1.Understanding pain heterogeneity in osteoarthritis patients: a narrative review.

The primary clinical manifestation of osteoarthritis (OA) is pain, yet considerable variability exists in the pain experience among OA patients. This narrative review aims to explore the mechanisms driving OA pain heterogeneity to inform the development of targeted interventions that improve treatment efficacy and patient outcomes. A comprehensive literature search was conducted across multiple databases (PubMed, Scopus, and Google Scholar) for papers published between January 1, 2020, and December 31, 2024. Inclusion criteria focused on studies addressing pain mechanisms and therapeutic interventions in OA. This review identifies key mechanisms of OA pain, including joint alterations, angiogenesis, nervous system involvement, peripheral and central sensitization, and psychosocial factors. It highlights the underlying distinct mechanisms in OA pain, which contribute to the variability in individuals' responses to treatment. It was suggested that interactions between neuroimmune and neurovascular systems are key contributors to chronic pain in OA. This narrative review emphasizes the complexity of OA pain, highlighting the importance of thoroughly understanding the underlying mechanisms for developing personalized and effective pain management strategies. Additional research is required to refine treatment approaches and explore long-term effects.

2.Panax notoginseng saponins alleviated oxidative stress damage of renal tubular epithelial cells induced by hydrogen peroxide

Objective:To observe the protective effect of Panax notoginseng total saponins(PNS)on oxidative stress damage induced by hydrogen peroxide(H 2O 2)in human renal tubular epithelial cells(HK-2)and its mechanism. Methods:HK-2 was cultured and divided into experimental groups: Normal group(without any intervention factors), H 2O 2 group(400 μmol/L H 2O 2 was added into the cell culture system for 6 h)and PNS group(which was divided into three groups: PNS 25 μg/ml, 50 μg/ml and 100 μg/ml were added into the cell culture system for 4 h, and then 400 μmol/l H 2O 2 was added into the cell culture system for 6 h), and the apoptosis of HK-2 induced by H 2O 2 was observed. Results:Compared with the normal group, the cell survival rate of H 2O 2 group decreased to(45.67±2.52)% and the cell apoptosis rate increased to(19.23±1.63)%(all P<0.01), while the cell survival rate of PNS group was(55.12±2.33)%, (65.37±4.72)% and(83.46±1.52)%, and the apoptosis rate was reduced to(15.53±0.70)%, (12.53±1.30)% and(7.17±0.35)%, respectively.The levels of reactive oxygen species(ROS)in HK-2 cells in 50 μg/ml and 100 μg/ml PNS groups were(845.7±17.79)and(353.3±26.1), respectively.With the increase of PNS dose, super oxide dismutase(SOD)and glutathione peroxidase(GSH-Px)activities increased and malonic dialdehyde(MDA)production decreased in the 3 PNS groups, compared with the H 2O 2 group, the differences were statistically significant(all P<0.01). Conclusions:PNS can protect HK-2 cells against oxidative damage by enhancing the activity of intracellular antioxidant enzymes, the effect is enhanced with the increase of the dose.

In view of the problem of slow development of intensive care medicine in Xizang, the research team made full use of the national partner assistance to Xizang, gathered resources across all cities in Xizang, and formed a national academic platform for critical care medicine in plateau areas. Adhering to the academic orientation with hemodynamics as the main topic, critical care ultrasound as the bedside dynamic monitoring and evaluation method, and blood flow-oxygen flow resuscitation as the core connotation, we have achieved the goals of improving the critical care talent echelon throughout Xizang, driving the overall progress of intensive care medicine in Xizang, making a figure in China, and focusing on training of top-notch talents.
4.Clinical phenotypic characteristics and follow-up of 26 children with progressive familial intrahepatic cholestasis

