Objective:To investigate the effect of tolerogenic dendritic cells(tolDC)on autophagy of synovial cells in collagen-induced arthritis(CIA)rats. Methods:Bone marrow mononuclear cells of rats were extracted and induced into tolDC using IL-4,GM-CSF and NF-κB oligonucleotide decoy, and loaded with BⅡC to become BⅡC-tolDC.SD rats were randomly divided into normal control group,CIA model group and BⅡC-tolDC intervention group, with 3 rats in each group.Normal female SD rats were immunized with bovine type Ⅱ collagen solution to construct CIA model.Rats in BⅡC-tolDC intervention group were infused with BⅡC-tolDC via tail vein on the 21st day after initial immunization for two weeks, arthritis indexes were recorded weekly. On the 35th day,the rats were sacrificed, and synovial histopathology of ankle joint of rats in each group were observed by HE staining;the number of osteo-clasts in cartilage of rats in each group were observed by TRAP staining. The number of autophagic of ankle synovial cells of rats in each group were observed by transmission electron microscopy.Levels of serum TNF-α and IL-1β of rats in each group were detected by ELISA.LC3,Beclin-1 and ATG5 proteins of synovial cells of ankle joints of rats in each group were detected by Immunohistochemical staining. Results:CIA rats were constructed successfully by immunization with bovine type Ⅱ collagen.BⅡC-tolDC intervention re-duced the arthritis index of CIA rats, inhibited synovial inflammation and abnormal proliferation of synovial tissue, improved joint bone and cartilage injury, and reduced the number of osteoclasts in cartilage tissue and the number of autophagosomes in synovial cells. At the same time, reduced levels of serum TNF-α,IL-1β,and protein expressions of LC3,Beclin-1 and ATG5 of synovial cell of CIA rats.Conclusion:BⅡC-tolDC may alleviate arthritis lesions of CIA rats by inhibiting synovial cell autophagy of CIA rats.

Objective To investigate the occurrence and characteristics of Antineutrophil cytoplasmic antibody(ANCA)associated vasculitis induced by methimazole, and to provide references for clinical safe drug use. Methods Case reports of ANCA associated vasculitis induced by methimazole published in Wanfang,CNKI,PubMed,and Web of Science were searched from the inception to October 31 st,2023.Demographic characteristics, drug use, complications, treatment and outcome were analyzed using descriptive statistical method. Results A total of 14 patients from 14literature were included. There were 3 males and 11 females with ages ranging from 8 to 79 years, with a mean age of(47.79±23.47)years.Four patients developed symptoms within 1 year, nine patients developed symptoms from 2 to 12 years, and 1 patient developed symptoms 24 years after medication.ANCA associated vasculitis affected kidney in 5 patients, lung and skin in 5patients,vision in 2 patients and heart in 2 patients. All patients discontinued methimazole,2 patients improved spontaneously without treatment,1 patient improved after anti-infection,and all others received hormonal or immunosuppressive therapy.1 patient developed death, and all others improved or were cured after treatment. Conclusions ANCA-associated vasculitis is a rare adverse reactionof methimazole. Most patients have a long latency period before the onset of disease, mainly involving multiple organs such asskin, kidney, lung, and eyesight.Clinicians should pay attention to differentiate it from primary vasculitis and discontinue the drug as soon as possible. When serious organ damage occurs, glucocorticoids and immunosuppressants should be adminstered promptly to avoid aggravation of the disease and endangerment of life.

Purpose To develop a nomogram combined radiomics, clinical and CT morphographic features for the prediction of the acquired T790M mutation in patients with advanced non-small cell lung cancer with resistance after the duration of first-line epidermal growth factor receptor(EGFR)-tyrosine kinase inhibitor(TKI)treatment.Materials and Methods The chest CT images and clinical data of 116 patients who underwent secondary biopsy after disease progression during first-line TKI treatment for advanced non-small cell lung cancer with EGFR sensitive mutations, from December 2016 to December 2022 in Shaoxing People's Hospital were retrospectively analyzed. All patients were randomly divided into a training cohort(n=81)and a validation cohort(n=35),at a ratio of 7︰3.The regions of interest of the lesion were delineated, and radiomics features were extracted.Feature selection was performed via the maximum relevance minimum redundancy and the least absolute shrinkage and selection operator methods.Clinical features were selected via univariate and multivariate Logistic regression.A nomogram combined clinical and radiomics features was subsequently constructed. The predictive ability of the combined model was evaluated via receiver operator characteristic curve, calibration curves and decision curve analysis. Results The progression-free survival of first-line EGFR-TKIs(OR=1.086,P=0.041),initial EGFR profile(OR=0.280,P=0.021),vascular convergence(OR=4.050,P=0.036)and air bronchogram(OR=3.265,P=0.030)were highly correlated with the acquired T790M mutation. The combined model demonstrated good predictive performance for acquired T790M mutation, both in the training set(AUC=0.867,95%CI 0.790-0.944)and the validation set(AUC=0.895,95%CI 0.786-1.000).The calibration curve showed good calibration power, and the decision curve analysis demonstrated a significant net benefit. Conclusion A radiomics-clinical nomogram based on CT radiomics has the potential to predict acquired T790M mutation and could be a complementary tool for T790M mutation detection after resistance to first-or second-generation EGFR-TKIs.

Objective:To investigate the prevalence and influencing factors of post-traumatic stress disorder (PTSD) in thyroid cancer patients. Methods:This cross-sectional study used convenience sampling to recruit thyroid cancer patients from the Thyroid Surgery Department of Harbin Medical University Cancer Hospital between July and September 2023. Data were collected using a general demographic questionnaire, the Post-Traumatic Stress Disorder Checklist-Civilian Version (PCL-C), the Perceived Stress Scale (PSS-10), and the Negative Cognitive Processing Bias Questionnaire (NCPBQ) .Results:A total of 210 questionnaires were distributed, and 204 valid responses were returned, yielding a response rate of 97.14% (204/210). Among the respondents, 44 patients (21.57%) tested positive for PTSD, with a PCL-C score of 23 (19, 32). Factors influencing PTSD included age, education level, marital status, monthly income, lateral neck lymph node metastasis, comorbid chronic diseases, perceived stress, and negative cognitive processing bias ( P<0.05) . Conclusions:PTSD symptoms were present in 21.57% of thyroid cancer patients and were influenced by various factors. Healthcare professionals should develop targeted interventions based on patient characteristics to alleviate PTSD symptoms in thyroid cancer patients.

Transradial access(TRA)for performing interventional diagnosis and treatment can reduce access site complications. However,TRA can still cause complications in the operated limb, including radial artery spasm, radial artery perforation, hemorrhage, hematoma, radial artery dissection, osteofascial compartment syndrome, nerve injury, pseudoaneurysm, arteriovenous fistula, and radial artery occlusion, among which radial artery spasm is the most commonly seen.At present, the clinical recognition, prevention and treatment of complications of surgical limbs are still insufficient. Therefore, this consensus aims to make a detailed introduction about the identification, prevention and nursing of nine major types of complications of the surgical limb in interventional diagnosis and treatment procedures via radial artery access,to propose 24 recommendations which may provide a reference for medical institutions in carrying out the nursing care for complications of the surgical limb in interventional diagnosis and treatment procedures via radial artery access, and to reduce the occurrence of complications in patients.(J Intervent Radiol,2024,33:465-471)
