1.Technological innovation and clinical practice driving high-quality development of cardiovascular CT and MRI

Over the past 40 years,cardiovascular CT and MRI have developed rapidly,achieving significant breakthroughs in technological innovation and clinical practice,and promoting the development of diagnosis and treatment of cardiovascular disease towards a precise and data-driven model.At present,cardiovascular CT has largely replaced invasive catheter-based angiography for diagnosing cardiovascular diseases,while cardiovascular MRI has gradually expanded from routine assessment of cardiac anatomy and function to analysis of myocardial microstructures and quantitation of histopathology and hemodynamics.The technological innovations,clinical application progresses and future development trends of cardiovascular CT and MRI were reviewed in this article.
2.Current status of the " Six-Good" Party branches for retired personnel in medical institutions and scientific research institutes

Objective:To analyze the current status, identify existing issues, and explore countermeasures for the development of " Six-Good" retirement Party branches in medical institutions and scientific research institutes.Methods:In August 2024, ten medical and research institutions were selected. Focusing on the current status and existing issues in Party branch building for retired personnel, questionnaire surveys were conducted among staff members of the Party office and the retired cadres department, as well as retired Party members. Experts in the field of Party building were selected for semi-structured interviews. Core themes were then extracted through content analysis.Results:The questionnaire survey included a total of 399 survey respondents, comprising 112 staff members from the Party office and the retired cadres department, as well as 287 retired Party members. 212 (53.1%) of the respondents belonged to Party branches with over 50 members, 280 (70.2%) of the respondents expressed a preference for establishing Party branches based on similar business functions or former departments prior to retirement. 373 (93.5%) of the respondents believed Party branch committee members were adequately appointed. 33.7% (127/377) of the respondents perceived ideological fluctuations among retirement Party members. 175 (43.9%) of the respondents indicated that learning content was insufficiently diverse. Through interviews with 15 experts, it was found that the construction of retired party branches was faced with issues such as low participation in offline organizational life, insufficient effectiveness in the education and management of party members, limited role play of party members, and prominent service and support pressures. In response, the experts proposed specific suggestions such as innovating organizational forms, strengthening the effectiveness of education and management, and building platforms for role play.Conclusions:The " Six-Good" Party branch building for retired personnel of the medical and scientific research institutions had achieved certain results, but there were also some problems. It is necessary to further optimize organizational settings, strengthen the branch leadership, innovate learning forms, build a platform for role play, strictly implement various systems, and strengthen assistance to difficult party members.

5.Protective effect and mechanism of Angong Niuhuang pill in sepsis-associated brain dysfunction of rats

Objective:To observe the protective effect of Angong Niuhuang pill on brain function of rats with sepsis, explore its protective mechanism, and provide the experimental basis for clinical application of Angong Niuhuang pill in the treatment of sepsis-associated encephalopathy (SAE).Methods:Thirty male Sprague-Dawley (SD) rats were divided into sham operation group, sepsis model group and Angong Niuhuang pill group according to random number table method, with 10 rats in each group. The sepsis model was established by cecal ligation and puncture (CLP); rats in sham operation group received open and closed abdomen. The rats in the Angong Niuhuang pill group were given Angong Niuhuang pill (0.3 g/kg) by gastric irrigation daily for 3 days before CLP, and the drugs were administrated 12 hours after modeling again. After 24 hours of CLP, the neuroreflex scores were evaluated, white blood cell count (WBC), the levels of serum neuron-specific enolase (NSE) and S100β were detected. Then the brain tissue was harvested. After hematoxylin-eosin (HE) staining, the pathological changes of brain tissue were observed under the light microscope. The mRNA expressions of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in brain tissue were detected by polymerase chain reaction.Results:Compared with the sham operation group, the total score of neuroreflex scores in the sepsis model group and the Angong Niuhuang pill group were significantly reduced (4.43±1.40, 6.57±1.90 vs. 9.40±0.84, both P < 0.05), WBC, serum NSE, S100β were significantly increased [WBC (×10 9/L): 8.07±1.32, 5.84±0.94 vs. 3.60±0.32; NSE (μg/L): 1.04±0.14, 0.61±0.07 vs. 0.16±0.04; S100β (ng/L): 255.624±30.25, 97.72±15.41 vs. 46.88±12.03, all P < 0.05], and the mRNA expressions of IL-6 and TNF-α in brain tissue were significantly increased [IL-6 mRNA (2 -ΔΔCt): 5.668±2.195, 3.605±1.014 vs. 0.997±0.329; TNF-α mRNA (2 -ΔΔCt): 18.996±0.913, 1.746±0.710 vs. 0.674±0.132, all P < 0.05]. Compared with the sepsis model group, the total score of neuroreflex scores in the Angong Niuhuang pill group was significantly increased (6.57±1.90 vs. 4.43±1.40, P < 0.05), WBC, serum NSE, S100β concentration, and the mRNA expressions of IL-6 and TNF-α in the brain were significantly reduced [WBC (×10 9/L): 5.84±0.94 vs. 8.07±1.32, NSE (μg/L): 0.61±0.07 vs. 1.04±0.14, S100β (ng/L): 97.72±15.41 vs. 255.62±30.25, IL-6 mRNA (2 -ΔΔCt): 3.605±1.014 vs. 5.668±2.195, TNF-α mRNA (2 -ΔΔCt): 1.746±0.710 vs. 18.996±0.913, all P < 0.05]. Brain histopathological observation showed that the hippocampal neurons in the sepsis model group were disordered arrangement, a large number of neuronal nuclei were contracted, and the tissue was loose with obvious edema. Compared with the sepsis model group, the Angong Niuhuang pill group had less nuclear shrinkage and tissue edema. Conclusions:The pretreatment of the Angong Niuhuang pill can improve the brain dysfunction of septic rats and reduce the expression of pro-inflammatory cytokines in the brain. It is speculated that the Angong Niuhuang pill can protect the brain function in sepsis by inhibiting the inflammatory reaction in the brain.
6.Exploration and practice of interdisciplinary collaboration between medicine and engineering in municipal hospitals: Take the First Hospital of Qinhuangdao as an example

