1.Clinical characteristics in patients with persistent positive pharyngeal swab of omicron variant and analysis on nucleic acid testing of anal swabs

Objective:To analyze the clinical characteristics in patients with persistent positive pharyngeal swab of 2019 novel coronavirus Omicron variant and results of nucleic acid testing of anal swabs to provide basis for prevention and control measures.Methods:This study included 93 patients whose pharyngeal swab nucleic acid test were persistent positive and admitted to the ward of Daping Hospital in the National Exhibition and Convention Center (Shanghai) Makeshift Hospital from May 1 to May 24, 2022. The gender, age, underlying diseases, vaccination status, clinical symptoms, interval between infection onset and anal sampling, length of hospital stay, the nucleic acid test result of pharyngeal swabs and anal swabs and the time turning negative were collected and analyzed.Results:The age of 93 patients ranged from 8 to 72 years old with a median of (46.0±16.0) years old. Among them, 30 cases (32.3%) were male and 63 cases (67.7%) were female. Sixty-five patients (69.9%) received 2-3 shots of vaccine, 2 patients (2.1%) received 1 shot, and 26 patients (28.0%) did not receive any vaccination. Twenty patients (21.5%) had underlying diseases, of which hypertension (13 cases, 14.0%) and type 2 diabetes mellitus (6 cases, 6.5%) were the most common. Twenty-four patients (25.8%) had asymptomatic infection and the rest (69 cases, 74.2%) had mild symptoms. Cough (50 cases, 53.8%) and sore throat (28 cases, 30.1%) were the most common clinical manifestations of the upper respiratory tract in these patients. Only 6 patients (6.5%) had gastrointestinal symptoms (including diarrhea in 5 patients and diarrhea with vomiting in 1 patient). Pharyngeal and anal swabs were collected simultaneously from all 93 patients at 8-16th days [(11.55±2.27) days] after 2019 novel coronavirus Omicron variant infection. The pharyngeal swabs were positive in 79 patients (85.0%) and the anal swabs were positive in 5 patients (5.4%). The time of pharyngeal swabs turning negative was (14.7±2.9) days, and that of anal swab turning positive was (14.2±1.9) days. The median length of hospital stay was (16.7±2.9) days.Conclusions:In patients with persistent positive nucleic acid of the 2019 novel coronavirus Omicron variant, there were more mild infection than asymptomatic. The upper respiratory tract symptoms such as cough and sore throat were the most. The likelihood of transmission of 2019 novel coronavirus Omicron variant through the digestive tract may be low. The correlation between gastrointestinal symptoms and 2019 novel coronavirus Omicron variant RNA in the digestive tract is uncertain.
2.Clinical research of heated humidified high-flow nasal cannula for prevention of extubation failure in preterm infants with respiratory distress syndrome

Objective To assess the clinical efficacy and safety of heated humidified high-flow nasal cannula (HHHFNC) and nasal continuous positive airway pressure (NCPAP) for prevention of extubation failure in preterm infants with respiratory distress syndrome (RDS).Methods From March 2016 to December 2017,75 preterm infants (≤32 weeks gestation) with RDS who needed noninvasive respiratory support after a period of mechanical ventilation with an endotracheal tube in neonatal intensive care unit were studied.They were randomly assigned into the HHHFNC group (38 cases) and the NCPAP group (37 cases) by using a random number table.The main observation was the success rate of removal of the ventilator,duration of noninvasive ventilation time,the total oxygen inhaling time,feeding conditions and incidence of adverse events.Results The baseline demographic characteristics of the two groups were similar in terms of gestational age,birth weight,gender,incidence of cesarean delivery,premature rupture of membranes,administrations of antenatal glucocorticoid prophylaxis,Apgar scores at 1 and 5 minutes (P > 0.05).No significant difference was found in the success rate of removal of the ventilator between HHHFNC group and NCPAP group(94.7 % vs.91.9%,P > 0.05).The time of noninvasive ventilation,the total oxygen inhaling time and hospital stay in the NCPAP group were shorter than those in the HHHFNC group,but there were no significant differences between two groups (P > 0.05).The occurrence of abdominal distention (10.5% vs.27.05%),nasal trauma(5.2% vs.21.6%),head shaping(0 vs.32.4%) were lower in HHHFNC group than those in NCPAP group (P <0.05).The time needed to achieve total enteral nutrition[(10.1 ± 1.2) d vs.(14.1 ± 1.6)d] and the incidence of feeding intolerance (13.1% vs.29.7%) were reduced in HHHFNC group than those in NCPAP group (P < 0.05).Conclusion As an respiratory support for the treatment of preterm infants with RDS after extubation,HHHFNC has the similar efficacy with NCPAP.However,HHHFNC has lower incidence of nasal trauma,abdominal distension and feeding intolerance,and further clinical research is needed.

