1.Clinical Advantages of Traditional Chinese Medicine in Treatment of Childhood Simple Obesity: Insights from Expert Consensus

Childhood simple obesity has become a significant public health issue in China. Modern medicine primarily relies on lifestyle interventions and often suffers from poor long-term compliance, while pharmacological options are limited and associated with potential adverse effects. Traditional Chinese Medicine (TCM) has a long history in the prevention and management of this condition, demonstrating eight distinct advantages, including systematic theoretical foundation, diversified therapeutic approaches, definite therapeutic efficacy, high safety profile, good patient compliance, comprehensive intervention strategies, emphasis on prevention, and stepwise treatment protocols. Additionally, TCM is characterized by six distinctive features: the use of natural medicinal substances, non-invasive external therapies, integration of medicinal dietetics, simple exercise regimens, precise syndrome differentiation, and diverse dosage forms. By combining internal and external treatments, TCM facilitates individualized regimen adjustment and holistic regulation, demonstrating remarkable effects in improving obesity-related metabolic indicators, regulating constitutional imbalance, and promoting healthy behaviors. However, challenges remain, such as inconsistent operational standards, insufficient high-quality clinical evidence, and a gap between basic research and clinical application. Future efforts should focus on accelerating the standardization of TCM diagnosis and treatment, conducting multicenter randomized controlled trials, and fostering interdisciplinary integration, so as to enhance the scientific validity and international recognition of TCM in the prevention and treatment of childhood obesity.

Objective:
To explore the related factors of type 2 diabetes mellitus (T2DM) in overweight and obese children and adolescents, so as to provide scientific evidence for early screening and making targeted comprehensive prevention and control strategies.
Methods:
A total of 332 overweight and obese children and adolescents aged 6-18 years who visited the department of pediatric endocrinology of a tertiary hospital in Liaoning Province from February 2023 to July 2025 were selected. A case control design was adopted, and the subjects were divided into the case group (overweight/obese children and adolescents with T2DM, n =166) and the control group (overweight/obese children and adolescents without diabetes, n =166) based on 1∶1 propensity score matching. Information on demographic characteristics, dietary and lifestyle behaviors was collected through questionnaires. Multivariate Logistic regression analysis was used to analyze the related factors of T2DM in overweight and obese children and adolescents.
Results:
The average body mass index of overweight and obese children and adolescents was (28.50±5.23)kg/m 2. Multivariate Logistic regression analysis showed that family history of diabetes ( OR=6.71, 95%CI =3.35-14.05), age at onset of weight gain <6 years ( OR=4.08, 95%CI =2.13-8.13), hyperphagia ( OR=2.46, 95%CI =1.25-4.97), and preference for fried foods ( OR= 2.42 , 95%CI =1.21-4.99) were associated with an increased risk of T2DM in overweight and obese children and adolescents (all P < 0.05). In contrast, vitamin D supplementation during pregnancy ( OR=0.50, 95%CI =0.25-0.99), frequent sun exposure during pregnancy of mothers ( OR=0.48, 95%CI =0.24-0.97), breastfeeding ( OR=0.35, 95%CI =0.18-0.66), vitamin D supplementation after 3 years of age ( OR=0.31, 95%CI =0.14-0.68), and active commuting to school ( OR=0.29, 95%CI =0.15-0.58) were associated with a reduced risk of T2DM in overweight and obese children and adolescents (all P <0.05).
Conclusions
The occurrence of T2DM in overweight and obese children and adolescents is closely related to early onset obesity, genetic background, dietary behaviors, and physical activity. A continuous comprehensive prevention and control strategy from pregnancy to school age should be established to reduce the risk of the disease.
3.Quality Research and Evaluation of Ketoconazole Lotion Based on National Drug Sampling and Testing

