1.A randomized controlled study on enhanced regimens of bowel preparation of linaclotide combined with compound polyethylene glycol electrolyte powder

Objective:To explore the efficacy of linaclotide combined with compound polyethylene glycol (PEG) on bowel preparation, and compare it with traditional PEG 3 L and oral sulfate solution (OSS) 3 L methods.Methods:Patients aged 18-70 years who underwent colonoscopy at the Digestive Diseases Hospital, Heilongjiang Provincial Hospital from January to June 2023 were continuously enrolled in the randomized controlled trial and randomly divided into 3 groups using the random number table. Intestinal preparation was conducted according to the protocols of each group, Group A: 3 L PEG; Group B: 3 L OSS; Group C: 290 μg of linaclotide + 2 L PEG. The effects of bowel preparation, adverse reactions, satisfaction, and willingness for repeated bowel preparation were compared.Results:A total of 360 patients were included in the analysis, with 120 patients in each group. There were no statistically significant differences in the median Boston bowel preparation score for each intestinal segment or the total score among the 3 groups (left colon: 3 VS 2 VS 3, H=0.371, P=0.831; transverse colon: 3 VS 3 VS 3, H=0.487, P=0.784; right colon: 2 VS 2 VS 2, H=1.088, P=0.580; total score: 8 VS 8 VS 8, H=0.017, P=0.991). Among the adverse reactions, the incidence of nausea and vomiting in Group B [3.33% (4/120)] was lower than that in Group A [12.50% (15/120), χ2=8.042, P=0.018], and there were no statistically significant differences in other adverse reactions among the 3 groups ( P>0.05). There was no significant difference in the satisfaction level among the 3 groups ( χ2=11.840, P=0.158). The willingness to undergo bowel preparation again in Group C [95.83% (115/120)] and Group B [96.67% (116/120)] was higher than that in Group A [85.00% (102/120)] (Group C VS Group A: χ2=8.127, P=0.004; Group B VS Group A: χ2=9.808, P=0.002), and there was no significant difference between Group C and B ( χ2=0.000, P=1.000). Conclusion:Linaclotide combined with 2 L PEG offers comparable bowel preparation efficacy and safety to 3 L PEG or 3 L OSS, with improved patient compliance due to reduced water intake, suggesting its potential as an enhanced bowel preparation regimen.

3.Relationship among professional identity, learning motivation and learning engagement in rehabilitation technology students in higher vocational education

ObjectiveTo investigate the overall level of professional identity, learning motivation and learning engagement in students majoring rehabilitation technology in higher vocational education, and to explore the relationship among them. MethodsFrom March to April, 2022, a questionnaire survey was conducted among 334 students majoring in rehabilitation technology in three public full-time higher vocational schools in Beijing, Shandong and Anhui with the Professional Identity Scale, Learning Motivation Scale and Learning Engagement Scale. ResultsA total of 334 valid questionnaires were collected. The average scores of professional identity, learning motivation and learning engagement were (3.64±0.66), (3.78±0.61) and (3.42±0.72), respectively. There was a significant positive correlation among them (r > 0.602, P < 0.01). The professional identity was different with schools and ages (P < 0.05), and the learning motivation was different with ages and grades (P < 0.01). There was no significant difference in learning engagement in different schools, genders, ages, or grades (P > 0.05). The learning motivation played a mediative role between professional recognition and learning engagement, accounting for about 55.3% of the total effect. ConclusionThe professional identity, learning motivation and learning engagement of the students majoring in rehabilitation technology in higher vocational colleges are at a moderate level, which needs to be improved. The professional identity is different among the schools of cities with different economic development, which suggests that there may be a decreasing effect of policy transmission behind it, which should be highly valued.
4.Efficacy and thoughts of endoscopic submucosal dissection for rectal neuroendocrine tumor

To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) for rectal neuroendocrine tumor (RNET). Data of 71 patients (43 males and 28 females, aged 46.66±10.15 years) with histologically diagnosed RNET ≤15 mm in diameter who underwent ESD in Department One of Gastroenterology, Digestive Diseases Hospital, Heilongjiang Provincial Hospital from December 2012 to January 2021 were retrospectively analyzed. Epidemiological characteristics, endoscopic characteristics, the operation time, complications, pathology results, the consistency of endoscopic ultrasound (EUS) and pathology and prognosis were analyzed. Sixty-nine patients had single lesion and 2 patients had multiple lesions (two of each). Seventy lesions (70/73, 95.89%) were?located?in?the?middle-lower?rectal?segment, and 3 lesions (3/73, 4.11%) in the upper segment. The maximum diameter was 8.54±3.12 mm. The en bloc resection rate was 100%. No adverse event was observed. The R0 resection rate was 87.67% (64/73), with 8 had undetermined vertical resection margin and 1 had lymphovascular invasion. EUS showed no case involving the muscularis propria with pathologic coincidence rate of 100.0%. The follow-up period was 4.52±3.85 years (1-9 years). No local recurrence or distant metastasis was found. ESD is safe and effective in RNET ≤15 mm in diameter. For?tumors of G1 without metastasis in preoperative evaluation, close?follow-up?seems?to?be?a?feasible?option with lymphovascular invasion or undetermined margin after?ESD.
5.Prophylactic effect of clipping after endoscopic mucosal resection of small colorectal polyps on delayed bleeding

