1.Colonization, drug resistance, and molecular epidemiological characteristics of methicillin-resistant Staphylococcus aureus among dairy farm workers in Xinjiang

Background Methicillin-resistant Staphylococcus aureus (MRSA) is an important pathogen for both human bloodstream infections and mastitis in cows. However, little attention has been paid to the cross-host transmission of MRSA from cows to high-risk groups in China. Objective To determine the MRSA colonization rates among dairy cows and dairy farm workers in Xinjiang, identify the antibiotic resistance profiles and molecular characteristics of the isolates, and provide scientific evidence for the formulation of targeted infection control strategies. Method A cross-sectional survey combined with laboratory pathogen analysis was conducted. From June to August 2024, large-scale dairy farms in Xinjiang region were selected as study sites. Nasal swabs (n=96) and skin swabs (n=39) were collected from workers, and bovine nasal swab samples (n=109) were collected simultaneously. All samples were subjected to MRSA isolation, cultivation, and identification, followed by antibiotic susceptibility testing to characterize resistance phenotypes. Staphylococcus aureus protein A (Spa) typing was performed to determine strain genotypes and elucidate MRSA colonization rates and molecular epidemiological patterns. Results A total of 35 MRSA strains was successfully isolated from 244 samples. The MRSA colonization rates among dairy farm workers and dairy cows were 20.83% (20/96) and 12.84% (14/109), respectively, with an overall isolation rate of 14.34% (35/244). Among the workers, the nasal colonization rate was 16.67% (16/96), and the skin colonization rate was 12.82% (5/39). One worker exhibited MRSA colonization at multiple body sites. All MRSA strains were resistant to cefoxitin (100%, 35/35). The resistance rates to erythromycin and clindamycin were 42.86% (15/35) and 34.29% (12/35), respectively. Thirteen strains showed a multidrug-resistant phenotype, whereas all strains were susceptible to vancomycin. The MRSA isolates exhibited high genetic diversity, with 13 Spa types identified, among which t441 was the most prevalent (8 strains). Both t441 and t034 types were detected in samples from both the dairy cows and their handlers. These two Spa types also carried and stably inherited specific resistance combinations, including erythromycin–clindamycin–cefoxitin and ciprofloxacin–erythromycin–clindamycin–gentamicin–cefoxitin–tetracycline, and a statistically significant association was also observed between the two resistance profiles and the bacterial types (P < 0.001). In addition, one novel Spa type strain was identified. Conclusion MRSA colonization rates among dairy cows and dairy farm workers in Xinjiang are relatively high, with evidence of multi-site colonization. The isolates exhibit high levels of multidrug resistance and genetic diversity, indicating a potential risk of cross-host transmission.
2.Application exploration of the construction of loose-leaf digital textbooks in Sino-foreign cooperative education program of clinical medicine

This study conducted in-depth interviews to analyze the use of textbooks and learning patterns among medical students from various countries and regions and then proposed loose-leaf digital textbooks as a novel pattern of textbooks. Loose-leaf digital textbooks can meet the requirements of both systematic theoretical education in domestic curricula and the self-directed learning approach in foreign education systems. Compared with traditional textbooks, such digital textbooks have the advantages of flexibility, timeliness, and strong interactivity, thereby enhancing learning efficiency and teaching effectiveness among students. The study further explored the modular design and application strategies for loose-leaf digital textbooks and suggested to achieve personalized education through the integration of digital technologies and improve the adaptability and international competitiveness of textbooks. This study provides innovative ideas for teaching reform in Sino-foreign cooperative education programs of clinical medicine.
3.Study on the diagnostic efficacy and influencing factors of quantitative infrapatellar fat pad fat fraction and T2* values using mDixon-Quant technique for knee osteoarthritis

Objective To investigate the diagnostic efficacy of fat fraction(FF)and T2* values of infrapatellar fat pad(IPFP)based on MR mDixon-Quant technique for knee osteoarthritis(KOA),and to analyze their related influencing factors.Methods A total of 178 knee joints from 114 volunteers were included and underwent knee joints X-ray examination and MR mDixon-Quant sequence scanning.The correlation between FF and T2* values of IPFP with gender,age,body mass index(BMI),and K-L grade was analyzed by Spearman's test and multiple linear regression.Receiver operating characteristic(ROC)curves were plotted to assess the diag-nostic efficacy of FF and T2* values for KOA.Results FF value of IPFP was negatively correlated with age,BMI,and K-L grade(r=-0.470,-0.276,-0.679,P<0.001),and T2* value was also negatively correlated with age,BMI,and K-L grade(r=-0.291,-0.173,-0.505,P<0.001).Multiple linear regression analysis results showed that only K-L grade had a statistically significant correlation with FF and T2* values(P<0.001).ROC curve analysis indicated that the area under the curve(AUC)for diagnosing KOA using the FF and T2* values of IPFP were 0.859 and 0.767,respectively,with sensitivity of 0.878 and 0.841,and specificity of 0.719 and 0.573,respectively.The combined diagnosis had an AUC of 0.855,a sensitivity of 0.625,and a specificity of 0.963,improving the diagnostic specificity for KOA.Conclusion FF and T2* values of IPFP can serve as objective imaging indicators for KOA assessment,and their reduction is correlated with the progress of age,BMI,and K-L grade,of which the K-L grade is an independent influencing factor.The combination of FF and T2 can improve the diagnostic specificity of KOA,and provide a quantitative basis for early KOA detection and severity stratification.
4.Exploration of the integrated general and specialist comprehensive management model for concomitant diseases of breast cancer: 2 cases analysis and literature review

