1.Research Advances of Traditional Chinese Medicine Diagnosis and Treatment of Metabolic Dysfunction-Associated Steatotic Liver Disease:Overview and Prospects

The pathogenesis of metabolic dysfunction-associated steatotic liver disease (MASLD) is fundamentally rooted in spleen deficiency and is closely associated with phlegm turbidity, damp-heat and blood stasis. Clinically, liver constraint with spleen deficiency and internal retention of damp turbidity represent the predominant traditional Chinese medicine (TCM) syndrome patterns. Researches have indicated intrinsic connections between the syndrome patterns and biological indicators such as gut microbiota and metabolic profiles. Regarding treatment, classical famous formulas, modern empirical formulas, and newly developed TCM drugs show positive effects in regulating glucose and lipid metabolism, improving insulin resistance, and alleviating metabolic inflammation, exhibiting multi-target mechanisms of action; acupuncture and other external therapies also provide adjunctive value. Nevertheless, current researches still have limitations such as the lack of high-quality clinical evidence and insufficient systematic elucidation of the uncerlying mechanisms. Future efforts should focus on conducting high-quality TCM clinical trials with hard endpoint outcomes such as hepatic histology outcomes, and utilizing modern technologies like multi-omics to elucidate TCM's mechanisms of action, thereby advancing the position of TCM as a first-line therapeutic strategy for MASLD.

This article systematically reviews the current research status as well as diagnosis and treatment strategies of traditional Chinese medicine (TCM) for gastroesophageal reflux disease (GERD). Studies demonstrate that TCM, based on the "disease-syndrome combination" approach, exhibits multi-target advantages in alleviating symptoms of various GERD subtypes, promoting mucosal repair, regulating emotions, and facilitating the reduction of western medication. To address clinical challenges such as symptom overlap and limited therapeutic efficacy, strategies have been proposed including "treating different diseases with the same method" and integrated regulation based on viscera correlation. Future efforts should focus on elucidating the mechanisms of compound prescriptions, promoting TCM drug development under the "three-combination" evaluation framework that integrates TCM theory, human experience and clinical trial evidence, and optimizing integrated traditional and western medicine models to enhance GERD management.

Irritable bowel syndrome (IBS) is a functional bowel disorder characterized primarily by abdominal pain and altered defecation habits. In recent years, traditional Chinese medicine (TCM) has made progress in multiple aspects of IBS research and treatment, including syndrome distribution, development of TCM formulas, clinical efficacy evaluation, external therapies, and psychosocial regulation. However, it still faces challenges such as over-reliance on symptomatic manifestations rather than biomarkers for diagnostic criteria, and the lack of high-quality evidence-based data supporting the efficacy of TCM formulas in treating IBS. This paper proposed that TCM diagnosis and treatment of IBS should adhere to the strategy of integrating the holistic concept with syndrome differentiation and treatment, combining TCM external therapies such as acupuncture, moxibustion and acupoint application), and emphasizing individualized diagnosis and treatment for psychosomatic abnormalities. Future research should integrate multi-omics technologies, artificial intelligence and other methods to deepen the understanding of the pathogenesis of IBS and the mechanisms of TCM formulas, so as to promote the standardization and internationalization of TCM in the diagnosis and treatment of IBS.
4.Current Status and Prospects of Research on Traditional Chinese Medicine Prevention and Treatment for Gastric Precancerous Lesions

Traditional Chinese medicine (TCM), through its multi-target and systematic regulatory effects, has demonstrated unique advantages in the treatment of gastric precancerous lesions (GPL). At present, TCM theoretical research on GPL is mainly reflected in three aspects, the integration of macroscopic syndrome differentiation, the inflammation-carcinoma transformation mechanism, as well as the systematization and scientization of theoretical inheritance from famous TCM practitioners. High-quality evidence-based research findings serve as the foundation for clinical practice guidelines on GPL, and TCM has gained international academic recognition in the field of GPL prevention and treatment. Research on TCM mechanisms has yielded a series of important outcomes in the aspects of signaling pathways, gene expression regulation, cellular epigenetics, histone modification, and intestinal microecology. It is proposed that future research on GPL should focus on four key directions, establishing multi-omics data, exploring targeted intervention strategies on key regulatory nodes, advancing the standardization process of integrated traditional Chinese and western medicine prevention and treatment technologies, and constructing stratified screening and intervention platforms. The in-depth integration of TCM microcosmic mechanism of action with its macroscopic syndrome differentiation and treatment system, coupled with interdisciplinary research, will provide valuable references for the clinical treatment and scientific research of GPL.
5.Inheritance and Current Research Status of Major Spleen-Stomach Theories in Traditional Chinese Medicine

