1.A study on the evaluation of left ventricular systolic dysfunction in patients with early coronary heart dis-ease by using three-dimensional speckle tracking imaging technology combined with left ventricular pres-sure strain loop

Objective We tried to analyze the value of three-dimensional speckle tracking imaging technology combined with left ventricular pressure strain loop for evaluation of left ventricular systolic dysfunction in early coronary heart disease patients.Methods 97 patients with early coronary heart disease admitted to our hospital from February 2022 to February 2024 were selected as the research subjects.According to the degree of left ventricular systolic dysfunction,they were divided into those without left ventricular systolic dysfunction(LVEF value≥50%)and those with left ventricular systolic dysfunction(LVEF value<50%).All patients under-went clinical data collection,left ventricular pressure strain loop,and three-dimensional speckle tracking imaging technology examination.Results Body mass index,gender,diabetes mellitus and hyperlipidemia were compared between the two groups(P>0.05).Compared with the group without left ventricular systolic dysfunction,the group with left ventricular systolic dysfunction is older,with a higher proportion of Killip grades Ⅲ~Ⅳ and multiple coronary artery lesions(P<0.05).Compared with the group without reduced left ventricular systolic function,the absolute levels of GWE,GCW,and GWI in the group with reduced left ventricular systolic function were lower,while the absolute level of GWW was higher(P<0.05).Compared with the group without left ventricular systolic dysfunction,the left ventricular systolic dysfunction group had lower levels of area,circumference,radial,and longitudinal strain(P<0.05).GWE、GCW、GWI、Area,circumference,radial,and longitudinal strain levels were negatively correlated with the incidence of left ventricular systolic dysfunction,while GWW was positively correlated with the incidence of left ventricular systolic dysfunction(P<0.05).The ROC analysis results showed that the sensitivity and accuracy of left ventricular pressure strain loop and three-dimensional speckle track-ing imaging technology in predicting left ventricular systolic dysfunction in early coronary heart disease patients were lower than combined detection(P<0.05).Conclusions Early coronary heart disease patients with reduced left ventricular systolic function had lower absolute levels of area,circumference,radial,longitudinal strain,GWE,GCW,and GWI,and higher absolute levels of GWW.These indicators are all related to the incidence of left ventricular systolic dysfunction.The combination of three-dimensional speckle tracking imaging technology and left ventricular pressure strain loop had high value in predicting left ventricular systolic dysfunction in early coronary heart disease patients.

Creutzfeldt-Jakob disease(CJD)is a rapidly progressive and fatal neurodegenerative disorder caused by prions,with certain infectivity and iatrogenic transmission risks.With the rapid progress and application of new dia-gnostic biomarkers and detection methods,as well as the construction and improvement of surveillance and reporting systems,the detection of CJD in patients domestically and internationally has shown an increasing trend year by year.Due to its long incubation period and heterogeneity of early symptoms,early identification and diagnosis of the disease is difficult,increasing the risk of transmission within medical institutions.Currently,there is a lack of con-sensus on the infection prevention and control of CJD.In order to timely identify and diagnose CJD as well as effec-tively block its transmission in medical institutions,this consensus summarizes 15 clinical concerns and formulates 24 specific recommendations based on the latest domestic and international research findings and clinical evidence,as well as combines with clinical practice,aiming to standardize healthcare-associated infection prevention and control measures for CJD and reduce its transmission risk in medical institutions.

5.Study on Endemic Medicinal Plants in Shaanxi Province

Abstract
OBJECTIVE To clarify the resource status and diversity of endemic medicinal plants in Shaanxi province. METHODS The species specificity, species composition, faunal composition, family and genus types, medicinal value and endangerment degree of endemic medicinal plants in Shaanxi province were studied by literature review.RESULTS There were 713 species of 331 genera and 101 families endemic to Chinese medicinal plants in the study area. Fifteen species were naturally distributed only in Shaanxi province, and the remaining 698 species were also naturally distributed in other provinces of China. Among the 713 species, 233 species(69 families, 159 genera) were not collected from the fourth resource census in Shaanxi province. There were 11 species of pteridophytes in 7 families and 11 genera, 14 species of gymnosperms in 4 families and 10 genera, 627 species of dicotyledons in 82 families and 278 genera, and 59 species of monocots in 8 families and 32 genera. The endemic life forms of medicinal plants in the study area were mostly herbaceous, followed by shrubs and trees, and semi-shrubs and epiphytes accounted for the least. There were 9 families with ≥ 20 species and 4 families with ≥ 10 species in the study area. The 90 families belonging to the endemic species of medicinal plants in Shaanxi province were divided into 13 distribution types and 9 variations, and the tropical distribution(2-7 categories) had a total of 34 families. There were 5 endemic species of medicinal plants in the study area under the national class I key protection, and 14 species under the national class II key protection. There were 26 species of plants under local key protection in Shaanxi province. There were 21 plants that could be used as original plants for medicinal materials included in the Pharmacopoeia of the People's Republic of China(2020 edition). CONCLUSION The endemic species of medicinal plants in Shaanxi province are rich in resources and have good medicinal value. However, the growing environment of some plants is harsh and human damage is serious. Multiple protection measures should be taken to maintain the species diversity and sustainable development of resources in the study area.
6.Curative effect of magnesium sulfate combined with labetalol on severe preeclampsia