Objective:To explore the prophylactic effect of clipping after endoscopic mucosal resection(EMR) of sessile colorectal polyps of diameter below 10 mm on delayed post-polypectomy bleeding (DPPB).Methods:Patients with sessile colorectal polyps of below 10 mm undergoing EMR from January 2017 to December 2019 in Digestive Disease Hospital, Heilongjiang Provincial Hospital were randomly divided into the clipping group (group A) and the non-clipping group (group B) by random number table, and DPPB rates of both groups were compared.Results:A total of 1 838 patients were included, 912 patients in group A and 926 patients in group B. The incidences of DPPB were 1.00% (9/912) and 1.10% (10/926) respectively, with no significant difference ( χ2=0.039, P>0.05). The proportion of bleeding polyps were 0.44%(9/2 029)and 0.49%(10/2 025) respectively, with no significant difference ( χ2=0.055, P>0.05). Polyp of 6-9 mm ( OR=11.032, 95% CI: 2.545-47.821, P<0.05) was the independent risk factor for delayed bleeding in small sessile colorectal polyps after EMR. Conclusion:Prophylactic clipping for sessile colorectal polyps below 10 mm after EMR may not significantly reduce the risk of DPPB.
6.Long non-coding RNA ARHGAP5-AS1 inhibits the proliferation, migration and invasion of breast cancer cells

[摘 要] 目的:检测长链非编码RNA(long non-coding RNA,lncRNA)ARHGAP5-AS1在乳腺癌组织及细胞中的表达,分析其表达与患者临床病理参数及预后的相关性,并初步探讨其对乳腺癌细胞体外增殖、迁移和侵袭的影响。方法:通过对TCGA数据库中乳腺癌相关数据集的生物信息学分析,筛选出在乳腺癌中低表达且与患者不良预后相关的lncRNA ARHGAP5-AS1,采用qPCR方法在江南大学附属医院肿瘤科从2010年4月至2016年10月收集的乳腺癌组织中验证其表达。采用χ2检验分析ARHGAP5-AS1表达与乳腺癌患者临床病理参数之间的关系,Kaplan-Meier生存分析构建生存曲线,比较高、低表达组的总生存期和无复发生存期。CCK-8实验、划痕实验和Transwell实验分别检测ARHGAP5-AS1敲低对乳腺癌细胞MDA-MB-231和BT-549的增殖、迁移和侵袭的影响。结果:TCGA数据库分析结果显示,ARHGAP5-AS1在乳腺癌组织中的表达水平显著低于正常乳腺组织(P<0.01),其低表达与较大肿瘤直径(T3)、远处转移(M1)、ER和PR阴性以及较短的总生存期显著相关(均P<0.05)。乳腺癌组织中ARHGAP5-AS1表达水平显著低于癌旁组织(P<0.05),其低表达与较大的肿瘤直径和淋巴结转移相关(均P<0.05)。同样,ARHGAP5-AS1在6株人乳腺癌细胞系(MDA-MB-231、BT-549、MDA-MB-468、MCF-7、HCC1937、Hs578T)中的表达水平也显著低于正常乳腺上皮细胞系(MCF-10A)(均P<0.05)。细胞功能实验显示,ARHGAP5-AS1敲低促进MDA-MB-231和BT-549细胞的增殖、迁移和侵袭(均P<0.05)。结论:ARHGAP5-AS1异常低表达可能通过促进乳腺癌细胞的增殖、迁移和侵袭影响乳腺癌的发生发展。
7.Application of multimedia health education in patients with endoscopic retrograde cholangio-pancreatography minimally invasive treatment

Objective? To evaluate the effect of mobile nursing workstation multimedia module applied to preoperative health education of patients with minimally invasive endoscopic retrograde cholangiopancreatography (ERCP). Methods? By convenience sampling, 100 patients who met the inclusion criteria in-hospitalized in Department of Digestive Medicine, Beijing Chao-Yang Hospital, Capital Medical University from January to June of 2018 were selected and divided into the observation group (n=44) and control group (n=44) according to the random number table. Conventional education approach of oral teaching and distribution of teaching materials was adopted in the control group, besides of that the observation group received the intensive teaching approach through mobile nursing platform multimedia and open communication. The knowledge mastering status, satisfactory degree and anxiety scores were compared between the two groups after health education. Results? In the observation group, the score of knowledge mastering status was (33.00±9.41), satisfactory degree was(23.45±1.18), both higher than the control group [(28.82±7.48),(17.61±1.95)] with statistical difference (P<0.05); the observation group achieved lower score in anxiety than the control group with statistical difference(P< 0.01). Conclusion? The use of mobile nursing platform multimedia teaching method can improve the quality of health education for patients with minimally invasive ERCP treatment and patient satisfaction, alleviate the anxiety of patients.
8.microRNA-18a Promotes Cell Migration and Invasion Through Inhibiting Dicer l Expression in Hepatocellular Carcinoma In Vitro.

