Tripterygium wilfordii is a traditional Chinese medicinal herb belonging to the genus Tripterygium in the Celastraceae family, which has the effects of clearing heat and detoxifying, dispelling wind and dampness, and invigorating blood circulation to relieve pain, and is used to treat diseases such as rheumatoid arthritis, glomerulonephritis, nephrotic syndrome, lupus erythematosus, scabies, and stubborn tinea. Its chemical composition is diverse. Among them, triptolide(TP) is one of the main active and toxic components of T. wilfordii. It has significant biological activities such as anti-inflammation, anti-tumor, and immunosuppression. However, it causes serious adverse reactions such as liver and kidney function damage and reproductive system disorders. At the same time, TP has poor water solubility and low bioavailability, and the enhancement of bioavailability by increasing the dosage undoubtedly improves the exposure of the drug in non-target organs, leading to the occurrence of adverse reactions, and these largely limit the clinical application of TP. Based on this, this article extracted relevant data from the Web of Science, PubMed, and China National Knowledge Infrastructure(CNKI) databases, summarized the research on the adverse reactions of TP in recent years, and reviewed the progress of toxicity reduction research from the perspectives of structural modification, novel drug delivery systems, and compatibility. Structural modification can precisely alter the chemical structure of TP, reduce the activity of its toxic groups, and retain its biological activity while fundamentally reducing the occurrence of adverse reactions. New drug delivery systems can achieve targeted delivery of TP, increase its concentration in target organs, and reduce its exposure in non-target organs, thereby enhancing therapeutic efficacy and reducing adverse effects. In addition, the combination of TP with Chinese medicine compound, single-flavored Chinese medicine or monomer can reduce the adverse effects of TP and enhance the efficacy to different degrees, which is of clinical value. This paper systematically explains attenuation research from the above three perspectives, aiming to provide a theoretical basis for the full utilization of biological activity and drug development of TP.

The gradient field, one of the core magnetic fields in magnetic resonance imaging (MRI) systems, is generated by gradient coils and plays a critical role in spatial encoding and the generation of echo signals. The uniformity or linearity of the gradient field directly impacts the quality and distortion level of MRI images. However, traditional point measurement methods lack accuracy in assessing the linearity of gradient fields, making it difficult to provide effective parameters for image distortion correction. This paper introduced a spherical measurement-based method that involved measuring the magnetic field distribution on a sphere, followed by detailed magnetic field calculations and linearity analysis. This study, applied to assess the nonlinearity of asymmetric head gradient coils, demonstrated more comprehensive and precise results compared to point measurement methods. This advancement not only strengthens the scientific basis for the design of gradient coils but also provides more reliable parameters and methods for the accurate correction of MRI image distortions.

4.Analysis of prognostic factors and construction of prognostic prediction models for hormone receptor-positive/human epidermal growth factor receptor 2-positive breast cancer

