1.Antibiotic-loaded bone cement enhances ability of tibial cortex transverse transport for treating infected wounds

BACKGROUND:Diabetic foot patients with wound infections constitute a large patient population,and there is currently no satisfactory treatment approach. OBJECTIVE:To investigate the clinical efficacy of a modified tibial cortex transverse transport combined with antibiotic-loaded bone cement for treating refractory diabetic foot ulcers. METHODS:A total of 46 diabetic foot ulcers patients,27 males and 19 females,with an average age of 64.37 years,were selected from Beijing Chaoyang Hospital,Capital Medical University and Beijing Chaoyang Integrative Medicine Rescue and First Aid Hospital from January 2020 to January 2023.All of them underwent the modified tibial cortex transverse transport combined with antibiotic-loaded bone cement treatment.Ankle-brachial index,WIFi(Wound/Ischemia/Foot infection)classification,pain visual analog scale score,and ulcer area were recorded before and 3 months after surgery. RESULTS AND CONCLUSION:(1)The mean ulcer healing time for the 46 patients was(58.07±24.82)days.At 3 months postoperatively,there were significant improvements in ankle-brachial index,pain visual analog scale score,ulcer area,and WIFi classification in 46 patients,as compared to the preoperative values,with statistically significant differences(P<0.05).Two patients experienced pin-tract infections,without infection or ulcer recurrence during the follow-up period.(2)These findings indicate that the modified tibial cortex transverse transport combined with antibiotic-loaded bone cement effectively alleviates patients'pain,improves lower limb circulation,controls infections,and promotes ulcer healing.
2.Analysis of the monitoring results of nail snails in the Tongjiang River channel of Zhenjiang section on the south bank of Yangtze River from 2019 to 2023

Objective To timely understand the current status and distribution of nail snails (Oncomelania hupensis) in Tongjiang River channels in Zhenjiang City, providing a scientific basis for achieving the goal of eliminating schistosomiasis. Methods From 2019 to 2023, nine Tongjiang River channels on the south bank of the Yangtze River under the jurisdiction of Zhenjiang City were selected as the monitoring area. Snail monitoring was carried out onshore and beach nail snails floats in the Tongjiang River channels, nail snails on attachments in hardened areas, snails induced by straw curtains, and snails carried by boats and domestic animals. Results The monitoring results of shoreline snail from 2019 to 2023 showed that the snail situation in the Tongjiang River channel and its outer river bank remained relatively stable from 2019 to 2020; however, in 2021, under the influence of the Yangtze River flooding disaster in 2020, the area of snails increased significantly. In 2021, the area with snails in the Tongjiang River channel and the outer river bank increased by 45.70% (11.95/26.15) and 100.00% (20.00/20.00) compared to 2020; the average density of nails snails in the Tongjiang River channel and the outer river bank and the emergence rate of snail frames both showed a significant increase, rising by 94.73% (0.18/0.19) and 68.08% (8.68/12.75) compared to 2020, and by 122.73% (0.81/0.66) and 102.78% (43.26/42.09), respectively. The differences in the increase in the occurrence rate of spiked frames in the Tongjiang River channel Chili River and Renmin River were not statistically significant (χ2=0.329, P>0.05; χ2=0.646, P>0.05). From 2022 to 2023, the density of nail snails and the occurrence rate of framed snails in the Tongjiang River channel showed a decreasing trend (F=4.72, P=0.04 and χ2=372.58, P<0.01). The area of nail snails, density of live snails, and occurrence rate of framed snails in the outer river bank showed a decreasing trend (F=13.96, P=0.02; F=23.43, P<0.01; χ2=1 029.69, P<0.01). During the five years, no nail snails were detected in the ancient canal and 11 tributaries. From 2019 to 2023, 180 times of 3 003 kg of floating objects were salvaged, with a total of 148 live snails detected. A total of 17 live snails were captured on attachments in the hardened berm area; a total of 11 live snails were captured by straw curtain snail baiting;112 boats were inspected, and no snails were found; there were 112 boats surveyed, and no snails were found; 97 cattle were observed, and 2 cattle were found to carry 1 live snail on their hooves; 321 sheep were observed, and no snails were found on their hooves; and no infectious snails were found in the monitoring area in 5 years. Conclusions Nail snails continue to exist in the Tongjiang River channel, and the risk factors for schistosomiasis transmission have not been completely eliminated. It is still necessary to carry out accurate monitoring of the snail situation in the Tongjiang River and the river bank, so as to grasp the risk of transmission in time and take emergency measures.
3.Micro-osteotomy bone transport combined with vacuum sealing drainage for the treatment of diabetic foot:A retrospective cohort study of 128 patients from two centers

