OBJECTIVES: Stress urinary incontinence (SUI) is a common condition among women that severely impairs quality of life. Pelvic floor proprioceptive training (PFPT) has attracted increasing attention for its potential to enhance pelvic floor muscle function and alleviate SUI symptoms. This study aims to observe and compare the clinical efficacy of PFPT combined with electroacupuncture, electrical stimulation, and biofeedback therapy versus conventional therapy consisting of electroacupuncture, electrical stimulation, and biofeedback alone in women with SUI, and to explore the role of PFPT in improving symptom and functional outcomes. METHODS: In this randomized controlled trial, 72 women with mild to moderate SUI were recruited from the Department of Rehabilitation Medicine at Third Xiangya Hospital, Central South University, between December 2021 and October 2023. Participants were randomly assigned to an experimental group (n=36) or a control group (n=36). Both groups received health education. The control group underwent electroacupuncture combined with electrical stimulation and biofeedback therapy, while the experimental group additionally received PFPT 3 times per week for 4 weeks. The primary outcome was assessed using the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF). Secondary outcomes included pelvic floor muscle strength, bladder neck mobility, and balance ability. The ICIQ-SF was reassessed at 1, 3, 6, and 12 months post-treatment. RESULTS: Both groups showed statistically significant improvements in all parameters after treatment (all P<0.05). However, there were no statistically significant differences between groups in most measures (all P>0.05). The experimental group demonstrated longer single-leg stance duration with eyes closed than the control group (left leg: P=0.026; right leg: P=0.006), with a significant increase from baseline (P<0.001). At 6 months post-treatment, the cure rate in the experimental group was significantly higher than that in the control group (P=0.037). CONCLUSIONS Conventional therapy effectively improves SUI symptoms, but adding PFPT provides notable additional benefits, including enhanced balance ability and sustained mid-term cure rates. These findings suggest that PFPT is a valuable adjunct to standard SUI management strategies.

To explore the application of humanistic care in children's fever clinics,the children's fever clinics of Guangzhou Women and Children's Medical Center conducted relevant research using narrative medicine as a guiding concept.This paper elaborated on the medical humanistic dilemmas and narrative care needs in children's fever clinics,and focused on exploring the practice paths of narrative medicine in pediatric with Chinese characteristics from five dimensions,including the cultivation of nursing staff's abilities of professional narrative and humanistic care,the establishment of health lectures featuring narrative patient education,the alleviation of medical anxiety for children and their families,related support of narrative nursing,and caring services.The aim was to improve the narrative care ability of nurses in children's fever clinics,develop methods for pediatric patients that can eliminate the fear of seeking medical treatment,and protect children's physical and mental health.A carrier of care,support,and emotional expression can be provided for parents.The foundation for nursing staff to achieve professional growth through narrative reflection can be laid.Thus,it can assist in establishing a life narrative community relationship between doctors and patients,and jointly explore the meaning of life.

This paper discussed how to set the limit of impurities in the quality standard when the impurities in small-molecular innovative drugs were metabolites.Firstly,through the analysis and interpretation of each relevant guideline,in combination with the review cases of FDA and EMA,found out the conditions for qualification of im-purity,and then establ ish the acceptable limit of impurities based on the test results of multiple batches,the incre-ments of production process and storage process.

