1.Factors influencing the occurrence of capsular contraction syndrome in cataract patients after phacoemulsification combined with intraocular lens implantation

AIM: To analyze the influencing factors of capsular constriction syndrome(CCS)in cataract patients after phacoemulsification(Phaco)combined with intraocular lens(IOL)implantation.METHODS: Retrospective study. The data of 2 900 cataract patients(2 900 eyes)in our hospital's information system from January 2021 to January 2024 were collected. All patients were treated with Phaco combined with IOL implantation, and the incidence of CCS within 30 wk after surgery was recorded. Patients were categorized into CCS(116 cases, 116 eyes)and N-CCS group(2 784 cases, 2 784 eyes)based on the occurrence of CCS. The basic data of the two groups were compared, and the influencing factors of CCS within 30 wk after Phaco combined with IOL implantation in cataract patients were analyzed by multivariate Logistic regression.RESULTS: Among 2 900 patients(2 900 eyes)included, 116 cataract patients(116 eyes)developed CCS within 30 wk after Phaco combined with IOL implantation, with an incidence rate of 4.00%. The single factor and multi-factor Logistic regression analysis showed that the complicated diabetes, high myopia, complicated glaucoma, and axial length(AL)>30 mm were the risk factors for the occurrence of CCS after Phaco IOL implantation in cataract patients(all P<0.05).CONCLUSION: Attention should be paid to cataract patients with diabetes, high myopia, glaucoma and AL>30 mm, which will increase the risk of CCS within 30 wk after Phaco combined with IOL implantation in cataract patients.

Objective: To analyze the demographic characteristics of patients with red blood cell transfusion reactions, the usage of red blood cell preparations, and the differences in the composition ratio of adverse reactions based on multi-center data from the Haemovigilance Network, in order to reveal the clinical characteristics of red blood cell transfusion and its underlying issues. Methods: Clinical data of patients who experienced transfusion reactions after red blood cell transfusion in the Haemovigilance Network from 2018 to 2023 were collected. The demographic characteristics of patients who experienced transfusion reactions with different types of red blood cell preparations, the utilization of these preparations, and the differences of the composition ratios of transfusion reactions were analyzed. Count data were expressed as numbers (n) or percentages (%), and comparisons between groups were performed using the Chi-square test. Results: Red blood cell transfusion reactions were more common in females (53.56%), with the majority of patients aged 50-69 years (35.54%). The Han polulation accounted for the vast majority of patients (92.77%), and patients in the hematology and obstetrics/gynecology departments had a relatively high proportion of transfusion reactions (13.26% and 14.26%, respectively). Leukocyte-reduced red blood cells and suspended red blood cells were the most common types of transfusion reactions reported among red blood cell preparations. Allergic reactions and non-hemolytic febrile reactions were the most common transfusion reactions, and there were significant differences in the composition ratios of allergic reactions (χ
=869.89, P<0.05) and non-hemolytic febrile reactions (χ
=812.75, P<0.05) across various types of red blood cell preparations. Conclusion: There are differences in the demographic characteristics and composition ratio of transfusion reactions among different red blood cell preparations. The management of red blood cell transfusion reactions should be tailored to patient characteristics and conditions, and the selection and use of blood products should be optimized to reduce or avoid the occurrence of transfusion reactions, such as considering the use of washed red blood cells for patients with a history of transfusion allergies or those prone to allergies.

The Compilation Instructions for Expert Consensus on Clinical Application of Dieda Huoxue capsules systematically expound the development methods and evidence-based basis of this consensus. In view of the weak clinical application evidence and ambiguous indications of Dieda Huoxue capsules, the Institute of Basic Research in Clinical Medicine of the China Academy of Chinese Medical Sciences and Wangjing Hospital took the lead and collaborated with 33 experts from 28 medical institutions nationwide. They strictly followed the World Health Organization (WHO) guideline-making norms and the Grading of Recommendations Assessment, Development and Evaluations (GRADE) evidence-grading system and completed the compilation through multidisciplinary cooperation. The workflow included constructing clinical questions (19 items were screened by the nominal group technique), retrieving evidence (from Chinese and English databases and grey literature), assessing safety (integrating drug monitoring data and clinical investigations), and forming recommendations and consensus suggestions (3 recommendations were reached via the GRADE grid method, and 16 consensus suggestions were reached by the majority vote rule). The results indicate that the consensus clearly states that this medicine (Dieda Huoxue capsules) is applicable to conditions like traumatic injury, blood stasis-induced pain, and sudden lumbar sprains. The recommended dose is 6 capsules each time, twice a day. Combining oral administration with external application can enhance the efficacy, and elderly patients should take the medicine at intervals. Safety monitoring suggests that it should be used with caution in people with a bleeding tendency and those with an allergic constitution. The compilation process involved three rounds of reviews by internal and external experts. Literature analysis, the Delphi method, and clinical applicability tests were employed to ensure methodological rigor. The compilation instructions comprehensively present key aspects such as project approval and registration, conflict-of-interest statements, and evidence evaluation through 12 appendices, providing methodological support for the clinical translation of the consensus. In the future, it will be continuously improved through a dynamic revision mechanism.
6.Application of Classic Prescription Xiao Chaihutang in Treatise on Febrile Diseases and Miscellaneous Diseases in Gynecology: A Review

