1.Body image disturbance status in AS patients and analysis of its influencing factors

Objective To understand the status of body image disturbance and its influencing factors in patients with ankylosing spondylitis (AS), so as to provide a scientific basis for the clinical management of AS. Methods A total of 353 AS patients admitted from January 2022 to December 2024 were selected as research subjects. Chinese version of Body Image Disturbance Questionnaire (BIDQ) was used to investigate the body image disturbance in AS patients. Single factor analysis was performed by t test and analysis of variance, and multiple factors were analyzed by multivariate linear regression. Results The total score of BIDQ in 342 AS patients was (25.01±4.22). Multivariate linear regression analysis results showed that self-paid medical expense, nighttime VAS score and negative emotion PANAS score could positively predict body image disturbance in AS patients (standardized regression coefficient=0.413, 0.413, 0.460, P<0.05), and PSSS score, positive emotion PANAS score and exercise management CDSSM score could negatively predict body image disturbance (standardized regression coefficient=-0.245, -0.134, -0.247, P<0.05). Conclusion The body image disturbance in AS patients is worthy of clinical attention. Nighttime pain, negative emotion and self-paid medical treatment can increase the risk of body image disturbance. Positive emotion, social support and high self-management level of exercise behavior can reduce the formation of body image disturbance, which can provide new ideas for clinical management of AS patients.
2.Study on the mechanism of Naozhenning granules in improving learning and memory impairment in multiple cerebral concussion model rats

OBJECTIVE To investigate the mechanism by which Naozhenning granules (NZN) improve learning and memory impairment in a rat model of multiple cerebral concussion (MCC). METHODS The MCC rat model was established using the closed controlled cortical impact method. The experiment was set up with a blank group (normal saline), a model group (normal saline), a piracetam group (positive control group, 0.324 g/kg), and high-, medium-, and low-dose NZN groups (5.4, 2.7, 1.35 g/kg), with 11 rats in each group. Drugs or normal saline were administered by gavage once daily for 28 consecutive days. General condition and body weight were monitored throughout the experiment. The sucrose preference rate and novel object recognition index were measured; Evans blue (EB) extravasation in the cerebral cortex was detected; pathological changes of cortical neurons were observed; the levels of B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), interleukin-6 (IL-6), IL-10, and tumor necrosis factor-α (TNF-α) in the cerebral cortex were determined; and the phosphorylation levels of AMP-activated protein kinase (AMPK), glycogen synthase kinase 3β (GSK3β), and Tau protein were detected. RESULTS Compared with the blank group, the model group showed poor mental state, sluggish response to external stimuli, reduced food and water intake, decreased limb flexibility, and disheveled fur. Body weight, sucrose preference rate, and novel object recognition index were significantly decreased ( P <0.05); EB extravasation in the cerebral cortex was significantly increased ( P <0.05), with severe neuronal damage. The positive area ratio of Bax protein, IL-6 and TNF-α levels, and Tau protein phosphorylation level were all significantly increased ( P <0.05), whereas the positive area ratio of Bcl-2 protein, IL-10 level, and AMPK and GSK3β protein phosphorylation levels were significantly decreased ( P <0.05). Compared with the model group, all NZN dose groups showed improvements in general condition and pathological damage, with quantitative indices partially restored, and the differences in quantitative indices in high-dose NZN group were statistically significant ( P <0.05). CONCLUSIONS NZN can effectively improve learning and memory impairment in MCC model rats. The mechanism may be related to activating the AMPK/GSK3β pathway, inhibiting inflammatory response, reducing Tau protein phosphorylation level, and then repairing the neuronal injury.
3.Strategies Study on DRG Subdivision of Gastrointestinal Surgery Cases

