Enamel demineralization, the formation of white spot lesions, is a common issue in clinical orthodontic treatment. The appearance of white spot lesions not only affects the texture and health of dental hard tissues but also impacts the health and aesthetics of teeth after orthodontic treatment. The prevention, diagnosis, and treatment of white spot lesions that occur throughout the orthodontic treatment process involve multiple dental specialties. This expert consensus will focus on providing guiding opinions on the management and prevention of white spot lesions during orthodontic treatment, advocating for proactive prevention, early detection, timely treatment, scientific follow-up, and multidisciplinary management of white spot lesions throughout the orthodontic process, thereby maintaining the dental health of patients during orthodontic treatment.

The mammalian target of rapamycin (mTOR) pathway is abnormally activated in lung cancer. However, the anti-lung cancer effect of mTOR inhibitors as monotherapy is modest. Here, we identified that ginsenoside Rh2, an active component of Panax ginseng C. A. Mey., enhanced the anti-cancer effect of the mTOR inhibitor everolimus both in vitro and in vivo. Moreover, ginsenoside Rh2 alleviated the hepatic fat accumulation caused by everolimus in xenograft nude mice models. The combination of everolimus and ginsenoside Rh2 (labeled Eve-Rh2) induced caspase-independent cell death and cytoplasmic vacuolation in lung cancer cells, indicating that Eve-Rh2 prevented tumor progression by triggering paraptosis. Eve-Rh2 up-regulated the expression of c-MYC in cancer cells as well as tumor tissues. The increased c-MYC mediated the accumulation of tribbles homolog 3 (TRIB3)/P62+ aggresomes and consequently triggered paraptosis, bypassing the classical c-MYC/MAX pathway. Our study offers a potential effective and safe strategy for the treatment of lung cancer. Moreover, we have identified a new mechanism of TRIB3/P62+ aggresomes-triggered paraptosis and revealed a unique function of c-MYC.

5.Establishment and Practice of Management of Weighting Recheck for Finished Intravenous Solution in PIVAS Based on Weight Analysis Method

OBJECTIVE:To management recheck and sorting weight for finished intravenous solutions in PIVAS ,to provide reference for reducing dispensing error and improving the safety and quality of intravenous infusion therapy. METHODS :The weight analysis method was used to determine the weight of main drug and solvent in the finished intravenous solutions and infusion bottle . The weight maintenance information was added in PIVAS information management system ,and the marked weight of finished intravenous solutions was calculated for the verification of finished intravenous solutions. Average daily check quantity of finished product ,checking time ,average checking time of finished products per bag ,detection rate of dispensing error ,external error and timeliness of finished infusion batch were compared before (Mar.-May,2019,n=83 006)and after (Jun.-Aug.,2019, n=83 173)management. The effects of weighting recheck management were evaluated. RESULTS :Compared with before the implementation of weighting recheck management ,there were no significant differences in the average daily check quantity of finished products ,the detection rate of dispensing errors caused by wrong labeling of liquid ,or the times of delayed drug delivery batches after the implementation of weighting recheck management (P>0.05). The checking time of finished products ,average checking time of finished products per bag ,the number of bags added or subtracted error ,detection rate of dose dispensing error , total error detection rate prolonged or increased significantly (P<0.05),and the number of external error was decreased significantly(P<0.05). CONCLUSIONS :The weighting recheck management improves the accuracy and safety of PIVAS preparation,effectively improves error detection rate ,reduces the occurrence of external error ,but prolongs the time of checking , which are urgent to be solved by information and automation means.

7.Application of Leichester assessment package in consultation skill training for general practitioners at outpatient clinic

Thirty-five general practitioners ( GPs) from Huangdu Subdistrict Community Health Center of Anting Township were trained for clinical skills and communication skills in consultation at outpatient department by using Leichester assessment package (LAP) from January 2016 to December 2016. The clinical skills and communication skills were assessed before and after 1 year of training.The results showed that compared to those before training the clinical skill and communication skill were significantly increased after the training.The scores of medical history taking , clinical reasoning and health education , as well as the scores of building doctor-patient relationship, collecting information and providing feedback , understanding complaints from patient perspective , sharing information and obtaining informed consents after the training were all higher than those before training (all P<0.05).However, there were no significant differences in scores of history writing and interview time before and after training .The study showed that Leichester assessment package can effectively improve the training of clinical skills and communication skills for GPs, and it can be applied for future training .
8.Analysis of Epidermal Growth Factor Receptor Gene Mutation in 274 Cases of Non-Small Cell Lung Cancer