Objective To investigate the effects of microRNA-18a (miR-18a) on migration and invasion of hepatocellular carcinoma (HCC) cells, and its possible mechanism associated with Dicer l.Methods HepG2 and HepG2.2.15 cells were transfected with miR-18a inhibitor using Lipofectamine. Cell invasion was evaluated by transwell invasion assay, and cell migration was detected by transwell migration and wound-healing assays. Moreover, luciferase reporter assay was used to identify whether Dicer expression was regulated by miR-18a. Real-time RT-PCR and western blot were performed to analyze Dicer 1 expression. In addition, a functional restoration assay was performed to investigate whether miR-18a promotes HCC cell migration and invasion by directly targeting Dicer 1.Results miR-18a inhibitor can suppress the migration and invasion of HCC cells. Furthermore, suppression of Dicer l expression by small interfering RNA essentially abolished the inhibition of cell migration and invasion induced by miR-18a inhibitor, restorating these activities to levels similar to the parental HCC cells. Interestingly, suppression of miR-18a in HCC cells resulted in enhanced expression of Dicer l. In addition, the results of a luciferase assay demonstrated targeted regulation of Dicer l by miR-18a.Conclusion Our findings suggest that miR-18a promotes migration and invasion of HCC cells by inhibiting Dicer l expression.

9.Comparison of maximum slope and deconvolution algorithms in multi-slice CT hepatic perfusion measurement

Objective To evaluate the reliability of CT perfusion parameter values of the normal hepatic segments and neoplasms, obtained with deconvolution (DC) and maximum slope (MS) algorithms. Methods Perfusion parameter values of 111 ROIs in 62 normal hepatic segments and 49 neoplasms derived from 62 CT perfusion studies performed with 320 multi-slice CT, were retrospectively analyzed by two experienced radiologists. BF,BV and PI according to DC and MS algorithms were compared with t paired test, Pearson correlation and Bland-Altman agreement analysis. Interobserver agreement for all perfusion parameters was calculated using intraclass correlation coefficients (ICC). Results Interobserver agreement measured with ICC was very good for all perfusion parameters (≥0.95). BFdc and BVdc exceeded the BFms and BVms in normal hepatic segments and neoplasms (P<0.05); PIdc significantly exceeded the PIms in normal hepatic segments(P<0.05) ,while no difference were found in hepatic neoplasms(P>0.05). Both pairs of perfusion measurements significantly correlated with each other(r>0.9, P<0.01),but the agreement of BF, BV and PI according to DC and MS algorithms was not good. Conclusions CT perfusion values such as BF,BV and PI obtained by DC and MS algorithms correlated significantly with each other, but with poor agreement.
10.Effect of rapamycin on infantile amnesia

Objective To verify the infantile amnesia phenomenon and discuss the effect of rapamycin injection on infantile amnesia and hippocampal neurogenesis.Methods (1) In experiment one,17-day-old (P17) and 60-day-old (P60) male Sprague-Dawley rats (n=84) were trained in a passive-avoidance task (they were randomly sub-divided into the shocked group and non-shocked group);all rats were tested immediately,2 and 7 days after training for memory retention.(2) In experiment two,P17 rats (n=84) were randomly divided into three groups:vehicle group,20 mg/kg rapamycin group and 40 mg/kg rapamycin group (they were sub-divided into the shocked group and non-shocked group);P17 rats were immediately injected with either vehicle or rapamycin after passive-avoidance training,and memory retention was evaluated 2 and 7 days after training.(3) In experiment three,P1 7 rats (n=48) were randomly divided into four groups:normal group,vehicle group,20 mg/kg rapamycin group and 40 mg/kg rapamycin group;immunofluorescence staining of BrdU/DCX was detected to observe the hippocampal neurogenesis at post-natal day 19 (P19) and post-natal day 24 (P24) after vehicle or rapamycin injection at P17 rats.Results In experiment one,P17 and P60 shocked rats exhibited comparable levels of crossing latencies when being tested immediately (P>0.05),when being tested after 2 days and 7 days,P17 rats forgot more rapidly than P60 rats (P<0.05);there were no age-related differences in crossing latencies between the non-shocked groups (P>0.05).In experiment two,three shocked groups exhibited comparable levels of crossing latencies when being tested 2 days after training (P>0.05),however,administration of rapamycin could enhance the memory retention 7 days after training;significant differences were noted between the vehicle group and each of the rapamycin treated groups (P<0.05);there were no dose-related differences in crossing latencies among the non-shocked groups (P>0.05).In experiment three,less BrdU-positive and BrdU/DCX-positive cells were found in the DG of the hippocampus at 20 mg/kg and 40 mg/kg rapamycin groups as compared with those in the vehicle group at P19 and P24 rats (P<0.05).Conclusion The infantile amnesia phenomenon is verified,and rapamycin may enhance the memory retention in infant rats by reducing the hippocampal neurogenesis.
