1.Clinical observation and insights on pathological changes during "deep cervical lymphaticovenous anastomosis+" in elderly patients with cognitive impairment

Objective:To investigate intraoperative pathological findings and the interventional effects of "deep cervical lymphaticovenous anastomosis+" (dcLVA+) on deep cervical lymphatic drainage as well as the blood flow of carotid artery and jugular vein in elderly patients with cognitive impairment, and to put forward the thoughts based on the findings from the surgery.Methods:Between May 2024 and December 2024, retrospective analysis of Microsurgery Hospital, Fengcheng Hospital, Xi'an Medical College performed dcLVA+ between the deep cervical lymphatics or lymph nodes and jugular veins in 50 elderly patients with cognitive impairment (19 males and 31 females, aged 55-88 years with 69.94 years in average). Nine patients were found with Clinical Dementia Rating (CDR) score at 1 (mild), 7 with CDR score at 2 (moderate) and 34 with CDR score at 3 (severe). Intraoperative observations based on literature reviews had identified anatomical relationships between the lymphatic sacs containing cervical lymphatic chain and the carotid sheath. The lymph node count, size, distribution, thickness of fat tissue and conditions of lymphatic vessels were documented. Ultrasound was used to compare the blood flow of carotid artery and jugular vein as well as the cross-sectional areas at the planes of hyoid and cricoid cartilage before and after the closure of incisions under anaesthesia in 39 patients. Correlation analyses for Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCa), Activities of Daily Living (ADL) and Agitation behavior score before and 1 week after surgery were performed using Spearman's correlation and Wilcoxon paired tests. P<0.05 was considered statistically significant. Postoperative follow-ups were conducted via the visit of outpatient clinic and WeChat and telephone interviews. Results:In surgery, the cervical lymphatic chain was found running within an almost enclosed sac surrounding the carotid sheath. There were enlarged lymph nodes, increased fat deposition, lymphatic dilation or fibrosis in the sac. The preoperative blood flow in the carotid artery at the planes of hyoid and cricoid cartilage in the 39 patients was recorded at 150.52 ml/min±40.33 ml/min and 358.29 ml/min±129.30 ml/min, while that in the jugular vein was at 172.50 ml/min±63.94 ml/min and 317.00 ml/min±105.21 ml/min, respectively, both were far lower than the normal blood flow. There were statistically significant differences in the blood flow of carotid artery and jugular vein before and after surgery ( P<0.01). It was found that the preoperative blood flow of the internal carotid artery at the plane of hyoid was positively correlated with the preoperative scores of MMSE ( P<0.01), MoCa ( P<0.05) and ADL ( P<0.01). The blood flow of the common carotid artery at the plane of cricoid cartilage was found significantly and positively correlated with the preoperative scores of MMSE ( P<0.05). It was also noted that the blood flow of the internal carotid artery at the plane of hyoid was significantly and positively correlated with the postoperative ADL ( P<0.01) but negatively correlated with the Agitation behavior score ( P<0.05). The blood flow of the common carotid artery at the plane of cricoid cartilage was significantly and positively correlated with the postoperative scores of MMSE and MoCa ( P<0.05). The blood flow of the internal jugular vein at the plane of hyoid was negatively correlated with the Agitation behavior score ( P<0.01). The cross-sectional area of carotid artery at the plane of left hyoid was significantly and positively correlated with the MMSE score ( P<0.05). Statistically significant differences were observed in MMSE, MoCa, ADL and Agitation behavior score before and after surgery ( P<0.01). Conclusion:dcLVA+ shows a certain therapeutic benefit to the elderly patients with cognitive impairment. The intraoperatively observed pathological changes in cervical lymphatic sacs affect deep cervical lymphatic drainage and the blood flow of carotid artery and jugular vein. Further studies are necessary to find out whether the findings from this study would be the specific pathological changes and the morbidity mechanisms among the elderly patients with cognitive impairment.
2.Expression and clinical significance of KLF4 and TOP2A in breast cancer

