"Dietary inquiry" is a core component of the diagnostic system in traditional Chinese medicine (TCM), which can be divided into three aspects including appetite, eating capacity, and food preference. Abnormalities in appetite are mainly attributed to dysfunction of the mind and impaired regulatory mechanisms. Clinical inquiry should focus on hunger sensation and the willingness to eat voluntarily. Treatment should aim to soothe the liver, regulate the spleen, nourish and calm the mind. Abnormalities in eating capacity are related to disorders of qi movement and structural dysfunction, for which inquiry should focus on whether food descends smoothly and on postprandial reactions, and the corresponding treatment is descending qi, relieving fullness, and promoting bowel movement and digestion. Abnormalities in food preference arise from damage caused by the five flavors and imbalance of visceral qi, for which inquiry should focus on dietary preferences and whether eating brings comfort. It is important to distinguish between "stomach preference" and "oral preference", and treatment should carefully differentiate flavor tendencies and correct imbalances through appropriate dietary flavors. By refining the content of dietary inquiry, this study explores how different dimensions of eating status reflect the holistic concept and syndrome differentiation-based treatment in TCM, providing a reference for the clinical diagnosis and treatment of spleen and stomach diseases and related disorders.

3.Establishment and optimization of the method for related substances in myristyl alcohol

Objective:To establish a GC method for the determination of related substances in myristyl alcohol.Methods:Use a column packed with polyethylene glycol as the stationary phase(HP-INNOWAX,0.25 mm×30 m,0.25 μm).Maintain the column temperature at 90℃,raise the temperature to 180℃by 5℃per minutes,maintain for 25 minutes.The temperatures of injection port is 270℃.The temperatures of detector is 280℃.Cal-culate the content by normalization method.Result:Lauryl alcohol,myristyl alcohol,pentadecanol,cetyl alcohol,stearyl alcohol and oleyl alcohol showed good linear relationships within the ranges of 6.2-123.8,4.2-134.2,5.2-104.2,4.5-90.3,4.8-96.0,4.8-95.2 μg·mL-1,respectively,with the correlation coefficient above 0.999 8.The limit of detection of aforementioned fatty alcohols were 0.2,0.2,0.2,0.3,0.5 and 0.5 μg·mL-1,which were more sensitive than the method in USP monotherapy Myristyl Alcohol.The content of lauryl alcohol was 0.2%-0.6%and that of cetyl alcohol was 0.1%-0.9%in 5 batches of samples.Sum of re-lated fatty alcohols and unspecified impurities was 0.7%-2.5%.Conclusion:The method established in this study has good separation effect,high specificity and high sensitivity,and solved the problem of difficult identifica-tion of related fatty alcohols caused by high noise at the baseline of the method in USP monotherapy myristyl alco-hol.It can be used for the the determination of related substances in myristyl alcohol.It also provides reference and guidance for the quality management and control of other fatty alcohols.
4.A network-based prognostic prediction model for gastric signet ring cell carcinoma after laparoscopic surgery

Objective:The purpose of this study was to develop a dynamic prediction model for patients with gastric signet ring cell cancer (GSRCC)following laparoscopic radical gastrectomy in order to improve the precision and usefulness of prognoses prediction for overall survival and disease-free survival.Methods:From 2011 to 2018, 914 National Cancer Center patients participated in the study. To find independent prognostic indicators and create a prognostic nomogram model, univariate and multivariate regression analyses were performed. Calibration curves, receiver operating characteristic curves, and consistency indices were used to assess the model's performance. To make clinical application more convenient, two web-based prediction tools were created.Results:A training set of 639 cases and a validation set of 275 instances were randomly selected from among the patients. Important predictive variables such as age, tumor size, location, pN and pT staging, and postoperative chemotherapy were all incorporated in the model (all P<0.05). The model's consistency index and area under the receiver operating characteristic curves were both higher than 0.7, and the calibration curves demonstrated a good fit between the expected and actual values, indicating high accuracy and consistency in postoperative survival prediction for patients with gastric signet ring cell carcinoma. Conclusion:We successfully developed two dynamic prediction models in this study, which improved its clinical practicability using web-based tools and is anticipated to be crucial to clinical practice going forward.
5.Construction of Clinical Questions and Outcome Indicators of Clinical Practice Guidelines of Acupuncture and Moxibustion Treatment for Irritable Bowel Syndrome Based on the Modified Delphi Method