This paper systematically reviews the core concepts and lines of theoretical inheritance of major spleen-stomach theories in traditional Chinese medicine (TCM), including spleen deficiency theory, spleen-stomach damp-heat theory, and liver-spleen disharmony theory. It is found that these theories have all undergone a developmental trajectory characterized by classical foundation, refinement of therapeutic methods, systematization of pathogenesis, and modern innovation. The evolution of spleen-stomach theory has achieved a shift from a singular focus on tonifying the spleen to regulating dynamic middle-jiao (焦) balance, and from localized spleen-stomach regulation to the circular movement of qi involving all five zang organs. In terms of modern disease-syndrome integrative research, spleen deficiency syndrome is shown to be closely associated with impairment of the gastrointestinal mucosal barrier, metabolic disorders, and gene polymorphisms related to Helicobacter pylori-associated gastric diseases. Spleen-stomach damp-heat syndrome is closely linked to hyperactive energy metabolism, inflammatory cytokines, and abnormal expression of aquaporins. Liver-spleen disharmony syndrome is mainly associated with dysregulation of the brain-gut axis and microbiota-related metabolic disorders. It is proposed that future research on spleen-stomach diseases and syndromes should further elucidate their potential multidimensional differential biological characteristics, thereby promoting the modernization of the TCM discipline of spleen-stomach studies.
6.BHB inhibits phagocytic function of CD14+monocytes in dairy cows via ROS-NL-RP3 pathway

The aim of this study was to investigate whether β-hydroxybutyric acid(BHB)inhibits the phagocytic function of CD14+monocytes in dairy cows through the ROS(reactive oxygen spe-cies)-NLRP3(NOD-like receptor thermal protein domain associated protein 3)pathway.CD14+monocytes in the blood of postpartum healthy cows were extracted,and 3 mmol/L BHB was added after transfection of small interfering RNA targeting NLRP3(si-NLRP3).Monocytes were pre-treated with 10 nmol/L NLRP3 inhibitor MCC950(CP-456773)or 10 mmol/L ROS scavenger N-acetyl cysteine(NAC),and then was treated with 3 mmol/L BHB for 24 h.The protein abundance of NLRP3 was detected by Western blot and the phagocytosis of monocytes was determined by flow cytometry and immunofluorescence.The results showed that compared with the si-Control+BHB group,the phagocytic function of monocytes in the si-NLRP3+BHB treatment group was significantly increased,while the protein abundance of NLRP3 was significantly decreased.Com-pared with DMSO+BHB group,the phagocytic function of monocytes in MCC950+BHB group was significantly increased.In addition,compared with DMSO+BHB group,the protein abundance of NLRP3 in monocytes was significantly decreased in MCC950+BHB group.Compared with the PBS+BHB group,the phagocytosis of monocytes was significantly increased after the addition of ROS scavenger NAC,while the protein abundance of NLRP3 was significantly decreased.These results indicated that BHB inhibited the phagocytosis of CD14+monocytes in cows via the ROS-NLRP3 pathway.Therefore,regulating the activation of NLRP3 inflammasome may be an effective method to improve the decrease of monocyte phagocytosis in perinatal dairy cows.
7.Mismatch analysis of individual identity alignments from STRtyper-21G DNA-typing database

Objective To investigate the loci in the STRtyper-21G kit that are prone to tolerance mismatches when compared with the GlobalFilerTM kit and the PowerPlex? 21 kit,and to analyze the underlying causes.Methods A total of 5,870 database comparison reports involving STRtyper-21G profiles and other autosomal STR kits were examined for identity alignment.Samples showing mismatched loci were re-tested using the STRtyper-21G,GlobalFilerTM,and PowerPlex? 21 kits.For loci with mismatches,primers were redesigned and sequencing was performed.Results Eight mismatched samples(8/5 870)were identified,involving the loci D18S51,D8S1179,and D2S1338.Sequencing revealed that the allele dropout at D18S51 was due to a G→A mutation at the 79th base upstream of the core sequence;at D8S1179,a C→A mutation at the 4th base upstream;and at D2S1338,a C→T mutation at the 22nd base downstream.Conclusion All mismatches were attributable to mutations in primer binding regions.These findings provide reference for interpreting mismatch results in the STRtyper-21G database.When mismatches occur at these loci and the profiles are homozygous,exclusion conclusions should be made with caution.
8.Predictive value of serum Hcy combined SDF-1 for early neurological deterioration in patients with acute ischemic stroke after thrombolysis

