1.Interpretation of expert consensus on diagnosis and treatment of esophagogastric variceal bleeding in cirrhotic portal hypertension (2025 edition)

Cirrhotic portal hypertension is a clinical syndrome caused by persistently elevated portal venous pressure due to liver cirrhosis and can lead to a series of complications, among which esophagogastric variceal bleeding has become one of the most severe complications due to sudden onset and a high mortality rate. Since the release of Expert consensus on diagnosis and treatment of esophagogastric variceal bleeding in cirrhotic portal hypertension (2019 edition), significant advances have been achieved in this field in China and globally. In order to formulate an expert consensus aligned with the situation of China, Chinese Society of Surgery, Chinese Medical Association organized and compiled Expert Consensus on diagnosis and treatment of esophagogastric variceal bleeding in cirrhotic portal hypertension (2025 edition). This article elaborates on the key updates in the new edition and explores the major differences between the old and new editions, in order to enhance the understanding of the new edition among clinicians and provide a reference for clinicians in clinical work.
2.Study on meal preferences of school aged children based on discrete choice experiment

Objective:
To explore the relative importance of different food attributes and levels in food decision making of school aged children, and to understand their meal preferences, so as to provide the evidence for formulating precise intervention strategies for dietary behaviours of school aged children.
Methods:
From May to June 2024, a total of 854 children aged 11 to 15 years old were selected from 2 middle schools (each school in urban and rural areas) in both Hubei Province and Anhui Province by stratified cluster random sampling method to conduct a D-optimal discrete choice experiment. The mixed Logit model was used to analyze children s preference for meal attributes and different levels, and to calculate the relative importance (RI) of attributes and willingness to pay (WTP) in meal choices.
Results:
The included five food attributes had statistical significance on meal choice of school aged children ( P <0.05). The relative importance of food attributes affecting the meal choices of school aged children in descending order were dining mode ( RI =31.26%), food varieties ( RI =30.56%), cooking method( RI =23.84%), taste( RI =8.06%) and price ( RI =6.27%). Among them, school aged children preferred home cooked meals ( β =0.74) (WTP=86.3 yuan),varied foods(grain/tubers+vegetables+fish, meat, eggs and beans) ( β =0.61) (WTP=71.9 yuan), fried/roasted cooking ( β =0.51) and spicy taste ( β =0.33).Price was negatively correlated with meal choices( β =-0.01) ( P <0.05). Based on residential area and body mass index (BMI), the stratified analysis showed that dining mode was highest in the relative importance for rural children with overweight and obese children ( RI =31.28%,34.17%), both of whom preferred home cooked meals ( β =0.76, 0.91), and meals containing fish, meat, eggs and beans with grain/tubers or grain/tubers and vegetables in terms of food choice (area: β =0.53, 0.53 ; BMI: β =0.55, 0.56) ( P <0.05).
Conclusions
School aged children have different preferences for different attributes of meals. The quality of school meals should be improved,the cost of buying healthy meals should be reduced,targeted family health education should be carried out,and healthy cooking methods should be advocated.
3.A case of occipitotemporal cavernous hemangioma

This report describes a 72-year-old male patient with occipitotemporal cavernous hemangioma(CCH).The patient presented with persistent pain in the right occipital and retro auricular areas for over one year.Physical examination detected a tender mass in the right occipital region.Imaging studies showed destruction of the right occipitotemporal bone with a heterogeneous signal mass and irregular enhancement on magnetic resonance imaging(MRI).Positron emission tomography/computed tomography(PET/CT)examination revealed increased fluorodeoxyglucose(FDG)uptake(SUVmax=5.9),suggesting a benign lesion.Complete surgical excision of the tumor and involved skull was performed,with pathological diagnosis confirming cavernous hemangioma.The patient's symptoms completely resolved with no recurrence during three months of follow-up.This case represents the first report of PET/CT application in diagnosing occipitotemporal CCH,providing valuable reference for improving diagnostic accuracy and reducing misdiagnosis rates for this rare condition.
4.Application value of needle-knife accurate fistulotomy for difficult biliary cannulation during endoscopic retrograde cholangiopancreatography