Objective To explore and construct the clinical questions and outcome indicators of the Clinical Practice Guideline of Acupuncture and Moxibustion Treatment for Irritable Bowel Syndrome;To provide a basis for the subsequent preparation of this guide to form recommendations.Methods First,by searching the databases of seven major Chinese and English journals,including CNKI,the preliminary list of clinical problems and outcome indicators in the Clinical Practice Guideline of Acupuncture and Moxibustion Treatment for Irritable Bowel Syndrome were sorted out,and then the clinical questions and outcome indicators that formed the recommendations of the guide were finally determined based on the modified Delphi method in the form of three rounds of online.The first two rounds were conducted in the form of online questionnaires filled out by experts,and the importance ratings of clinical issues and outcome indicators were imported into the SPSS 27.0 software for statistical analysis.The first and second rounds of clinical questions and outcome indicators were rated as the average score≥4,full score frequency≥30%,and the coefficient of variation≤25%,respectively;the inclusion criteria for entering the second round of evaluation were an average score of≥7 and an average score of≤25%.The third round would be further discussed and voted on by experts in an online consensus meeting,with a voting rate of≥80%as the standard to determine the final items to be included in the guidelines.Results A total of 109 questionnaires were distributed nationwide in the first round of inquiry,and 107 were collected;a total of 20 questionnaires were distributed for the second round of expert research,and 20 were collected.The positive coefficients of the first and second rounds of experts were 98.17%and 100%;the Cronbach coefficients of clinical questions were 0.937 and 0.943,respectively;the Cronbach coefficients of the outcome indicators were 0.970 and 0.940,respectively.In the third round,a total of 22 experts participated in the meeting and all voted,resulting in a positive coefficient of 100%and an authority coefficient of 0.88.13 clinical questions and 17 outcome indicators were finally included in Clinical Practice Guideline of Acupuncture and Moxibustion Treatment for Irritable Bowel Syndrome.Conclusion According to the results of the above three rounds of modified Delphi method,it indicates that the questionnaire survey in the process of formulating the guidelines is highly reliable,which can provide a reliable basis for the writing of this guide,and to provide a reference for the development of acupuncture guidelines in related fields.
6.Analysis on Acupoint Selection Law for Acupuncture Treatment of Sinusitis Based on Data Mining

Objective To analyze the acupoint selection law in acupuncture treatment for sinusitis using data mining techniques.Methods Relevant literature on acupuncture treatment for sinusitis was retrieved from CNKI,Wanfang Data,VIP and CBM from January 1,2000 to December 20,2024.Descriptive statistics were performed on the frequency of acupoint usage,meridian tropism,and regional distribution.SPSS Modeler 18.0,Cytoscape 3.10.3 and Origin Pro were employed to conduct association rule mining,co-occurrence network analysis,and systematic cluster analysis on the acupoints.Results A total of 69 articles were included,yielding 114 acupoint prescriptions involving 65 distinct acupoints.The most frequently used acupoints were Yingxiang,Yintang and Hegu,etc.;the most commonly used meridians were large intestine meridian,Governor Vessel,gallbladder meridian and bladder meridian.Acupoint selection was predominantly concentrated in the head/neck region,lower limbs and upper limbs.Among the specific acupoint categories,Jiaohui acupoint,Wushu acupoint and Yuan acupoint were used most frequently.The acupoint combinations with the strongest associations were Hegu-Yingxiang,Hegu-Yintang-Yingxiang and Fengchi-Yingxiang.The top 22 high-frequency acupoints could be grouped into 5 clusters.Conclusion Acupuncture treatment for sinusitis can exert the functions to disperse wind and unblock the orifices,drain heat and resolve turbidity,expel pathogens and alleviate pain,tonify deficiency and dissipate cold,as well as harmonize qi and blood.The characteristics of acupoint combination include a primary focus on local points supplemented by distal points,pattern differentiation-based selection,and a propensity for unblocking yang qi.The core acupuncture formula is Yingxiang-Yintang-Hegu-Fengchi.