Objective:To explore the clinicopathological characteristics and prognostic factors of hormone receptor-positive/human epidermal growth factor receptor 2-positive (HR +/HER2 +) breast cancer, and to construct a nomogram model for predicting the prognosis of patients. Methods:SEER*Stat 8.3.5 software was used to screen out the data of 11 664 HR +/HER2 + and 5,054 HR -/HER2 + breast cancer patients from 2010 to 2015 from the Surveillance, Epidemiology, and End Results (SEER) database. The clinicopathological data of the both groups were compared and the overall survival of the both groups was analyzed by using the Kaplan-Meier method and log-rank test was also performed; the factors influencing the overall survival in HR +/HER2 + patients with breast cancer were analyzed by using the Cox proportional hazards model. According to multivariate Cox regression analysis results, the median of the probability values of combined variables (linear predictive values) was calculated by using MedCalc software, with ≥ the median classified as high-risk group and < the median as low-risk group; the risk factor affinity diagrams were constructed by using R4.0.3 software. Based on the independent influencing factors of the overall survival in HR +/HER2 + breast cancer patients, a nomogram model to predict the 1-, 3-, 5-year overall survival of patients was established; the receiver operating characteristic (ROC) curve and calibration curve were plotted, and the predictive efficacy of the model was verified. Results:The differences in the proportions of HR +/HER2 + and HR -/HER2 + breast cancer patients with different age, race, histological grade, surgical method, whether receiving radiotherapy or chemotherapy, T stage, N stage, and M stage were statistically significant (all P < 0.05). The overall survival of HR +/HER2 + patients was better than that of HR -/HER2 + patients ( P < 0.001). Cox regression analysis showed that age, race, histological grade, surgical method, whether receiving radiotherapy or chemotherapy, first primary tumor or not, T stage, N stage, and M stage were independent influencing factors of overall survival in HR +/HER2 + patients (all P < 0.05). MedCalc software was used to obtain the median of the linear predictive values (0.394), with ≥ 0.394 classified as the high-risk group (5 838 cases) and < 0.394 as the low-risk group (5 826 cases). The risk factor affinity diagrams showed that all risk factors were more highly expressed in high-risk group compared with those in low-risk group; the overall survival of patients in high-risk group was poorer than that in low-risk group ( P < 0.001). The area under the ROC curve of a nomogram model for predicting the 1-, 3-, and 5-year overall survival rates was 0.900, 0.760, 0.760, respectively; and C index was 0.799, and the calibration curve was highly matched with the ideal curve. Conclusions:The prognosis of HR +/HER2 + patients is better than that of HR -/HER2 + patients. The constructed risk factor affinity diagrams and nomograms can well predict the prognosis of HR +/HER2 + breast cancer patients, which have a high clinical reference value.
5.Investigation of airborne allergenic pollen in Handan area

OBJECTIVE To determine the species,quantity,and dispersion patterns of airborne pollen in two districts and five counties of Handan City,Hebei Province.METHODS The gravity sedimentation method was employed from October 1,2023,to September 30,2024,for daily and consecutive pollen exposure slides in two districts(Congtai District and Yongnian District)and five counties(Qiu County,Daming County,Cheng'an County,Cixian County,and She County)of Handan City,Hebei Province.The types of pollen were counted and identified under an optical microscope,and the pollen data were statistically analyzed.RESULTS The annual pollen quantity in the two districts and five counties of Handan City ranged from 24 031 grains/1 000 mm2 to 85 131 grains/1 000 mm2.The dissemination of major allergenic pollen presented two peaks.The first peak occurred in spring(March to April),mainly composed of Platanus(25.15%),Broussonetia(13.61%),Pinus(8.48%);the second peak occurred in(August to September),mainly consisting of Humulus/Cannabis(13.71%)and Artemisia(8.44%).Pollen in spring was significantly higher than that in summer and autumn.CONCLUSION The major allergenic airborne pollen in the two districts and five counties of Handan City exhibited two peaks in spring and summer-autumn,with Platanus pollen,Broussonetia pollen,Pinus pollen,Humulus/Cannabis pollen,and Artemisia pollne being the dominant pollen types.Among them,Platanus pollen was the absolute dominant pollen in this region.
6.Study on quality control of Dalitong Granule Extract based on one test multiple evaluation method combined with chemical recognition pattern

Objective:To establish the fingerprint of the intermediate of Dalitong Granules; To control its quality with QAMS method.Methods:Gradient elution was performed by HPLC on Wondasil C18 superb column (4.6 mm × 250 mm, 5 μm). The mobile phase was 0.02% acetonitrile - acetic acid aqueous solution, gradient elution. The detection wavelength was 325 nm and the injection volume was 10 μl. The column temperature was 30 ℃, and the flow rate was 1.0 ml/min. Chemical recognition patterns were combined for analysis, and QAMS method was used for content determination.Results:Through methodological investigation, the fingerprint of Dalitong Granule Extract had good repeatability. The 10 batches of Dalitong Granule Extract could be divided into 3 categories through clustering analysis and principal component analysis. The relative correction factors of four components in Dalitong Granule Extract were established. Compared with external standard method, the established QAMS method has higher reliability.Conclusion:The established fingerprint and QAMS method can be used for the quality control of Dalitong Granule Extract.