Objective:To investigate the clinical phenotype and prognosis among different genotypes of progressive familial intrahepatic cholestasis(PFIC) by cases analysis.Methods:The PFIC cases diagnosed at Beijing Children′s Hospital from 2015 to 2022 were collected, and the clinical phenotypic characteristics, treatment and prognosis were compared and analyzed.Results:A total of 628 cases of cholestatic liver disease were diagnosed, and 26 cases of PFIC were found, accounting for 4.1%.The number of PFIC 2 were the most, 14(53.8%)cases; three(11.5%) cases were PFIC 1; five(19.2%)cases were PFIC 3; while two(7.7%) cases were PFIC 4 and PFIC 6, respectively, and there was no case of PFIC 5.Type 1, 2, 4, and 6 had early onset ages(2 days to 21 months), while type 3 had a wide range of onset ages(8 to 145 months). The symptoms included jaundice(96.2%), pruritus(42.3%), and mucosal bleeding(15.4%). All three cases of type 1 had extrahepatic manifestations of diarrhea and malnutrition.Two cases of type 3 were found to have end-stage liver disease.Cases of PFIC 3 had increased serum γ-glutamyltransferase(97.2-439.5 U/L), while those of other types were normal.The bile acids were all increased(10.1-599.6 μmol/L). Abdominal ultrasound mainly showed liver enlargement(80.8%)and enhanced echogenicity of liver parenchyma(73.1%), enlargement of the spleen(61.5%). Ultrasound liver elastography ranged from 6.3 kPa to 23.1 kPa, there were 21(80.8%) cases ≥9 kPa.Among 26 cases, one case was lost to follow-up, and 11 cases were effective by oral medication alone.Fourteen children were still suffering from relapse or progress after drug treatment: four cases received liver transplantation (three cases had a good prognosis and one case died), two cases received biliary drainage, six cases were still taking drugs orally, and two cases died without active intervention in disease progress.Conclusion:Type 2 is the most common type in PFIC.The onset of most cases is in infancy.Jaundice, pruritus and hepatosplenomegaly are common clinical manifestations, and extrahepatic manifestations can be seen in type 1 cases.Type 3 cases can start with end-stage liver disease.Bile acid of all cases are increased.Except for type 3, the serum γ-glutamyltransferase of cases are normal.Oral medication has certain effects on some cases, but more than half progress, and some need biliary diversion or liver transplantation.
5.Correlation between the waiting time for radiotherapy and prognosis after induction chemotherapy for locally intermediate and advanced nasopharyngeal carcinoma

Objective:To investigate the correlation between the waiting time for radiotherapy after induction chemotherapy and the prognosis of locally intermediate and advanced nasopharyngeal carcinoma, as well as its optimal time.Methods:Retrospective analysis of 101 patients with locally intermediate and advanced nasopharyngeal carcinoma admitted to the Fifth Affiliated Hospital of Sun Yat-sen University from 2017 to 2020 was performed. All patients received at least 2 courses of induction chemotherapy followed by radical radiotherapy. The waiting time for radiotherapy was defined as the time from the end of induction chemotherapy to the start of the first radiotherapy. The relationship between waiting time for radiotherapy and other factors (age, gender and stage, etc.) with progression-free survival (PFS), local recurrence-free survival (LRFS) and distant metastasis-free survival (DMFS) was analyzed through Cox model. The median waiting time for radiotherapy with 3 weeks was used as the boundary, and all patients were divided into ≤3 weeks and>3 weeks groups. The PFS, LRFS and DMFS between two groups were compared using Kaplan-Meier survival analysis. P<0.05 was considered as statistical significance. Results:Cox-regression analysis showed that the waiting time was correlated with PFS, LRFS, and DMFS (all P<0.05). Kaplan-Meier survival analysis suggested that the PFS, LRFS and DMFS in the ≤3 weeks group were significantly better than those in the >3 weeks group (all P<0.05). Under the premises of the T 3 stage, N 2 stage and the increased EB virus DNA replication levels before treatment, the PFS, LRFS and DMFS in the ≤3 weeks group were significantly better than those in the >3 weeks group (all P<0.05). Conclusions:The waiting time for radiotherapy is one of the factors affecting clinical prognosis of locally intermediate and advanced nasopharyngeal carcinoma. The earlier the time, the better the prognosis. Radiotherapy should be delivered within 3 weeks.
6.Effects of WTAPP1 on the proliferation,invasion,migration and Wnt/β-catenin signaling pathway of nephroblastoma cells

Objective:To investigate the effects of long non-coding RNA Wilms tumor 1 associated protein pseudogene 1(WTAPP1)on the proliferation,invasion,migration and Wnt/β-catenin signaling pathway of nephroblastoma cells. Methods:Real-time fluorescence quantitative PCR was used to detect the relative expression level of WTAPP1 in 48 cases of nephroblastoma tumor tissues and their matched adjacent tissues,human nephroblastoma cells(SK-NEP-1)and normal renal epithelial cells(PCS-400-01 0,PCS-400-011 and PCS-400-01 2).The clinicopathological characteristics and prognosis of nephroblastoma patients with high or low WTAPP1 expression of were analyzed and compared.SK-NEP-1 cells were infected with lentivirus carrying the full-length WTAPP]gene(WTAPP1 overexpression)or shRNA targeting WTAPP1(shWTAPP1).Then,colony formation assay and CCK-8 assay were used to assess the proliferation of SK-NEP-1 cells,wound healing assay and Transwell assay were used to evaluate the migration and invasion activity of SK-NEP-1 cells respectively,and Western blotting was used to examine the relative expression levels of Wnt3a,β-catenin,C-myc and Survivin proteins. Results:The relative expression level of WTAPP1 in cancer tissues was higher than that in adjacent tissues(P<0.05).The relative expression level of WTAPP1 in SK-NEP-1 cells was higher than that in normal renal epithelial cells(PCS-400-01 0,PCS-400-01 1 and PCS-400-01 2;P<0.05).WTAPP1 expression was associated with the clinical stages of nephroblastoma(P<0.05)but not with the age,gender,tumor diameter or lymph node metastasis of nephroblastoma patients(all P>0.05).The 5-year survival rate of WTAPP1-high patients was significantly lower than that of WTAPP1-low patients(P<0.05).The proliferation,migration and invasion activities of SK-NEP-1 cells were significantly increased after WTAPP1 overexpression(all P<0.05)while decreased after WTAPP1 silencing(all P<0.05).The relative expression levels of Wnt3a,β-catenin,C-myc and Survivin proteins were significantly upregulated after WTAPP1 overexpression(all P<0.05)while decreased after WTAPP1 silencing(all P<0.05). Conclusion:WTAPP1 is correlated with the clinical stages and prognosis of nephroblastoma patients and may promote the proliferation,invasion and migration of tumor cells by activating Wnt/β-catenin signaling pathway.
7.Predictive Value of Depth of Invasion of Tongue Squamous Cell Carcinoma for Cervical Lymph Node Metastasis and Prognosis

