1.Professional self-concept of nurses: the influence of work stressors and related factors

BackgroundThe professional self-concept of nurses is a crucial indicator for their personal growth, development and career planning. Previous studies have shown that work stressors during the internship period may lead to a decrease in the level of professional self-concept among nursing students. Given the existing differences in social roles between nursing students and clinical nurses, the influencing factors of professional self-concept in clinical nurses and its relationship with work stressors require further research. ObjectiveTo explore the relationship between work stressors and professional self-concept among nurses, so as to provide references for improving the level of professional self-concept of clinical nurses. MethodsFrom December 2022 to February 2023, a stratified random sampling method was employed to select 260 in-service nurses from Mianyang Hospital of Traditional Chinese Medicine as the study subjects. Chinese Nurses Stressor Scale (CNSS) and Professional Self-concept of Nurses Instrument (PSCNI) were used for assessment. Pearson correlation analysis was conducted to examine the correlation between CNSS scores and PSCNI scores of nurses. Multiple linear regression analysis was utilized to explore the factors influencing the professional self-concept of nurses. ResultsA total of 238 nurses (91.54%) completed valid questionnaires. PSCNI total score yielded a statistical difference among nurses with different marital statuses (F=8.947, P<0.05). PSCNI total scores were significantly higher in nurses with emergency medical service experience than those without such experience (t=2.208, P<0.05), and were significantly lower in nurses with abnormal physical examination findings in the past year than those without abnormal findings (t=-2.584, P<0.05). Correlation analysis revealed that CNSS total score and subscale scores were negatively correlated with PSCNI total score (r=-0.275~-0.169, P<0.01). Multiple linear regression analysis indicated that work expectation-related stressors, marital status and emergency medical service experience might be influencing factors of their professional self-concept (β=-0.350, 0.345, 0.183, P<0.01). ConclusionNurses' work stressors are closely correlated with their professional self-concept, and high levels of stressors related to nurses' expectations may lead to a decrease in their professional self-concept levels. China's Medical Service Price(MSP)reform has entered a new phase,implementing management measures.The compilation,implementation,and coordination of MSP item guidelines and the existing contradictions and problems were reviewed.It proposes strengthening price item management in planning under the background of the new era,itemizing in the alignment particle size,and comparing prices in the same coordinate system.This can offer systematic approaches and innovative strategies for improving the quality and efficiency of deepening MSP reform.
3.Prevalence and influencing factors of post-traumatic stress disorder in thyroid cancer patients

Objective:To investigate the prevalence and influencing factors of post-traumatic stress disorder (PTSD) in thyroid cancer patients.Methods:This cross-sectional study used convenience sampling to recruit thyroid cancer patients from the Thyroid Surgery Department of Harbin Medical University Cancer Hospital between July and September 2023. Data were collected using a general demographic questionnaire, the Post-Traumatic Stress Disorder Checklist-Civilian Version (PCL-C), the Perceived Stress Scale (PSS-10), and the Negative Cognitive Processing Bias Questionnaire (NCPBQ) .Results:A total of 210 questionnaires were distributed, and 204 valid responses were returned, yielding a response rate of 97.14% (204/210). Among the respondents, 44 patients (21.57%) tested positive for PTSD, with a PCL-C score of 23 (19, 32). Factors influencing PTSD included age, education level, marital status, monthly income, lateral neck lymph node metastasis, comorbid chronic diseases, perceived stress, and negative cognitive processing bias ( P<0.05) . Conclusions:PTSD symptoms were present in 21.57% of thyroid cancer patients and were influenced by various factors. Healthcare professionals should develop targeted interventions based on patient characteristics to alleviate PTSD symptoms in thyroid cancer patients.

6.Analysis of resting-state functional brain network in schizophrenia patients based on graph theory

Objective To explore the brain network and its topological properties in schizophrenia patients using resting-state functional magnetic resonance imaging(rs-fMRI).Methods Thirty-five schizophrenia patients(patient group)and 35 healthy subjects(control group)from the Center of Biomedical Research Excellence were collected,and the regional homogeneities of the two groups were calculated for statistical analysis.Dosenbach's 160 atlas was used to examine the whole brain functional network,and extract two sub networks of the whole brain network(default mode network and somatic motor network)for constructing a new network.The topological properties of the whole brain network and the new network were calculated.Results In schizophrenia patients,the functional connectivity between default mode network and somatic motor network and that within the default mode networks were significantly weakened(P<0.05,FDR corrected);the clustering coefficient of the whole brain network was decreased;and the global and local efficiencies of the network composed of default mode network and somatic motor network were reduced(P<0.05).Conclusion Abnormal functional connectivity is found between default mode network and somatic motor network and within default mode network in schizophrenia patients,and the significant changes of topological properties between default mode network and somatic motor network may be the key factor.In addition,this conclusion can be applied to qualitative analysis of the brain neuron stochastic differential equations corresponding to default mode network and somatic motor network,which is helpful for the physical therapy for schizophrenia.
7.Giant acinic cell carcinoma complicated with severe mediastinum deviation: A case report

