1.Antimicrobial resistance and genomic characteristics of Salmonella enterica serovar Derby isolates from clinical patients and food sources in Hangzhou

Objective:To analyze the antimicrobial resistance and genomic characteristics of Salmonella enterica serovar Derby strains isolated from human and food sources in Hangzhou. Methods:A total of 60 Salmonella enterica serovar Derby strains isolated in Hangzhou during the period from 2015 to 2020 were subjected to antimicrobial susceptibility testing, pulsed field gel electrophoresis (PFGE) typing and whole-genome sequencing. Multilocus sequence typing (MLST), core genome multilocus sequence typing (cgMLST) and the identification of antimicrobial resistance genes were performed using the sequencing data. Phylogenetic tree based on the single nucleotide polymorphism (SNP) sites in the 60 genomes from Hangzhou and 379 genomes from public databases was constructed. Results:No significant difference was observed in the drug resistance rates between the clinical strains and food strains in Hangzhou. The multidrug resistance (MDR) rate was 76.7% (46/60). All of the 60 Salmonella Derby strains were positive for the antimicrobial resistance genes aac(6′)- Iaa and fosA7. The 60 strains were subtyped into 46 molecular types by PFGE and 53 molecular types by cgMLST(HC2). Except for one strain belonging to ST3220, the other Salmonella Derby strains were ST40. The phylogenetic analysis showed that some strains isolated in Hangzhou were close to the strains in Southeast Asia, suggesting the possibility of cross-border transmission of ST40 strains, with the main food sources being pork and fish; other strains were close to those circulating in Beijing, Guangzhou, Hubei, Chongqing and other provinces, suggesting the possibility of cross-province transmission of the strains, with the main food sources being pork, beef and chicken. Conclusions:The epidemic of Salmonella Derby in Hangzhou was mainly caused by the spread of ST40 strains and MDR was common. Clinical infections might be closely related to the consumption of pork, beef, chicken and fish. There was the possibility of cross-border transmission of Salmonella Derby between Hangzhou and Southeast Asia and cross-province transmission in China.
2.Effects of high-frequency transcranial magnetic stimulation on cellular senescence in Parkinson′s disease

Objective:To investigate any anti-aging effect of repeated transcranial magnetic stimulation (rTMS) and explore the relationship between the effect and relief of clinical symptoms in patients with Parkinson′s disease (PD).Methods:A total of 108 PD patients were randomly divided into an rTMS group and a control group, each of 54, while another 54 healthy counterparts were selected to form a normal group. In addition to anti-PD drug therapy, the rTMS group was given daily rTMS treatment, 5 days a week for 4 weeks, while the control group received sham rTMS treatment, with no treatment of the normal group. Before the treatment and after 4 weeks of treatment as well as and 1 month after the ending of the treatment, the subjects′ clinical exercise symptoms were evaluated using the Unified Parkinson′s Disease Rating Scale (UPDRS), a timed exercise test and the 10m re-entry exercise test. Non-exercise symptoms were assessed using the Hamilton Depression Scale (HAMD), the Hamilton Anxiety Scale (HAMA) and the Mini-mental State Examination (MMSE). Fasting venous blood samples were analyzed to quantify the serum levels of tumor necrosis factor (TNF), interleukin-6 (IL-6), interleukin-1β (IL-1β) and matrix metalloproteinase-3 (MMP-3).Results:Four weeks and 1 month after the treatment, the average UPDRS scores, exercise test times and 10m re-entry exercise test results of the rTMS group were significantly better than those before treatment and significantly better than those of the control group at the same time point. The rTMS group′s average HAMA, HAMD and MMSE scores, as well as its average P300 latency and amplitude were also significantly better than those of the control group at the same time point and significantly better than those before treatment. After 4 weeks, the average MMP-3 content in the rTMS group was significantly lower than the control group′s average, and after a month the average levels of TNF, IL-6, IL-1β and MMP-3 of the rTMS group were all significantly different from those before treatment and those of the control group. The TNF, IL-6, IL-1β and MMP-3 levels were all positively correlated with the average UPDRS total score.Conclusion:High-frequency rTMS therapy can change the phenotypes related to cell senescence, and thus has good therapeutic effect on motor and non-motor symptoms of PD.
3.Role of angiopoietin-like proteins in the development of nonalcoholic fatty liver disease