OBJECTIVE:To study the gene mutation status of epidermal growth factor receptor(EGFR)in non-small cell lung cancer(NSCLC)patients and its relationship with clinical indexes,and to provide reference for individualized administration of EGFR-TKI in NSCLC patients. METHODS:Totally of 274 NSCLC patients from the northern of Jiangsu area were selected from our hospital during Jan. 2015-Dec. 2017. Mutation status of EGFR gene in lung tissue was determined by amplification refractory mutation system (ARMS)-TaqMan PCR assay. The relationship of EGFR gene mutation with clinical indexes as gender,age, smoking status, staging, tumor differentiation and pathological type were analyzed retrospectively. Compared with related literatures,the regional differences of EGFR gene mutation were analyzed. RESULTS:Among 274 NSCLC patients,112 patients suffered from EGFR gene mutation with total mutation rate of 40.88%. There were 50,57,3,2 cases of exon 19,exon 21,exon 20 and exon 19+21 mutation,and the types of EGFR gene mutation were delE746-A750,L858R and insH773-V774H,etc. The mutation rates of EGFR gene exon 19,exon 21 in non-smoking,early,well-differentiated and adenocarcinoma patients were 52.50%,47.24%,46.36% and 45.00%,which were significantly higher than smoking (28.57%),advanced (27.03%), poor-differentiated(31.71%)and squamous cell carcinoma(27.66%)patients,with statistical significance(P<0.05). There was no statistical significance in mutation rates of EGFR gene exon 19 and exon 21 between male and female,≥65 year-old and <65 year-old patients (P>0.05). EGFR mutation rate of NSCLC subjects from the northern of Jiangsu area was significantly higher than Shanghai area(P<0.05);there was no statistical significance compared with Yunnan area(P>0.05)but mutation types were different. CONCLUSIONS:There is the highest EGFR gene mutation rate in its exon 21,lesser in exon 19,rare in exon 20 and exon 19+21 among NSCLC patients from the Northern of Jiangsu area. There are obvious regional differences. The mutation rate of EGFR gene mutation exon 19 and exon 21 are associated with smoking status,staging,tumor differentiation and pathological type of NSCLC patients. The non-smoking, early stage, well-differentiated and adenocarcinoma patients are more likely to benefit from EGFR-TKI targeted therapy.
9.Using standardized patients in training of assistant general practitioners

Thirty assistant general practitioners (GPs) of Jiading District Shanghai were trained for clinical skills and communication skills by using standardized patients (SP) in outpatient department from January to April 2014.The scores of clinical skills and communication skills were evaluated and compared before and two months after training.The results showed that the score of clinical skills [(72 ± 5) vs (56 ± 7), t=7.871, P<0.05] and communication skills [(71 ±4) vs (45 ±5), t =9.573, P<0.05] were significantly improved after training.After training, the scores of medical history taking [(36 ±5) vs (28 ± 6), t =4.352], clinical thinking [(17 ± 2) vs (14 ± 3), t =3.974], health education [(3 ± 1) vs (2 ± 1), t =3.090] and medical record v,riting [(13 ± 1) vs (8 ±2), t =5.317] were significantly higher than those before training (all P < 0.05).But there was no difference in reception time management [(4 ± 0) vs (4 ± 0), t =1.025] before and after training (P > 0.05).After training, the scores and the proportions of the trainees qualified for building physician-patient relationship [(20 ± 3) vs (15 ± 4), t =4.056],collecting information and providing feedback [(22 ±4) vs (13 ±3), t =4.682], understanding complaints from patients' perspective [(5 ± 2) vs (3 ± 2), t =3.107], sharing information and obtaining informed consent [(19 ± 4) vs (11 ± 5), t =4.322] and closing consultation [(6 ± 2) vs (3 ± 1), t =2.987] were higher than before training (all P < 0.05).The results indicate that training by using SP can effectively improve the clinical skills and communication skills of assistant GPs.
10.A new sesquiterpenoid from fungus Colletotrichum sp. and its cytotoxicity.

A novel sesquiterpenoid (1) and three known compounds identified as isoaltenuene (2), altenuene (3), and alternariol 4, 10-O-dimethyl ether (4), were isolated in our investigation of the cytotoxic constituents from solid cultures of the endophytic fungus Colletotrichum sp. The structures of these compounds were elucidated through spectroscopic data analysis. All compounds exhibited cytotoxic activity against lung cancer cell line A549, breast cancer cell line MDA-MB-231 and pancreatic cancer cell line PANC-1. Compound 4 could induce the PANC-1 cells inflation or death, but couldn't induce apoptosis at the IC50 of 60.2 microg x mL(-1).
