Background In 2021, chronic obstructive pulmonary disease (COPD) emerged as the forth leading cause of death in the world. However, the impact of air pollutants on COPD is still inconsistent across current studies. Objective To analyze the relationship between ambient sulfur dioxide (SO₂) exposure and hospital admissions for COPD in Lanzhou, and to examine the modified effects of SO₂ across different genders, age groups, and seasons. Methods A total of 20718 hospital admission records for COPD were collected from Lanzhou between 2016 and 2020, alongside synchronized data on SO₂ concentration and meteorological variables. A generalized additive model integrated with a distributed lag nonlinear model was used to assess the relationship and the lag effects between SO₂ exposure and COPD admissions. Potential confounders, including meteorological factors, day-of-the-week effect, and holidays, were controlled for, with a maximum lag period set at 7 d. Two-pollutant models and sensitivity analyses were conducted to ensure robustness. Results are expressed as relative risks (RR) with 95% confidence intervals (95%CI). Results During the study period, the average daily number of COPD admissions was 11.34±7.03. The average mass concentration of SO₂ was (23.72±12.35) μg·m⁻³. SO₂ exposure was positively correlated with COPD admissions; specifically, each 10 μg·m⁻³ increase in SO₂ concentration was associated with an RR of 1.440 (95%CI: 1.317, 1.573). Stratified analyses found that at a cumulative lag of 7 d, the RRs were higher among females, individuals aged ≥65 years, and during the cold season. Conversely, no significant increase in COPD admission risk related to SO₂ was observed during the warm season. For every 10 μg·m⁻³ increase in SO₂ concentration, the RR was 1.483 (95%CI: 1.311, 1.678) for females, 1.441 (95%CI: 1.311, 1.583) for those aged > 65 years， and 1.595 (95%CI: 1.313, 1.937) during the cold season. Conclusion Short-term exposure to ambient SO₂ exposure in Lanzhou is associated with an increased risk of COPD admission. The association is more pronounced among females, the elderly (aged> 65 years) and during the cold season.

Objective To investigate the impact of blood sampling from different sites on coagula-tion results during systemic heparin anticoagulation period treated by continuous renal replacement therapy(CRRT).Methods Seventy-eight patients undergoing CRRT with systemic heparin anticoag-ulation were selected.Using a self-control method,blood samples were simultaneously collected from sampling port at input end of the CRRT extracorporeal circuit,the arterial pressure monitoring cathe-ter,and the peripheral vein of the patients.The four coagulation parameters were tested,and the differences in coagulation results among the blood sampling sites were compared.Results During CRRT with systemic heparin anticoagulation,the four coagulation parameters using blood samples col-lected from the sampling port at the input end of the CRRT extracorporeal circuit,the arterial pressure monitoring catheter,and the peripheral vein showed no statistically significant differences(P＞0.05).Subgroup analyses based on different hemofiltration machines and heparin doses also showed no statisti-cally significant differences(P＞0.05).Conclusion During CRRT,blood sampling from the sam-pling port at the input end of the circuit or the arterial pressure monitoring catheter can be used as an alternative to peripheral venous blood sampling,with no impact on coagulation results.