Angiopoietin-like proteins (ANGPTLs) are a family of secretory glycoproteins recently found to be constitutionally homologous with angiogenin and play a role in the regulation of angiogenesis. In recent years, more and more studies have shown that ANGPTLs play an important role in glucose and lipid metabolism, inflammation, and tumor. As we all know, nonalcoholic fatty liver disease and its disease spectrum are closely associated with metabolism, inflammation, and tumor. This article reviews the role of ANGPTLs in various diseases associated with nonalcoholic fatty liver disease, in order to provide new ideas for the prevention and treatment of nonalcoholic fatty liver disease in clinical practice.
4.Analysis of genomic characteristics and virulence factors of emetic-type Bacillus cereus strains isola-ted in Hangzhou

Objective To investigate the genomic characteristics and virulence factors of emetic-type Bacillus cereus strains isolated from food in Hangzhou for better understanding their pathogenic potential. Methods Real-time PCR was performed to detect the ces gene cluster ( cereulide) in 132 Bacillus cereus strains isolated from food from 2015 to 2017. Genomes of cereulide-positive strains were sequenced using Illumina MiSeq sequencing platform. Genome annotation, virulence factor detection, comparative and evolu-tionary analysis were performed after the sequences of genomes were assembled. Results Twelve strains (9. 09%) carried the ces gene. Their genome sizes ranged from 5. 35 to 5. 75 Mb and GC contents from 35. 25 to 35. 43 mol%. All of them harbored the full cereulide biosynthesis gene cluster, nonhemolytic ente-rotoxin ( NHE)-encoding gene cluster ( nheA, nheB and nheC) and hemolysinⅢ( hlyⅢ) . The average nu-cleotide identity ( ANI ) between the 12 isolates and the reference strain NC7401 ( Accession number:AP007209) was over 99. 35%. Phylogenetic analysis demonstrated these strains were clustered into the same branch with local clinical isolates and the emetic-type Bacillus cereus strains of NC7401 and AH187. Con-clusions The genomic sequences of the emetic-type Bacillus cereus strains isolated from food in Hangzhou area were highly similar to that of the reference strain NC7401. Results of the genomic analysis suggested that these isolates carried many virulence factors that were related to pathogenicity.
5.Construction of correlation index system on hospital inpatient suicide and public environment

ObjectiveTo determine the weights of the primary and secondary index in the correlation index system on the relationship between hospital inpatients suicide and the public environment and to construct the system. MethodsFrom January to March in 2018, 40 experts were finally selected as the consultants according to the admission criteria. The Delphi method was used to complete two rounds of effective inquiry to 40 experts, investigate the index weight, construct the hierarchical structure model, establish the judgment matrix, and to calculate the index weight at all levels and to test the consistency rate of the questionnaire. ResultsThe recovery rates of the two rounds of expert consultation were 100 % and 100%, respectively, and the authority coefficient of the experts was 0.85-0.87. Finally, by consistency testing, the public environmental index system of hospital inpatient suicide related to public environment included 2 primary indexes (i.e. external environmental factors, internal environmental related factors), 12 secondary indicators and 18 tertiary indicators. The weight coefficient of the primary index was 0.540 6-0.459 4, the weight of the secondary index was 0.024 4-0.203 1, the weight of the tertiary index was 0.008 4-0.058 3, and the CR value of each judgment matrix was 0-0.011 0. Conclusions40 experts have a high degree of authority and good coordination of opinions. Through Delphi method, the public environment correlation index system of hospital inpatient suicide is established. The index system is objective and scientific, and can provide objective quantifiable reference for the hospital in the effective prevention of patients' suicide and ensure the patient safety.
6. Occupational acute mixed gas poisoning due to improper disposal of hazardous waste