8.Clinical effect of upper lip aesthetic unit filled with autogenous dermis

Objective:To explore the clinical effect of upper lip aesthetic unit filled with autogenous dermis.Methods:The clinical data of patients with poor upper lip morphology admitted to the Department of Plastic Surgery of the Second Hospital of Nanjing from January 2022 to July 2024 were analyzed retrospectively. A longitudinal fusiform incision was made approximately 5.0 cm long in the sacrococcygeal region; the required dermal tissue was excised; and the dermis was trimmed and shaped according to the filling location. For thickening the vermilion border, the dermis was trimmed into a fusiform shape. For filling the philtral columns, the dermis was trimmed into a flat strip shape. A longitudinal incision (approximately 0.5 cm) was made in the center of vermilion. Then the tissue was separated in the layer between the orbicularis oris muscle and the mucosa to the direction of oral commissure (filling the vermilion) or separated to the philtrum ridge (filling the philtrum ridge), forming a tunnel and filling the dermal tissue in the tunnel. For subtle vermilion tubercle, the dermis could be overlapped and filled in an appropriate location in vermilion tubercle. The incision healing and complications were observed, and the morphology of upper lip and philtrum ridge was followed up to investigate whether the patients were satisfied with the surgical outcomes.Results:A total of 11 patients, all female, aged 19-57 years (mean, 38.1 years) were included. In 7 patients, the upper lip was filled with dermal tissue, the upper lip and the philtrum ridge were filled in 3 cases, and the upper lip lifting was performed in 1 case with philtrum ridge. Except for one case where the vermilion tubercle was not prominent and a two-layer dermal overlay was used to fill the vermilion tubercle, all other cases received single-layer dermis filling. The patients were followed up for 6 to 17 months. The lip swelling occurred in the early postoperative period, which could be improved after 2 weeks. The incisions were healed primarily, and no complications such as hematoma and infection occurred. The appearance of the upper lip was smooth, natural and soft. The fullness of the upper lip, the protuberance of the upper lip, the stereopsis of the vermilion tubercle and the philtrum ridge were significantly improved. No dermal contracture, cyst or other long-term complications occurred. All patients were satisfied with the appearance of upper lip.Conclusion:The use of autologous dermis to fill the upper lip aesthetic unit can effectively improve the upper lip apperance, with stable long-term results, fewer complications and high patient satisfaction.
9.Dilemmas and strategies for cross-regional medical insurance fund collaborative supervision: based on the revised SFIC model

With the expansion of cross-regional medical services in China, the supervision of medical insurance funds is facing systemic challenges including insufficient inter-regional coordination and ambiguous multi-stakeholder responsibilities, necessitating the establishment of a collaborative regulatory framework tailored to China′s governance context. The authors constructed a revised " 4+ 1" collaborative regulatory model by integrating complex regulatory network spatial theory with the collaborative governance SFIC model, which included incorporating initial conditions, intervention leadership, institutional design, collaborative processes, and outcome feedback. The research systematically deconstructed China′s medical insurance fund regulatory practices, found reveal persistent challenges in the current collaborative regulatory system, including institutional friction among stakeholders, blocked cross-regional rule transmission, and insufficient dynamic feedback mechanisms, which collectively contributed to policy implementation deviations and diminished regional coordination efficacy. The authors proposed specific optimization paths, including innovating three-dimensional driven institutional supply, restructuring rights and responsibilities to assist collaborative leadership, implementing dynamic negotiation driven effective collaboration, and constructing a feedback loop through dual dimensional coupling.
10.Research progress of exosomal non-coding RNA in diabetes and its complications

The high incidence of diabetes mellitus(DM)and its chronic complications imposed a heavy burden on families and society,yet the specific mechanism remain unclear.Exosomal non-coding RNA(ncRNA)is involved in various mechanisms of DM pathogenesis,playing a crucial role in the autoimmune process of type 1 diabetes mellitus and the development of insulin resistance and β-cell dysfunction in type 2 diabetes mellitus.Exosomal ncRNA shold potential for improving DM and its complications,particularly those derived from mesenchymal stem cell.This article reviews the research on exosomal ncRNA in DM and its complications.