Nonunion of osteoporotic vertebral fractures (OVF), predominantly affecting the elderly, can lead to intractable pain, vertebral collapse, progressive kyphotic deformity, and neurological impairment, significantly compromising patients′ quality of life. There exists considerable debate on diagnosis and management of OVF, encompassing key issues such as clinical diagnosis and staging criteria for nonunion, surgical indications and procedure selection, and postoperative rehabilitation planning. Currently, there lacks standardized clinical guideline and expert consensus on the diagnosis and management of OVF nonunion in China. To address this gap, Minimally Invasive Surgery Group of Chinese Orthopedic Association, Osteoporosis Committee of Chinese Association of Orthopedic Surgeons, Prevention and Rehabilitation Committee for Osteoporosis of Chinese Association of Rehabilitation Medicine and Minimally Invasive Orthopedic Surgery Branch of China Association for Geriatric Care jointly organized domestic experts in spinal surgery, endocrinology, and rehabilitation to formulate the Clinical guideline for the diagnosis and treatment for nonunion of osteoporotic vertebral fractures ( version 2025), based on existing literature and clinical experience and adhering to principles of scientific rigor and practicality. The guideline provided 13 evidence-based recommendations encompassing diagnosis and treatment of OVF nonunion, aiming to standardize its clinical management.
8.Delayed physical growth and related factors in pediatric patients with transfusion-dependent thalassemia

Objectives:To investigate the physical growth status of pediatric patients with transfusion-dependent thalassemia (TDT) and analyze the effects of treatment-related and socioeconomic factors on physical growth.Methods:Based on the specialized thalassemia database from gene therapy clinical research at the Institute of Hematology & Hospital of Blood Diseases, Chinese Academy of Medical Sciences & Peking Union Medical College, we collected data on height and weight development, family economic status, and medical records of 338 pediatric patients with TDT from October 2023 to May 2024. The length/height-for-age and body mass index (BMI) -for-age were classified based on the Growth Standard for Children under 7 Years of Age, Standard for Height Level Classification among Children and Adolescents Aged 7-18 Years, and Dietary Guidelines for Chinese Residents. Logistic regression analysis was conducted to assess the effects of family economic status and disease-related treatment on length/height-for-age and BMI-for-age.Results:Among the 338 patients, 118 were children and 220 were adolescents (192 males and 146 females), with a median age of 12 years (range: 0.8-18) and a median diagnosis duration of 10.3 years (range: 0.5-17.9). Subtypes included α-thalassemia [21 cases (6.2%) ], β-thalassemia [288 cases (85.2%) ], and combined αβ-thalassemia[29 cases (8.6%) ]. The monthly household income of patients was concentrated in 3 000-5 000 yuan (39.9%) and 5 001-10 000 yuan (34.9%), whereas 67.2% of the families had monthly medical expenses of <3 000 yuan. Of the patients, 75.5% received their first transfusion before 1 year of age. The proportions of children and adolescents with pretransfusion hemoglobin (HGB) of ≤70 g/L were 4.2% and 6.4%, respectively. Adolescents demonstrated significantly higher rates of transfusion frequency of <4 weeks/session, monthly red blood cell infusion of >2 U, serum ferritin (SF) of ≥5 000 μg/L, iron chelation therapy, and splenectomy compared with children (all P<0.05). Of the 338 patients, 26.0%, 22.8%, and 8.9% demonstrated stunted growth, underweight, and concurrent stunted growth with underweight, respectively. No significant difference was observed in the stunted growth rates between children (22.9%) and adolescents (27.7%) ( P=0.402). However, the underweight rate in adolescents (26.8%) was significantly higher than that in children (15.3%) ( P=0.023). The multivariate analysis determined the following risk factors for stunted growth: monthly household income of <10 000 yuan (5 001-10 000 yuan: OR=5.49, 95% CI: 1.48-35.76; 3 000-5 000 yuan: OR=6.87, 95% CI: 1.88-44.60; <3 000 yuan: OR=9.29, 95% CI: 2.20-64.77), pretransfusion HGB of ≤70 g/L ( OR=3.25, 95% CI: 1.07-10.18), and SF of ≥5 000 μg/L ( OR = 3.04, 95% CI: 1.20-7.70). Longer diagnostic duration was associated with underweight ( OR=1.10, 95% CI: 1.01-1.20) . Conclusions:Children and adolescents with TDT with pretransfusion SF of ≥5 000 μg/L, HGB of ≤70 g/L, low monthly household income, or longer diagnosis duration were significantly more likely to experience delayed physical growth.
9.Analysis of four seroepidemiological surveys on hepatitis B in healthy population of Tongchuan city

