1.Prevalence rates of healthcare-associated infection in a tertiary first-class hospital in the northwest of Hunan Province in 2015-2024

Objective To understand the current situation and dynamic changing trends of healthcare-associated infection(HAI)in a tertiary first-class hospital in the northwest of Hunan Province from 2015 to 2024,and provide scientific basis for optimizing infection control strategies.Methods A single-day cross-sectional survey method was employed to investigate the HAI prevalence rates of hospitalized patients on the given survey day each year from 2015 to 2024.The standardized survey protocol on prevalence rate issued by the National Medical Institution Infec-tion Surveillance Network was strictly adhered,lanqingting real-time HAI monitoring management platform was adopted to retrieve cases from the hospital information system,and R4.2.2 was applied for statistical analysis.Results From 2015 to 2024,the prevalence rate of HAI decreased from 3.03%in 2015 to 1.76%in 2024(Z=-3.37,P<0.001),and the HAI case prevalence rate decreased from 3.55%in 2015 to 2.20%in 2024(Z=-2.81,P=0.005).Department of critical care medicine continuously had the highest HAI case prevalence rate,which presented a downward trend over time(Z=-2.84,P=0.004).The main site of HAI was lower respiratory tract,accounting for 39.36%to 48.15%,bloodstream infection increased from 3.57%in 2015-2016 to 10.60%in 2023-2024(Z=2.41,P=0.016).A total of 302 strains of HAI pathogens were detected,including 212 strains(70.20%)of Gram-negative bacteria,mainly Pseudomonas aeruginosa(n=55,18.21%),Escherichia coli(n=45,14.90%),Acinetobacter baumannii(n=33,10.93%),and Klebsiella pneumoniae(n=31i,10.26%).65 strains(21.52%)of Gram-positive bacteria were identified,with Enterococcus faecium(n=19,6.29%)and Staphylococcus aureus(n=18,5.96%)accounting for the highest proportions.25 fungal strains(8.28%)were detected,mainly Candi-da albicans(n=11,3.64%).The use rate of antimicrobial agents showed a downward trend over the past decade(Z=-4.01,P<0.001).Therapeutic antimicrobial use accounting for 82.42%,and its proportion increased over time(Z=6.02,P<0.001).Prophylactic antimicrobial use accounted for 16.42%,showing a decreasing trend(Z=-2.75,P<0.001).The pathogen detection rate presented an upward trend over the past decade(Z=13.01,P<0.001).Conclusion The prevalence rate and case prevalence rate of HAI present a downward trend in this hospi-tal.In the future,it is necessary to establish a monitoring data-based dynamic analysis mechanism,achieve timely feedback and intervention in data monitoring,pay attention to high-risk links in department of critical care medicine,implement precise prevention and control mearsures,perform targeted prevention and control for lower respiratory tract,urinary tract,and bloodstream infection,optimize diagnosis and treatment processes,use antimicrobial agents rationally,and pay attention to the prevalence trend of Gram-negative bacteria.

3.Construction and validation of a predictive model for septic shock based on propensity score matching

Objective To construct a predictive model for septic shock based on the propensity score matching (PSM) method and validate its effectiveness. Methods A total of 114 patients with sepsis were enrolled as study objects, and were divided into septic shock group (40 patients) and sepsis group (74 patients) according to whether they developed septic shock. PSM was performed with a ratio of septic shock to sepsis of 1∶2, resulting in the inclusion of 30 patients in the septic shock group and 60 patients in the sepsis group after matching. The levels of C-reactive protein (CRP), procalcitonin (PCT), interleukin-6 (IL-6), serum amyloid A (SAA), soluble endothelial protein C receptor (sEPCR), endothelial cell-specific molecule 1 (ESM-1), clusterin (CLU), and the Acute Physiology and Chronic Health Evaluation Ⅱ (APACHE Ⅱ) score and Sequential Organ Failure Assessment (SOFA) score at admission were compared between the two groups. Cox proportional hazards regression analysis was used to identify the factors influencing septic shock, and a predictive model for septic shock was constructed and internally validated using the receiver operating characteristic (ROC) curve. Kaplan-Meier survival curves were plotted to analyze the differences in survival prognosis among patients with different expression levels of the indicators. Objective? To explore the effects of family participation in the respiratory rehabilitation of the patients with chronic obstructive pulmonary disease (COPD) in terms of their treatment compliance and outcomes. Methods? Totally 80 COPD patients admitted in The First Affiliated Hospital of Guangzhou Medical University from March 2016 to August 2017 were selected and divided into the control group (n=40) and the observation group (n=40) according to the random number table. Patients in the control group received conventional respiratory rehabilitation intervention, while the patients had their family members participated in the rehabilitation process in the observation group. Treatment compliance and effects were compared between the two groups. Results? The compliance of the observation group totaled 97.50%, while that of the control group totaled 87.50% (χ2=7.207, P< 0.05). After 3 months' intervention, the walking distance within 6 min of the observation group was (278.72±45.89) m; the PEF pred% was (93.46±13.47)%; and FEV1% was (87.42±14.71)%, better than those of the control group (t=4.127,7.156,3.859;P< 0.05). Conclusions? Family participation in respiratory rehabilitation process can improve the compliance of COPD patients significantly, thus improve the treatment effects, which is worth promoting in clinical practice.

