1.Study the influencing factors on the therapeutic effect of glucocorticoid impact in the treatment of active thyroid-related eye diseases

Objective:To analyze the influencing factors on the therapeutic effect of glucocorticoid impact in the treatment of active thyroid-related eye diseases.Methods:A total of 106 patients with thyroid-related eye disease admitted to Lishui People's Hospital from December 2020 to January 2024 were retrospectively selected, all of whom were treated with glucocorticoid impact therapy. According to the efficacy, they were divided into poor efficacy group (34 cases) and good efficacy group (72 cases), and relevant clinical data of the two groups were compared. Single factor and multiple factor Logistic regression were used to analyze the risk factors affecting the curative effect.Results:The proportion of smoking history in the poor efficacy group was higher than that in the good efficacy group:73.53% (25/34) vs. 29.17% (21/72), there was statistical difference ( χ2 = 18.50, P<0.01). The thyroid stimulating hormone (TSH) and lymphocyte (LYM) in the poor efficacy group were lower than those in the good efficacy group: (0.08 ± 0.02) mU/L vs. (0.10 ± 0.02) mU/L, 0.175 ± 0.028 vs. 0.192 ± 0.031;the free triiodothyronine (FT 3), free thyroxine (FT 4), tumor necrosis factor - α (TNF- α), interleukin (IL) -6, IL-8, neutrophil-to-lymphocyte ratio (NLR), degree of proptosis, and clinical activity score (CAS) were higher than those in the good efficacy group: (5.28 ± 1.21) pmol/L vs. (4.73 ± 1.03) pmol/L, (15.52 ± 2.35) pmol/L vs. (14.28 ± 2.31) pmol/L, (11.82 ± 2.11) ng/L vs. (10.05 ± 2.04) ng/L, (2.12 ± 0.21) ng/L vs. (1.96 ± 0.18) ng/L, (38.56 ± 4.35) ng/L vs. (35.46 ± 4.21) ng/L, 2.52 ± 0.41 vs. 1.67 ± 0.37, (17.42 ± 2.44) mm vs. (15.32 ± 2.27) mm, (4.34 ± 0.65) scores vs. (3.98 ± 0.63) scores, there were statistical differences ( P<0.05). Multiple Logistic regression analysis showed that smoking history, TSH, FT 3, FT 4, TNF-ɑ, IL-6, IL-8, LYM, NLR, exophthalmia and CAS score were all included in the regression equation, and they were all influential factors affecting the efficacy of glucocorticoid shock therapy for thyroid-related eye diseases ( P<0.05). Conclusions:In active thyroid-related eye disease patients receiving glucocorticoid impact therapy, the efficacy may be affected by many factors, and comprehensive intervention should be implemented according to the actual situation to further improve the efficacy.
2.Analysis of risk factors for pegaspargase-associated acute pancreatitis in childhood with B-cell acute lym-phoblastic leukemia

Objective To analyze the clinical characteristics and risk factors of children with B-cell acute lymphoblastic leukemia(B-ALL)who developed acute pancreatitis(AP)after treatment with pegaspargase(PEG-ASP).Methods A retrospective analysis was conducted on the general data,clinical data,blood routine data,albumin concentration,and cumulative dose of PEG-ASP of 272 children with ALL complicated with AP who received PEG-ASP treatment in the hospital from January 2021 to February 2023.The correlations between gender,age,risk stratification,cumulative dose of pegaspargase,blood routine indicators,albumin concentration and the progression of pancreatitis were analyzed.Results Among the 272 children,the incidence of AP was 8.5%(23/272).There was no statistically significant correlation between AP and gender,age,body mass index(BMI),risk stratification,cumulative dose of pegaspargase,hemoglobin concentration,platelet count and monocyte count(P>0.05),but there was a significant correlation with white blood cell count,neutrophil count,lymphocyte count,neutrophil/lymphocyte ratio,platelet/lymphocyte ratio and albumin concentration(P<0.05).Logistic regression analysis further showed that white blood cell count,neutrophil count,lymphocyte count and albumin concentration were related to the occurrence of PEG-ASP-related AP(P<0.05).ROC analysis found that white blood cell count,lymphocyte count and albumin concentration could predict the occurrence of PEG-ASP-related AP.Conclusions White blood cell count,neutrophil count,lymphocyte count and albumin concentration are risk factors for PEG-ASP-related AP in children with B-ALL.Especially,abnormal white blood cell count,lymphocyte count and albumin concentration in blood routine examination can help identify high-risk children with B-ALL complicated with PEG-ASP-related AP at an early stage.
3.Motion compensation algorithm for multi-degree-freedom luminal surgical instruments