4.Bundles of care prevents postoperative delirium in elderly patients with hip fractures

Objective To evaluate the effect of bundles of care on the prevention of postoperative delirium among elderly patients with hip fractures.Methods A prospective randomized case control study was conducted on 80 patients (≥65 years old) with hip fractures from March 2017 to June 2017.The patients were divided into experimental group (n =43) and control group (n =37) according to the random number table method.The experimental group received bundles of care,while the control group received routine nursing.The patients in both groups were all surgically treated,and the confusion assessment method (CAM) was applied to diagnose delirium after surgery.Gender,age,fracture type,duration from injury to operation,internal fixation type,operation time,total amount of bleeding,visual analogue scale (VAS),incidence of delirium,complications,and adverse events were compared between the two groups.Results Among the 80 patients,there were 11 males and 69 females,aged averagely 79.3 years (range,65-95 years).No significant differences were found between experiment group and control group in terms of gender (male:6 cases vs.5 cases,females:37 cases vs.32 cases),age [(79.8 ± 7.8) years vs.(78.7 ± 8.9) years],cause of injury (traffic injuries:7 cases vs.4 cases;falling injuries:36 cases vs.33 cases),duration from injury to operation [(66.1 ±14.3)hours vs.(63.4 ±14.9) hours],fracture type (femoral neck:13 cases vs.10 cases;intertrochanteric:26 cases vs.24 cases;subtrochanteric:4 cases vs.3 cases),internal fixation type (artificial total hip:5 cases vs.5 cases;artificial femoral head:8 cases vs.5 cases;PFNA:29 cases vs.27 cases),operation time [(55.5 ± 16.8) minutes vs.(51.6 ± 17.0) minutes],total blood loss [(114.4 ± 73.9) ml vs.(108.1 ±72.0) ml] (P > 0.05).After bundles of care intervention,the postoperative VAS [(2.2 ± 0.8) points vs.(4.3 ± 1.2) points],postoperative delirium incidence (9% vs.32%),incidence of complications and adverse events (2% vs.19%) in experimental group were significantly lower than those in control group (P < 0.05).Conclusion Bundles of care can relieve the pain and effectively reduce the incidences of postoperative delirium,complications,and adverse events in elderly patients with hip fracture.
5.Developed for the Determination of Serum Bilirubin between Oxidase Method and Vanadate Oxidation Method

Objective To study reaction principle of bilirubin vanadate oxidation method and bilirubin oxidase method through comparison of the determination results,and discuss similarities and difference between the two methods .Methods 310 ca-ses were measured and analyzed with each method.Abnormal samples were further investigated.Results Fortotal bilirubin, the regression equation obtained wasY=1.065 1X+1.197 2,the correlation coefficientr=0.997 0.For direct bilirubin of a-dults and children greater than 30 days,the regression equation wasY=0.945 9X+0.599 5 and the correlation coefficient r=0.994 4.For neonatal direct bilirubin,the regression equation wasY=0.410 4X+2.756 3 and the correlation coefficient r=0.883 5.The results from vanadate oxidation method were unacceptable for abnormal neonatal serum measurement after serial dilution.Conclusion The overall conclusions were that for the measurement of total bilirubin,and direct bilirubin for adults and children older than 30 days.The correlation between these two methods is in an acceptable range,for measure-ment of neonatal direct bilirubin,the correlation between thesetwo methods was not acceptable.It is not recommended to measure neonatal direct bilirubin by vanadate oxidation method.
6.Protective effect of rosuvastatin on acute kidney injury in the patients after coronary artery bypass grafting