Objective:To analyze the infection status of HBV among the healthy population in Tongchuan city in 2023, and compare it against the data from 2006, 2014, and 2020, thereby elucidating epidemiological trends and providing reference for optimizing hepatitis B prevention and control strategies in Tongchuan city.Methods:By using the method of multi-stage stratified random sampling, healthy people aged 1-59 from eight villages/communities in the four districts and counties of Tongchuan city were selected for on-site investigation and blood sample collection. ELISA was used to detect the five hepatitis B markers of the survey respondents, including HBsAg and HBsAb, and the seroprevalence rates of the populations with different characteristics were analyzed. Statistical analysis was performed using Chi-square test.Results:In 2023, a total of 1 926 healthy people were surveyed. After standardization, the positive rates of HBsAg, HBsAb, and HBV, and the rate of all five hepatitis B markers being negative were 0.56%, 54.57%, 26.77%, and 33.05%, respectively. Children aged 1-4 had the lowest positive rates of HBsAg, HBcAb, HBeAb, and HBV, as well as the lowest rate of all five hepatitis B markers being negative, while they had the highest positive rate of HBsAb. In the population aged 1-29, the positive rates of HBsAg and HBV, and the rate of all five hepatitis B markers being negative showed an upward trend with increasing age, while the positive rate of HBsAb showed a downward trend. In particular, the positive rate of HBsAb dropped sharply in the 5-14 age group. In 2023, the positive rate of HBV in the population aged 1-29 was 10.93% (111/1 016), and the rate of all five hepatitis B markers being negative was 38.39% (390/1 016), both lower than the results of the previous three surveys. The positive rate of HBsAb was 56.69% (576/1 016) in people aged 1-29 in 2023, which was higher than the results of the previous three surveys.Conclusions:Although certain achievements have been made in the prevention and control of hepatitis B in Tongchuan city in 2023, with more than one-third of the population showing all negative results in the five hepatitis B tests, the positive rate of HBsAb in the 5-14 age group has dropped sharply. Further actions required for hepatitis B prevention and control in Tongchuan city include optimizing the childhood immunization schedule for national immunization program vaccines, implementing booster programs for adolescents, and developing tailored immunization strategies for adults.
10.Qingre Lidan Jiedu Recipe improves high copper load-induced cognitive dysfunction in rats by regulating mitophagy

Objective To explore the mechanisms of Qingre Lidan Jiedu Recipe(QLJR)for improving cognitive dysfunction in rats with high copper load.Methods Seventy-five male SD rats were randomized into normal control group,model group,QLJR group,penicillamine(PCA)group,and QLJR+PCA group.Except for those in the control group,all the rats were fed a high-copper diet for 12 weeks.The effects of the treatments on cognitive function of the rats were assessed using the Barnes maze and passive avoidance tests.Hippocampal expressions of NIX,FUNDC1 and LC3 of the rats were detected using Western blotting and immunofluorescence staining,and changes in mitochondrial morphology were observed with transmission electron microscopy.Results Behavioral tests showed prolonged target hole latency,shortened latency to enter the dark chamber,and increased error counts of the rats in the model group,which were significantly improved in QLJR+PCA group;the error counts were significantly lower in QLJR+PCA group than in either QLJR or PCA group.Among all the groups,the hippocampal expressions of NIX and FUNDC1 were the lowest and LC3 I/II expression the highest in the model group;NIX and FUNDC1 expressions were significantly higher and LC3 I expression was lower in QLJR+PCA group than in QLJR group and PCA group.Immunofluorescence staining revealed weakened NIX and FUNDC1 expressions and enhanced LC3 expression in the hippocampus of the rats in the model group as compared with those in the normal control and QLJR+PCA groups,but their expressions did not differ significantly between QLJR and PCA groups.The rats in the model group showed obvious structural disarray of the mitochondria,which were improved in all the treatment groups.Conclusion QLJR improves cognitive dysfunction in rats with high copper load possibly by regulating mitophagy.