Objective To investigate the protective effect of recombinant Schistosoma japonicum cystatin(rSj-Cys)against acute liver injury induced by lipopolysaccharide(LPS)and D-GalN in mice.Methods Adult male C57BL/6J mice with or without LPS/D-GaIN-induced acute liver injury were given intraperitoneal injections of rSj-Cys or PBS 30 min after modeling(n=18),and serum and liver tissues samples were collected from 8 mice in each group 6 h after modeling.The survival of the remaining 10 mice in each group within 24 h was observed.Serum levels of ALT,AST,TNF-α and IL-6 of the mice were measured,and liver pathologies was observed with HE staining.The hepatic expressions of macrophage marker CD68,Bax,Bcl-2 and endoplasmic reticulum stress(ERS)-related proteins were detected using immunohistochemistry or immunoblotting,and TUNEL staining was used to detect hepatocyte apoptosis.Results The survival rates of PBS-and rSj-Cys-treated mouse models of acute liver injury were 30%and 80%at 12 h and were 10%and 60%at 24 h after modeling,respectively;no death occurred in the two control groups within 24 h.The mouse models showed significantly increased serum levels of AST,ALT,IL-6 and TNF-α and serious liver pathologies with increased hepatic expressions of CD68 and Bax,lowered expression of Bcl-2,increased hepatocyte apoptosis,and up-regulated expressions of ERS-related signaling pathway proteins GRP78,CHOP and NF-κB p-p65.Treatment of the mouse models significantly lowered the levels of AST,ALT,IL-6 and TNF-α,alleviated liver pathologies,reduced hepatic expressions of CD68,Bax,GRP78,CHOP and NF-κB p-p65,and enhanced the expression of Bcl-2.In the normal control mice,rSj-Cys injection did not produce any significant changes in these parameters compared with PBS.Conclusion rSj-Cys alleviates LPS/D-GalN-induced acute liver injury in mice by suppressing ERS,attenuating inflammation and inhibiting hepatocyte apoptosis.

Objective:To analyze the monitoring data of Kashin-Beck disease (KBD) in Gansu Province and learn about the long-term trend of KBD incidence.Methods:The X-ray detection rate of children with KBD in Gansu Province from 1990 to 2021 was collected through Gansu Provincial Center for Disease Control and Prevention. Joinpoint regression model was used to analyze the trend of X-ray detection rate of KBD in children. The annual percent change (APC) and average annual percent change (AAPC) were used to analyze the trend of the observed indicators. In addition, a polynomial regression model was constructed to fit the change of X-ray detection rate of children with KBD over time.Results:From 1990 to 2021, a total of 126 726 children were examined for KBD by X-ray in Gansu Province, with 3 011 positive cases, X-ray detection rate of KBD in children was 0 in 2019 - 2021. Joinpoint regression model analysis showed that the X-ray detection rate of children with KBD in Gansu Province from 1990 to 2018 showed a decreasing trend (AAPC = - 14.40%, P = 0.001). The APC of 1990 - 1998, 2002 - 2008 and 2008 - 2012 was - 17.75%, - 21.89% and - 49.89%, respectively, showing a decreasing trend ( P < 0.05). The APC of 2012 - 2018 was 6.15%, but the trend change was not statistically significant ( P = 0.475). The curve fitting of X-ray detection rate of KBD in children in Gansu Province over time was carried out, and the quadric polynomial equation was y = 0.000 314 5 x4 - 0.021 37 x3 + 0.487 1 x2 - 4.635 x + 22.08. Conclusions:The condition of KBD in children of Gansu Province has been effectively controlled. The X-ray detection rate has remained at a relatively low level since 2012, and it has reached a state of elimination since 2019.

Objective To investigate the correlation between serum long non-coding RNA FGD5-AS1(ln-cRNA FGD5-AS1),microRNA-103a-3p(miR-103a-3p)and puerperal infection(PI)in patients with gesta-tional diabetes mellitus(GDM)in late pregnancy.Methods A total of 168 late pregnancy GDM patients who were hospitalized and delivered in the hospital from January 2022 to June 2023 were retrospectively selected as the experimental group,and the patients were separated into an infected group(96 cases)and an uninfected group(72 cases)based on whether they had PI.At the same time,120 late pregnant women who underwent prenatal examination in the hospital and had normal gestational blood glucose were selected as the control group.Real-time fluorescence quantitative PCR(qRT-PCR)was applied to detect the expression levels of ln-cRNA FGD5-AS1 and miR-103a-3p.Multivariate Logistic regression was applied to analyze the influencing factors of PI in late pregnancy GDM patients.StarBase website was applied to analyze the relationship between lncRNA FGD5-AS1 and miR-103a-3p.Pearson was applied to analyze the correlation between lncRNA FGD5-AS1 and miR-103a-3p.Receiver operating characteristic(ROC)curve was applied to evaluate the value of ln-cRNA FGD5-AS1 and miR-103a-3p in predicting the occurrence of PI.Results There was a statistically sig-nificant difference in the expression levels of serum lncRNA FGD5-AS1 and miR-103a-3p between the experi-mental group and the control group(P＜0.05),the expression level of serum lncRNA FGD5-AS1 in the infec-ted group was obviously higher than that in the uninfected group(P＜0.05),but the expression level of ser-um miR-103a-3p in the infected group was obviously lower than that in the uninfected group(P＜0.05).The expression level of lncRNA FGD5-AS1 was an independent risk factor for PI in late-pregnancy GDM patients(P＜0.05),and the expression level of miR-103a-3p was an independent protective factor for PI in late-preg-nancy GDM patients(P＜0.05).There was a negative correlation between lncRNA FGD5-AS1 and miR-103a-3p expression level(r=-0.409,P＜0.001).The efficacy of the combined detection of lncRNA FGD5-AS1 and miR-103a-3p for predicting PI in late pregnancy GDM patients was superior to that of serum lncRNA FGD5-AS1 and miR-103a-3p alone(P＜0.05).Conclusion LncRNA FGD5-AS1 is an independent risk factor for PI in late pregnancy GDM patients,while miR-103a-3p is an independent protective factor for PI in late pregnancy GDM patients.The combined detection has higher value for predicting PI in late pregnancy GDM patients.