Objective To investigate the prevention of acute kidney injury (AKI) by earlier application of rosuvastatin in patients after coronary artery bypass grafting (CABG).Methods A total of 200 patients with CABG were enrolled from May 2013 to April 2017.According to whether rosuvastatin were used routinely before operation or not,all patients were divided into the trial group (n =136) and the control group (n =64).Demographics,and clinical data were collected before and after CABG.The renal function markers including blood urea nitrogen (BUN),serum creatinine (sCr),endogenous creatinine clearance rate (GFR),emergence of AKI of two groups were documented and compared.Enumeration data were analyzed with x2 test,measurement data were analyzed with t test,and P < 0.05 was considered to be significant.Results There were no differences in sCr (t =-1.156,P > 0.05) but differences in BUN and eGFR (t =-2.915,3.690,respectively,P < 0.05) before operation between two groups.After operation,the BUN was decreased (t =2.486,P < 0.05) compared with that of pre-operation in the trial group,but there were no significant difference in sCr and eGFR (t =-1.877,-0.752,respectively,P >0.05).The BUN and sCr were increased (t =-3.792,-5.027,respectively,P < 0.05) after operation compared with that of pre-operation in the control group,while the eGFR was decreased (t =5.540,P <0.05).Compared with the control group,BUN,sCr and the incidence of AKI were significantly decreased in the trial group (t/x2 =5.759,4.196,15.506,respectively,P <0.05),while the eGFR was increased (t =-6.215,P < 0.05).Conclusions Earlier application of rosuvastatin before CABG can effectively protect renal function and reduce the incidence of AKI.
7.A Hydrogen Peroxide Sensor Based on Palladium Nanoparticles Loading to Poly(amidoamine) Dendrimers Functionalized Carbon Nanotubes

The fourth generation poly( amidoamine) dendrimers ( G4. 0 PAMAM) functionalized multiwalled carbon nanotube ( G4 . 0-MWCNTs ) was prepared by amidation between carboxylated multiwalled carbon nanotube (MWCNTs) and G4. 0 PAMAM. Then a novel hydrogen peroxide (H2O2) sensor was fabricated by electrodepositing Pd nanoparticles (NPs) on a glassy carbon electrode (GCE) modified with G4. 0-MWCNTs composites. The modified electrode was characterized by field emission scanning electron microscopy ( FESEM) , cyclic voltammetry ( CV) and electrochemical impedance spectroscopy ( EIS) . A large amounts of highly dispersion PdNPs could be well loaded on the surface of the G4. 0-MWCNTs, and the modified electrode exhibited excellent electrocatalytic activity towards the reduction of H2 O2 . Under the optimized conditions, the reduction peak currents of H2 O2 were linear to their concentrations in the range from 1. 0 × 10-9 mol/L to 1. 0×10-3 mol/L and the limit of detection of 2. 3×10-8 mol/L was obtained. The recovery of standard addition for human serum samples was 96 . 7%-103 . 1%.
8.Effect of overexpression of vascular cell adhesion molecule-1 on migration of murine mesenchymal stem cells

