Hearing loss is the most prevalent disabling disease. Cochlear implantation(CI) serves as the primary intervention for severe to profound hearing loss. This consensus systematically explores the value of genetic diagnosis in the pre-operative assessment and efficacy prognosis for CI. Drawing upon domestic and international research and clinical experience, it proposes an evidence-based medicine three-tiered prognostic classification system(Favorable, Marginal, Poor). The consensus focuses on common hereditary non-syndromic hearing loss(such as that caused by mutations in genes like GJB2, SLC26A4, OTOF, LOXHD1) and syndromic hereditary hearing loss(such as Jervell & Lange-Nielsen syndrome and Waardenburg syndrome), which are closely associated with congenital hearing loss, analyzing the impact of their pathological mechanisms on CI outcomes. The consensus provides recommendations based on multiple round of expert discussion and voting. It emphasizes that genetic diagnosis can optimize patient selection, predict prognosis, guide post-operative rehabilitation, offer stratified management strategies for patients with different genotypes, and advance the application of precision medicine in the field of CI.

2.Effects of treadmill exercise on hippocampal autophagy-induced apoptosis in ovariectomized stressed rats

BACKGROUND:Apoptosis and autophagy imbalance in the hippocampal region of perimenopausal depressed rats are closely related to cognitive decline.Whether aerobic exercise can reduce apoptosis by promoting hippocampal autophagy and thus improve the learning and memory abilities of perimenopausal depressed rats is not clear.OBJECTIVE:To investigate the possible mechanism by which 4-week moderate-intensity treadmill exercise improves learning memory ability in ovariectomized stressed rats.METHODS:Forty Sprague-Dawely rats were randomly divided into four groups,namely,sham operation group(n=10),ovariectomized group(n=10),ovariectomized stress group(n=10)and ovariectomized stress exercise group(n=10).Except for the sham operation group,the ovaries were removed in the other three groups to establish a perimenopausal rat model,and then a depressed rat model was established by chronic unpredictable stress in the latter two groups.The rats in the ovariectomized stress exercise group underwent a 4-week moderate-intensity treadmill exercise.Tail suspension test and sucrose preference test were performed to text depression-like behaviors in rats after exercise and stress.The eight-arm maze experiment was used to test the learning and memory behaviors of rats after exercise and stress.Western blot was used to detect the protein levels of AMP-activated protein kinase/UNC-51 like autophagy activating kinase 1/mammalian target of rapamycin(AMPK/mTOR/ULK1),hippocampus apoptotic factor Caspase-3 and the protein expression of autophagy markers LC-3II/Beclin-1 in the hippocampus.RESULTS AND CONCLUSION:(1)Compared with the sham operation group,rats in the ovariectomized and ovariectomized stress groups had prolonged resting time in the tail suspension test and decreased sugar-water intake and sugar-water preference in the sucrose preference test.(2)Ovary removal reduced the learning memory capacity of rats,as evidenced behaviorally by a significant increase in the number of working memory errors,the number of reference memory errors,and the completion time,and an even more pronounced increase in the above measures in the ovariectomized stress group.(3)Compared with the ovariectomized group,there was a significant reduction in the number of working memory errors,the number of reference memory errors,and the completion time in the ovariectomized stress group.(4)Compared with the sham operation group,in the ovariectomized and ovariectomized stress groups,the expression of hippocampal apoptotic factor Caspase 3 protein was significantly elevated,the expression of autophagy-related factors proteins Beclin-1 and LC3II,as well as the protein expression of AMPK and ULK1,was decreased,whereas the expression of mTOR protein was elevated.Changes in the above indicators were more significant in the ovariectomized stress group.(5)Compared with the ovariectomized stress group,in the ovariectomized stress exercise group,the protein expression of Caspase 3 was significantly decreased,the protein expression of Beclin-1 and LC3II was significantly increased,the protein expression of AMPK and ULK1 was significantly increased,and the protein expression of mTOR was significantly reduced.To conclude,4-week moderate-intensity treadmill exercise may promote cellular autophagy and reduce apoptosis through the AMPK/mTOR/ULK1 autophagy signaling pathway,thereby enhancing the learning and memory capacity of rats with ovariectomized depression
3.Study on the Effect and Mechanisms of Jianpi Shengqing Jiangzhuo Recipe on Proteinuria in Mice with Diabetic Kidney Dis-ease

