Pan-vascular diseases are a group of diseases involving the systemic vascular system, with vascular lesions (especially atherosclerosis) as a common pathological feature. Pan-vascular diseases have attracted widespread attention due to the high healthcare burden that accompanies high morbidity. Inflammation is the core pathogenesis of pan-vascular diseases. Targeted anti-inflammatory strategies have achieved certain success in basic research. However, due to the increased risk of infection, their clinical application has been hindered. Simiao Yong'an Tang (SMYAT), which has the effects of nourishing Yin, detoxifying, and activating blood, demonstrates unique therapeutic advantages. Clinical evidence indicates that SMYAT alone or in combination with conventional Western Medicine/other compound formulas/non-drug therapies can effectively alleviate the clinical symptoms and traditional Chinese medicine (TCM) syndromes of pan-vascular diseases. At the same time, it can reduce the serum levels of inflammatory factors and the risk of infection, improve glucose and lipid metabolism and blood viscosity, and promote wound healing, thereby enhancing the quality of life. The research on the pharmacological material basis indicates that when SMYAT enters the bloodstream, it can be converted into various active components such as harpagide, harpagoside, and chlorogenic acid. Basic research suggests that SMYAT can exert anti-inflammatory effects through multiple pathways and targets, such as interfering with immune cell adhesion and aggregation, macrophage polarization, foam cell autophagy, neovascularization in the outer membrane and plaque, lipid metabolism and cholesterol reverse transport. The current research lacks comprehensive discussion on vascular diseases. In-depth studies on the effects and mechanisms of SMYAT can provide new therapeutic approaches for pan-vascular diseases. Moreover, they help to gain an overall understanding of the disease progression, demonstrating the superiority of the TCM concept of treating different diseases with the same method. This paper provides a systematic overview of the TCM and Western Medicine understanding of pan-vascular diseases, the theoretical framework and material basis of SMYAT, and the progress of clinical and basic research on SMYAT treatment of pan-vascular diseases. The ultimate goal is to provide theoretical evidence and guidance for the development and application of SMYAT in the field of pan-vascular diseases. In the future, further research should be conducted to standardize the compound formula and explore the mechanism of treatment with integrated TCM and Western Medicine.

It is believed that diabetes complicated with coronary heart disease is closely related to the functional interplay of the heart, spleen, and kidneys. This paper proposed the concept of the heart-spleen-kidney as a unified system for understanding and treating the disease. At the early stage, spleen and kidney deficiency leads to the internal accumulation of phlegm, dampness, and turbid lipids, causing impaired blood circulation and vascular obstruction, so treatment should focus on tonify the kidneys and strengthening the spleen, activating blood circulation and resolving stasis, using the self-prescribed Tangxin Maiwen Formula (糖心脉温方). As the disease progresses, further decline of spleen and kidney function results in inadequate nourishment of the heart, leading to blood stasis and the accumulation of phlegm, dampness, and turbid lipids, which may transform into pathogenic heat and toxins, causing heart damage, then treatment should emphasize on boosting qi and nourishing yin, clearing heat, activating blood and resolving toxins, using the self-prescribed Tangxin Maiqing Formula (糖心脉清方). In advanced stages, three zang organs, the heart, spleen, and kidneys, become severely impaired, leading to mental activity fail to be nourished and abnormal cognitive functions, so treatment should focus on harmonizing the three zang organs simultaneously, using the self-prescribed Yunpi Tiaoxin Decoction (运脾调心汤). This approach aims to provide a clinical framework for the diagnosis and treatment of diabetes with coronary heart disease.

Journal of Korean Neurosurgical Society 2025;68(2):184-201
Objective:
: Baculovirus inhibitory of apoptosis repeat-containing 5 (BIRC5) is critically implicated in various types of tumors. However, the specific mechanisms by which it operates in glioma are yet to be fully understood.
Methods:
: The data sourced from The Cancer Genome Atlas and Gene Expression Omnibus were merged and analyzed using the R software to investigate the relationship between BIRC5 expression and prognosis and diagnosis outcomes. This exploration was conducted utilizing various biological information repositories. The correlation between BIRC5 and immunity was obtained based on TIMER and TISIDB databases.
Results:
: Gliomas displayed a markedly elevated level of BIRC5 expression compared to adjacent tissues. Patients with glioma who exhibit elevated levels of BIRC5 experience poorer prognoses and shorter survival times. Subgroup classification further revealed that heightened expression of BIRC5 led to diminished overall survival. Analysis of logistic regression and COX indicated that expression of BIRC5 serves as a risk factor in glioma development. Functional enrichment pathways showed that the 72 hub genes related to BIRC5 were mainly closely related to nuclear division, spindle, tubulin binding, and cell cycle in glioma patients. BBIRC5 methylation suggested that BIRC5 might influence the immune response regulation and the tumor microenvironment within gliomas. BIRC5 is associated with many chemicals. Additionally, studies conducted using cell experiments and pathological sections have consistently shown that BIRC5 expression is higher in tumor cells compared to normal cells and tissues.
Conclusion
: BIRC5 holds promise as a valuable tool in the diagnosis, prognosis, and management of gliomas.