Objective To explore strategies for further subdivision of DRG in gastrointestinal surgery cases,providing references to enhance the differentiation of DRG subgroups.Methods A total of 5 108 gastrointestinal surgery cases were selected from a tertiary grade A hospital and a tertiary hospital in Hubei Province,spanning from January 2019 to June 30,2023,and another secondary hospital's data from 2020 and 2021.It employs single factor analysis and multiple linear regression analysis to identify factors influencing case costs.Additionally,the opinions of nine clinicans were gathered regarding factors affecting resource consumption in gastrointestinal surgery cases.The four selected case groups were further subdivided considers the peak characteristics of disease costs.It compares subdivided groups with the DRG Payment Subgroups Scheme(Version 2.0).Results Groups GB1,GB2,GC1,and GC2 were subdivided into 7,4,7,and 6 DRG groups,respectively.The coefficient of variation of each subdivided DRG were reduced,homogeneity was increased,and inter-group differentiation was increased.The results were consistent with the DRG Payment Subgroups Scheme(Version 2.0).Conclusion Based on DRG grouping,the DRG groups can be further subdivided according to the peak characteristics presented by case costs.This subdivision strategy is helpful to provide new ideas for case grouping of Medicare payment.
4.Effects of Bushen Huoxue Formula on myelin and cognitive function of mice with chronic cerebral hypoperfusion

Objective To determine the effects of Bushen Huoxue Formula(BHF),a clinically vali-dated traditional Chinese herbal compound,on myelin integrity and cognitive function in mice with chronic cerebral hypoperfusion(CCH).Methods A total of 30 SPF male C57BL/6J mice were randomly divided into sham operation group(n=8)and model group(n=22).Bilateral com-mon carotid artery stenosis was inflicted to establish a mouse model of CCH,while the mice in the sham operation group only received separation of bilateral vagus nerves.In the model group,6 mice died in 2 d after surgery,the left 16 mice were then randomly divided into a model group and a treatment group,with 8 animals in each group.In 2 weeks after surgery,BHF(0.2 ml/d)or pure water was administered via gavage for 60 d to the corresponding groups.Cognitive function was assessed using the Morris water maze(MWM)test.White matter lesions were evaluated by diffu-sion tensor imaging-fractional anisotropy(DTI-FA).Demyelination in the corpus callosum was visualized using Luxol fast blue(LFB)staining,while neuronal damage in the hippocampal CA1 region was analyzed with Nissl staining.Results Compared to the sham group,the model group exhibited significantly prolonged swimming distance and escape latency on day 5 and reduced platform crossings on day 6 in Morris water maze test,decreased DTI-FA value,lower absorbance value of myelin in LFB staining,and fewer Nissl-stained neurons in the hippocampal region(P<0.05,P<0.01).In contrast,the treatment group showed significantly shorter swimming distance[188.14(105.19,342.00)cm vs 280.22(168.47,501.37)cm,P<0.05]and escape latency[10.22(5.77,19.47)s vs 19.39(13.57,31.09)s,P<0.01]on day 5,increased platform crossings on day 6[3.00(2.00,4.00)times vs 2.00(1.00,3.00)times,P<0.05],elevated DTI-FA value(0.340±0.014 vs 0.313±0.012,P<0.01),enhanced absorbance value of myelin[0.353(0.328,0.364)vs 0.305(0.290,0.350),P<0.05],and greater neuronal counts(9.94±2.22 cells vs 7.11±2.02 cells,P<0.01)when compared to the model group.Conclusion BHF can alleviate CCH-induced cogni-tive deficits in mice by promoting repair of white matter microstructure and rescue of neuronal loss.
5.Research progress of Vitamin D in chronic rhinosinusitis with nasal polyps

Chronic rhinosinusitis with nasal polyps is a common chronic inflammatory disease involving the nasal cavity and paranasal sinuses, with unknown etiology and poor clinical treatment effect. In addition to calcium and phosphorus metabolism, vitamin D is involved in immune modulation and anti-inflammatory responses. We review recent research on the improvement of tissue remodeling, innate and adaptive immune changes caused by long-term chronic inflammation with vitamin D, and summarizes the effects and molecular mechanisms of vitamin D on the inflammatory process of sinusitis.
6.Left atrial volumetric/mechanical coupling index for predicting left atrial dysfunction in patients with chronic kidney disease

