1.Conventional MRI and synthetic MRI quantitative parameter histogram features contributing to predicting tumor infiltrating lymphocyte level of breast cancer

Objective To explore conventional MRI and synthetic MRI(syMRI)quantitative parameter histogram features contributing to predicting tumor infiltrating lymphocyte(TIL)level of breast cancer.Methods A total of 114 female patients with solitary breast cancer were retrospectively included and divided into the low-TIL group(L group,0-10%,n=70)and high-TIL group(H group,>10%,n=44).Conventional MRI features of breast cancer were analyzed based on dynamic contrast-enhanced MRI(DCE-MRI),and histogram features of syMRI quantitative parameters(T1,T2 and proton density[PD])images were extracted,including the 10th and 90th percentile(abbreviated to 10th and 90th,respectively),mean,entropy,kurtosis and skewness,which were then compared between groups.Multivariate logistic regression was used to screen the independent predictors for TIL level among histogram features of syMRI quantitative parameters.Results The proportion of Luminal A breast cancer was lower,while the proportion of HER2-positive breast cancer was higher in H group than those in L group(both adjusted P<0.05).H group had a higher proportion of lesions with regular margin than L group(P<0.05).H group exhibited higher value of T1-10th,T1-90th,T1-mean,T2-10th,T2-mean and lower value of PD-skewness compared to those in L group(all P<0.05).T2-mean was identified as an independent predictor of TIL level of breast cancer(P<0.05),and its area under the curve(AUC)of receiver operating characteristic(ROC)was 0.780.Conclusion Tumor margin characteristics on conventional MRI and histogram feature T2-mean based on syMRI contributed to predicting TIL level of breast cancer.
2.NLUS-VQA: construction and evaluation of a visual question answering model for neonatal lung ultrasound diagnosis

Objective:To develop and evaluate a medical visual question answering (VQA) model for neonatal lung ultrasound (LUS) images to enhance intelligent auxiliary diagnosis of neonatal pulmonary diseases.Methods:Using data from neonates admitted to Beijing Obstetrics and Gynecology Hospital, Capital Medical University (January 2023 to December 2024), an image-question-answer dataset comprising 251 LUS images was constructed [43 pneumonia (17.1%), 42 neonatal respiratory distress syndrome (16.7%), 83 transient tachypnea (33.1%), and 83 normal (33.1%) images] with a four-tier medical question-answer framework. Building upon the Qwen2.5-VL-7B base model and integrating LoRA fine-tuning with chain-of-thought prompting, we developed the NLUS-VQA model to enhance visual-language semantic alignment and enable stepwise clinical reasoning, achieving efficient small-sample adaptation. Model performance was comprehensively assessed through natural language generation metrics (BLEU-4, ROUGE-1/2/L), qualitative evaluation of characteristic recognition, and clinical consistency analysis.Results:(1) Quantitative evaluation demonstrated that NLUS-VQA achieved scores of 22.38 (BLEU-4), 48.26 (ROUGE-1), 22.40 (ROUGE-2), and 37.20 (ROUGE-L), representing significant improvements over baseline models. (2) Qualitatively, the model exhibited strong performance in identifying lung consolidation, coalescent B-lines, and snowflake signs, with its chain-of-thought strategy enhancing clinical interpretability and answer accuracy. (3) Clinically, NLUS-VQA achieved a Cohen's Kappa coefficient of 0.78 and diagnostic accuracy of 80.8% (21/26), indicating substantial agreement with clinical experts.Conclusion:The NLUS-VQA model demonstrates robust interpretability in recognizing key sonographic patterns (e.g. lung consolidation, confluent B-lines, and snowflake signs), providing a scalable framework for small-sample medical image analysis, though diagnostic performance on complex conditions remains limited by dataset scale and minority class representation.
3.Influenza vaccination among children during the flu season of 2023-2024 in Xiqing District of Tianjin