OBJECTIVE To investigate the effect of overexpression of vascular cell adhesion molecule-1(VCAM-1)on the migration in vitro of the murine mesenchymal stem cells(MSCs)and its possible mechanism. METHODS The migration ability of normal mouse MSC (C3) ,empty vector-transfected MSC(C3+N) and VCAM-1 transfected MSC(C3+VCAM-1)was assessed by Transwell culture system in vitro after incubation for 8 and 12 h,respectively. The fetal bovine serum (FBS) was used as the chemotactic agent to induce MSC migration. The transmigrated cells were detected with methylosaniliam chloride(crystal violet)as well as DAPI staining.Furthermore,the specific chemical inhibitors of mitogen-activation protein kinase (MAPK) pathway ( SB203580,PD98059 and JNK inhibitorⅡ)were added to the Transwell system for 12 h and the alteration of the MSC migration ability was evaluated. RESULTS After incubation with FBS for 8 and 12 h,the absolute migrated cell number(7467 ± 485 and 8795 ± 255)and migration rate〔(14.9 ± 1.0)% and(17.6 ± 0.5)%〕of MSC in C3+VCAM-1 group were significantly increased compared with C3 group〔2731±562 and 4779±224, (5.5 ± 1.1)%and(9.6 ± 0.4)%〕and C3+N group〔2539 ± 321 and 5645 ± 1080,(5.1 ± 0.6)%and(11.3 ± 1.1)%〕(P<0.05,P<0.01),but there was no significant difference between C3 and C3+N groups. Moreover,the MSC migration ability of C3+VCAM-1 group was partially suppressed by addition of JNK inhibitorⅡ. The transmigrated cell number(4843 ± 167)and migration rate〔(9.7 ± 0.3)%〕were decreased compared with those of C3+VCAM-1 group without JNK inhibitorⅡ(P<0.01). SB203580 and PD98059,as specific chemical inhibitors of MAPK pathway,had no effect on MSC migration. CONCLUSION VCAM-1 can enhance mouse MSC migration in vitro and th4e mechanism may be related to JNK/MAPK pathway activation.
9.A Hydrogen Peroxide Sensor Based on Pt@Au Nanoparticles Loading to Polyethyleneimine Functionalized Carbon Nanotubes

A novel hydrogen peroxide sensor was fabricated by the seed-mediated growth method. First, polyethyleneimine(PEI) functionalized multiwalled carbon nanotubes(MWNTs) were used as growth scaffold on the glass carbon electrode ( GCE). Then, Au nanoparticles were electrodeposited uniformly as seeds. Finally, Pt nanoparticles ( PtNPs ) grew on Au nanoparticles to form Pt @ Au core-shell structure nanocomposite. A new type of electrochemical sensor based on Pt @ Au / PEI-MWNTs nanocomposites for detection of hydrogen peroxide was developed, and the designed Pt@ Au / PEI-MWNTs/ GCE was characterized by electrochemical methods and field emission scanning electron microscopy (FESEM). The Differential pulse experimental results showed that the modified electrode exhibited excellent electrocatalytic activity towards the reduction of H2 O2 with the wide linear range from 9. 2 ×10-8 mol/ L to 1. 3 ×10-3 mol/ L. The correlation coefficient was 0. 9994 and the low detection limit was 3. 1×10-8 mol/ L at the signal-to-noise of 3.
10.Nosocomial Infection Pathogens in an Orthopedic Hospital:Their Drug Resistance and Distribution

OBJECTIVE To investigate the drug resistance and distribution of nosocomial infection (NI) pathogens. METHODS A total of 519 strains of NI pathogens during from Jan 2006 to Dec 2007 were completely surveyed. RESULTS Among all pathogens,Gram-positive cocci were 209 strains (40.27%),the main pathogen was Staphylococcus aureus. Gram-negative bacilli were 256 strains (49.33%). Fungi were 54 strains (8.28%). resistance the rate in nosocomial infectionis was increased year by year. CONCLUSIONS The Gram-negatives of NI pathogens are markedly increased year by year. The drug resistance rate of pathogens is higher and the clinically selected anti-bacterial drugs should be based on the bacterial culture and drug susceptibility test.
