1.Regulatory effect of histone lactylation modification in hepatic fibrosis

Hepatic fibrosis is a reversible pathological process in various chronic liver diseases and is closely associated with the development and progression of severe liver diseases such as liver cirrhosis and hepatocellular carcinoma, and it has emerged as a significant global health challenge. In recent years, studies have shown that histone lactylation, a newly discovered epigenetic modification, actively participates in regulating the progression of hepatic fibrosis. This article systematically reviews the core regulatory effect of histone lactylation modification in the interaction between inflammatory microenvironment and hepatic fibrosis, in order to clarify the cascade regulatory mechanism of “inflammation-hepatic fibrosis” and provide new insights for early diagnosis, targeted intervention, and prevention of malignant transformation in hepatic fibrosis.

To explore the advantages of traditional Chinese medicine (TCM) and integrative TCM-Western medicine approaches in the treatment of diabetic microvascular complications (DMC), refine key pathophysiological insights and treatment principles, and promote academic innovation and strategic research planning in the prevention and treatment of DMC. The 38th session of the Expert Salon on Diseases Responding Specifically to Traditional Chinese Medicine, hosted by the China Association of Chinese Medicine, was held in Beijing, 2024. Experts in TCM, Western medicine, and interdisciplinary fields convened to conduct a systematic discussion on the pathogenesis, diagnostic and treatment challenges, and mechanism research related to DMC, ultimately forming a consensus on key directions. Four major research recommendations were proposed. The first is addressing clinical bottlenecks in the prevention and control of DMC by optimizing TCM-based evidence evaluation systems. The second is refining TCM core pathogenesis across DMC stages and establishing corresponding "disease-pattern-time" framework. The third is innovating mechanism research strategies to facilitate a shift from holistic regulation to targeted intervention in TCM. The fourth is advancing interdisciplinary collaboration to enhance the role of TCM in new drug development, research prioritization, and guideline formulation. TCM and integrative approaches offer distinct advantages in managing DMC. With a focus on the diseases responding specifically to TCM, strengthening evidence-based support and mechanism interpretation and promoting the integration of clinical care and research innovation will provide strong momentum for the modernization of TCM and the advancement of national health strategies.
3.Enhanced Dissolution Behavior and Stability of Felodipine by Cocrystal Technology

Objective To develop four novel cocrystals of felodipine aimed at improving its poor solubility.Methods Felodipine-caproamide,felodipine-2,4-dihydroxybenzoic acid,felodipine-2,5-dihydroxybenzoic acid,and felodipine-3,5-dihydroxybenzoic acid cocrystals were prepared using the slurry method.The cocrystals were characterized by powder X-ray diffraction(PXRD),differential scanning calorimetry(DSC),thermogravimetric analysis(TGA),and Fourier-transform infrared spectroscopy(FT-IR).Stability tests and in vitro dissolution studies were conducted to assess their pharmaceutical properties.Results All four cocrystals demonstrated improved solubility and dissolution rates compared to pure felodipine.Notably,the felodipine-3,5-dihydroxybenzoic acid cocrystal showed superior stability and a marked increase in solubility.Conclusion Cocrystallization technology significantly enhanced the stability and dissolution profile of felodipine,underscoring its potential for pharmaceutical formulation development.
4.Research Progress of New State of Pharmaceutical Substance of the Proton Pump Inhibitors

Currently,the first-generation of proton pump inhibitors(PPIs)commonly utilized in clinical practice for the treatment of peptic ulcer include omeprazole,lansoprazole,and pantoprazole,and the second generation include rabeprazole and esomeprazole.The predominant mode of PPI administration is oral,which poses numerous challenges in clinical settings owing to their poor solubility,susceptibility to gastric acid degradation,short half-life,and limited stability.This article presents a comprehensive review of studies exploring the novel states of these five PPIs,include polymorphism,co-crystal drugs,and nanodrugs.Furthermore,it summarizes the advancements in research aimed at modifying the physicochemical properties of these drugs through innovative solid states,thereby enhancing their bioavailability and stability.This work offers a new perspective for researchers endeavoring to develop new states of pharmaceutical substances that incorporate PPIs.
5.Advances in Mechanistic and Translational Applications of Anti-CGRP/CGRP Receptor Antibodies

Calcitonin gene-related peptides(CGRP)are peptides that are ubiquitous in the central and peripheral nervous systems,which has been shown to be associated with pain,setting off a boom in the development of chemical small molecule antagonists targeting CGRP receptors,but most of them have failed due to greater hepatotoxicity.Anti-CGRP/CGRPR antibodies have attracted widespread attention due to their high specificity and safety,especially lower hepatotoxicity.This article reviews the structure and function of CGRP and its receptors,as well as the research progress of anti-CGRP-related neutralizing antibodies in order to provide a reference for the exploration of disease mechanisms and drug development related to CGRP.
6.Diagnostic value of cardiac color Doppler ultrasound for left ventricular hypertrophy complicated left heart failure