Objective:
To understand the influenza vaccination rate and related factors of children in Xiqing District of Tianjin, so as to provide a scientific basis for improving the influenza vaccination coverage rate among children.
Methods:
A stratified random cluster sampling method was employed to select junior high school students and below students from 15 schools in five directions(east, west, south, north, midland) in Xiqing District, Tianjin, involving 13 075 students. A questionnaire survey was conducted among parents of students to analyze the influenza vaccination rate of children in 2023-2024 flu season (October to March of the following year). A binary Logistic regression model was used to analyze the factors affecting influenza vaccination.
Results:
The influenza vaccination rate was 16.66% for children in Xiqing District of Tianjin, in 2023-2024 flu season, among them, the vaccination rate in kindergartens was the highest (28.86%), followed by primary schools (18.87%) and junior high schools (12.61%). Binary Logistic regression analysis showed that having received the influenza vaccine during the 2020-2022 ( OR=10.55, 95%CI =9.48-11.73), parents awareness of influenza related knowledge ( OR=1.38, 95%CI =1.24-1.54), kindergarten children ( OR= 2.33 , 95%CI =1.91-2.83), primary school students ( OR=1.26, 95%CI =1.12-1.42), parents with a postgraduate degree or above ( OR=1.62, 95%CI =1.13-2.31), and a monthly average family income of 10 000-<20 000 yuan ( OR=1.67, 95%CI = 1.41- 1.99) were positive correlation factors with influenza vaccination of children ( P <0.05). Among parents of children who have not received the flu vaccine, 61.61% fully accepted flu vaccination or accepted but still considered. The most popular way for parents to obtain flu knowledge was flu prevention and control information from professional institutions during the flu season ( 77.38 %).
Conclusions
The influenza vaccination rate among kindergarten, as well as primary and junior high school students in Xiqing District of Tianjin is low. Promotion of influenza vaccination among children and increase the vaccination rate of influenza among children might help reduce the risk of campus flu outbreak.
4.Exploration on Therapy of Tonifying Heart and Spleen in Improving the Emotions of Patients with Postpartum Depression of Heart and Spleen Deficiency Syndrome Based on the Intestinal Flora-Neurotransmitter-Brain Axis

Objective To observe the clinical efficacy of therapy of tonifying heart and spleen for the treatment of patients with postpartum depression of heart and spleen deficiency syndrome based on the theory of intestinal flora-neurotransmitter-brain axis(shortened as brain-intestine axis),and to explore its effect on the improvement of patients'depression and anxiety.Methods A total of 136 patients with postpartum depression of heart and spleen deficiency syndrome who admitted to The 908th Hospital of the Joint Logistics Support Force of the People's Liberation Army of China from June 2021 to July 2023 were randomly divided into the conventional treatment group(control group)and the tonifying heart-spleen group(observation group)according to the random number table method,with 68 cases in each group.The control group was treated with conventional antidepressant drugs,and the observation group was treated with therapy of tonifying heart and spleen by utilization of Shen Qi Jieyu Prescription Granules)on the basis of treatment for the control group.Both groups were treated for six continuous weeks.Before and after the treatment,the two groups were observed in the changes of main symptom scores of traditional Chinese medicine(TCM)syndrome,Hamilton Depression Scale(HAMD)scores,7-Item Generalized Anxiety Disorder Scale(GAD-7)scores,and the levels of serum neurotransmitters of 5-hydroxytryptamine(5-HT),dopamine(DA),norepinephrine(NE),as well as microflora metabolites such as short-chain fatty acids(SCFAs),glutamic acid(GLU),gamma-aminobutyric acid(GABA).Moreover,the clinical efficacy and safety of the two groups of patients were evaluated.Results(1)After six weeks of treatment,the total effective rate of the observation group was 89.71%(61/68)and that of the control group was 73.53%(50/68),and the intergroup comparison(tested by chi-square test)showed that the efficacy of the observation group was significantly superior to that of the control group(P<0.05).(2)After treatment,the scores of TCM syndrome main symptoms such as emotional upset,slow thinking,poor appetite,abdominal distension and loose stools in the two groups were decreased compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.01).(3)After treatment,the HAMD scores and GAD-7 scores in the two groups of patients were all decreased compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.01).(4)After treatment,the serum levels of neurotransmitters 5-HT,DA,and NE in the two groups of patients were all increased compared with those before treatment(P<0.05),and the increase in the observation group was significantly superior to that in the control group(P<0.01).(5)After treatment,the serum levels of microflora metabolites SCFAs,GLU and GABA in the two groups of patients were all increased compared with those before treatment(P<0.05),and the increase in the observation group was significantly superior to that in the control group(P<0.01).(6)The incidence of adverse reactions in the observation group was 8.82%(6/68)and that in the control group was 5.88%(4/68),and the intergroup comparison showed that the difference was not statistically significant(P>0.05).Conclusion The application of therapy of tonifying heart and spleen for the treatment of patients with postpartum depression of heart and spleen deficiency syndrome based on brain-intestine axis exerts certain efficacy and safety through correcting the imbalance of neurotransmitters and regulating the microflora metabolites of the patients,thus to alleviate depression and anxiety.

7.To investigate the effect of Astragaloside Ⅳ on high-glucose-induced cardiomyocytes pyroptosis