Objective To assess the effectiveness of enhanced tibial transverse transport(TTT)in con-junction with vacuum-assisted closure(VAC)therapy for managing recalcitrant diabetic foot ulcers.Methods A retrospective analysis was conducted on data from diabetic foot patients with Wagner grade≥2 who were treated at Beijing Chaoyang Hospital and Beijing Chaoyang Hospital of Integrated Traditional Chinese and Western Medicine between July 2020 and December 2022.The patients were categorized into three groups based on their treatment regimen:VSD treatment(VSD group),modified TTT treatment(TTT group),and combined application of TTT and VSD(combined group).A one-year follow-up was performed to assess general data,ulcer area before and three months after surgery,ankle brachial index,visual analog pain score,as well as adverse events within one year post-surgery among the three groups.Results The VSD group consisted of 43 patients,while the TTT group consisted of 43 patients,and the combined group consisted of 42 patients.There were no statistically significant differences in baseline characteristics among the three groups(P>0.05).Patients in the VSD group had longer ulcer healing time,higher pain scores,lower ankle brachial index(P<0.05),larger ulcer area(P=0.029),and higher one-year ulcer recurrence rate compared to those in the TTT group.On the other hand,patients in the combined group had shorter ulcer healing time compared to those in the TTT group(P=0.046).However,there were no significant differences observed between these two groups regarding ulcer area(P=0.362),pain scores(P=0.932),ankle brachial index(P=0.671),and one-year ulcer recurrence rate(P=0.710).Conclusions The efficacy of modified TTT surpasses that of VSD in promoting ulcer healing,alleviating pain,and enhancing lower limb circulation.Furthermore,the combination of VSD with modified TTT demonstrates a potential to further expedite wound healing time.
4.Risk factors for postoperative pulmonary infection in patients with esophageal cancer: A systematic review and meta-analysis

Objective To systematically evaluate the risk factors for postoperative pulmonary infection in patients with esophageal cancer. Methods CNKI, Wangfang Data, VIP, CBM, PubMed, EMbase, The Cochrane Library were searched from inception to January 2021 to collect case-control studies, cohort studies and cross-sectional studies about risk factors for postoperative pulmonary infection in patients with esophageal cancer. Two researchers independently conducted literature screening, data extraction and quality assessment. RevMan 5.3 software and Stata 15.0 software were used for meta-analysis. Results A total of 20 articles were included, covering 5 409 patients of esophageal cancer. The quality score of included studies was 6-8 points. Meta-analysis results showed that age (MD=1.99, 95%CI 0.10 to 3.88, P=0.04), age≥60 years (OR=2.68, 95%CI 1.46 to 4.91, P=0.001), smoking history (OR=2.41, 95%CI 1.77 to 3.28, P<0.001), diabetes (OR=2.30, 95%CI 1.90 to 2.77, P<0.001), chronic obstructive pulmonary disease (OR=3.69, 95%CI 2.09 to 6.52, P<0.001), pulmonary disease (OR=2.22, 95%CI 1.16 to 4.26, P=0.02), thoracotomy (OR=1.77, 95%CI 1.32 to 2.37, P<0.001), operation time (MD=14.08, 95%CI 9.64 to 18.52, P<0.001), operation time>4 h (OR=3.09, 95%CI 1.46 to 6.55, P=0.003), single lung ventilation (OR=3.46, 95%CI 1.61 to 7.44, P=0.001), recurrent laryngeal nerve injury (OR=5.66, 95%CI 1.63 to 19.71, P=0.006), and no use of patient-controlled epidural analgesia (PCEA) (OR=2.81, 95%CI 1.71 to 4.61, P<0.001) were risk factors for postoperative pulmonary infection in patients with esophageal cancer. Conclusion The existing evidence shows that age, age≥60 years, smoking history, diabetes, chronic obstructive pulmonary disease, pulmonary disease, thoracotomy, operation time, operation time>4 h, single lung ventilation, recurrent laryngeal nerve injury, and no use of PCEA are risk factors for postoperative pulmonary infection in patients with esophageal cancer. Due to the limitation of the quantity and quality of included literature, the conclusion of this study still needs to be confirmed by more high-quality studies.
5.A nomogram model predicting futile recanalization in patients with acute anterior circulation large vessel occlusion after endovascular mechanical thrombectomy