Objective To observe the value of left atrial volume/mechanical coupling index(LACI)for predicting left atrial(LA)dysfunction in patients with chronic kidney disease(CKD).Methods Totally 213 CKD patients(CKD group)and 50 healthy controls(control group)were enrolled.Clinical data,laboratory indicators and echocardiographic parameters were compared between groups.According to quartile values of LACI,patients in CKD group were further divided into 4 subgroups,and their clinical,laboratory indicators and echocardiographic parameters were compared.The correlations between LACI and laboratory markers of myocardial injury as well as echocardiographic parameters were analyzed.Taken LAVI>34 ml/m2 as LA dysfunction,the efficacy of LACI,LA strain and LA stiffness index(LASI)for predicting LA dysfunction in CKD patients were evaluated.Results Statistical differences of gender,blood pressure,creatinine,estimated glomerular filtration rate(eGFR),cardia troponinT(cTnT),creatine kinase isoenzymes(CK-MB)and N-terminal pro-B-type natriuretic peptide(NT-proBNP)were found between groups(all P<0.05).In CKD group,with the increase of LACI,the prevalence of blood pressure and diabetes,the levels of cTnT and NT-proBNP in different subgroups showed increasing trend,while eGFR showed a decreasing trend.LACI was correlated with cTnT,CK-MB,NT-proBNP and multiple echocardiographic parameters(all P<0.001).The AUC of LACI for predicting LA dysfunction in CKD patients was 0.884,higher than that of LA strain during reservoir phase,conduit phase,contraction phase and LASI(AUC=0.652,0.621,0.611,0.746,all P<0.05).Conclusion LACI could be used to effectively predict LA dysfunction in CKD patients.
7.Predictive value of a model based on clinical features and plasma biomarkers for AF after pacemaker implantation surgery

Objective To construct a prediction model for atrial fibrillation(AF)after pacemaker implantation based on clinical features and plasma atrial natriuretic peptide(ANP)and brain na-triuretic peptide(BNP).Methods A retrospective analysis was conducted on 242 patients under-going pacemaker implantation in our department from January 2020 to October 2023.According to the occurrence of postoperative AF or not,they were divided into an AF group(61 cases)and a non-AF group(181 cases).The risk factors of AF after pacemaker implantation were analyzed,and a risk prediction model of AF after pacemaker implantation was constructed based on clinical features and plasma ANP and BNP levels.Results The AF group had significantly advanced age,larger proportions of hypertension and coronary heart disease,larger left ventricular diameter,and higher ANP,BNP,IL-6 and IL-8 levels,but lower proportion of using calcium antagonists when compared with the non-AF group(P<0.01).Binary logistic regression analysis showed that hy-pertension,coronary heart disease,ANP,BNP and IL-6 were risk factors(P<0.05,P<0.01),and taking calcium antagonists was protective factor for AF after pacemaker implantation(P<0.05).Hosmer Lemeshow fitting test indicated the model had a good fitness(x2=7.264,P=0.508).ROC curve analysis showed that the area under curve(AUC)value of the risk model for AF after pacemaker implantation in the training set was 0.826(95%CI:0.768-0.884),with an accuracy of 79.3%(192/242),and the AUC value of the model in the validation set was 0.835(95%CI:0.733-0.938).Conclusion Our AF prediction model based on clinical features and plasma ANP and BNP had good performance,and can provide auxiliary reference in predicting AF in patients undergoing pacemaker implantation.
8.Application of shear wave elastography in the diagnosis of diabetes with peripheral neuropathy

Objective To evaluate the application of shear wave elastography(SWE)in the diagnosis of peripheral neuropathy in patients with diabetes.Methods Totally 85 patients with type 2 diabetes(T2DM)were selected from the Chengdu Office Hospital of People's Government of Tibetan Autonomous Region,including 46 patients with peripheral neuropathy(DPN)and 39 patients without peripheral neuropathy(NDPN).Compared for clinical data(gender,age,disease duration),cross-sectional area of the median nerve measured by high-frequency ultra-sound(CSA)and shear wave elastography(SWE)parameters(mean Young's modulus value,Emean)and shear wave velocity(SWV)between two groups of patients.Multifactor Logistic regression analysis was carried out on the indicators between the above groups to screen independent predictors in the diagnosis of peripheral neuropathy in diabetes patients,and a combined model was constructed.The area under the operating characteristic curve(AUC),sensitivity and specificity of the subjects were used to evaluate the diagnostic efficacy of the single model and com?bined model of the quantitative parameters(CSA,Emean,SWV)measured by clinical data,high?frequency ultra?sound and SWE in the diagnosis of peripheral neuropathy in diabetes patients.Results Age,course of disease,Emean,SWV and CSA were statistically significant in the diagnosis of peripheral neuropathy in diabetes patients(all P<0.05).AUC was 0.658,0.754,0.839,0.822 and 0.736,respectively.The combination model based on disease course,CSA and SWV showed the highest diagnostic efficiency,with AUC,sensitivity,and specificity of 0.887(0.800-0.946),80.43%,and 84.62%,respectively.Conclusions The combined model based on the course of disease,CSA and SWV have a high diagnostic efficiency in peripheral neuropathy of diabetes patients,and has good clinical application value.
9.Beta-hydroxybutyric acid improves energy dysfunction of mouse hippocampal neuron HT22 cells induced by amyloid-β protein 1-42