Objective:
To investigate an occupational hazardous gas poisoning incident caused by gas leakage in the process of hazardous waste treatment.
Methods:
An investigation was conducted on a case of occupational acute hazardous gas poisoning caused by waste treatment gas leakage in Shandong province in December 2017. Meanwhile, the clinical data of 5 cases of poisoning patients were analyzed, and the accident related poison test report and other relevant data were analyzed.
Results:
The incident was caused by the toxic waste did not do labeling work, the workers' protection measures were not in place, the illegal operation and the blind rescue, resulting in a total of 5 people died on the spot, 12 people were hospitalized with poisoning. Among them, 5 patients admitted to our hospital showed varying degrees of damage to the nervous system and respiratory system. After active treatment, they all got better and were discharged.
Conclusion
The poisoning is mainly caused by hydrogen sulfide dichloromethane hydrogen cyanide gas leakage serious production liability accident, clinical main performance for the nervous system circulatory system respiratory system and other system damage.
7. Bromadione poisoning with multiple hemorrhage after trauma as the first symptom

Objective:
To report a case of rat poison with multiple hemorrhage after trauma.
Methods:
The clinical data of a case of rodenticide poisoning with hemorrhage as the first symptom admitted to a third-class a hospital in July 2018 were analyzed and summarized.
Results:
This patient is a rodent drug poisoning patient with hemorrhage as the first symptom.The disease was diagnosed as bromohamelin and bromadiolone poisoning through the analysis of poison detection because the rodent drug was taken in the market and the history of taking poison was concealed. The patient was given active comprehensive treatment of vitamin K1 and other drugs for clinical cure.
Conclusion
For patients with clinically unexplained hemorrhage, the possibility of rodenticide poisoning should be considered and the toxicant detection should be improved actively.
8.Construction and implementation of gastroenterology nursing quality monitoring index

Objective To construct and implement the gastroenterology nursing quality monitoring index, and to improve the quality of nursing in the specialty. Methods A research team was formed to analyze the major problems in nursing quality according to the top 5 diseases in gastroenterology department. The research process was based on the characteristics of gastroenterology, focusing on nursing operating skills, diseases and complications and patient safety, in the methods of literature review and brain storm. Finally, a total of 6 specialist nursing quality monitoring indicators were established: the implementation rate of digestive endoscopy preoperative preparation, the incidence of hematemesis asphyxia, the plugging rate of nasal biliary (pancreatic) drainage; the incidence of perianal skin injury in diarrhea patients; the incidence of hypoglycemia in fasting patients and the satisfaction of pain management. The key content and scientific measurement of the indicators were clarified by retrospectively analyzing and comparing 6 monitoring indicators between 2014 and 2015, 2015 and 2016 respectively. The statistical methed used was χ2test. Results In 2015, all the indicators improved except the incidence of hematemesis asphyxia than those in 2014, and the differences were statistically significant (χ2=34.710,9.223,7.834,5.547,32.899;P<0.05). In addition, all the 6 indicators improved than those in 2015, and the differences were statistically significant (χ2=102.334,5.496,4.257, 3.953,39.191,192.910;P< 0.05). Conclusions The establishment of gastroenterology nursing quality monitoring indicators, the development of targeted care measures, and the continuous improvement in the quality of specialist nursing can improve the quality of nursing management.

10. An incident of toadstool poisoning

Objective:
To investigate a poisonous mushroom poisoning incident and analyze its clinical data.
Methods:
Investigate a poisonous mushroom poisoning incident in a place in Shandong in July 2018, at the same time, the clinical data of 2 cases of mushroom poisoning were analyzed and summarized.
Results:
The incident was caused by a poisoning incident caused by residents eating poisonous mushrooms. The poisonous mushroom in this incident was identified as a scaled white goose cream. Two patients with poisoning developed gastrointestinal symptoms such as nausea and vomiting, abdominal pain and diarrhea, and liver damage occurred later. After active rescue and treatment, one patient was discharged from hospital, and the other patient developed acute pulmonary embolism during the treatment. He was discharged after interventional thrombolysis and follow-up treatment.
Conclusion
After investigation, the incident was caused by the ingestion of poisonous mushrooms mainly based on the scalloped white goose cream. After active treatment, they were cured and discharged.