Objective:To explore the diagnostic value of cardiac color Doppler ultrasonography for left ventricular hypertrophy(LVH)complicated left heart failure(LHF).Methods:A total of 117 LVH patients complicated LHF admitted in Ningde City Hospital Affiliated to Ningde Normal College between January 2019 and January 2023(heart failure group)and 100 healthy people(control group)who underwent general physical examination in our hospital simultaneously were selected for the study.Left ventricular ejection fraction(LVEF),left atrial diameter(LAD),left ventricular end-diastolic diameter(LVEDd),early diastolic peak flow velocity/late diastolic peak flow velocity(E/A)were measured by cardiac color Doppler ultrasound in two groups.Receiver operating charac-teristic(ROC)curve was used to analyze the diagnostic value of cardiac color Doppler ultrasound indexes for LVH complicated LHF.In addition,the 117 patients with LVH and LHF were divided into mild group(n=54)and moderate to severe group(n=63)according to cardiac function class.Spearman correlation analysis was used to an-alyze the associationof cardiac color ultrasound parameters with cardiac function class.Results:Compared with par-ticipants in control group,those in heart failure group had significant lower LVEF[(47.88±4.75)% vs.(69.81±5.64)%],and significant higher LAD[(44.03±4.88)mm vs.(27.56±2.76)mm vs.],LVEDd[(55.68±5.04)mm vs.(42.19±1.38)mm],E/A[(13.04±3.58)vs.(6.60±1.67)](P<0.001 all).ROC analysis indicated that the area under the curve(AUC)of the combination of various parameters of cardiac ultrasound for diagnosing LVH with LHF was significantly higher than those of the single tests(Combined detection:AUC=0.901,95%CI 0.853~0.937 vs LVEF:AUC=0.644,95%CI 0.577~0.708,LAD:AUC=0.703,95%CI 0.637~0.763,LVEDd:AUC=0.633,95%CI 0.565~0.697,E/A:AUC=0.748,95%CI 0.685~0.804,Z=7.062,5.764,7.292,4.864,P<0.001 all).Compared with patients in mild group,those in moderate to severe group had signifi-cant lower LVEF[(45.67±3.37)% vs.(50.47±4.86)%],and significant higher LAD[(46.31±4.42)mm vs.(41.36±3.98)mm],LVEDd[(60.09±1.75)mm vs.(50.53±1.41)mm]and E/A[(13.99±2.96)vs.(11.93±3.92)](P<0.01 all).Spearman correlation analysis indicated that LVEF was negatively correlated with cardiac function class(r=-0.474),while LAD(r=0.511),LVEDd(r=0.863),E/A(r=0.269)were positively corre-lated with cardiac function class(P<0.01 all).Conclusion:Cardiac color Doppler ultrasound could better diagnose LVH complicated LHF,and also could effectively predict the cardiac function of patients with LVH and LHF.
7.Clinical application of compound cold and heat ablation system in adrenal functional tumors

The preferred treatment for adrenal functional tumors is laparoscopic surgical resection. However, in recent years, various ablation techniques have gradually been applied in the treatment of adrenal tumors. This report presents the successful diagnosis and treatment experience of 4 aldosterone-producing adenomas and 1 cortisol-secreting adenoma treated with a combined cold and hot ablation system at Xi’an Daxing Hospital from Dec. 2023 to Dec. 2024, providing a new treatment approach for clinical exploration.
8.Interpretation of the Guidelines for Integrated Traditional Chinese and Western Medicine Diagnosis and Treatment of Liver Cirrhosis

Liver cirrhosis is the terminal stage of various acute and chronic liver diseases and ranks 11th among the most common causes of death worldwide.In recent years,with the progress of clinical research,there has been increasing support from evidence-based medicine for the treatment of liver cirrhosis with integrated traditional Chinese and Western medicine.In 2023,the Chinese Association of Integrative Medicine,the China Association of Chinese Medicine,and the Chinese Medical Association jointly released the first evidence-based guideline in this field,the Guidelines for Integrated Traditional Chinese and Western Medicine Diagnosis and Treatment of Liver Cirrhosis.By combining the latest research at home and abroad,this article provides a detailed interpretation of the highlights in the guideline,including traditional Chinese medicine etiology and pathogenesis,diagnostic progress,disease and syndrome combination,stage-based diagnostic mode,and treatment strategies of integrated traditional Chinese and Western medicine.The aim is to enhance understanding of this guideline among health workers and promote the improvement of the diagnosis and treatment of liver cirrhosis with integrated traditional Chinese and Western medicine.

10.Association between handgrip strength and chronic kidney disease in adult residents in Anhui Province

Objective:To explore the independent association between handgrip strength and chronic kidney disease (CKD) in adult residents in Anhui Province using data from the China Adult Chronic Disease and Risk Factor Surveillance (2023).Methods:A multi-stage stratified cluster random sampling method was used to select residents aged ≥18 years for surveys, physical measurements, and laboratory tests. Relevant covariates were adjusted, and a multivariable logistic regression model was established to infer the association between handgrip strength and CKD, followed by subgroup analysis.Results:A total of 7 295 participants were included in the study, with age of (61.5±13.2) years, and 55.6% of the study participants were women. The results of the multivariate logistic regression analysis showed that with each 1.0 kg increase in handgrip strength, the risk for CKD decreased by 1.3% ( OR=0.987, 95% CI: 0.978-0.997). Compared with those with low handgrip strength, the people with moderate hasdgrip strength ( OR=0.818, 95% CI: 0.694-0.964) and high handgrip strength ( OR=0.729, 95% CI: 0.598-0.989) had lower risk for CKD. In the subgroup analysis, the association between handgrip strength and risk for CKD remained unchanged regardless age, sex, smoking status, and alcohol consumption statuys, and the prevalence of hypertension and hyperlipidemia (interaction P>0.05), except BMI and diabetes. Conclusion:The decline in handgrip strength is associated with an increased risk for CKD in adult residents in Anhui.