Objective:The paper aims to analyze achievements and problems of interdisciplinary collaborative research between medicine and engineering carried out jointly by the First Hospital of Qinhuangdao and Yanshan University, to provide further reference for municipal hospitals.Methods:The data of scientific research of the medicine-engineering interdisciplinary collaboration in past 8 years were analyzed with specific focus on the characteristics, academic level and discipline distribution.Results:Academic level of the First Hospital of Qinhuangdao was significantly improved through such medicine-engineering interdisciplinary scientific research collaboration, especially in disciplines such as orthopaedics, rehabilitation medicine, medical imaging that are closely related to engineering disciplines.Conclusions:Deficiency of scientific research resources faced by municipal hospitals can be mitigated by medicine-engineering interdisciplinary collaboration with universities, and medicine-engineering interdisciplinary collaboration can be a possible way to improve academic level of municipal hospitals.
7.Design and application of Checklist for quality control in intensive care unit

Objective:To design a Checklist for quality control in intensive care unit and observe the effect of clinical application.Methods:By consulting guidelines and literature, such as Critical care medicine professional medical quality control index (2015 edition), the quality control Checklist of intensive care unit was designed. It included four parts: quality control data collection, medical record quality verification, special diagnosis and treatment, and hospital infection prevention and control supervision. Every month, a doctor with a senior professional title served as the quality control director, and was responsible for the quality control of the department's medical care, including collecting data of the past 24 hours during the morning handover, discussing and registering special diagnosis and treatment behaviors that would be performed on the day, and coordinating with the nursing team leader, controlling the quality of the whole department throughout the day, such as supervising each medical staff if they had unreasonable behaviors, checking the running and discharge medical records, and inspecting the status of the staff on duty. The data in 2018, 2019 (Checklist implemented) and 2017 (Checklist not implemented) were retrospectively analyzed, including the status of admitted patients, department management information, length of intensive care unit (ICU) stay, and the incidence of three-tube infection [ventilator-associated pneumonia (VAP), catheter-related bloodstream infection (CRBSI), catheter-associated urinary tract infection (CAUTI)], and standardized mortality, etc. Results:From 2017 to 2019, the number of patients admitted was 373, 446, and 480, with annual growth of 19.57% and 7.62% in 2018 and 2019, respectively, and an increase of 28.69% in 2019 compared with 2017. There was no statistically significant difference in the average age and acute physiology and chronic health evaluationⅡ (APACHEⅡ) of patients in the three years. Compared with 2017, the length of ICU stay of patients in 2018 and 2019 were significantly shortened (days: 8.99±6.12, 9.14±7.02 vs. 10.20±7.21), and the incidence of VAP, CRBSI and CAUTI were significantly reduced [VAP (cases/1 000 ventilation days): 12.97±3.60, 9.62±3.14 vs. 17.48±4.89, CRBSI (cases/1 000 catheter days): 3.75±2.19, 3.87±1.87 vs. 6.19±3.13, CAUTI (cases/1 000 catheter days): 3.29±2.18, 3.28±1.87 vs. 5.61±3.18]. The standardized mortality were also significantly reduced [(77.27±7.24)%, (70.61±7.49)% vs. (84.41±9.05)%], the number of non-compliance with hospital infection prevention per month decreased significantly (person times: 54.00±6.30, 41.08±10.76 vs. 72.08±19.68), and the number of special diagnosis and treatment per month increased significantly (person times: 1 056.67±235.27, 1 361.75±278.48 vs. 722.25±145.96), the rate of etiology submission before antimicrobial treatment [(93.21±3.68)%, (96.59±2.49)% vs. (87.86±5.28)%] and deep vein thrombosis (DVT) prevention rate [(91.13±6.36)%, (96.23±2.99)% vs. (85.58±7.68)%] were significantly improved, and all the differences were statistically significant (all P < 0.05). All medical records in the three years were Grade A, but the average scores in 2018 and 2019 were higher than those in 2017 (96.82±2.84, 96.73±2.94 vs. 93.70±3.33, both P < 0.01). Compared with 2018, the incidence of VAP, the rate of etiology submission before antimicrobial treatment, the DVT prevention rate, and the standardized mortality rate in 2019 were further improved, and the number of non-compliance with hospital infection prevention per month decreased and the number of special diagnosis and treatment per month increased, and the differences were statistically significant (all P < 0.05). Conclusion:The application of quality control Checklist in intensive care unit can build an effective quality control system, reduce the incidence of three-tube infection, standardized mortality and length of ICU stay, improve the quality control awareness and execution of medical staff, and promote the improvement of medical quality.
8.Targeted suture anchor repair technique for deltoid ligament rupture