Xiao Chaihutang,an ancient classic prescription,originated from the Treatise on Febrile Diseases and Miscellaneous Diseases (hereinafter referred to as Treatise on Febrile Diseases) written by ZHANG Zhongjing in the Eastern Han Dynasty. It is a representative prescription for reconciling Shaoyang. It can strengthening the body resistance to eliminate pathogenic factors,clear away the evil heat,and harmonize the stomach Qi and can be used to treat various diseases caused by insufficient vital energy,lingering evil energy,dysregulation of ascending and descending,disharmony between the exterior and interior,and dysfunction of the liver,gallbladder,and stomach,such as liver-gallbladder damp heat syndrome,disharmony between the exterior and interior (intermittent chills and fever,malaria),abnormal ascending and descending (vomiting,thirst,irritability,constipation, and depression),and disharmony between Yin and Yang. The core point of the pathogenesis is that the lack of positive energy and the invasion of evil energy,with an unfavorable Shaoyang cardinal system,which is consistent with the pathophysiological characteristics of women. In gynecology,on the one hand,women are prone to Qi and blood deficiency,which affects the body's ability to fight off evil influences. On the other hand,women's liver function is the most important,and they are prone to the pathological manifestations of catharsis dysfunction,Qi stagnation,and loss of function. Therefore,Xiao Chaihutang is used in the treatment of various gynecological diseases such as menstruation,leukorrhea,childbirth,and lactation. The key to using ancient formulas lies in flexible application. As long as the additions and subtractions are appropriate,different diseases can be treated in the same way. If the clinical syndrome can be adjusted according to the changes,the ratio of monarch drug and ministerial drug group,the nature of cold and warm,and the ability to tonify and diarrhea,it can be widely used in the treatment of a variety of gynecological diseases based on the reconciliation of the pathogenesis of Shaoyang,the essence of the same treatment of different diseases,and flexible tailoring. This article aims to study the application of the classic prescription Xiao Chaihutang from the Treatise on Febrile Diseases in gynecology,summarize its characteristics of pathological mechanism integration,and help peers in the treatment of gynecological diseases.
7.Characteristics of malignant tumor incidence and mortality in cancer registration areas of Sichuan Province in 2021

Objective:
To investigate the characteristics of malignant tumor incidence and mortality in cancer registration areas of Sichuan Province in 2021.
Methods:
Cancer registration data from 142 registries in Sichuan Province in 2021 were collected via the China Cancer Registry Platform. Crude incidence and crude mortality were calculated. The Chinese population-standardized incidence and world population-standardized incidence were standardized using the age structure of the standard population from the Fifth National Population Census in 2000 and Segi's world standard population. Descriptive analyses examined the distribution of rates by genders, urban/rural areas, and ages, and the ranking of leading cancer sites.
Results:
In 2021, there were 248 600 new malignant tumor cases reported in Sichuan Province, with a crude incidence of 296.37/100 000. The Chinese population-standardized incidence and world population-standardized incidence were 164.67/100 000 and 160.47/100 000, respectively. There were 158 673 malignant tumor deaths, with a crude mortality of 189.16/100 000. The Chinese population-standardized mortality and world population-standardized mortality were 92.47/100 000 and 92.00/100 000, respectively. The Chinese population-standardized incidence and mortality in males were higher than in females (179.56/100 000 vs. 151.62/100 000, 125.09/100 000 vs. 60.35/100 000). The Chinese population-standardized incidence and mortality in urban areas were higher than in rural areas (175.74/100 000 vs. 157.54/100 000, 93.63/100 000 vs. 91.82/100 000). Both the crude incidence and crude mortality increased with age. The top ten malignant tumors by crude incidence were lung cancer, colorectal cancer, liver cancer, breast cancer, esophageal cancer, gastric cancer, cervical cancer, prostate cancer, thyroid cancer, and corpus uteri cancer, accounting for 76.33% of all new cases. The top ten by crude mortality were lung cancer, liver cancer, esophageal cancer, colorectal cancer, gastric cancer, breast cancer, pancreatic cancer, cervical cancer, prostate cancer, and brain tumors, accounting for 82.39% of all cancer deaths.
Conclusions
In registration areas of Sichuan Province, the incidence and mortality of malignant tumors are relatively low. Key populations such as males, urban residents, and the elderly require focused prevention and control efforts. Comprehensive measures should be prioritized for malignant tumors including lung cancer, liver cancer, esophageal cancer, gastric cancer, colorectal cancer, and breast cancer.