OBJECTIVE To explore the protective effect of Jianpi Shengqing Jiangzhuo Recipe on db/db mice with diabetic kid-ney disease(DKD)and its possible mechanism.METHODS Thirty-two 8-week-old male db/db mice were randomly divided into four groups(n=8),including the model group,the Western medicine group[Dapagliflozin(1.0 mg·kg-1·d-1)],low-dose Jianpi Shengqing Jiangzhuo group(19.63 g·kg-1·d-1),and high-dose Jianpi Shengqing Jiangzhuo group(58.89 g·kg-1·d-1).Addi-tionally,8 db/m mice were used as the normal group.The mice were orally administered once a day for 10 consecutive weeks.The general survival status of the mice was observed,and the body weight,fasting blood glucose(FBG),and urine volume of the mice were dynamically monitored;urine creatinine and urine microalbumin were detected,and urine microalbumin excretion(UAE)and protein-creatinine ratio(ACR)were calculated.After the last intervention,mice were fasted for 12 h,blood was collected under anesthesia,and kidney tissue was separated;blood creatinine(Scr),serum urea nitrogen(BUN),triglycerides(TG),total cholesterol(TC),low-density lipoprotein(LDL-C),and high-density lipoprotein(HDL-C)were tested;HE and Masson staining were used to observe pathological changes in renal tissue;Oil Red O staining was used to observe the deposition of lipid droplets in the kidneys,and Image J was used for quantitative analysis;ELISA was used to detect the levels of TNF-α and IL-1β in renal tissue;Western blot was used to detect the expression of SIRT1,SREBP-1,and PPAR-α proteins in renal tissue.RESULTS Compared with the normal group mice,the body weight,FBG,Scr,TG,TC,HDL-C,LDL-C,urine volume,UAE,and ACR of the model group mice were signifi-cantly increased(P<0.05,P<0.01);the glomerular volume was significantly increased,renal fibrosis was altered,and renal lipid droplet deposition increased(P<0.01);renal TNF-α,IL-1β,SREBP-1 expression increased(P<0.01),and SIRT1 and PPAR-α expression decreased(P<0.01).Compared with the model group,the FBG levels in the Western medicine group was significantly de-creased at the 5th and 10th weeks of intervention(P<0.05,P<0.01);after the intervention,the Scr,UAE and ACR in the Western medicine group and high-dose Jianpi Shengqing Jiangzhuo group were significantly decreased(P<0.05,P<0.01),and the serum TG level in the high-dose Jianpi Shengqing Jiangzhuo group was also significantly decreased(P<0.01);the Western medicine group and the high-dose group of Jianpi Shengqing Jiangzhuo group improved renal pathological changes and lipid deposition(P<0.05,P<0.01),with decreased levels of TNF-α and IL-1β and increased expression of SIRT1 and PPAR-α proteins(P<0.05,P<0.01);high-dose group of Jianpi Shengqing Jiangzhuo decreased the expression of SREBP-1(P<0.01).CONCLUSION Jianpi Shengqing Jiangzhuo Recipe can improve proteinuria,kidney injury,renal lipid deposition as well as inflammatory reaction in DKD,which may be related to the activation of the SIRT1/SREBP-1/PPAR-α pathway to regulate lipid homeostasis.

In recent years,significant breakthroughs have been achieved in the immunotherapy for Alzheimer's disease.In line with global advancements,two anti-amyloid-β monoclonal antibodies have been approved and successfully launched in China for clinical use.Lecanemab and Donanemab were officially used in June 2024 and April 2025 in China,respectively.In order to standardize the rational and safe application of anti-amyloid-β monoclonal antibodies for Alzheimer's disease in China,this article integrates recom-mendations from the clinical trials and real-world experience from the author's team and domestic peers to further update the recom-mendations for the clinical use of anti-amyloid-β monoclonal antibody based on the 2024 version.It includes indications for therapy,pre-treatment evaluation and preparation,administration protocols and safety measures during treatment,and post-treatment monitor-ing strategies.

7.Common pathogenesis of gout and rheumatoid arthritis based on bioinformatics analysis