Objective:To investigate the application value of needle-knife accurate fistulo-tomy (NKAF) for difficult biliary cannulation during endoscopic retrograde cholangiopancreato-graphy (ERCP).Methods:The retrospective and descriptive study was conducted. The clinicopatho-logical data of 137 patients with difficult biliary cannulation during ERCP at Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine between January 2021 and December 2022 were collected. There were 51 males and 86 females, aged (69±13)years. All 137 patients received NKAF for cannulation during ERCP. Observation indicators: (1) surgical situations; (2) complications. Measurement data with normal distribution were represented as Mean± SD. Count data were repre-sented as absolute numbers. Results:(1) Surgical situations. Of 137 patients, 136 cases had succe-ssful cannulation, 1 case had failed cannulation with NKAF following unsuccessful double-guidewire technique. In the 136 successful cases, the endoscope was straightened in 42 cases, left in a long position in 37 cases, and maintained in the standard position in 57 cases. The cannulation time was (90±8)s. (2) Complications. The serum amylase at postoperative 6 hours in the 136 successful cases was (163±23)U/L. No patient developed post-ERCP pancreatitis. Of the 136 patients with successful cannulation, one case experienced post-sphincterotomy bleeding, which was observed oozing from the papillary orifice on emergency gastroscopy. The patient was successfully controlled with endoscopic clips.Conclusion:NKAF is safe and effective for difficult biliary cannulation during ERCP.
5.Effects of different feeding patterns on growth performance,blood physiological and biochemical indices and intestinal flora of beef calves

The aim of this experiment was to study the effects of different feeding modes on growth performance,blood biochemical indexes and intestinal flora of lactating Holstein male calves.Twenty-four newborn Holstein male calves with body mass of(40.00±1.01)kg and similar day old were selected and randomly divided into four groups of six calves each.The subgroups were low-milk group(LM),high-milk group(HM),high-milk milk replacer feeding group(HMR),and low-milk switching to high-milk milk replacer feeding group(CMR).The results showed that:At 45 d,the body mass of calves in the HM group was significantly higher than that of calves in the other groups(P<0.05),and at 60 d,the body mass of calves in the HM group was significantly higher than that of calves in the LM &.CMR groups(P<0.05).At 90 d,the body mass of calves in the LM group was significantly higher than that of calves in the HM group.Throughout the ex-perimental period,the average daily weight gain and average pellet feed intake of calves in the LM group were significantly higher than that of calves in the HM group(P<0.05).The calf globulin level in the HMR group was significantly higher than that in the LM and HM groups(P<0.05);the plasma immunoglobulin A level of calves in the HM group was significantly lower than that of calves in the LM and HMR groups(P<0.05);and the plasma immunoglobulin M level of calves in the HM group was significantly higher than that of calves in the LM and CMR groups(P<0.05),and HMR group was also significantly higher than that of LM group(P<0.05);plasma glutathione peroxidase level of calves in HMR group was significantly higher than that of LM group(P<0.05);plasma malondialdehyde level of calves in LM group was significantly higher than that of calves in HMR and HM groups(P<0.05),and CMR group was also significantly higher than that of HM group(P<0.05).Relative abundance of Thermodesulfovibrio was higher in the HM group(P<0.05),relative abundance of Bacteroidetes in the LM group was significantly higher than that in the HMR and HM groups(P<0.05),relative abundance of Blautia in the HM group(P<0.05),and relative abundance of Corynebacterium in the CMR group was significantly higher than that in the LM and HM groups(P<0.05).In summary,calves in the LM group had better weaning weights and pellet feed intake;calves in the CMR group could compensate for growth by supplemental feeding of milk replacer to obtain more optimal weaning weights and pel-let feed intake;the HMR group proved that milk-free feeding could ensure stable growth of calves;and calves in the HM group had a better pre-lactation growth performance,lower levels of oxida-tive stress,and a healthier fecal flora.
6.Analysis of potential changes in symptom characteristics of breast cancer patients before and after chemotherapy and nursing countermeasures

