1.Analysis of risk factors for piracetam-associated thrombocytopenia and the establishment of risk prediction model

OBJECTIVE To analyze the risk factors contributing to piracetam-associated thrombocytopenia and develop a predictive model for risk prediction. METHODS The electronic medical record information of inpatients treated with piracetam was collected retrospectively from the First Affiliated Hospital of Guangxi Medical University from January 2021 to December 2023, including gender, age, underlying diseases, combined medication, and laboratory data, etc. Patients were divided into the occurrence group and the non-occurrence group according to whether thrombocytopenia occurred, and the differences in clinical data between the two groups were compared. The independent risk factors were determined through univariate/multivariate Logistic regression analysis. A nomogram was drawn to visually present the independent risk factors, and a risk prediction model was constructed. The predictive efficacy of the model was evaluated using the receiver operating characteristic (ROC) curve, Bootstrap internal validation and calibration curve. RESULTS A total of 224 patients were included, among which 196 cases were in the non- occurrence group and 28 cases in the occurrence group. The incidence of thrombocytopenia was 12.50%. The results of the univariate Logistic regression analysis showed that the proportion of patients using three or more combined antibiotics and the level of serum creatinine in the occurrence group were significantly higher than those in the non-occurrence group, while the level of hemoglobin was significantly lower (P<0.05). The results of the multivariate Logistic regression analysis revealed that the use of three or more combined antibiotics, low hemoglobin level and high serum creatinine level were independent risk factors for piracetam-associated thrombocytopenia (P<0.05). The constructed risk prediction model was LogitP= -1.114+1.256×three or more combined antibiotics-0.017×hemoglobin level+0.009×serum creatinine level. The AUC of the ROC curve of this model was 0.757, and the optimal cut-off value was 0.474; the AUC of the ROC curve of the Bootstrap internal validation was 0.733; the apparent curve and the bias-corrected curve were close to the ideal curve. CONCLUSIONS The use of three or more antibiotics, along with low hemoglobin level and high serum creatinine level, are identified as independent risk factors for piracetam-associated thrombocytopenia. The developed risk prediction model demonstrates good predictive value.
2.Present situation of health education on school myopia prevention and control in primary and middle schools in Beijing

Objective:
To investigate the present situation of health education on school myopia prevention and control in primary and middle schools in Beijing, so as to provide scientific basis and suggestions for effective implementation of health education on school myopia prevention and control.
Methods:
From November 6-17, 2023, a total of 551 school doctors and health care teachers from public primary and secondary schools in seven districts of Beijing were investigated by using stratified random cluster sampling method. The contents included basic information, the basic situation of school health work, and the status of myopia prevention and control in schools. The data was described by frequency and proportion. Chisquare test was used to analyze the influencing factors.
Results:
A total of 89.8% of the respondents were found in the school of offering health education courses about myopia prevention and control. Among these respondents, 54.5% were equipped with health education course material of professional myopia prevention and control, and the teachers were primarily class teachers (57.6%), health care teachers (45.5%), and physical education teachers (45.1%), and most of the classes were conducted once a month (33.7%) and once a semester (28.1%). A total of 95.6% of the subjects had health education activities about myopia prevention and control at their schools. The frequency of activities was once a month (38.5%) and once a semester (27.9%). There were statistically significant differences of the report rate in setting up health education courses on myopia prevention and control, and the frequency of activities in different areas and different types of schools (χ2=19.53, 13.15, 34.30, 20.32, P<0.05).
Conclusions
Health education on school myopia prevention and control in primary and middle schools in Beijing is supportive. But health education courses taught by professional teachers should pay attention to the norms of the course material to complete a certain amount of class time, and development of health education activities should be diversified, so as to effectively implement health education of myopia prevention and control at school.
3.Anti-interference hemoglobin analysis system by high performance liquid chromatography.

High performance liquid chromatography (HPLC) is currently the mainstream technology for detecting hemoglobin. Glycated hemoglobin (HbA1c) is a gold indicator for diagnosing diabetes, however, the accuracy of HbA1c test is affected by thalassemia factor hemoglobin F (HbF)/hemoglobin A2 (HbA2) and variant hemoglobin during HPLC analysis. In this study, a new anti-interference hemoglobin analysis system of HPLC is proposed. In this system, the high-pressure three-gradient elution method was improved, and the particle size and sieve plate aperture in the high-pressure chromatography column and the structure of the double-plunger reciprocating series high-pressure pump were optimized. The system could diagnose both HbA1c and thalassemia factor HbF/HbA2 and variant hemoglobin, and the performance of the system was anti-interference and stable. It is expected to achieve industrialization. In this study, the HbA1c and thalassemia factor HbF/HbA2 detection performance was compared between this system and the world's first-line brand products such as Tosoh G8, Bio-Rad Ⅶ and D10 glycosylated hemoglobin analysis system. The results showed that the linear correlation between this system and the world-class system was good. The system is the first domestic hemoglobin analysis system by HPLC for screening of HbA1c and thalassemia factor HbF/HbA2 rapidly and accurately.