Objective To evaluate the quality of ketoconazole lotion produced by different domestic companies.Methods Legal standards and exploratory research were used to conduct a comprehensive evaluation of 45 batches(40 batch numbers)of ketoconazole lotion for national drug sampling inspection in 2024,including related substances,antioxidant content,packaging oxygen permeability,in vitro permeation test,and viscosity,antibacterial efficacy,irritation,microstructure,etc.Results The legal standard inspection pass rate was 100.0%.Correlation analysis found that the main factors affecting the quality of this product are prescription technology and packaging.Conclusion It is recommended that manufacturers optimize the prescription process as soon as possible,and pay attention to choose suitable packaging materials,effectively improve the quality of ketoconazole lotion.
4.Research on Analysis Method of Ketoconazole Related Substancse Based on National Drug Sampling and Testing

Objective To establish the HPLC analysis method for ketoconazole related substances,and to provide technical support for the improvement of the quality standard and the purity of ketoconazole.Methods Gradient elution conditions were optimized based on the chromatographic parameters outlined in the 2024"British Pharmacopoeia"for ketoconazole cream.A C18 column(4.6 mm×150 mm,3 μm)was used to facilitate the elution process using a gradient of acetonitrile and acetate buffer,and detection performed at a wavelength of 230 nm.Results The known impurities and potentially genotoxic impurities in the material drugs of ketoconazole can be more effectively separated using the established methods,which are both robust and highly sensitive.The content of the two potentially geno toxic impurities,six known impurities and unknown impurities in the 21 batches of ketoconazole sourced from five raw material pharmaceutical companies is below the specified limit requirements.Conclusion This method contributes to improving the quality of ketoconazole ingredients,ensuring the safety of ketoconazole preparations,and better supporting the regulatory supervision.
5.Characteristics of electroencephalogram power spectrum in cerebral infarction of various sites and neurological deficit serverites

Objective To investigate the characteristics of the relative electroencephalogram(EEG)power spectrum in cerebral in-farction patients with different lesion locations and neurological deficit severities.Methods From February,2024 to May,2025,70 patients with cerebral infarction who completed EEG examinations at Shijiazhuang People's Hospital were enrolled.They were divided into cortical group(n=19)and subcortical group(n=51)based on admission MRI/CT imaging,and divided into mild group(n=53)and moderate-to-se-vere group(n=17)according to the scores of National Institutes of Health Stroke Scale(NIHSS)and Barthel In-dex.All patients underwent 32-channel video-EEG monitoring.The power spectral density of α and θ waves,and the(δ+θ)/(α+β)ratio(DTABR)were analyzed in eight electrodes of F3,F4,P3,P4,T3,T4,O1 and O2.Results DTABR was higher in the cortical group than in the subcortical group across all eight electrodes(|Z|≥2.047,P<0.05),while the relative power decreased in α waves(|Z|≥1.968,P<0.05),and increased in θ waves(|Z|≥1.988,P<0.05)in the cortical group.DTABR was higher in the moderate-to-severe group than in the mild group(|Z|≥2.263,P<0.05).Conclusion Elevated DTABR suggests cortical involvement or more severe neurological impairment,and decreased αwave and increased θ wave activity are primarily observed in patients with cortical lesions,potentially serving as EEG indicators for identifying cortical involvement.

9.The early clinical outcome of ACDF under a cervical soft endoscopic minimally invasive system in the treatment of two-segment cervical myelopathy