Primary acinic cell carcinoma (ACC) of the lung is extremely rare. The World Health Organization tumor classification defines ACC as "a malignant epithelial neoplasm that demonstrates some cytological differentiation towards (serous) acinar cells". It is considered to be a low-grade malignant tumor. Since the first case described by Fechner in 1972, less than 30 cases have been reported in the literature. The rarity of this tumor may leads it to be confused with other primary lung tumors and incorrectly diagnosed. We reported a female patient with primary ACC of the lung with mediastinum deviation at age of 27 years received a right pneumonectomy. She was followed up for 12 months postoperatively and remains well.
8.Comparative study on the incidence of retinopathy of prematurity in premature infants born in the hospital and transported from other hospitals

Objective:To explore the incidence, diagnosis and treatment of retinopathy of prematurity(ROP) in preterm infants born in the Guangdong Women and Children′s Hospital and transported from other hospital.Method:s Clinical data of 755 premature infants with ROP at Neonatal Intensive Care Unit, Guangdong Women and Children′s Hospital from January 2013 to December 2015 were retrospectively analyzed.There were 239 cases born in the hospital and 516 cases transported from other hospitals.Their gestational age, birth weight, gender, severity of ROP lesion and clinical data were collected and compared.Result:s The birth weight in the group of transported from other hospital was lower than that in the group of born in the hospital[(1 290.64±392.87) g vs.(1 586.21±512.74) g], and the difference was statistically significant( P<0.001). The ROP diagnosis of gestational age in the group of transported from other hospital was higher than that in the group of born in the hospital[(35.53±2.81)weeks vs.(34.51±2.17)weeks], and the difference was statistically significant( P<0.001). On the proportion of severe condition [such as lesion area Ⅰ, aggressive posterior retinopathy of prematurity(AP-ROP) and plus combined lesions], in the group of transported from other hospital was higher than that in the group of born in the hospital, and the differences was statistically significant( P<0.001). In the comparison of the proportion of laser photocoagulation, vitreous injection, combination of the two operations and supplementary laser therapy, in the group of transported from other hospital were higher than those in the group of born in the hospital[60.1%(310/516 cases) vs.20.9%(50/239 cases); 10.9%(56/516 cases) vs.2.5%(6/239 cases); 8.1%(42/516 cases) vs.1.7%(4/239 cases); 4.5%(23/516 cases) vs.1.3%(3/239 cases)], and the differences were statistically significant(all P<0.001). Conclusions:Premature infants with ROP transported from other hospitals have lower birth weight, severe ROP lesions and high surgical intervention rate.Improving ROP screening level in primary hospitals, timely diagnosis and efficient transportation can help to effectively prevent the deterioration of ROP in premature infants and improve their quality of life.
9.Mastery of disease-related knowledge among tuberculosis children's parents

ObjectiveTo explore the mastery of disease-related knowledge among tuberculosis children's parents so as to provide a basis for carrying out health education for children's parents. MethodsFrom November to June 2016, we selected tuberculosis children's parents at West China Second University Hospital of Sichuan University as subjects by convenience sampling. A total of 112 children's parents were investigated with the self-designed questionnaire by interview. A total of 112 questionnaire were sent out and all of them were collected with 100% for the recovery rate. ResultsAmong 112 children's parents, they mastered well on key points in the treatment, whether could be cured, medicine taking time and requirements for review of tuberculosis with above 90% for the accuracy rate. There were statistical differences in the mastery of disease-related knowledge among children's parents with different education levels and nations (P< 0.05). ConclusionsWe should focus on strengthening health education of tuberculosis knowledge for parents with low educational levels, in rural areas and minority nationalities, and health education of drug-related knowledge for all parents. We should develop targeted health education to improve the compliance of tuberculosis treatment in children.
10.Correlations among hope level, coping style and life quality in myasthenia gravis patients

Objective To explore the relationships among hope level, coping style and quality of life (QOL) in myasthenia gravis (MG) patients. Methods From September 2016 to September 2017, 139 MG patients of Shaoxing People's Hospital were selected by convenience sampling. The hope level, coping style and QOL of patients were investigated with the Herth Hope Index (HHI), Medical Coping Modes Questionnaire (MCMQ) and Quality of Life Questionnaire-Core 30 (QLQ-C30). Pearson correlation and multiple linear regression were used to analyze the correlations among them and influencing factors of QOL respectively. Results The total score of HHI, score of confrontation dimension, avoidance dimension, yield dimension of MCMQ and score of total QOL of MG patients was (35.85±3.14), (18.78±3.67), (15.82±2.37), (9.94±2.79) and (52.33±11.83) respectively. Correlation analysis showed the total score of HHI of MG patients was positively associated with the total score of total QOL (r=0.675, P<0.05); the confrontation coping style was positively associated with the total QOL (r=0.613, P<0.05); the yield coping style had a negative correlation with the total QOL (r=-0.582, P<0.05). Multiple linear regression analysis revealed the influencing factors of QOL of MG patients included the burden of medical expense, classification of Myasthenia Gravis Foundation of America (MGFA), total score of HHI and yield. Conclusions The QOL of MG patients is in a low level. The main influencing factors of QOL of patients involve the burden of medical expense, classification of MGFA, hope level and yield coping style.