4.The follow-up value of D/S ratio in CHF patients with treatment

Objective To explore the follow-up value of phonocardiogram D/S ratio in CHF patients with treatment.Methods Forty-eight CHF patients who had never under-went regular treatment were collected in our hospital,given the regular anti-heart failure treatment for 3 months.NYHA heart failure classification,resting heart rate(RHR),and D/S ratio of phonocardiogram were recorded,six-minutes walk distance(6MWD),Left ventricular end diastolic diameter(LVEDd),the left ventricular ejection(LVEF) were tested before and after treatment.After 3-month treatment,the resting heart rate 65 times as the watershed,the patients were classified into two groups:one was up to standard,and the other was not up to standard.D/S ratio,RHR,6MWD,LVEDD,LVEF and the improvement of the heart function were compared between the two group.Results Exception the resting heart rate,the in-dicators had no significant improvement in the group of not up to standard compared with the before treatment,and D/S ratio, 6MWD,LVEF was improved significantly after three months treatment(P <0.05).In the group of up to standard,the D/S ratio was improved significantly after one months treatment(P <0.05),while after three months treatment all indicators were improved significantly(P <0.05).Comparison between the two groups,the RHR,D/S ratio were significantly improved(P <0.05 ),LVEF was significantly improved after three months treatment(P <0.05).At the same time,the D/S ratio,6MWD and NYHA classifica-tion of cardiac function was significant negatively correlated(P <0.05).Conclusion The D/S ratio could reflect heart functional im-provements in patients with CHF after anti-heart failure treatment,and it could reflect the heart functional improvement earlier and more significant in the group up to standard,it has a good clinic follow-up value in CHF patients with anti-heart failure treatment.
5.The expression and significance of serum c-met in esophageal squamous cell carcinoma of two different nationalities in Xinjiang

Objective To explore the relationship between serum expression of c-met protein and clinic pathological features in esophageal squamous cell carcinoma (ESCC) of Kazakh and Han people. Methods A total of 50 samples of ESCC and its corresponding adjacent tissues of Kazak and Han patients were collected. The expression of c-met protein was detected by immunohistochemistry assay. The serum c-met protein was detected in 40 Kazak and Han ESCC samples and 40 Kazak and Han controls using ELISA method. Results The positive expression rates of c-met protein were higher in Kazak and Han esophageal squamous cell carcinoma patients than those in its corresponding adjacent tissues respectively. And the serum expression rates of c-met protein were higher in Kazak and Han esophageal squamous cell carcinoma patients than those of healthy controls. There were significant differences in the different tumor invasion, lymph node metastasis and TNM stage of esophageal squamous cell carcinoma tissue and serum between groups, in which the infiltrated deep muscle and se?rous breakthrough (T3+T4) were higher than the infiltration of the mucosa and submucosa group (T1+T2), with lymph node metastasis was higher than that without lymph node metastasis, and TNM stage of ⅡB+Ⅲwas higher than that ofⅠ+ⅡA. There was no significant difference in the expression of c-met protein between ethnicity, gender, age and degree of differenti?ation. Conclusion The c-met protein expression is related to the occurrence and development, the invasion, metastasis and TNM stage of ESCC in Kazak and Han nationalities. The high expression of c-met protein in tissue and serum may indicate the invasion and metastasis of esophageal cancer and prompt a late clinical stage.
6.Clinical features of thyroglossal duct carcinoma.

OJECTIVE:
To analyze the cinicopathological features, treatments, and prognosis of patients with papillary TDCa.
METHOD:
A retrospective study was conducted of the medical records of our hospital for cases of TDCa. General clinical information including diagnostic criteria and treatments was obtained and analyzed. A literature review was also conducted.
RESULT:
There were 160 cases of thyroglossal duct anomalies hospitalized in Peking Union Medical College Hospital in the past 20 years, and TDCa was diagnosed in 3 (1. 88%) cases. All 3 cases underwent local radical resections, and papillary TDCa was diagnosed based on the pathology examination. Selective neck dissection was chosen in one which was confirmed with cervical lymphatic metastasis. All patients were followed up with no recurrences or metastasis.
CONCLUSION
The incidence of TDCa is very low and the pathology examination is the only way to confirm the diagnosis. Surgery, especially Sistrunk's procedure, is the treatment of choice, with low complications and recurrence. In the presence of cervical lymphadenopathy, selective neck dissection should be considered.