With the extended survival period of breast cancer patients and the increasing health demands, the concomitant diseases of breast cancer have gradually attracted the attention of both doctors and patients, and it is imperative to conduct comprehensive management of these diseases, in which the general practitioners, as the more comprehensive and complex medical talents, have not yet played their due roles. In this article, we report two cases of comprehensive management of concomitant diseases of breast cancer through collaboration of general practitioners and specialists (integrated general and specialist care). The role and function of general practitioners in this process were deeply analyzed, and the establishment of a consultation-liaison general practice model to further promote the role of integrated general and specialist care in integrated oncology care was advocated.

With the improvement of diagnosis and treatment level, most breast cancer patients survive in a chronic state for a long time, and the issue of concomitant diseases of breast cancer (CDBC) has become increasingly prominent. All-round and full-cycle management of these comorbidities can help improve patients’ quality of life and prognosis. General practice, with its long-term, comprehensive and responsible health care that is person-centered, family-based, community-wide and oriented to the maintenance and promotion of overall health, presents new opportunities for the health management of CDBC. This article will explore the application of consultation-liaison general practice through the integrated general and specialist care in the comprehensive management of CDBC, aiming to raise people’s awareness of it and promote the development of consultation-liaison general practice and the management model of the "Internet plus general practitioner team" , which will surely contribute to the all-round management of concomitant diseases in breast cancer patients.
6.Research progress on the bidirectional relationship between sleep-related breathing disorders and pulmonary hypertension

Sleep-related breathing disorders(SBDs)are a group of disorders characterized by abnormal breathing patterns during sleep,with obstructive sleep apnea(OSA)being the most common type.Pulmonary hypertension(PH)is characterized pathophysiologically by pulmonary vasoconstriction and vascular remodeling,with initial clinical diagnosis typically relying on echocardiography.SBDs and PH have a bidirectional relationship in many aspects,with shared pathophysiological mechanisms such as hypoxia,inflammation and oxidative stress being common.The percentage of sleep time with nocturnal oxygen saturation<0.90(T90)can assess the risk of pulmonary hemodynamic and right ventricular function deterioration in OSA patients,but this indicator still has limitation in clinical application.Currently,there is limited domestic research on the bidirectional relationship between SBDs and PH in China.This article reviews the bidirectional relationship between SBDs and PH in terms of epidemiology,potential mechanisms,screening and treatment.It also analyzes the relevance of OSA and PH based on current domestic and international clinical studies,providing a theoretical basis for further exploration of these two diseases.
7.Effects of human umbilical cord-derived mesenchymal stem cells on chronic intermittent hypoxia in mice

Objective To investigate the therapeutic potential of human umbilical cord-derived mesenchymal stem cells(hUCMSCs)in modulating the cGAS-STING-NF-κB signaling pathway in chronic intermittent hypoxia(CIH)mice.Methods Twenty-four C57BL/6 mice were divided into the control group,the model group,the hUCMSCs group and the hUCMSCs+STING agonist(DMXAA)group,with 6 mice in each group.Except for the control group,the other groups were exposed to hypoxic conditions for 8 hours daily for a total of 8 weeks to establish the CIH mouse model.After 8 weeks,mice were anesthetized for cardiac blood collection followed by euthanasia and lung tissue collection.Serum levels of IL-6,TNF-α,IL-1β and IL-17A were measured by ELISA.Pulmonary inflammatory infiltration and collagen deposition were assessed by HE and Masson staining.E-Cadherin and α-SMA expression levels were evaluated by immunohistochemistry.Expression levels of cGAS,STING and NF-κB mRNA were detected by RT-qPCR,while protein expression levels of E-Cadherin,N-Cadherin,α-SMA,Vimentin,cGAS,STING and NF-κB were analyzed by Western blot assay.Results Compared with the control group,levels of IL-6,IL-1β,TNF-α and IL-17A increased in the model group,inflammation and fibrosis scores increased,mRNA expression levels of cGAS,STING and NF-κB increased,and protein expression levels of N-Cadherin,α-SMA,Vimentin,cGAS,STING and NF-κB increased.In contrast,E-Cadherin protein expression was significantly decreased(P<0.05).Compared with the model group,IL-6,IL-1β,TNF-α and IL-17A decreased in the hUCMSCs group,mRNA expression levels of cGAS,STING and NF-κB were decreased,protein expression levels of N-Cadherin,α-SMA,Vimentin,cGAS,STING and NF-κB were also decreased.Meanwhile,E-Cadherin protein expression was significantly increased(P<0.05).STING activator DMXAA reversed the protective effects of hUCMSCs in CIH mice(P<0.05).Conclusion Intravenous administration of hUCMSCs alleviates pulmonary inflammatory infiltration and epithelial-mesenchymal transition in mouse model of intermittent hypoxia,which may be related to the down-regulation of the cGAS-STING-NF-κBsignaling pathway.