Objective The aim of this study was to analyze the incidence trend and age-specific characteristics of esophage-al cancer in cancer registration areas of Gansu Province from 2010 to 2021,and predict the incidence of esophageal cancer from 2022 to 2030.Methods Based on the incidence data of esophageal cancer in 15 cancer registries in Gansu Province from 2010 to 2021,the incidence was calculated by age,sex,urban and rural areas.The age-standardized incidence by Chinese standard population(ASIRC)was standardized using the 2000 Chinese standard population.Joinpoint regression model was used to analyze the change trend of esophageal cancer incidence,and the average annual percentage change(AAPC)was calculated to quantify the overall rate of change.A birth cohort model was constructed to analyze incidence trend of different birth groups from 1930 to 2021,and the Bayesian age-period-cohort(BAPC)model was used to predict incidence.Results From 2010 to 2021,the ASIRC of esophageal cancer in Gansu Province showed a significant downward trend(AAPC=-14.47％,95％CI:-18.72％--9.99％,P＜0.001).The ASIRC in men decreased at an annual rate of 13.02％(AAPC=-13.02％,95％CI:-17.28％--8.54％,P＜0.001),the incidence of esophage-al cancer in women at a rate of 15.80％per year(AAPC=-15.80％,95％CI:-20.69％--10.61％,P＜0.001).The ASIRC of e-sophageal cancer in urban areas decreased at an annual rate of 15.32％(AAPC=-15.32％,95％CI:-21.04％--9.18％,P＜0.001);ASIRC of esophageal cancer in rural areas decreased at a rate of 5.33％per year(AAPC=-5.33％,95％CI:-8.94％--1.58％,P＜0.001).From 2010 to 2021,the incidence of esophageal cancer in all age groups showed a significant downward trend(AAPC:-19.59％to-9.18％).The birth cohort analysis revealed that the incidence of esophageal cancer in people over 40 years old in the province,men,women,urban and rural areas showed a downward trend with increase of birth years.BAPC model predicted that that the ASIRC of esophageal cancer for the total province population,men and women would decrease from 6.47/100,000,10.02/100,000,and 2.95/100,000 in 2021 to 1.31/100,000,1.72/100,000,and 0.91/100,000 in 2030,respectively.Conclu-sions The incidence of esophageal cancer in Gansu Province showed a downward trend from 2010 to 2021,but men and rural areas were still the focus of high incidence.The prediction indicates that the incidence of esophageal cancer in Gansu Province will further decline by 2030,suggesting that the prevention and control measures have achieved results;however,it is still necessary to strengthen the intervention and long-term monitoring of high-risk groups.

[Purpose]To analyze the incidence and mortality of thyroid cancer in Gansu cancer registration areas in 2019 and the trends from 2010 to 2019.[Methods]The data of thyroid can-cer from 2010 to 2019 were collected from 23 cancer registries in Gansu Province and the data quality was evaluated.The crude incidence/mortality rates,age-standardized incidence/mortality rates by Chinese standard population(ASIRC,ASMRC)and world standard population(ASIRW,ASMRW)and cumulative rate(0～74 years old)were calculated.The annual percentage change(APC)and average annual percentage change(AAPC)were calculated with Joinpoint software to analyze the trends of the incidence and mortality from 2010 to 2019.[Results]In 2019,the crude incidence of thyroid cancer was 6.89/105,the ASIRC was 5.50/105,the ASIRW was 5.33/105 and the cumulative rate(0～74 years old)was 0.48％in 2019.The crude mortality rate of thy-roid cancer was 0.42/105,ASMRC was 0.27/105,ASMRW was 0.31/105,cumulative rate(0～74 years old)was 0.03％.Incidence and mortality rates gradually increasing at age 30 and 55,re-spectively,reaching the peaks in the 45～49 and 60～64 age groups,respectively.Chengguan Dis-trict of Lanzhou City had the highest incidence rate,and Gulang County had the highest mortality rate.From 2010 to 2019,the AAPC of ASIRC of thyroid cancer was 17.12％(95％CI:7.52％～30.42％,P＜0.05)showing an upwards trend;while that of ASMRC was-3.64％(95％CI:-15.67％～10.10％,P＞0.05)showing a downward trend,but the change was not statistically significant.[Conclusion]The overall incidence of thyroid cancer in Gansu cancer registration areas is on the rise,and the prevention of high-risk groups should be strengthened.

177Lu-prostate specific membrane antigen(PSMA)radio-ligand therapy has been approved abroad for advanced prostate cancer and has been in several clinical trials in China.Based on domestic clinical practice and experimental data and referred to international experience and viewpoints,the expert group forms a consensus on the clinical application of 177Lu-PSMA radio-ligand therapy in prostate cancer to guide clinical practice.