Objective:To investigate the effect of Astragaloside Ⅳ on high glucose-induced cardiomyocyte pyroptosis.Methods:H9c2 cells were cultured in vitro and divided into control group(5.5 mmol/L glucose), high glucose group(33.3 mmol/L glucose), Astragaloside Ⅳ group(33.3 mmol/L glucose+ 100μmol/L Astragaloside Ⅳ), and NLRP3 inhibitor group(33.3 mmol/L glucose+ 1μmol/L MCC950). Cell counting kit 8(CCK-8)was used to detect the activity of H9c2 cells.Lactate dehydrogenase(LDH)kit was used to detect the content of LDH in cell supernatant.Superoxide anion fluorescent probe(DHE)was used to detect the level of intracellular reactive oxygen species(ROS). Real-time fluorescence quantitative polymerase chain reaction(RT-qPCR)and Western blot were used to detect the mRNA and protein expression levels of pyroptosis-related genes.Immunofluorescence was used to detect the fluorescence intensity of NLRP3.Enzyme-linked immunosorbent assay(ELISA)was used to detect the level of inflammatory factors in cell supernatant.Results:When the concentration of Astragaloside Ⅳ was 100 μmol/L, it could significantly inhibit the decrease of cardiomyocyte viability induced by high glucose( P<0.01)and reduce LDH release( P<0.01). Compared with the control group, the level of ROS was increased( P<0.01), the mRNA and protein expressions of pyroptosis-related molecules were up-regulated( P<0.01 for all), the fluorescence intensity of NLRP3 was increased( P<0.01), and the levels of inflammatory factors in the cell supernatant were increased in the high glucose group( P<0.01). Compared with the high glucose group, the ROS level was decreased( P<0.01), the mRNA and protein expressions of pyroptosis-related molecules were down-regulated( P<0.05 or P<0.01), the fluorescence intensity of NLRP3 was decreased( P<0.01), and the levels of inflammatory factors in cell supernatant were decreased( P<0.05 or P<0.01)in Astragaloside Ⅳ group and inhibitor group. Conclusions:Astragaloside Ⅳ plays a protective role in high glucose-induced cardiomyocyte injury by inhibiting NLRP3/Caspase-1 signaling pathway and inhibiting pyroptosis.Moreover, it can improve the anti-inflammatory and antioxidant properties in cell models.
8.Ultrasensitive quantification of trace amines based on N-phosphorylation labeling chip 2D LC-QQQ/MS

Trace amines(TAs)are metabolically related to catecholamine and associated with cancer and neuro-logical disorders.Comprehensive measurement of TAs is essential for understanding pathological pro-cesses and providing proper drug intervention.However,the trace amounts and chemical instability of TAs challenge quantification.Here,diisopropyl phosphite coupled with chip two-dimensional(2D)liquid chromatography tandem triple-quadrupole mass spectrometry(LC-QQQ/MS)was developed to simul-taneously determine TAs and associated metabolites.The results showed that the sensitivities of TAs increased up to 5520 times compared with those using nonderivatized LC-QQQ/MS.This sensitive method was utilized to investigate their alterations in hepatoma cells after treatment with sorafenib.The significantly altered TAs and associated metabolites suggested that phenylalanine and tyrosine metabolic pathways were related to sorafenib treatment in Hep3B cells.This sensitive method has great potential to elucidate the mechanism and diagnose diseases considering that an increasing number of physiological functions of TAs have been discovered in recent decades.
9.Establishment of basic tests and extended tests list for clinical laboratories in Shanghai community health service centers

Objective:To develop a list of basic and expanded medical laboratory tests in community health service centers in Shanghai.Methods:The status quo of human and equipment resource allocation, the test items and quality control currently performed, the perspectives of various stakeholders, the capacity building of community clinical laboratory in community health service centers in Shanghai were investigated by quantitative survey and qualitative interview; and the rating scores of each test item were assessed by expert consultation using Delphi method. The expert focus discussion was conducted, and each test item was rated and classified. Finally a list of the basic tests and expanded tests in clinical laboratories of community health service center was developed.Results:A total of 247 questionnaires were distributed and 192 (77.7%) were answered. A list of 94 laboratory test items was screened out based on the questionnaire survey of the laboratories of the community health centers. Thirty one experts in the relevant areas were invited to rate the test items, the average authority coefficient of experts was 0.90, with which the weighted average of the expert ratings was made. There were 45 (47.9%) items scored 7 or higher, 38 (40.4%) scored between 5 and 7, and 11 (11.7%) scored less than 5. Based on the results of the expert focus discussion, 48 items were recommended as the basic tests and 46 items as the extended tests.Conclusion:In this study a list of tests recommended to clinical laboratories in Shanghai community health service centers has been developed, which contains 48 basic tests and 46 extended tests.
10.Evaluation method of evidence body for acupuncture and moxibustion clinical practice guidelines: the stratified evidence evaluation method.

To explore the evaluation method of evidence body for acupuncture and moxibustion clinical practice guidelines (CPGs), and to provide methodological support for the development of acupuncture and moxibustion CPGs and to promote the establishment and application of acupuncture and moxibustion standard. Based on the academic characteristics of TCM acupuncture, the stratified evidence evaluation method was proposed. Firstly, evidences were collected from the ancient literature, modern literature and clinical experience, and were scientifically distinguished and evaluated. Secondly, all the evidences were integrated as body of evidence by analytic hierarchy process (AHP). Lastly, based on the general index (GI), the clinical recommendations for acupuncture were proposed. Based on the stratified evidence evaluation method, acupuncture and moxibustion CPGs were developed in a more scientific and objectiveway with the comprehensive and sufficient evidences.