Objective:To investigate the risk factors for futile recanalization in patients with acute anterior circulation large vessel occlusion after endovascular mechanical thrombectomy (EMT), and establish a nomogram model predicting futile recanalization in these patients.Methods:One hundred and eighty-eight patients with acute anterior circulation large vessel occlusion after EMT were enrolled from Department of Neurology, Zhongshan Hospital of Chinese Medicine, Guangzhou University of Chinese Medicine from January 2018 to December 2021. Patients were divided into futile recanalization group and effective recanalization group according to whether futile recanalization occurred; futile recanalization was defined as successful recanalization showed by immediate postoperative DSA (modified thrombolysis in cerebral infarction [mTICI] ≥2b), but poor functional outcome (modified Rankin Scale [mRS] scores>2 90 d after surgery). The clinical data were compared between futile recanalization group and effective recanalization group. Multivariate Logistic regression analysis was used to screen the risk factors for futile recanalization. R software was used to establish a nomogram model for futile recanalization. C-index was used to evaluate the differentiation of the model, and correction curve was used to evaluate the accuracy of the line graph model prediction. Receiver operating characteristic (ROC) curve was used to evaluate the predictive efficiency of the nomogram model.Results:Ninety-two patients had futile recanalization. Compared with the effective recanalization group, futile recanalization group had significantly older age, significantly higher hemoglobin A1c on admission, significantly longer time from puncture to vascular recanalization, significantly higher proportions of patients with hypertension history and poor collateral circulation, and significantly different severities of stroke and white matter hyperintensities (WMHs) and distribution of occlusive sites ( P<0.05). Multivariate Logistic regression analysis showed that age, history of hypertension, time from puncture to vascular recanalization, poor collateral circulation, severe WMHs, and moderate-severe stroke were independent risk factors for futile recanalization after EMT in patients with acute anterior circulation large vessel occlusion ( P<0.05). A nomogram model of futile recanalization was established according to the above 6 factors, with C-index of 0.862 ( 95%CI: 0.809-0.914); the calibration curve of the model was highly coordinated to the ideal model curve; area under the ROC curve of futile recanalization was 0.862 ( 95%CI: 0.809-0.914, P<0.001). Conclusions:Acute anterior circulation large vessel occlusion patients with older age, history of hypertension, longer time from puncture to vascular recanalization, poor collateral circulation, severe WMHs, and moderate-severe stroke trend to have futile recanalization after EMT. This nomogram can predict futile recanalization after EMT.
6.Investigation and suggestions on the smoothness of reporting channels for adverse drug reaction of pharmaceutical manufacturers in China based on the public perspective

OBJECTIVE To provide the suggestions for improving t he enthusiasm of the public to report adverse drug reactions (ADRs),promoting pharmaceutical manufacturers to improve the smoothness of ADR reporting channels by the public and the enthusiasm of assuming the main responsibility for drug safety ,and to provide reference for the performance of drug safety supervision by regulatory departments in China. METHODS Taking 180 pharmaceutical manufacturers that had entered the top 1 000 in the world as the objects ,the questionnaire was prepared to investigate the smoothness of ADR reporting channels through 4 channels:contact number ,e-mail,official website and new media (including Wechat and microblog ) of enterprise . The questionnaire involved the establishment of public reporting channels ,the records and the feedback of enterprises. The existing problems were analyzed and suggestions were put forward. RESULTS & CONCLUSIONS More than 70% of pharmaceutical manufacturers in China had established the channels for reporting ADR by the public ,which were mainly regular channels such as contact numbers and e-mail ,and each channel had the phenomenon that ADR information couldn ’t be reported. More than 60% of the public channels established by manufactures lacked inquiry and supplement for the miss ing part of th e reported information ; only 24 pharmaceutical manufacturers provided feedback on ADR information ,and the feedback contents were monotonous. gywf2021-11It is suggested that pharmaceutical manufacturers should pay more attention to ADR monitoring among the public ,consider increasing multiple reporting channels while ensuring the smoothness of channels ,strengthen the training of employee ’s information collection ability to improve the quality of information ,timely feed back the ADR information reported by the public , and increase the feedback content concerned by the public.
7.Relationship between primiparas′postpartum fatigue and paternal involvement

Objective:To investigate the current status of primiparas′ postpartum fatigue and paternal involvement, and to explore the relationship between primiparas′ postpartum fatigue and paternal involvement, and to provide reference basis for developing targeted intervention measures to alleviate postpartum fatigue of primiparas.Methods:A cross-sectional survey was conducted on 347 primiparas from Affiliated Hospital of Yangzhou University, Yangzhou Maternal and Child Health Care Hospital from September to December 2020 by convenience sampling. The survey instruments included the general information questionnaire, the Parenting Alliance Inventory (PAI), and the Postpartum Fatigue Scale (PFS).Results:The total score of PAI was (86.51 ± 12.07) points, and the level of paternal involvement was high. The total score of PFS was (16.68 ± 4.12) points. 95.97% (333/347) of primiparas had varying degrees of postpartum fatigue. There was a significant negative correlation between paternal involvement and primiparas′ postpartum fatigue ( r=-0.327, P<0.01). The results of multiple stratified regression analysis showed that paternal involvement was included in the influencing factor model of primiparas′ postpartum fatigue, which could independently explain 9.7% variation of primiparas′ postpartum fatigue. Conclusions:The higher level of paternal involvement could predict the lower level of primiparas′ postpartum fatigue. Medical staff should pay attention to the participation level of the spouses of primiparas in childcare, and improve the participation level of the spouses of primiparas in scientific ways to alleviate the postpartum fatigue of primiparas.
8.Meta-analysis of the effects of telemedicine on the quality of life in patients with inflammatory bowel disease

