chronic heart failure (CHF) in the elderly. Methods The enrolled patients were divided into four CHF groups according to the New York Heare Association(NYHA) functional class: NYHA class I (n=23), Ⅱ (n=27), Ⅲ (n = 20) and IV group (n= 16) , and there were 41 healthy controls over the same period. The levels of peripheral blood CD34+ cells were measured, and the tumor necrosis factor-α (TNF-α). its soluble receptors (sTNFR-1 and sTNFR-2) and vascular endothelial growth factor (VEGF) were also measured. Results The levels of CD34+ cells were elevated in the early CHF and depressed in the advanced CHF in elderly patients. The levels of CD34+ cells were (0. 6± 0.2) 109/L in control group, (2.4±0. 4) 109/L in NYHA class I group, (1.9±0.2)×109/L in NYHA class Ⅱ group, (1.3±0.1)×109/L in NYHA class Ⅲ group and (0.5±0.2)×109/L in NYHA class Ⅳ group, respectively (all P<0.01).And TNF-α, sTNFR-1, sTNFR-2 and VEGF were increased in severe chronic left ventricular failure CNYHA class IV vs. I group:TNF-α: (61.4± 15.7) ng/L vs. (28.4±10.8)ng/L; sTNFR-1: (2820.9±1282.8)ng/L vs. (690.8±62.7) ng/L; sTNFR-2: (4113.1±1102.2) ng/L vs. (740.8± 112.3)ng/L; VEGF: (996.3±487.1)ng/L vs. (423. 3±147. 9)ng/L, all P<0. 013. Conclusions The changes of CD34+ cell level may predict various degrees of chronic heart failure in elderly patients.

Objective: To investigate the variation of γδ T cells from healthy human peripheral blood(PB)and neonatus cord blood (CB)in proliferation and subtypes with isopentenyl pyrophosphate(IPP), and to acquire enough γδ T cells possessing distinct characteristics for experimental study.Methods: Mononuclear-cells from peripheral blood and cord blood induced by IPP were stained separately with monoclonal antibodies,which were fluorescein-labeled,and then used for assaying the expressing condition of surfaco molecules,as well as to evaluate the variation of γδ T cells on the percentage, subtypes and pbenotypes by FCM.Results:γδ T cells only account for a small proportion in both PB and CB.However,there was a significant difference in the heterogeneity between peripheral blood and cord blood γδ T cells.Vγ9Vδ2 T cells were dominant in peripheral blood γδ T cells.Most of Vγ9Vδ2 T cells in fresh isolated PBMC were central memory-type(CD27~+ CD45RA~-)and effector memory-type(CD27~-CD45RA~-)with IPP, PB γδ T cells proliferated strongly;The effector memory-typo(CD27~-CD45RA~-)turned into the main subtype in all Vγ9Vδ2 T cells,and HLA-DR and B7 molecules were highly expressed on the populations.But the cord blood γδ T cells showed rather complex subgroup heterogeneity,and Vγ9Vδ2 T cells were almost na(i)ve-type(CD27~+ CD45RA~+); Though γδ T cells were expanded(the percent of γδ T cells was increased),and Vγ9Vδ2 T cells were differentiated towards central memory-type and effector memory-type on day 14 with IPP,most of γδ T celLs still remained in the phase of na(i)ve-type(CD27~+ CD45RA~+).Conclusion:Tbere lies great differences of γδ T cells in quantity and subtypes between healthy person peripheral blood and neonatus cord blood.Peripheral blood γδ T cells can be activated and proliferated with IPP, while cord blood γδ T cells have the potential to deferentiate into director memory-type which can be used for experimental and clinical study with the synergy of corresponding cytokines.The immuno-regulation and effector function will be reported in other papers.