BACKGROUND:Patients with Alzheimer's disease have severe brain energy disorders.In recent years,brain energy rescue strategies based on ketone body intervention have attracted more and more attention in the treatment of Alzheimer's disease.OBJECTIVE:To investigate whether β-hydroxybutyric acid can improve energy dysfunction by improving mitochondrial bioenergy function in HT22 cells of mouse hippocampal neurons induced by amyloid-β protein 1-42 (Aβ1-42).METHODS:HT22 cells were divided into four groups:Control,β-hydroxybutyric acid,Aβ1-42,Aβ1-42+β-hydroxybutyric acid.Related detection kits were respectively used to detect HT22 cell survival rate,adenosine triphosphate level,α-ketoglutarate dehydrogenase activity,Na+K+-ATPase activity,mitochondrial membrane potential,and reactive oxygen species levels.RESULTS AND CONCLUSION:Compared with the control group,the survival rate,adenosine triphosphate level,α-ketoglutarate dehydrogenase activity,Na+K+-ATPase activity,and mitochondrial membrane potential of HT22 cells were significantly decreased (P<0.05),and the level of reactive oxygen species was significantly increased (P<0.05) in the Aβ1-42 group.Compared with the Aβ1-42 group,the survival rate,adenosine triphosphate level,α-ketoglutarate dehydrogenase activity,Na+K+-ATPase activity,and mitochondrial membrane potential of HT22 cells were significantly increased (P<0.05),and the reactive oxygen species level was significantly decreased (P<0.05) in the Aβ1-42+β-hydroxybutyric acid group.These results showed that β-hydroxybutyric acid improved mitochondrial bioenergetic function and ultimately improved Aβ1-42-induced energy impairment and survival rate in HT22 cells.
10.A comparison and prediction study of wide-field swept-source optical coher-ence tomography angiography in identifying non-perfusion areas in diabetic retinopathy

Objective To compare the differences between swept-source optical coherence tomography angiography(SS-OCTA)and ultra-wide-field fluorescein angiography(UWFA)in detecting non-perfusion areas(NPs)in patients with diabetic retinopathy(DR),to evaluate the accuracy of SS-OCTA in predicting NPs outside its visible range,and to explore the distribution patterns of NPs.Methods A retrospective analysis was made on 69 DR patients(88 eyes)who under-went both UWFA and SS-OCTA examinations at the Ophthalmology Department of Sichuan Provincial People's Hospital from December 2022 to September 2024.Manual NP labeling was conducted to compare the detection rate of NPs between the two imaging techniques.The distribution patterns of NPs and the accuracy of SS-OCTA for predicting NPs outside its visible range were also analyzed.Results In a scanning area of 20 mm x 24 mm,the overall NP detection rate by SS-OCTA was 47.40%,with UWFA taken as the standard.The NP detection rate by SS-OCTA was 51.56%in the superotemporal quad-rant,58.35%in the inferotemporal quadrant,45.50%in the superonasal quadrant,and 43.17%in the inferonasal quad-rant.Most NPs occurred in the inferonasal quadrant,accounting for 41.71%of the total NP.The accuracy of SS-OCTA in predicting NPs was 75.00%in the superonasal quadrant and 78.41%in the inferonasal quadrant.The ischemic indices(ISI)of the two imaging techniques were highly positively correlated(r2=0.74).Conclusion Although SS-OCTA can-not yet fully replace UWFA for NP detection in DR patients due to a small visible range,it is still an effective tool to assess retinal ischemia.SS-OCTA has the ability to predict NPs outside its visible range in its scanning range.The inferonasal quadrant is the region where NPs occur most frequently in DR patients,so it is suggested that special attention should be paid to this region in early diagnosis and follow-up periods.