ObjectiveTo observe the efficacy of Huoxue Jiedu Formulas (活血解毒方药, HJF) as an adjunctive treatement for patients with binding of stasis and toxin syndrome during the vulnerable period after acute myocardial infarction (AMI) percutaneous coronary intervention (PCI) surgery, and to explore its potential mechanism from the perspective of serum neutrophil extracellular traps (NETs). MethodsA total of 129 patients with binding of stasis and toxin syndrome within 6 months after PCI for AMI were enrolled and divided into a treatment group (65 cases) and a control group (64 cases) based on patients' willingness to take Chinese herbal medicine. The control group received standard western medical therapy alone, while the treatment group additionally received HJF, one dose daily. Both groups were treated for four weeks. Before and after treatment, TCM syndrome scores were assessed. Seattle angina questionnaire (SAQ) was used to record angina stability and frequency scores, while the short form-36 health survey (SF-36) was employed to assess quality of life across eight dimensions, including physical functioning, role-physical, bodily pain, general health, vitality, social functioning, role-emotional, and mental health. The Pittsburgh sleep quality index (PSQI) was used to evaluate sleep quality, and the patient health questionnaire-15 (PHQ-15) was used to assess psychosomatic symptoms; Duke activity status index (DASI) was used to measure daily physical activity. Serum levels of neutrophil extracellular traps (NET) markers including myeloperoxidase-DNA (MPO-DNA), neutrophil elastase-DNA (NE-DNA), and citrullinated histone H3 (CitH3) were measured in 20 patients from the treatment group. ResultsAfter treatment, TCM syndrome score, PSQI score and PHQ-15 score in both groups significantly decreased, while DASI score, angina stability and frequency scores, and all eight dimensions of the SF-36 scale significantly increased (P<0.05). Compared to the control group, the treatment group had significantly lower TCM syndrome scores and significantly higher DASI, angina stability and frequency scores (P<0.05), as well as higher scores in the SF-36 dimensions of physical functioning, role-physical, social functioning, bodily pain, and vitality (P<0.05). After treatment, serum levels of MPO-DNA, CitH3, and NE-DNA in the treatment group were significantly reduced (P<0.05). ConclusionHJF combined with conventional therapy can significantly improve angina symptoms, TCM syndrome scores, and psychosomatic conditions in patients with binding of stasis and toxin syndrome during the vulnerable period after AMI. It also enhances quality of life, sleep quality, and daily physical activity. The underlying mechanism may be associated with the inhibition of serum NETs level.

This article summarized clinical experience in differentiating and treating non-obstructive hypertrophic cardiomyopathy (HCM) based on the concept of nourishing the heart and softening the hardness. It is considered that HCM belongs to the category of "heart accumulation", with the fundamental cause being depletion of the spleen and kidney, and phlegm-stasis accumulation, as well as qi-yin exhaustion, serving as the manifestations. Spleen and kidney depletion leads to the transformation of phlegm and stasis, which accumulate in the heart; over time, this phlegm-stasis accumulation consumes heart qi and yin, resulting in the heart being deprived of nourishment, which eventually leads to the damage to both the function and structure of heart. Therefore, the method of nourishing the heart and softening the hardness is proposed for the treatment of non-obstructive HCM. Emphasis is placed on softening hardness and dissipating masses throughout the entire treatment process, often using Modified Siwei Ruanjian Formula (四味软坚方加减). During periods with prominent symptoms, the main treatment is boosting qi and nourishing yin to soften hardness and dissipate masses with self-made Yuxin Ruanjian Formula (自拟育心软坚方) in modifications; in stable periods, the main treatment is boosting kidney and fortifying spleen to soften hardness and dissipate masses with self-made Pishen Tongzhi Formula (脾肾同治方) in modifications.

Chinese Acupuncture & Moxibustion 2025;45(9):1299-1304
The paper introduces Professor FAN Gangqi's clinical experience in treatment of migraine. Regarding the syndrome/pattern differentiation of TCM, a four-approach framework is established, identifying the nature of illness, analyzing the syndrome/pattern and pathogenesis, determining the stage of illness, and identifying body constitution. In treatment, the principle of treatment is determined in line with syndrome/pattern differentiation, so as to ensure the therapeutic effect by means of "four dimensions". The acupuncture regimens are formulated in terms of the illness stages, "strong needling stimulation in acute stage for analgesia, and needle retaining in chronic stage for long-term effect". "Focusing on neuovascular pathway" is the effective approach to treatment of migraine with acupuncture and moxiubstion. The clinical holistic model by combining acupuncture with medication is advocated because that "the single acupuncture is weak in therapeutic effect, but with medication combined, the effect is enhanced". The different acupuncture techniques are provided comprehensively in treatment of migraine such as horizontal and row-like needling, collateral needling at Taiyang (EX-HN5), acupuncture at Sankong (Yuyao [EX-HN4], Sibai [ST2] and Jiachengjiang [Extra]), acupoint injection at Tianyou (TE16) and Renying (ST9), and acupoint embedding therapy at Fengchi (GB20).