Objective To explore the predictive value of serum homocysteine(Hcy)and stromal cell-derived factor(SDF)-1 levels in patients with acute ischemic stroke(AIS)for early neurological deterioration(EDN)after thrombolysis.Methods From April 2023 to July 2024,134 AIS patients treated in Jiaxing Second Hospital were selected as subjects.According to the National Institutes of Health stroke scale(NIHSS)evaluation 24 hours after thrombolysis,they were divided into EDN group(n=45)and non EDN group(n=89).Enzyme-linked immunosorbent assay(ELISA)method was applied to detect serum Hcy and SDF-1 levels.Pearson method was applied to analyze correlation.Logistic regression was applied to analyze and test the influencing factors.The receiver operating characteristic curve was applied to analyze the predictive value of indicators for EDN.Results The serum Hcy,SDF-1 levels,admission to puncture time,and baseline NIHSS score in EDN group were higher than those in non EDN group(P<0.05).The levels of serum Hcy and SDF-1 in AIS patients were positively correlated with admission to puncture time and baseline NIHSS score(P<0.05).Hcy and SDF-1 were independent influencing factors for EDN in AIS patients after thrombolysis(P<0.05).The area under the curve of Hcy and SDF-1 combination for predicting EDN was 0.963,with sensitivities of 88.98%,and specificities of 94.43%,respectively.The prediction critical values of Hcy and SDF-1 were 25.31μmol/L and 159.10μg/L.Conclusion Serum Hcy and SDF-1 levels increase in AIS patients with EDN after thrombolysis.The combination of the two has important clinical predictive value for EDN.
9.Status and influencing factors of elevated blood pressure among manufacturing workers in Foshan City

Objective To analyze the elevated blood pressure status and its influencing factors among manufacturing workers in Foshan City. Methods A total of 565 795 manufacturing workers who underwent occupational medical examinations in Foshan City from 2017 to 2023 were included. Data of workers were obtained from the Guangdong Provincial Key Occupational Disease Monitoring and Management Platform. The influencing factors of elevated blood pressure were analyzed using descriptive epidemiological methods. Results A total of 89 526 cases of elevated blood pressure were detected among the 565 795 workers, with a detection rate of 15.82%. From 2017 to 2023, the annual detection rate of elevated blood pressure was 14.11%, 15.00%, 14.69%, 15.94%, 17.00%, 16.90%, and 16.68%, respectively, showing an overall upward trend (P<0.01). The top three industries with the highest detection rates of elevated blood pressure were instrument and meter manufacturing; wood processing and the manufacture of wood, bamboo, rattan, palm, and straw products; as well as agricultural and sideline food processing. Binary logistic regression analysis showed that male workers had a higher risk of elevated blood pressure than female workers (P<0.01). The risk of elevated blood pressure in workers increased with age, years of occupational hazard exposure, and duration of dust exposure (all P<0.05). Workers exposed to noise for more than three years had a higher risk of elevated blood pressure than those without noise exposure (P<0.05). The risk of elevated blood pressure among workers in Nanhai, Shunde, and Gaoming districts was higher than that in Chancheng District (all P<0.01). Workers originating from Northeast and Northwest China had a higher risk of elevated blood pressure than those from North China (all P<0.05). Workers from smaller-scale enterprises had higher risk of elevated blood pressure (all P<0.01). Conclusion Targeted blood pressure health interventions in Foshan City should be strengthened for male manufacturing workers who are older, have longer working years, and originate from Northeast and Northwest China. The development of exemplary health enterprises should be promoted, particularly among micro- and small-sized enterprises and key industries. Enhanced occupational health management is especially needed for positions with high intensity noise exposure and high concentration dust exposure.
10.Impact of health education interventions on the proper use of respiratory protective equipment among dust-exposed workers

Objective To investigate the impact of various health education intervention strategies on the proper use of personal respiratory protective equipment (RPE) among workers exposed to dust. Methods Dust-exposed workers were recruited from 60 selected enterprises in Guangdong Province using cluster random sampling method. They were randomly allocated to the control, low-intensity intervention, and high-intensity intervention groups, with 358, 346, and 371 workers in each group, respectively. Workers in the control group received no designed intervention. Workers in the low-intensity intervention group received traditional plus mobile health education on the proper use of RPE. Workers in the high-intensity intervention group received all components of the low-intensity intervention, supplemented with peer education. The intervention lasted for six months. RPE usage was compared among the three groups of workers before and after the intervention. Results Workers in the control, low-intensity intervention, and high-intensity intervention groups showed higher rates of both RPE wearing and correct RPE wearing after the intervention than before it within their respective groups (RPE wearing rate: 94.1% vs 99.2%, 95.7% vs 100.0%, 94.6% vs 100.0%, all P<0.01; correct RPE wearing rate: 66.8% vs 91.1%, 67.3% vs 95.7%, 66.6% vs 96.5%, all P<0.01). Post-intervention correct RPE wearing rates were highest in the high-intensity intervention group, followed by the low-intensity intervention group, and the control group, with the percentage of 96.50%, 95.66% and 91.06%, respectively (P<0.01). Binary logistic regression analysis result showed that different intervention strategies affected the correct use of personal RPE among dust-exposed workers after adjusting for gender, age, and other confounding factors (P<0.05). Compared with the control group, the rates of correct RPE use increased in the low-intensity intervention group and the high-intensity intervention group (odd ratio was 2.14 and 3.01; 95% confidence interval was 1.12 - 4.10 and 1.53 - 5.91, respectively). Conclusion The implementation of traditional plus mobile health education interventions on the proper use of RPE can promote correct RPE utilization among dust-exposed workers, and integrating peer education further enhances the intervention effectiveness.