Due to the constraints of the surgical environment and operational space,laparoscopic surgical instruments employ wire-driven mechanisms.However,factors such as wire rigidity,hysteresis,and motor drive limitations result in the end-effector accuracy of surgical instruments failing to meet ideal requirements.To address the shortcomings of existing multi-degree-of-freedom laparoscopic surgical instruments in achieving end-effector precision,a motion compensation algorithm based on the Autogluon algorithm for a 4-degree-freedom laparoscopic surgical instrument is proposed.A single-degree-of-freedom surgical instrument driven by wire ropes was constructed,and machine learning was utilized to estimate the end-effector position.This estimated position served as a feedback compensation condition to control the end-effector of the surgical instrument.To validate the correctness of this method,it was compared with approaches such as neural networks,linear regression,decision trees,Gaussian processes,and support vector machines.The results demonstrated that the proposed method achieved the smallest mean squared error,maximum error,and mean absolute error,thereby verifying its effectiveness.

5.Intervention of Tongfenglian capsule on inflammation and intestinal microecology in gout arthritis model rats

Objective:To observe the regulatory effects of Tongfenglian capsule on intestinal flora,inflammatory factors and other indicators of gout in rats with gout arthritis(GA),and to explore possible mechanism of Tongfengli capsule on GA.Methods:A total of 36 male SD rats were randomly divided into blank group,Tongfenglian low,medium and high doses(0.216 g/kg,0.432 g/kg,0.864 g/kg)groups,colchicine group(0.18 mg/kg)and model group.Blank group and model group were given the same amount of distilled water,other groups were given the corresponding drug once a day for 14 days.On the 13th day after gavage,GA rat models were prepared according to Coderre modeling method,0.2 ml normal saline was injected into the right ankle joint for blank group,and general conditions of rats were dynamically observed to determine whether the model was established,and the time was recorded.Rate of foot swelling in each group was calculated.Abdominal aorta blood,colon tissue and intestinal contents of rats were collected,and colon length,pathological changes of colon tissue,serum levels of inflammatory factors(IL-1β,TLR4),mRNA expressions of NF-κB and TLR4 inflammatory pathway in intestinal tissue,and intestinal flora of rats were detected.Results:Compared with blank group,swelling rate of ankle joint,serum IL-1β and TLR4 levels,and mRNA expressions of TLR4 and NF-κB in intestinal tis-sue were significantly increased in model group,and a large number of inflammatory cells were infiltrated in the colon by HE staining,showing typical inflammatory pathological changes,colon length decreased significantly,intestinal flora richness and diversity de-creased,ratio of Firmicutes to Bacteroidetes increased,the abundance of Lactobacillus and Bifidobacterium decreased,and the abun-dance of Prevotella and Escherichia coli-Shigella increased.Compared with model group,all the indexes in all Tongfenglian groups were improved,the richness and diversity of intestinal flora were increased,and the reduction of intestinal microbial diversity and mi-crobial community structure caused by monosodiumurate were changed.Conclusion:Mechanism of Tongfenglian capsule alleviating inflammation may be related to the regulation of TLRs/NF-κB signaling pathway.Through the mediation of intestinal flora,Tongfengli-an capsule plays a role in regulating intestinal homeostasis,improving the abundance and diversity of intestinal microorganisms,and playing an intervention role in GA.This microecological intervention mechanism may be a potential means to prevent and treat GA.
6.Survey on oral health status of second-grade primary school students in Suzhou City