Objective:To investigate the expression and clinical significance of zinc finger protein Kruppel-like factor 4 (KLF4) and topoisomeraseⅡalpha (TOP2A) in breast cancer.Methods:From Jan. 2022 to Jun. 2023, 126 patients with breast cancer were selected for examination in the clinical Laboratory of the First Affiliated Hospital of Inner Mongolia Medical University. Breast cancer tissues and adjacent tissues of all patients were collected. The expressions of KLF4 and TOP2A were detected by immunohistochemical staining, and the relationship of the expressions of KLF4 and TOP2A with the pathological features of patients was analyzed. Patients were divided into recurrence group and non-recurrence group according to whether they had recurrence after surgery. The expression of KLF4 and TOP2A in the two groups were compared. Spearman correlation analysis was used to analyze the relationship of KLF4 and TOP2A with breast cancer recurrence, and multivariate Cox risk regression was used to analyze the risk factors of breast cancer recurrence after surgery.Results:The positive expression rate of KLF4 in tumor tissues was lower than that in paracancer tissues, and the positive expression rate of TOP2A was higher ( χ 2=35.24, 53.89, P<0.001) ; The levels of KLF4 and TOP2A in tumor tissues were correlated with TNM stage, differentiation degree, borderline state and lymph node metastasis ( χ 2=10.98, 8.49, 4.28, 7.28, P=0.001, 0.004, 0.039, 0.007; χ 2=8.15, 8.78, 6.39, 7.20, P=0.004, 0.003, 0.011, 0.007), and the expression of TOP2A was correlated with the pathological type ( χ 2=4.31, P=0.04) ; The recurrence rate in KLF4 negative expression group and TOP2A positive expression group was higher than that in KLF4 positive expression group and TOP2A negative expression group ( χ 2=4.53, 5.92, P=0.033, 0.015) ; Spearman correlation analysis showed that positive expression of KLF4 was negatively correlated with breast cancer recurrence ( r=-0.43, P<0.001), and positive expression of TOP2A was positively correlated with breast cancer recurrence ( r=0.51, P<0.001) ; Multivariate Cox regression analysis showed that TNM stage, lymph node metastasis, negative KLF4 expression and positive TOP2A expression were independent risk factors for breast cancer recurrence ( P<0.05) . Conclusion:KLF4 is negatively expressed in breast cancer tissues, while TOP2A is positively expressed, and its expression level is closely related to pathological characteristics and has certain evaluation value for postoperative recurrence of breast cancer.

4.Emergency primary reconstruction of bone and soft tissue defects in Gustilo Ⅲ C fracture of both legs: a case report

This is a case report of bilateral Gustilo ⅢC fracture with tibial defects in both legs and combined with defects of soft tissue. The injury was treated by emergency surgery in November 2021 in the Hand and Foot Microsurgery Hospital, Xi'an Fengcheng Hospital. It was found that annular avulsion injury with partial defect of soft tissue in left leg from under the knee to ankle (10.0 cm × 4.0 cm, 14.0 cm × 8.0 cm), distal tibia-fibula fractures and partial bone defect of tibia (6.0 cm), anterior tibial artery and posterior tibial artery rupture, and posterior tibial nerve and foot were both intact. Of the right leg, it was found that there was an annular avulsion injury with partial defect of soft tissue between 1/3 of proximal leg and above ankle (25.0 cm × 10.0 cm), distal tibia-fibula fractures and partial tibial defect (4.0 cm), anterior tibial artery was ruptured and the posterior tibial artery was embolised, but posterior tibial nerve and foot were both intact. There was no blood supply in both feet. A chimeric fibular flap of right peroneal artery (15.0 cm × 6.0 cm, and the excised fibula was 13.0 cm long) was harvested and further divided into 2 chimeric flaps for reconstruction of the defects of bilateral tibia and soft tissue. A chimeric flap pedicled with descending branch of left lateral circumflex femoral artery (29.0 cm × 8.0 cm) was harvested and further divided into 2 chimeric tissue flaps pedicled with the descending branch of lateral circumflex femoral artery to cover the remaining wounds of both calves. The proximal peroneal artery was anastomosed with anterior tibial artery, and the distal peroneal artery was anastomosed with the lateral circumflex femoral artery carried by chimeric flap, and bilateral posterior tibial arteries were bridge anastomosed with the great saphenous veins. All patients were entered in the scheduled follow-up at 4, 8 and 12 weeks after surgery. The walking and the movement of ankles and toes were gradually recovered. At 1 year after surgery, the patient was able to get of the bed by himself. Plantar sensation regained to S 4 and there was a slight limb in walking, but without pain, wear or ulceration in feet.

7.Effect of white noise reduction earplugs combined with eye mask on sleep quality of ICU patients

Objective:To investigate the effect of white noise reduction earplugs combined with eye mask on the sleep quality of ICU patients.Methods:A total of 136 patients who were hospitalized in the ICU ward of the Second People′s Hospital of Liaocheng from April to November 2019 were selected and divided into 3 groups according to the random number table method. Group 1 (3M earplug + eye mask, 45 cases), group 2 (white noise reduction earplug + eye mask, 46 cases), and group 3 (control group, 45 cases). On the first night after enrollment, all patients were given routine care only. On the second and third nights, group 1 was given 3M ear plugs and eye masks, group 2 was given white noise reduction earplugs and eye masks, and group 3 was given routine care only. On the morning of the second and fourth day, patients′ baseline Richards Campbell Sleep Questionnaire (RCSQ), Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) scores were assessed.Results:After the intervention, the sleep depth, sleep latency, wake at night, return to sleep, sleep quality, nocturnal subjective noise perception and total score in RCSQ in group 1 were (55.98 ± 19.65), (57.66 ± 19.56), (55.68 ± 16.25), (58.17 ± 11.32), (60.54 ± 16.33), (60.65 ± 12.58), (57.60 ± 17.21) respectively, in group 2 were (56.65 ± 18.36),(61.33 ± 20.33), (65.35 ± 23.22), (60.36 ± 19.35), (63.78 ± 20.36), (66.56 ± 10.21) and (61.49 ± 17.52), in group 3 were (45.96 ± 17.79), (48.36 ± 13.65), (48.21 ± 13.49), (51.36 ± 13.65), (51.33 ± 16.45), (43.06 ± 17.15) and (49.04 ± 15.32). There were statistically significant differences in each dimension and its total scores among the three groups (F values were 4.33-8.35, all P<0.05). Compared with group 3, each dimension and total scores of groups 1 and group 2 were higher than those of group 3, and the differences were statistically significant (t values were 2.37-7.96, all P<0.05). After the intervention, the SAS score of group 2 was (43.32 ± 21.68), which was lower than that of group 1 (57.75 ± 22.68) and group 3 (60.65 ± 19.88), and the difference were statistically significant (t=3.09, 3.99, both P<0.05).Conclusions:3M earplugs or white noise reduction earplugs combined with eye masks can effectively improve the sleep quality of ICU patients, and white noise reduction earplugs combined with eye masks can also relieve the anxiety of patients.
8.Reconstruction of multiple metatarsus and soft tissue defects with fibular flap and fibular artery chimeric: A case report