Objective To investigate the predictive value of depth of invasion (DOI) of tongue squamous cell carcinoma (TSCC) for cervical lymph node metastasis and prognosis. Methods We retrospectively analyzed the clinical and pathological data of 73 patients with T1/2 TSCC. ROC curve was used to determine the optimal cut-off value of DOI for predicting cervical lymph node metastasis, and logistic regression analysis was performed to analyze the related factors affecting cervical lymph node metastasis of TSCC. Kaplan-Meier method and Cox regression analysis were used for survival analysis. Results Among 73 patients, 18 patients were with lymph node metastasis and 55 patients were without lymph node metastasis. Cow′s milk protein allergy (CMPA) is one of the most common presentations of food allergy seen in early childhood.It is an abnormal immune response caused by cow′s milk protein.CMPA can be clinically subdivided into either immediate-onset IgE mediated or delayed onset non-IgE mediated, or both.At present, concerns regarding the early and timely diagnosis of CMPA have been high-lighted over the years and there are many expert consensus on CMPA in China, but these consensus did not distinguish IgE mediated or non-IgE mediated CMPA.In view of the obvious clinical differences between the two type of CMPA and non-IgE mediated CMPA is more common in infancy, experts focus on pediatric gastroenterology, allergy/immunology, dermatology, nutrition and child healthcare convened by the Allergy Prevention and Control Professional Committee of Chinese Preventive Medicine Association present this guideline to help practitioners in primary care settings to early recognize and make suitable management of non-IgE mediated CMPA in China.The guideline incorporates the cutting-edge international guidance and the actual situation of Chinese children describing in detail the types, clinical features, diagnosis and nutritional intervention of non-IgE mediated CMPA.There are 42 recommendations in 7 categories in total referring to the common questions related to non-IgE mediated CMPA.
9.Genome mining of fungal globin-like enzymes for catalyzing the synthesis of linear terpenes.

Genome mining for the search and discovery of two new globin-like enzymes, TriB fromFusarium poae and TutaA from Schizophyllum commne, are involved in the synthesis of two linear terpenes tricinonoic acid (1) and 2-butenedioic acid (3). Both in vivo heterologous biosynthesis and in vitro biochemical assays showed that these two enzymes catalyzed the C-C double bond cleavage of a cyclic sesquiterpene precursor (-)-germacrene D (7) and a linear diterpene backbone schizostain (2), respectively. Our work presents an unusual formation mechanism of linear terpenes from fungi and expands the functional skills of globin-like enzymes in the synthesis of terpene compounds.

10.Visual analysis of research progress on nurses′ turnover intentions in China

Objective:To summarize the research progress of domestic nurses′ turnover intentions in the past ten years, and to provide references for related research on improving nurses′ turnover intentions.Methods:In May 2021, the documents related to nurses′ resignation intentions were retrieved in China National Knowledge Infrastructure (CNKI), Wanfang and VIP database and Citespace5.7.R5 software was used to perform keyword visualization analysis to form keyword clustering graphs and tables, emergent words, and corresponding time zones figure, the current research status and hot trends of nurses′ turnover intentions were analyzed.Results:A total of 2 029 valid documents were retrieved, the number of publications reached the highest peak in 2019. Among them, "nurse", "willingness to leave", "job satisfaction", "turnover intention" and "influencing factors" were the top 5 hot keywords and 23 emergent words were generated; research directions mainly included influencing factors and research objects and workplace related topics.Conclusions:Managers should actively play a leadership role to reduce nurses′ turnover intention through various effective intervention studies.