Objective:To investigate the efficacy of magnesium sulfate combined with labetalol on severe preeclampsia and the effects on serum levels of soluble intercellular adhesion molecule-1 (sICAM-1), vascular endothelial growth factor (VEGF) and pregnancy associated plasma protein-A (PAPP-A).Methods:Eighty-five patients with severe preeclampsia who received treatment in Jinhua Municipal Central Hospital between January 2019 and June 2020 were included in this study. They were randomly assigned to receive treatment either with magnesium sulfate (control group, n = 42) or magnesium sulfate combined with labetalol (observation group, n = 43). Before and after treatment, 24-hour dynamic blood pressure, 24-hour urinary protein excretion and umbilical artery hemodynamics were determined. In addition, serum levels of sICAM-1, VEGF and PAPP-A were detected. Results:After treatment, 24-hour mean arterial pressure, 24-hour urine protein excretion in the control and observation groups were significantly decreased compared with before treatment. After treatment, the 24-hour mean arterial pressure [(116.45 ± 9.63) mmHg vs. (120.74 ± 9.48) mmHg] and 24-hour urine protein excretion [(1.85 ± 0.52) g vs. (2.33 ± 0.75) g] were significantly lower than those in the control group ( t = 2.069, 3.436, both P < 0.05). The umbilical artery resistance index in the control group, and the peak systolic velocity/end-diastolic velocity ratio and the umbilical artery resistance index in the observation group were significantly decreased after treatment compared with before treatment (all P < 0.05). After treatment, the peak systolic velocity/end-diastolic velocity ratio in the observation group was significantly lower than that in the control group [(2.80 ± 0.50) vs. (3.01 ± 0.45), t = 2.034, P < 0.05]. After treatment, serum sICAM-1 and PAPP-A levels were significantly decreased, and serum VEGF level was significantly increased, in each group (all P < 0.05). After treatment, serum levels of sICAM-1 [(548.52 ± 102.74) mg/L vs. (647.52 ± 124.15) mg/L, t = 4.009, P < 0.05] and PAPP-A [(111.74 ± 28.52) U/L vs. (134.24 ± 35.96) U/L, t = 3.200, P < 0.05] in the observation group were significantly lower than those in the control group, and serum VEGF level in the observation group was significantly higher than that in the control group [(32.12 ± 7.45) ng/L vs. (28.17 ± 6.23) ng/L, t =2.659, P < 0.05]. There was no significant difference in the incidence of poor pregnancy outcome between the control and study groups ( P > 0.05). Conclusion:Magnesium sulfate combined with labetalol exhibits good therapeutic effects on severe preeclampsia. The treatment method can improve umbilical artery hemodynamics, and its mechanism may be related to its effect on vascular endothelial function.
7. The application value of transrectal ultrasonography in the diagnosis of anorectal malignant melanoma

Objective:
To investigate the value of transrectal ultrasonography(TRUS) in the preoperative diagnosis and tumor T-staging of anorectal malignant melanoma(ARMM).
Methods:
The clinical and ultrasound data of 19 patients(22 lesions) with ARMM confirmed by pathology from February 2008 to Apirl 2019 in Fujian Medical University Affiliated Union Hospital were collected. TRUS was performed within one week before pathological examination. The sonographic features were summarized, ultrasonic typing was performed according to the lesion size and growth pattern, and the accuracies of preoperative ultrasound diagnosis and tumor staging were analyzed.
Results:
Among the 22 lesions, 63.64%(14/22) were with a distance of ≤3 cm between the anal verge and the lower margin of tumor, 68.18%(15/22) with regular shape, 81.82%(18/22) with clear boundary, and 63.64%(14/22) with Adler blood flow grading from Ⅱ to Ⅲ. The preoperative TRUS diagnostic coincidence rate was 73.68%(14/19), and the tumor T-staging accuracy was 75%(12/16), respectively.
Conclusions
TRUS exhibits certain sonographic characteristics, with high preoperative diagnosis rate and tumor T-staging accuracy in anorectal malignant melanoma, which is of great guiding significance for clinical diagnosis and treatment.
8.The value of color Doppler ultrasound in diagnosis and differential diagnosis of ovarian granulosa cell tumor