7.Research on biometric method of heart sound signal based on GMM.

OBJECTIVEExtraction of cepstral coefficients combined with Gaussian Mixture Model (GMM) is used to propose a biometric method based on heart sound signal.
METHODSFirstly, the original heart sounds signal was preprocessed by wavelet denoising. Then, Linear Prediction Cepstral Coefficients (LPCC) and Mel Frequency Cepstral Coefficients (MFCC) are compared to extract representative features and develops hidden Markov model (HMM) for signal classification. At last, the experiment collects 100 heart sounds from 50 people to test the proposed algorithm.
RESULTSThe comparative experiments prove that LPCC is more suitable than MFCC for heart sound biometric, and by wavelet denoising in each piece of heart sound signal, the system achieves higher recognition rate than traditional GMM.
CONCLUSIONThose results show that this method can effectively improve the recognition performance of the system and achieve a satisfactory effect.

8.Analysis of the heart sound with arrhythmia based on nonlinear chaos theory.

In this paper, a new method based on the nonlinear chaos theory was proposed to study the arrhythmia with the combination of the correlation dimension and largest Lyapunov exponent, through computing and analyzing these two parameters of 30 cases normal heart sound and 30 cases with arrhythmia. The results showed that the two parameters of the heart sounds with arrhythmia were higher than those with the normal, and there was significant difference between these two kinds of heart sounds. That is probably due to the irregularity of the arrhythmia which causes the decrease of predictability, and it's more complex than the normal heart sound. Therefore, the correlation dimension and the largest Lyapunov exponent can be used to analyze the arrhythmia and for its feature extraction.

9.A clinical study of cardiac reserve mobilizing condition for pregnant women.

This study was conducted on the basis of informed consent of subjects and was approved by the ethical review committee concerned. 527 pregnant women voluntarily participated in this project for the investigation of cardiac reserve mobilizing condition. Using the digital technique of heart sound signal processing, we measured the heart rate (HR), the ratio of the first heart sound to the second heart sound (S1/S2)and the ratio of diastolic to systolic duration (D/S) during pregnancy. There was significant difference of HR and S1/S2 between the group of non-pregnant (G1), the group of 28-36 pregnant weeks (G2), and the group of 37-42 pregnant weeks (G3) (HR, S1/S2: G1 vs G2, G1 vs G3: P < 0.01). HR of the pregnant women increased with the increase of pregnant weeks. D/S decreased with the increase of pregnant weeks. There was significant difference of D/S between G1, G2, and G3 (G1 vs. G2: P < 0.01; G1 vs. G3: P < 0.01). There was also significant difference of D/S between G2 and G3 (G2 vs. G3: P < 0.05). Everybody in the non-pregnant women group was found to have D/S > or = 1.30; 64.33% of pregnant women were found to have 1.00 < D/S < 1.30, whereas 3.05% of pregnant women were found to have D/S < 1.00. These data revealed that the heart burden of the pregnant woman increased with the increase of pregnant weeks and the mobilization of cardiac reserve.

10.Research and application of cardiac reserve data processing and information management system

BACKGROUND: Cardiac reserve indicators obtained under quiescent condition are used as evaluating index for cardiac reserve functions. However, most important aspect of cardiac dysfunction in heart failure is not the depressed cardiac performance noted at basal resting states, but rather the loss of cardiac reserve. OBJECTIVE: To develop a data processing and information management system for detecting cardiac reserve indicator with the widely used B mode ultrasound instrument in the ultrasonography field, and to provide guidance for B mode ultrasound instrument perfection. METHODS: General B mode ultrasound instrument as peripherals was connected with the pc by image acquisition card; ultrasound heart image diagnosis and management system including professional heart image acquisition and process and perfect data bank was develop. RESULTS AND CONCLUSION: An efficient and flexible mini picture archiving and communication system system was achieved, which realized image enhancement, geometric parameter measuring, convenient diagnosis report output, archive management and print function. Results demonstrated that with the powerful data process ability of PC, the system could improve the performance of the traditional ultrasound instrument in detecting cardiac reserve, which provide a powerful support for the further study.