Objective To explore the time-varying pattern of potential symptoms in breast cancer patient before and after chemotherapy,and further analyze its influencing factors,aims to provide reference for clinical nursing practice.Methods 233 cases of breast cancer in a tertiary A hospital in Ningxia from June to October,2023 were selected as the research subjects.By the general information questionnaire and the Chinese version of Anderson Symptom Assessment Scale,the potential change of symptoms of breast cancer patients undergoing chemotherapy 1-3 days before the first chemotherapy(T1,233 cases)and 7-14 days after the third or fourth chemotherapy(T2,225 cases)was analyzed.Results 225 cases of breast cancer patients completed 2 surveys,and their symptom characteristics before and after chemotherapy can be divided into 2 categories:a high symptom troubled group and a low symptom troubled group.The high-symptom troubled group has strong stability,and the probability of maintaining the original group is 85.20%,while the low-symptom troubled group is more inclined to change to the high-symptom troubled group,and the transition probability is 26.60%.Logistic regression analysis showed that the mass was located on the left side,and the patients who obtained disease knowledge mainly through the Internet were more likely to change from the high symptom troubled group to the low symptom troubled group,while the patients who were employed,with unsatisfactory sleep at night,and never exercised were more likely to change from the low symptom troubled group to the high symptom troubled group.Conclusion The symptoms of breast cancer patients before and after chemotherapy are heterogeneous.Patients who are on the job,have unsatisfied sleep at night and never exercise are more likely to change from the low-symptom troubled group to the high-symptom troubled group,which suggests that medical staff should identify potential high-risk groups at an early stage,and make full use of the predictive function of symptom characteristics to carry out individualized management based on the dynamic monitoring of symptoms according to different types of population characteristics,so as to reduce the burden of symptoms and improve their quality of life.
7.Potential profile analysis of symptoms and quality of life of patients with early breast cancer undergoing postoperative chemotherapy

Objective:To explore the potential categories of symptom characteristics of early breast cancer patients undergoing postoperative chemotherapy, analyze the differences of symptoms among different categories of patients, and compare their quality of life according to different categories.Methods:This was a cross-sectional study. From January to June 2023, 240 patients with early breast cancer who received chemotherapy after surgery in General Hospital of Ningxia Medical University were selected by convenience sampling. They were investigated by general information questionnaire, Memory Symptom Assessment Scale, and Functional Assessment of Caner Therapy-Breast Cancer. Mplus 8.3 was used to carry out potential profile analysis and identify the potential categories of symptom characteristics of early breast cancer patients undergoing postoperative chemotherapy. Binary Logistic regression analysis was used to determine the influencing factors of symptom characteristics. SPSS 26.0 software was used to compare the differences in quality of life scores of patients undergoing postoperative chemotherapy for different categories of early breast cancer.Results:Totally 233 valid questionnaires were ultimately collected, with an age of 50.85 ± 8.09. There were two potential categories of postoperative chemotherapy patients with early breast cancer: low symptom troubled group (159 cases, 68.2%) and high symptom troubled group (74 cases, 31.8%). The quality of life score of the high symptom distress group was (76.84 ± 13.15) points, which was significantly lower than (89.56 ± 11.29) points of the low symptom distress group, and the difference was statistically significant ( t=7.18, P<0.01). Logistic regression analysis showed that patients with early breast cancer who had financial burden after postoperative chemotherapy were more likely to belong to the group with high symptoms [ OR (95% CI) was 3.028 (1.434-6.394), P<0.01]. Postoperative chemotherapy patients with early breast cancer who had satisfactory sleep at night are more likely to belong to the group with low symptoms [ OR(95% CI) was 0.446 (0.217-0.920), P<0.05]. Conclusions:There is population heterogeneity in the symptom characteristics of patients with early breast cancer undergoing postoperative chemotherapy. and economic burden and night sleep are the influencing factors of symptom distress categories of breast cancer patients. It is suggested that medical staff should give individualized and targeted intervention measures centered on patients′ needs according to different types of population characteristics, so as to reduce the symptom burden and improve their quality of life.
8.Meta-analysis of Reduning in the treatment of respiratory syncytial virus pneumonia in children