Objective:To investigate the early clinical outcomes of anterior cervical discectomy and fusion (ACDF) in the treatment of two-segment cervical spondylotic myelopathy (CSM) using a cervical soft endoscopic minimally invasive system.Methods:A retrospective follow-up study was conducted on the medical records of 23 patients with two-segment cervical myelopathy who underwent ACDF using a soft endoscopic cervical spine minimally invasive system at the Third Affiliated Hospital of Southern Medical University between October 2022 and December 2023. The cohort included 15 males and 8 females, aged 37-79 years (58.52±11.77 years). The affected cervical segments included: C 3, 4 and C 4, 5 in 2 cases; C 3, 4 and C 5, 6 in 3 cases; C 4, 5 and C 5, 6 in 10 cases; C 5, 6 and C 6, 7 in 7 cases; and C 4, 5 and C 6, 7 in 1 case. Clinical outcomes were evaluated based on the Japanese Orthopaedic Association (JOA) scores and visual analog scale (VAS) for neck and shoulder pain, assessed preoperatively and at 1 week, 1 month, and 3 months postoperatively. Additional data recorded included incision length, intraoperative blood loss, operative time, postoperative complications, and the presence of prevertebral soft tissue edema. The improvement rate of JOA scores at the final follow-up was also calculated. Results:All patients successfully underwent surgery and completed follow-up, with follow-up durations ranging from 3 to 6 months (4.01±0.98 months). The mean operative time was 80.09±22.66 min (range: 53-127 min), and the mean incision length was 3.25±0.32 cm (range: 3-4 cm). Estimated blood loss ranged from 10 to 100 ml, with a mean of 34.78±24.1 ml. Postoperative drainage ranged from 0 to 80 ml (mean: 23.13±26.1 ml), and postoperative hospitalization durations ranged from 4 to 12 days (6.83±2.59 days). JOA scores improved significantly from a preoperative median of 9.00(8.00, 10.00) to 12.00(11.00, 14.00) at 1 week, 13.00(12.00, 14.00) at 1 month, and 15.00(15.00, 16.00) at 3 months postoperatively (χ 2=220.492, P<0.001). VAS scores for neck and shoulder pain also improved significantly from a preoperative median of 5.00(4.00, 6.00) to 3.00(2.00, 3.00) at 1 week, 2.00(2.00, 3.00) at 1 month, and 2.00(1.00, 2.00) at 3 months postoperatively (χ 2=170.869, P<0.001). No postoperative complications such as dysphagia, hoarseness, nerve injury, cerebrospinal fluid leakage, or intraspinal hematoma were observed. Imaging revealed no significant prevertebral soft tissue edema. At the final follow-up, the improvement rate of JOA scores resulted in 14 cases rated as excellent and 9 as good. Conclusions:ACDF using a cervical soft endoscopic minimally invasive system demonstrates satisfactory clinical outcomes for the treatment of two-segment CSM. This technique reduces the incidence of common complications associated with both open and traditional endoscopic surgeries.
10.Efficacy and safety of liposomal bupivacaine in transversus abdominis plane block for post-cesarean analgesia

Objective To comparatively evaluate the clinical efficacy of liposomal bupivacaine and ropiva-caine in transversus abdominis plane(TAP)block combined with intravenous patient-controlled analgesia(PCA)following cesarean section,and to explore the analgesic advantages of liposomal bupivacaine.Methods Eighty parturients scheduled for elective cesarean section were recruited and randomly allocated into two groups via a random number table:the liposomal bupivacaine group and the ropivacaine group.At the conclusion of the surgical procedure,both groups underwent ultrasound-guided bilateral TAP block.In the ropivacaine group,20 mL of 0.5%ropivacaine was administered per side.In the liposomal bupivacaine group,266 mg of liposomal bupivacaine was dissolved in 0.9%normal saline to a total volume of 40 mL,with 20 mL injected per side.The following parameters were compared between the two groups:Visual Analog Scale(VAS)scores at rest and during movement at various postop-erative time points,the overall scores of the 15-item Quality of Recovery(QoR-15)scale,postoperative opioid consumption,the time to first ambulation,the time to first flatus,and the incidence of adverse drug reactions such as nausea,vomiting,constipation,and pruritus.Results In comparison with the ropivacaine group,the liposomal bupivacaine group exhibited significantly lower Visual Analogue Scale(VAS)scores both at rest and during move-ment at 12 hours,24 hours,and 48 hours postoperatively(P<0.001).Significantly higher Quality of Recovery-15(QoR-15)scores were recorded in the liposomal bupivacaine group at 24 hours and during the 24-48-hour period postoperatively(P<0.001).The postoperative opioid consumption within 48 hours was markedly lower in the liposo-mal bupivacaine group(P<0.001).The time to first flatus was significantly shorter in the liposomal bupivacaine group(P<0.001).No significant differences were detected in the incidence of nausea,vomiting,or constipation between the two groups(P>0.05),and no cases of pruritus or other severe adverse reactions were observed.Conclusion Liposomal bupivacaine used for TAP block following cesarean section offers extended analgesia,reduces the need for opioids,enhances the quality of postoperative recovery,promotes gastrointestinal motility,and demonstrates excellent safety.