AIM: To understand the screening and correction of myopia in children and adolescents from the Gannan region of Gansu Province, and to provide guidance for the prevention and control of myopia.METHODS: A cross-sectional stratified cluster sampling study was conducted to select 2 kindergartens and 12 primary and secondary schools in Hezuo City and Zhouqu County, Gannan region of Gansu Province, two classes were randomly selected from each grade, and the whole class was used as a unit for screening. The screening and correction of myopia in children and adolescents were collected for statistical analysis.RESULTS: A total of 5 072 children and adolescents were selected, and 4 806 valid data were finally included after excluding unqualified records. The overall prevalence of myopia was 45.55%, and the prevalence of myopia showed an increasing trend with the increase of grade(P<0.001). The prevalence of myopia in girls(48.66%)was higher than that in boys(42.18%; P<0.001). The prevalence of myopia increased with age(P<0.001), and the age group of 10-12 years old was the fastest growing for myopia, increasing from 25.62% to 60.57%. Furthermore, moderate myopia and high myopia showed an increasing tread with the increase of the grade(all P<0.001). The overall glasses wearing rate of the Gannan region was 28.55%, with a full correction rate of 50.72%, and the glasses wearing rate showed an increasing trend with the increase of grades(P<0.001). The glasses wearing rate of female students(30.84%)was higher than that of male students(26.69%; P=0.008). The full correction rates of low, moderate and high myopia in junior high were the lowest among the 3 phases of studying. The full correction rate of high myopia was the lowest in all phases of studying.CONCLUSION: The prevalence of myopia in children and adolescents from the Gannan region is lower than the national average, but the myopia of children and adolescents is still a trend of young age and high incidence, and the glasses wearing rate of myopia and full correction rate are low.

Objective: To understand the prevalence and its influencing factors of metabolic syndrome (MS) among employees in petrochemical enterprises, so as to provide insights into the prevention and control of MS among employees in petrochemical enterprises. Methods: The employees in petrochemical enterprises who underwent health examinations at the Fourth Affiliated Hospital of Gansu University of Traditional Chinese Medicine from May 2021 to December 2022 were selected as the survey subjects. Demographic information, lifestyle behaviors and occupational exposure were collected using questionnaires, and the blood biochemical indicators were measured through laboratory testing. Factors affecting MS were identified using a multivariable logistic regression model. Results: A total of 2 479 individuals were included, with a mean age of (44.84±7.87) years. There were 1 684 males (67.93%) and 795 females (32.07%). There were 905 cases of MS, with a detection rate of 36.51%. Multivariable logistic regression analysis showed that gender (male, OR=2.246, 95%CI: 1.353-3.728), age (≥40 years, OR=3.523, 95%CI: 2.003-6.194), noise exposure (OR=1.894, 95%CI: 1.272-2.821), smoking index (>0~200 cigarette-years, OR=1.907, 95%CI: 1.155-3.149; >200 cigarette-years, OR=2.257, 95%CI: 1.320-3.859), hyperuricemia (OR=3.013, 95%CI: 1.852-4.900) and γ-glutamyltransferase (abnormal, OR=2.691, 95%CI: 1.589-4.559) were the influencing factors of MS among employees in petrochemical enterprises. Conclusion The risk of MS occurrence among employees in petrochemical enterprises is related to gender, age, noise exposure, smoking index, hyperuricemia and γ-glutamyltransferase level.