Objective To evaluate the clinical efficacy,safety and economy of Reduning injection in the treatment of respiratory syncytial virus pneumonia in children.Methods Randomized controlled trials that met the inclusion and exclusion criteria in database were searched.The data extraction and quality evaluation of the literature were performed independently by two people.Meta-analysis was performed.A total of 6 studies involving 514 children were included.Results The total effective rate of Reduning injection combined with western medicine in the treatment of children with respiratory syncytial virus pneumonia was higher than that in western medicine group.The cough,dyspnea,wheezing disappearance time,hospitalization time were shorter than those in western medicine group,and the levels of inflammatory factors and immune cells were better than those in western medicine group.The incidence of adverse events in Reduning injection combined with western medicine group was lower than that in western medicine group,and the hospitalization cost was lower than that in western medicine group.The above results were statistically significant(P<0.05).Conclusion Reduning injection combined with western medicine in the treatment of children with respiratory syncytial virus pneumonia can improve the clinical efficacy,with good safety and less hospitalization costs.
9.Construction and application effect evaluation of supportive care intervention scheme for breast cancer radiotherapy patients based on empowerment theory

Objective:To construct the supportive care intervention scheme for breast cancer radiotherapy patients based on the empowerment theory and discuss its application effect, so as to provide the reference for the clinical nursing intervention.Methods:According to the investigation results of the current situation of supportive care needs of patients with breast cancer radiotherapy and the results of literature review, a supportive care intervention scheme for breast cancer radiotherapy patients based on the empowerment theory was constructed by Delphi expert consultation method.Adopting experimental research, breast cancer radiotherapy patients in General Hospital of Ningxia Medical University were selected by convenient sampling method. They were divided into the control group from April to May 2023 (40 cases) for routine nursing care, and the experimental group from August to September 2023 (40 cases) for the supportive care intervention scheme for breast cancer radiotherapy patients based on the empowerment theory on the basis of routine nursing care according to the admission time. The 34-item Short-Form Supportive Cancer Needs Survey (SCNS-SF34), Strategies Used by People to Promte Health (SUPPH), Cancer Fatigue Scale(CFS) and Function Assessment of Cancer Therapy-Breast (FACT-B) were collected on the day of admission, the end of radiotherapy and 3 months after radiotherapy to evaluate the effects before and after intervention.Results:After 1, 2 rounds of expert correspondence, the positivity of experts were 95.24%(20/21) and 100.00%(20/20), the expert authority coefficient was 0.804 and 0.825, and the Kendall harmony coefficient was 0.129 and 0.138 (both P<0.05). Finally, a supportive care intervention scheme for breast cancer radiotherapy patients based on the enabling theory was formed, which included 5 primary items, 11 secondary items and 32 tertiary items.The application results showed that there were 37 cases in the control group with an age of (48.16 ± 10.21) years, and 38 cases in the experimental group with an age of (44.50 ± 9.81) years. All patients were female. There was no statistically significant difference in the scores of SCNS-SF34, SUPPH, CFS, and FACT-B between the two groups on the day of admission(all P>0.05). Three months after the end of radiotherapy, the score of SCNS-SF34 in the experimental group was (73.24 ± 11.99) points, which was lower than (79.91 ± 14.79) points in the control group, and the difference was statistically significant ( t=4.43, P<0.05). At the end of radiotherapy and 3 months after the end of radiotherapy, the scores of SUPPH in the experimental group were (80.39 ± 8.55) and (92.37 ± 8.14) points, respectively, which were higher than (73.95 ± 9.86) and (86.24 ± 7.10) points in the control group, and the difference was statistically significant ( t=-3.03, -3.47, both P<0.05). At the end of radiotherapy and 3 months after the end of radiotherapy, the scores of CFS in the experimental group were 30.00 (28.75, 33.00) and (25.29 ± 3.26) points, respectively, which were lower than 34.00 (31.50, 35.00) and (27.59 ± 3.43) points in the control group, and the difference was statistically significant ( Z=-3.51, t=2.98, both P<0.05). At the end of radiotherapy and 3 months after the end of radiotherapy, the scores of FACT-B in the experimental group were (85.08 ± 12.74) and (105.53 ± 6.53) points, respectively, which were higher than (79.03 ± 7.90) and (95.73 ± 7.61) points in the control group, and the difference was statistically significant ( t=-2.48, -5.99, both P<0.05). Conclusions:The supportive care intervention scheme for patients with breast cancer radiotherapy based on the empowerment theory established in this study is scientific and feasible, which can reduce patients' needs for supportive care, alleviate cancer-related fatigue, and improve self-management efficacy and quality of life.