Objective:To evaluate the effects of telemedicine on the quality of life in patients with inflammatory bowel disease (IBD) and provide effective evidence for the implementation of telemedicine in patients with IBD.Methods:Randomized controlled trails (RCT) involving telemedicine on improving the quality of life in patients with IBD which were published since the databases were established to September 23, 2018 were retrieved from PubMed, Web of Science, Embase, the Cochrane Library, Cumulative Index of Nursing & Allied Health Literature (CINAHL) , China Biology Medicine (CBM) , VIP, Wanfang Data and China National Knowledge Internet (CNKI) . Meta-analysis was performed by two researchers using RevMan 5.3 after independent selection of literature, extraction of data, and quality evaluation according to the inclusion and exclusion criteria of the literature.Results:Among the 741 articles identified in the initial search, totally 8 RCT were included, involving a total of 1 966 patients, including 1 067 patients in the intervention group and 899 patients in the control group. One study was of high quality and the remaining seven were of medium quality. Meta-analysis showed that the quality of life scores of patients in the telemedicine group were higher than those in the control group after intervention [ SMD=0.26, 95% CI (0.06, 0.46) , P=0.01]. Subgroup analysis of the follow-up evaluation time revealed that within 3 months of intervention, telemedicine could better improve the quality of life of patients compared with the control group [ SMD=0.56, 95% CI (0.02, 1.10) , P=0.04]. Subgroup analysis of age showed that telemedicine could better improve the quality of life of adult patients compared with the control group [ SMD=0.29, 95% CI (0.06, 0.52) , P=0.01]. Conclusions:The effects of telemedicine on the quality of life in patients with IBD are better than that of routine/standard care, but high-quality research is still needed to verify it. Telemedicine can be an effective alternative for the long-term disease monitoring, management and education of patients, and may also provide a reference for the formation of a scientific and effective community-family management model for patients with IBD.
9.Bibliometric analysis of status and trend of studies on self-management in patients with inflammatory bowel disease in China

Objective To analyze the studies on self-management in patients with inflammatory bowel disease (IBD) in China and its development status, and provide a direction and idea for the further development of studies in this regard. MethodsRelevant literatures on self-management in IBD patients published since the database was established to May 2018 were retrieved from CNKI, WanFang Data, VIP and CBM. NoteExpress 3.2.0 and Excel 2016 were used to establish a database. External characteristics such as the year, journal and author as well as internal characteristics such as the research type, research subject and intervention measures were extracted from the literatures, and methodological quality assessment was conducted over the randomized controlled trails (RCT). ResultsTotally 87 articles about self-management in IBD patients were included in this study. The number of publications rose perpendicularly from 2014 to 2018. Totally 52 articles (65.82%) were published on the top 5 journals in terms of the total number of articles. The top 3 areas in terms of the total number of articles were Jiangsu (42), Zhejiang (10) and Guangdong (7). There were 64 (73.56%) co-authored articles in total, and 29 articles (36.71%) were supported by a grant. There were various types of intervention studies, but there was huge difference between them in intervention time and frequency and there was no complete assessment tool for outcome indicators yet. The blind method and random distribution were not stated in any RCT, and their quality needed to be improved. ConclusionsCurrently, China's studies on self-management in IBD patients are still in a rapid development stage. Both core journals, forums and authors have taken form, but researchers still need to develop a complete specificity assessment tool, apply complete outcome assessment indicators, and enhance the scientific and preciseness of research design and implementation,thus improving the quality of literatures.
10.The situation of hospital humanistic caring practice in China:a bibliometric study

Objective To analyze the situation of the hospital humanistic caring in our country and provide a reference for the later humanistic nursing practice and research.Methods Relevant literature were searched in Chinese Biomedical Database,Wanfang Database,China National Knowledge Infrastructure from their establishment to June 2016 and analyzed with bibliometric methods.Results A total of 181 studies were included.The data analysis showed an increasing trend since 2004.Humanistic nursing was more applied to the vast general hospitals in the eastern coastal region of China(60%,108/181).Besides,the humanistic nursing researches were mainly in single department.Of the total,random control trial accounted for the most substantial proportion(61%,111/181).40%of the researchers considered the human environment,psychological care and communication as the most crucial humanistic nursing measures.Assessment index including satisfaction,psychology was mostly used to evaluate the effect of these measures.Conclusions Humanistic nursing is practiced widely.However,there are still some problems in the research methods.There are no standards of practice and evaluation,so these need to be further explored.