Objective To discuss the value of color Doppler ultrasound in the diagnosis and differential diagnosis of ovarian granulosa cell tumor (OGCT) . Methods Fourteen cases were diagnosed as OGCT by pathology postoperatively had been examined by color Doppler ultrasound check-up before the surgery . In image appearance ,the tumors were divided into cystic mass ,solid mass or mixed cystic-solid mass respectively according to their own pathological features . Additionally ,the blood flowing signal of OGCT was labeled from 0 to 3 grade with Adler standard based on the blood flowing imaging observed by color Doppler flow imaging ( CDFI) . The CDFI results were compared with the pathological diagnosis . Results Ultrasound image appearance of OGCT varied in the 14 cases .Two of them could be characterized as cystic mass ,5 solid mass and 7 mixed cystic-solid mass . The mixed cystic-solid OGCT contained solid mass in multiple cystic degeneration area . There was a significant difference in the diameter among three kinds of OGCT ( P = 0 .0006) . The more visible cystic lesion indicated the longer diameter of OGCT . According to the CDFI ,4 ,5 ,3 and 2 cases were 0 ,1 ,2 ,3 grade respectively in Adler standard . No significant difference was found among the four standards groups ( P =0 .056) . The localization diagnosis accordance rate of the Doppler imaging in OGCT was 71 .4% (10/14) and the qualitative diagnosis accordance rate was 35 .7% (5/14) . Conclusions The color Doppler ultrasonography can be the first-line preoperative imaging investigation in the diagnosis and differential diagnosis of OGCT ,as well as in the long-term following after surgery . It can provide more details and evidences for the treatment and prognosis assessment of OGCT .
9.Effects of human milk fortifier on premature infant's cardiac function

Objective To investigate the effects of human milk fortifier on premature infant's cardiac function.Methods From January 2014 to December 2016, sixty premature infants of paediatric preterm infants were selected from Daqing Longnan Hospital,Heilongjiang province. The random number table method was divided into 30 cases (milk feeding and strengthening agent) and the control group of 30 cases (pure milk feeding).The heart rate of premature infant was measured before, during and after using human milk fortifier. After using human mike fortifier for 1 week, heart rate, electrocardiogram (p-wave duration, QRS wave duration, P-R interval, incidence of arrhythmia), echocardiogram (LVEF, RVEF, E/A ratio of mitral and tricuspid valves) and myocardial enzymes (the quantity of AST, LDH, CK and CK-MB) were compared between experimental group and control group.Results The heart rate of premature infant during using human milk fortifier was significantly faster than those before and after using human milk fortifier and that of children in the control group (P<0.05). There were no statistically significant differences in electrocardiogram (p-wave duration, QRS wave duration, P-R interval), the systolic and diastolic function of the heart and myocardial enzymes between the experimental and control groups (P>0.05).Conclusions Human milk fortifier can increase the heart rate of premature infant, and has no harmful effect on cardiac electrical activity, pump function and myocardial enzymes.
10.Value of color Doppler ultrasonography in the diagnosis and differential diagnosis of urinary tract urothelial carcinoma

Objective To discuss the value of color Doppler ultrasonography in the diagnosis and differential diagnosis of urinary tract urothelial carcinoma (UTUC).Methods Color Doppler ultrasonography exam was practiced in 84 UTUC patients preoperatively.According to ultrasound characteristics,lesions was classified into nodular type,wall thickening type and solid mass type,which were confirmed by pathology in all cases postoperatively.Results In the total of 84 cases (76 transitional-cell carcinoma,5 adenocarcinoma and 3 epidermoid carcinoma),lesions infiltrated laminae propria and surrounding tissue in 65 cases.In the nodular type of UTUC,there were nodules raising into lumen.Wall thickening type of UTUC was characterized by irregularly wall thickening,lumen narrowing,truncating and breaking.Similarly,obstruction broke off the ureteral lumen suddenly in solid mass type of UTUC.A clear demarcation could be found between the pipe wall and surrounding tissue in nodular type.However,such clear demarcation could not be found in wall thickening type and solid mass type.The accordance rate of ultrasonographic located diagnosis were 92.9 % (78/84),with 6 missed diagnosis cases.The accordance rate of qualitative diagnosis were 89.7% (70/78).Conclusions As result of color Doppler ultrasonography,there were different relationship between lesion and the peripheral tissue in different types of UTUC.In types of wall thickening and solid mass,the demarcation was unclear and it is easier to violate the surrounding tissue.Color Doppler ultrasonography can provide more effective information for diagnosis and therapy UTUC.