BACKGROUNDInsulin resistance (IR) plays an important pathophysiological role in the development of diabetes, dyslipidemia, hypertension, and cardiovascular disease. Moreover, IR can occur even in non-obese people without diabetes. However, direct detection of IR is complicated. In order to find a simple surrogate marker of IR early in non-obese people, we investigate the association of commonly-used biochemical markers (liver enzymes and lipid profiles) with IR in urban middle-aged and older non-obese Chinese without diabetes.
METHODSThis cross-sectional study included 1 987 subjects (1 473 women). Fasting blood samples were collected for measurement of glucose, insulin, liver enzymes, lipid profiles and creatinine. Subjects whose homeostasis model of assessment-IR (HOMA-IR) index values exceeded the 75th percentile (2.67 for women and 2.48 for men) of the population were considered to have IR. The area under the receiver operating characteristic curve (ROC) was used to compare the power of potential markers in identifying IR.
RESULTSTriglycerides (TG) and ratio of TG to high-density lipoprotein cholesterol (TG/HDL-C) discriminated IR better than other indexes for both sexes; areas under the receiver operating characteristic (ROC) curves (AUC) values were 0.770 (95% confidence interval 0.733-0.807) and 0.772 (0.736-0.809), respectively, for women and 0.754 (0.664-0.844) and 0.756 (0.672-0.840), respectively, for men. To identify IR, the optimal cut-offs for TG and TG/HDL-C ratio were 1.315 mmol/L (sensitivity 74.3%, specificity 71.0%) and 0.873 (sensitivity 70.1%, specificity 73.4%), respectively, for women, and 1.275 mmol/L (sensitivity 66.7%, specificity 74.4%) and 0.812 (sensitivity 75.8%, specificity 69.2%), respectively, for men.
CONCLUSIONTG and TG/HDL-C ratio could be used to identify IR in urban middle-aged and older non-obese Chinese without diabetes.

8.Effect of 15-HETE on the hypoxic pulmonary arterial rings

Objective To inveatigate the effect of 15-HETE, a metabolite of arachidonic acid, on isolated hypoxic pulmonary arterial ringa ( PARs) , trying to find appropriate treatment for pulmonary hypertension and its complications during anesthesia in order to avoid hypoxemia. Methods Sixteen healthy Wistar rats of either sex weighing (230 ? 10) g were randomly divided into two groups : A control group breathing fresh air (FiO2 =21%) and B hypoxia group breathing hypoxic air (N2 = 90% , O2 = 10% ) in a hypoxic box. After breathing hypoxic air for 9 days the animals were anesthetized. Heart and lungs were immediately removed and PARs (0.5-1.0 mm in diameter and 3 mm in length) were prepared. Four PARs were prepared from each animal. The PARs were suspended in baths filled with Krebs-Hensleit (K-H) solution maintained at 37℃ and aerated with 95% O2 and 5% CO2. Preload was gradually increased to 0.3 g in 30 min. The isometric tension was measured using a four-channel force-displacement transducer. 15-HETE was added to K-H solution and the concentration was gradually increased from 10-8 to 10-6 mol?L-1 at 5 min intervals. Contractility of PARs was analyzed by a software of Medlab 6.0. Concentration-tension curve was drawn and contraction rates were calculated. 2 mmol?L-1 4-AP, 10-2 mol?L-1 TEA and 10-6 mol?L-1 GLYB were added to separate K-H solution baths and 40 min later 15-HETE was added in order to detennine the effect of difierent potassium channel blockers on contraction response of PARs to 15-HETE. Results With increasing concentration from 10-8 to 10-6 mol?L-1 , 15-HETE increased PARs tension gradually in a dose-dependent manner from 106% ?6% to 139% ? 4% in group A and from 113% ?6% to 163% ?6% in group B.