Objective To summarize the characteristics of deltoid ligament rupture and explore the feasibility and shortterm clinical outcomes of targeted suture anchor repair technique according to the rupture site.Methods From May 2011 to October 2014,19 cases of complete deltoid ligament rupture (17 males and 2 females) were recruited in this study,with an average age of 34.15± 1.23 years (ranged from 15 to 60 years).According to Lauge-Hansen classification,there were 7 cases of pronation external rotation grade ⅣV injury,including 3 cases of Maisonnuve fracture;1 case of pronation abduction type Ⅲ degree injury,1 case of pronation abduction grade ⅣV injury;and 10 cases of supination external rotation grade ⅣV injury.According to AO / OTA classification,there were 9 cases of 43B type injury and 10 cases of 43C type injury.According to the rupture site of deltoid ligament,the targeted suture anchor repair surgery was operated respectively.Early mobilization with the help of hinged ankle brace was encouraged.The evaluation at last follow-up was based on the American Orthopedic and Ankle Association (AOFAS) criteria of ankle and hindfoot,and the visual analogue scale (VAS) scoring system.Results Nineteen patients were all followed up for 24 to 48 months,with an average of 30.42±5.11 months.Fourteen cases (73.7%,14/19) with talus end avulsion were treated by double suture anchor in the talus,with continuous locking suture of the avulsed end.Four cases (21.1%,4/19) with middle part rupture were treated by double suture anchor in the talus,with the sutures crossing three bone tunnels at the medial malleolus.One case (5.3%,1/19) with medial malleolus end avulsion was treated by single suture anchor at the medial malleolus,with continuous locking suture of the avulsed end.At the last follow-up,the AOFAS score was ranged from 70 points to 96 points,with an average of 90.53 points,and excellent in 16 cases,good in 2 cases,fair in 1 case,excellent and good rate was 94.7%.The VAS score was ranged from 0 to 2 points,with an average of 0.42 point.No wide medical clear space was detected.But traumatic arthritis was happened in 2 patients.Conclusion The targeted suture anchor repair technique according to the rupture site was a save technique in treating deltoid ligament rupture,which is conducive to early postoperative functional exercise,with excellent short-term clinical outcomes and few complications.
9.Expression and clinical significance of T cell immunoglobulin and mucin domain-3 and programmed death-1 on T lymphocytes of peripheral blood in breast invasive ductal carcinoma patients

Objective To explore the expression of T cell immunoglobulin and mucin domain-3 (Tim-3)and programmed death-1(PD-1) on T lymphocytes in breast invasive ductal carcinoma patients and their clinical significance. Methods All peripheral blood of 40 breast cancer patients and 10 healthy people was obtained.The expressions of TIM-1 and PD-1 on T lymphocytes were analyzed by flow cytometry, and the relationship between them and clinical pathological features were analyzed. Results The frequency of TIM-3 cells among T lymphocytes in breast cancer patients peripheral blood was significantly higher than that in healthy people: (2.01 ± 0.62)% vs. (0.26 ± 0.08)%, P=0.03. The frequency of TIM-3 cells among T lymphocytes in peripheral blood of low differentiated breast cancer patients was also higher than that in middle- high differentiated patients: (4.45 ± 1.22)% vs. (1.02 ± 0.27)%,P=0.00.The number of PD-1 and Tim-3 cells showed a significant positive correlation (r=0.47,P=0.02). Conclusions TIM-3 cells may suppress immune to tumor cells and accelerate the development of breast cancer.
10.Analysis of scientific research system in a pharmaceutical department at hospital

Objective By researching the cognition and attitudes of the pharmacists of the Department of Pharmacy to further improve and perfect the scientific research policy of the department,and promote the construction of scientific research work.Methods The questionnaire used Microsoft Excel software for statistical analysis.Results 24.7% of the pharmacists engaged in scientific research to reflect the individual academic and technical ability level;only 14.7% of pharmacists are more willing to participate in the competition for scientific research positions;35.3% of the pharmacists are satisfied with the current policy;6.5% of the pharmacists are not satisfied with the rewards and penaltypolicy.Conclusions The Pharmaceutical Department of Central South University Xiangya Hospital should give proper guidance for pharmacist in research purpose and direction and make them clear that the scientific research purposes is to improve rational medication.Our scientific research system still exist some shortcomings.We need to increase investment in scientific research advertisement,optimize the reward system,set up research teams for employees for their development.