Gastroesophageal reflux disease （GERD） is a frequently and commonly occurring disease in clinic. In recent decades， with the development in pathophysiology and drug researches， modern medicine has achieved remarkable progress and results in diagnosis and treatment. However， the treatments for non-erosive reflux disease， refractory gastroesophageal reflux disease， proton pump inhibitor resistance， overlap of disease symptoms， and extraesophageal symptoms are limited and ineffective. Traditional Chinese medicine （TCM） was widely used in clinical practice， which has been proved effective in relieving symptoms and improving the quality of life. Sponsored by China Association of Chinese Medicine （CACM） and undertaken by the Spleen and Stomach Disease Branch of CACM， "the 12^th Youth Salon of Clinical Predominance Disease Series （GERD）" invited 18 authoritative digestive experts of TCM and western medicine to discuss "the difficulties of clinical diagnosis and treatment of GERD and TCM advantages". The focus issues such as modern medical diagnosis and treatment achievements and contributions， improvement and maintenance of symptoms， response to overlapping disease symptoms， reduction and withdrawal of acid suppressors， and treatment of extra-esophageal symptoms were discussed in depth. Objective:To observe the diagnostic value of six classification intelligent auxiliary diagnosis lightweight model for common fundus diseases based on fundus color photography.Methods:A applied research. A dataset of 2 400 color fundus images from Nanjing Medical University Eye Hospital and Zhejiang Mathematical Medical Society Smart Eye Database was collected, which was desensitized and labeled by a fundus specialist. Of these, 400 each were for diabetic retinopathy, glaucoma, retinal vein occlusion, high myopia, age-related macular degeneration, and normal fundus. The parameters obtained from the classical classification models VGGNet16, ResNet50, DenseNet121 and lightweight classification models MobileNet3, ShuffleNet2, GhostNet trained on the ImageNet dataset were migrated to the six-classified common fundus disease intelligent aid diagnostic model using a migration learning approach during training as initialization parameters for training to obtain the latest model. 1 315 color fundus images of clinical patients were used as the test set. Evaluation metrics included sensitivity, specificity, accuracy, F1-Score and agreement of diagnostic tests (Kappa value); comparison of subject working characteristic curves as well as area under the curve values for different models.Result:Compared with the classical classification model, the storage size and number of parameters of the three lightweight classification models were significantly reduced, with ShuffleNetV2 having an average recognition time per sheet 438.08 ms faster than the classical classification model VGGNet16. All 3 lightweight classification models had Accuracy > 80.0%; Kappa values > 70.0% with significant agreement; sensitivity, specificity, and F1-Score for the diagnosis of normal fundus images were ≥ 98.0%; Macro-F1 was 78.2%, 79.4%, and 81.5%, respectively.Conclusion:The intelligent assisted diagnosis of common fundus diseases based on fundus color photography is a lightweight model with high recognition accuracy and speed; the storage size and number of parameters are significantly reduced compared with the classical classification model.