In May 2019, a patient who suffered with multiple foot metatarsus injury with soft tissues defect was treated in the Hand and Foot Microsurgery of Xi'an Fengcheng Hospital, by using chimeric peroneal artery for phase-1 reconstruction of metatarsus with folded fibular flap, foot cross arch and soft tissue defects. After 2 years, the arch of foot as intact without collapse. There was no arthritic lesions. The height, arc and width of metatarsus were very close to the healthy side. The flap and plantar were smooth without abrasion and ulceration. The patient could walk, run and stand with single-foot stand, and able to carry out heavy physical work continuously.
9.Analysis of clinical nurses' attitude toward vital sign monitoring and influencing factors in ClassⅢ hospitals in Liaocheng

Objective:To explore the attitude of clinical nurses toward vital sign monitoring and analyze its influencing factors.Methods:From May to July, 2020, clinical nurses from three Class Ⅲ general hospitals in Liaocheng City were selected by convenient sampling and surveyed using a general information questionnaire, the Chinese version of the Vital Signs Monitoring Attitude Scale, and the Chinese version of the NASA Task Load Index (NASA-TLX) . A total of 747 questionnaires were collected in this study, and 721 of them were valid, accounting for an effective recovery rate of 96.5%.Results:The total score of the 721 clinical nurses' attitude toward vital signs monitoring was (56.48±6.72) . The results of multivariate analysis showed that working years, department, and workload were the influencing factors of clinical nurses' attitude toward vital signs monitoring ( P<0.05) . Conclusions:The positive attitude of clinical nurses toward vital monitoring needs to be improved, and nursing managers can start from human resource allocation to reduce their workload, encourage them to perform vital sign monitoring timely and properly, and identify changes in patients' conditions early.

Objective To investigate the inhibitory effects of radioactive 125I seeds on the growth and apoptosis of human lung adenocarcinoma cells A 549 in nude mice.Methods Human lung adenocarcinoma A549 cells were cultured in vitro and subcutaneously transplanted in BALA/c nude mice.When the tumor size reached(300 ±50)mm3,40 tumor-bearing mice were divided into 4 groups by the random number table method as 0,0.6,0.8 mCi(1 Ci=3.7×1010Bq)groups and blank control group,with 10 in each group.The 125I seeds of 0,0.6,and 0.8 mCi were implanted into the transplanted tumors in nude mouse,respectively.The blank control group received no treatment.The weight of nude mice was measured regularly every 4 days.The mice were sacrificed on the 32 days after 125I seeds implication.The transplanted tumors were weighed and the weight gain curve for nude mice was plotted.Hematoxylin-eosin(HE)staining was used to observe the histopathological changes of the tumor tissue.Cell apoptosis was detected by TUNEL assay,and the expressions of the P21,Caspase-9,Survivin and Livin proteins were detected by immunohistochemical assay.Results There was no nude mice dead in each group.On the day 28 and 32 after 125I seeds treatment,the body weights of nude mice of 0.6 and 0.8 mCi groups became lighter than those of the blank control group(q=4.26,9.19,4.11,11.59,P<0.05),the tumor weights of the 0.6 and 0.8 mCi groups were significantly decreased(q=5.021,5.692,P<0.05)with tumor inhibition rates of about 49%and 62%.In the 0.6 and 0.8 mCi groups,a large number of tumor cells degenerated to be necrotic cells.In addition,the apoptotic indexes were(50.00 ±2.58)%and(62.33 ± 4.51)%in the 0.6 and 0.8 mCi groups,respectively,and higher than that of blank control group(27.00 ±4.69)%.The expressions of P21 and Caspase-9 proteins in the 0.6 and 0.8 mCi groups were significantly higher than that in the blank control group(χ2=11.380,24.310,11.380,20.376,P<0.05).The expressions of Survivin and Livin proteins in the 0.6 and 0.8 mCi groups was significantly lower than that in the blank control group(χ2=9.643,23.254,15.429,26.667,P<0.05).Conclusions Radioactive 125I seeds can inhibit the proliferation of tumor cells and promote the apoptosis of A 549 cells probably by up-regulating the expressions of P21 and Caspase-9 but down-regulating the expressions of Survivin and Livin.