Objective: To examine the effects of obesity and central obesity on hypertension, so as to provide insights into the prevention and control measures of hypertension. Methods: From September to December 2018, residents at ages of 35 to 75 years were sampled using the multi-stage random sampling method in Baiyin District, Baiyin City, Gansu Province, and subjected to questionnaire surveys and physical examinations. The interaction between obesity/central obesity and hypertension was evaluated using logistic regression analysis. The synergy index ( SI ), relative excess risk due to interaction ( RERI ) and attributable proportion due to interaction ( AP ) were calculated using Excel compiled by Andersson et al. Results: A total of 6 246 questionnaires were allocated and 6 169 valid questionnaires were recovered, with an effective recovery rate of 98.77%. The respondents included 3 038 men ( 49.25% ) and 3 131 women (50.75%), with a mean age of ( 52.05±8.78 ) years. There were 832 respondents with obesity ( 13.49% ) and 2 278 with central obesity ( 36.93% ). The crude and standardized prevalence rates of hypertension were 35.89% and 33.05%, respectively. Multivariable logistic regression analysis showed that obesity ( OR=2.020, 95%CI: 1.705-2.393 ) and central obesity ( OR=1.622, 95%CI: 1.433-1.836 ) were statistically associated with hypertension. There was no multiplicative interaction between obesity or central obesity and hypertension ( OR=1.011, 95%CI: 0.655-1.560 ), and no additive interaction was detected between obesity or central obesityand hypertension ( SI=1.405, 95%CI: 0.815-2.424; RERI=0.658, 95%CI: -0.298 to 1.614; AP=0.201, 95%CI: -0.075 to 0.476 ). Conclusions Obesity and central obesity increase the risk of hypertension; however, no interaction is detected between obesity or central obesity and hypertension.

Objective:To explore the clinical value of surgical resection combined with 131Ⅰ therapy in the treatment of differentiated thyroid carcinoma(DTC)in the elderly. Methods:From January 2010 to December 2015, 168 elderly patients with DTC admitted to the general surgery department of our hospital were divided into the combined treatment group(n=97)and the surgery group(n=71). During the same period, 68 young and middle-aged patients with DTC were enrolled as the control group.The combined treatment and control groups were treated with total or subtotal thyroidectomy combined with 131Ⅰ therapy, while the surgery group received surgical resection only.An enzyme-linked immunosorbent assay(ELISA)was used to monitor the thyroglobulin(Tg)level of patients in the three groups before and 1 week after treatment and 1, 3, 6 months after treatment, and adverse reactions and clinical efficacy grading within 6 months after treatment were evaluated.Tumor recurrence and metastasis and patient survival in the three groups were followed up for 5 years. Results:After 1 month of treatment, serum Tg levels showed significant differences between the three groups( F=15.329, P=0.011), and Tg levels were lower in the control group than in the combined treatment and surgery groups[(30.47±6.97) μg/L vs.(47.13±7.34) μg/L, (46.12±6.55) μg/L, t=3.462 and 4.791, P=0.017 and 0.023)]. After 3 and 6 months of treatment, serum Tg levels still had significant differences between the three groups( F=22.641 and 34.182, P=0.000 and 0.000), and Tg levels were lower in the combined treatment and control groups than in the surgery group[(28.27±10.16) μg/L, (41.82±9.83) μg/L vs.(13.17±2.69), t=3.164 and 4.649, P=0.012 and 0.005; (11.16±5.43) μg/L, (28.67±9.19) μg/L vs.(1.45±0.28) μg/L, t=4.251 and 7.283, P=0.009 and 0.000)], and the control group had lower Tg levels than the combined treatment group( t=2.184 and 4.537, P=0.011 and 0.000). After 6 months of treatment, the incidences of myelosuppression(21.6%, 0.0% vs.11.8%, χ2=17.851, P=0.000)and gastrointestinal reaction(37.1%, 11.3% vs.27.9%, χ2=14.070, P=0.001)were higher in the combined treatment group than in the surgery and control groups.The control group had a higher rate of effectiveness than the other two groups(73.5% vs.62.9%, 31.0%, χ2=8.796 and 33.834, P=0.032 and 0.000)and the combined treatment group had a higher effectiveness rate than the surgery group, (62.9% vs.31.0%, χ2=18.825, P=0.000). During the follow-up, the tumor recurrence-free time and the survival time were better in the control group than in the other two groups( Log Rankχ2=30.335 and 20.628, P=0.000 and 0.000). Conclusions:Surgical resection combined with 131Ⅰ therapy can reduce local progression, recurrence and metastasis of DTC, but its clinical effectiveness is worse in elderly patients than in young and middle-aged patients.