9.Development of a droplet digital polymerase chain reaction assay for the sensitive detection of total and integrated HIV-1 DNA

Background::Total human immunodeficiency virus (HIV) DNA and integrated HIV DNA are widely used markers of HIV persistence. Droplet digital polymerase chain reaction (ddPCR) can be used for absolute quantification without needing a standard curve. Here, we developed duplex ddPCR assays to detect and quantify total HIV DNA and integrated HIV DNA.Methods::The limit of detection, dynamic ranges, sensitivity, and reproducibility were evaluated by plasmid constructs containing both the HIV long terminal repeat (LTR) and human CD3 gene (for total HIV DNA) and ACH-2 cells (for integrated HIV DNA). Forty-two cases on stable suppressive antiretroviral therapy (ART) were assayed in total HIV DNA and integrated HIV DNA. Correlation coefficient analysis was performed on the data related to DNA copies and cluster of differentiation 4 positive (CD4 +) T-cell counts, CD8 + T-cell counts and CD4/CD8 T-cell ratio, respectively. The assay linear dynamic range and lower limit of detection (LLOD) were also assessed. Results::The assay could detect the presence of HIV-1 copies 100% at concentrations of 6.3 copies/reaction, and the estimated LLOD of the ddPCR assay was 4.4 HIV DNA copies/reaction (95% confidence intervals [CI]: 3.6-6.5 copies/reaction) with linearity over a 5-log 10-unit range in total HIV DNA assay. For the integrated HIV DNA assay, the LLOD was 8.0 copies/reaction (95% CI: 5.8-16.6 copies/reaction) with linearity over a 3-log 10-unit range. Total HIV DNA in CD4 + T cells was positively associated with integrated HIV DNA ( r = 0.76, P <0.0001). Meanwhile, both total HIV DNA and integrated HIV DNA in CD4 + T cells were inversely correlated with the ratio of CD4/CD8 but positively correlated with the CD8 + T-cell counts. Conclusions::This ddPCR assay can quantify total HIV DNA and integrated HIV DNA efficiently with robustness and sensitivity. It can be readily adapted for measuring HIV DNA with non-B clades, and it could be beneficial for testing in clinical trials.
10.MiR-372 regulates the proliferation, apoptosis and migration of 786-O cell by inhibiting PI3K/AKT/mTOR signaling pathway

Objective:To explore whether miR-372 can affect the proliferation, migration and apoptosis of renal clear cell carcinoma (ccRCC) cell under different expression conditions, and to study the effect of miR-372 on PI3K/AKT/mTOR signaling pathway.Methods:ccRCC cell line (786-O) and normal renal epithelial cell line (HK-2) were selected to detect the expression level of miR-372 in 786-O cell and HK-2 cell by real-time fluorescence quantitative polymerase chain reaction (RT-qPCR). The mimic group and negative control group were prepared by transfection with the mimic agent, and the inhibitor group and negative control group were prepared by transfection with the inhibitor. After stable transcription of cell lines was obtained, cell migration was detected by cell scratch assay, cell proliferation was detected by CCK8 method, and cell apoptosis was detected by flow cytometry, and the expression levels of proliferation and apoptosis related proteins and PI3K/AKT/mTOR signaling pathway-related proteins were detected by Western blotting. Measurement data were expressed as mean ± standard deviation ( ± s), and independent sample t-test was used for comparison between groups. Results:The expression level of miR-372 was decreased in 786-O cell. Overexpression of miR-372 inhibited the expression of proliferating cell nuclear antigen, promoted the expression of Bcl-2 and Caspase3, and reduced the phosphorylation level of PI3K/AKT/mTOR signaling pathway in 786-O cell.Conclusions:miR-372 can inhibit the migration and proliferation of 786-O cell, and promote cell apoptosis. In addition, miR-372 can inhibit the PI3K/AKT/mTOR signaling pathway.