Objective To investigate the oral health status of second-grade primary school students in Suzhou,provide baseline data for evaluating Suzhou's"Bright Eyes and Healthy Teeth"public health project,and offer reference for formulating oral health policies for students in Suzhou.Methods A multi-stage,stratified,random sampling method was used to select 1 640 second-grade primary school students from 10 districts(county-level cities)across Suzhou.The oral health survey of primary school students in Suzhou was designedbased on WHO"Basic Methods for Oral Health Surveys"(5th edition).A survey on the students' oral health status was con-ducted.Results The caries prevalence,mean DMFT(Decayed,Missing,Filled Teeth),and caries filling rate for primary teeth among second-grade students in Suzhou were 80.73%,4.25,and 24.48%,respectively.For permanent teeth,the caries prevalence,mean DMFT(Decayed,Missing,Filled Teeth),and caries filling rate were 30.30%,0.62,and 16.96%,respectively.Specifically,the caries prevalence,mean DMFT,and filling rate for the first permanent molar were 29.88%,0.59,and 16.94%,respectively.The pit and fissure sealing rate for the first permanent molar was 13.74%.Additionally,the detection rate for gingival bleeding was 58.05%,and for dental calculus,49.15%.Conclusion The oral health status of second-grade primary school students in Suzhou is concerning.The caries prevalence rate for both primary and permanent teeth is high,while the filling rate is relatively low.Prior to the implementation of oral health programs,the pit and fissure sealing rate for the first permanent molars was low,with significant differ-ences observed between urban and rural areas,as well as among different districts(or county-level cities).

8.A randomized controlled study of deproteinized calf blood extract in the treatment of diabetic cataract with dry eye after phacoemulsification

Objective:To compare the clinical effect of deproteinized calf blood extract (DCBE) eye drops and 0.3% sodium hyaluronate eye drops in the treatment of diabetic cataract patients with dry eye after phacoemulsification.Methods:A randomized controlled study was performed.A total of 112 patients (116 eyes) with type Ⅱ diabetes combined with cataract and dry eye who underwent phacoemulsification in the Eye Hospital of Shandong First Medical University from January 2022 to September 2022 were selected.Patients were divided into DCBE group (49 cases, 51 eyes) and sodium hyaluronate group (63 cases, 65 eyes) by the random number table method.Patients received DCBE eye drops and 0.3% sodium hyaluronate eye drops after surgery according to grouping.Preoperative and postoperative 1 week, 1 month subjective symptoms of dry eye, corneal edema grade, tear film breakup time (TBUT), corneal fluorescein staining (CFS) and Schirmer Ⅰ test (SⅠt) results were recorded and compared.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Eye Hospital of Shandong First Medical University (No.SDSYKYY202112-1).Written informed consent was obtained from each subject.Results:There were significant differences in dry eye symptom score, TBUT and SⅠt at different time points between before and after surgery ( Ftime=323.202, 102.771, 151.876; all P<0.001).Dry eye symptom score decreased and SⅠt increased in the two groups at 1 week and 1 month after surgery compared with before surgery, and the differences were statistically significant (all P<0.017).TBUT was longer at 1 week and 1 month after surgery compared with before surgery in the DCBE group, and TBUT at 1 month after surgery was longer than that before surgery in the sodium hyaluronate group, and the differences were statistically significant (all P<0.017).There were statistically significant differences in CFS scores between the two groups at different time points before and after surgery ( Fgroup=5.391, P=0.022; Ftime=142.402, P<0.001).The CFS scores of both groups at 1 week and 1 month after surgery were lower than those before surgery, and CFS scores in the DCBE group were lower than those in the sodium hyaluronate group, with statistically significant differences (all P<0.017).One week after surgery, the proportion of patients with corneal edema in the DCBE group was 5.9%(3/51), which was significantly lower than 20.0%(13/65) in the sodium hyaluronate group ( χ2=4.790, P=0.029). Conclusions:DCBE eye drops can promote corneal repair, stabilize tear film and improve dry eye discomfort in early postoperative period in diabetic cataract, and its overall effect is better than 0.3% sodium hyaluronate eye drops.