Objective:To study the common pathogenesis of gout and rheumatoid arthritis(RA)by bioinformatics analysis.Methods:Microarray expression profiles of peripheral blood mononuclear cells in gout and RA were obtained from the GEO public da-tabase.R language and other tools were used to re-annotates the chip,and then the differential genes(DEGs)of the two were screened and the intersection was taken.The protein-protein interaction(PPI)network and topology analysis of common differential genes(CO-DEGs)were constructed by STRING database and Cytoscape software(including CytoNCA plug-in).The HubGene was screened and validated by ROC curve.Finally,the DAVID online analysis tool was used to perform GO and KEGG functional enrichment analysis of HubGene.Results:There were 9 HubGene screened,they were TNF,RGS1,CD69,IL7R,DDX3X,SOCS3,IFIT1,IFIT3,CCL3.GO enrichment showed that HubGene was mainly involves the regulation of virus,STAT receptor signaling pathway and positive regu-lation of neuroinflammatory response.KEGG enrichment showed that HubGene was mainly involved in Toll like receptor signaling pathway,TNF signaling pathway,JAK-STAT signaling pathway,adipocytokine signaling pathway,RIG-Ⅰ-like receptor signaling pathway and osteoclast differentiation.Conclusion:Using bioinformatics analysis,nine HubGene and related signaling pathways in-volved in the pathogenesis of gout and RA have been identified,which may serve as novel biomarkers and potential targets.
8.A phenomenological study on benefit finding of patients with age-related macular degeneration under adaptive challenges

Objective:To find benefit of adaptive challenges in age-related macular degeneration (AMD) patients of long-term treatment in the challenge of vision decline, and provide new ideas for improving the quality of life of patients.Methods:It was a phenomenological study from qualitative research, selected by purpose sampling method and semi-structured interview which involved 20 AMD patients who received intravitreal treatment of drug in the Ophthalmology Department of Affiliated Hospital of Nantong University from January to March 2023.Two interviewees comprehensively read and analyzed the transcript of the interview and its meaning, subsequently Colaizzi seven-step analysis method was used to classify and encode the transcribed data.Results:Among 20 patients, there were 9 males and 11 females, aged 56-81 years old. Patients with AMD experience a sense of benefit in their long-term management of the condition, adapting to the challenges posed by declining strength, which could be summarized into four main themes: moderate sense of decision control, satisfactory social support, active self-management of the disease, and an improvement in quality of life.Conclusions:Patients with AMD experience a sense of disease-related benefit during the long-term management of visual acuity challenges. Healthcare professionals should guide patients to seek this benefit from adverse events, identify the adaptive challenges patients face, and leverage their adaptive leadership skills. Furthermore, offering multi-channel low vision rehabilitation services can assist patients in enhancing their adaptive capabilities, ultimately contributing to further improving their quality of life.
9.Prevalence and influencing factors analysis of the polycystic ovary syndrome among female college students in Fuzhou City, Jiangxi Province

ObjectiveTo investigate the prevalence of polycystic ovary syndrome (PCOS) among female college students at a university in Fuzhou City, Jiangxi Province, and to facilitate early detection and intervention of PCOS. MethodsUsing a stratified sampling method, a total of 450 female freshmen were randomly selected for PCOS screening. A self-designed questionnaire was used for data collection, covering menstrual status, high androgen signs, lifestyle, dietary habits, and awareness of PCOS. Sample t test and χ2 test were used to compare the basic information and dietary habits between PCOS and non-PCOS cases. The correlation between various indicators and the prevalence of PCOS was analyzed by a logistic regression model. ResultsA total of 12 PCOS cases were identified, with a prevalence rate of 2.99%. PCOS cases exhibited statistically significant differences compared to non-PCOS cases in terms of waist-to-hip ratio, waist circumference, abdominal obesity, the proportion of overweight or obese individuals, and a preference for sweet food (all P<0.05). Multivariate logistic regression analysis revealed a significant correlation between preference for sweet food and the occurrence of PCOS (OR=4.858, 95%CI=1.199‒19.675,P=0.027), as well as a significant correlation with PCOS accompanied by abdominal obesity (OR=7.083, 95%CI=0.773‒64.937, P=0.048). Among the female college students surveyed, 37.90% had never heard of PCOS, 51.62% were only familiar with the name of the disease, and 10.47% had attempted to search for PCOS-related information. ConclusionThe prevalence of PCOS among female college students should not be overlooked and unhealthy dietary habits may be a crucial factor contributing to the occurrence of PCOS during this period. Early screening for PCOS during puberty is crucial.

Objective To investigate the correlation between the ratio of lesion volume to uterine volume(T/U),the expression levels of Ki-67 and p16 proteins in lesion tissue,and the recurrence risk of endometrial cancer.Methods A total of 150 patients diagnosed with endometrial carcinoma through pathological examination at Qiandongnan Prefecture People's Hospital were enrolled for follow-up observation.Among them,28 patients experienced recurrence after a 2-year follow-up period,while 122 patients remained recurrence-free.The expression differences of Ki-67 protein and p16 protein in T/U and lesion tissues during surgery were compared between the two groups.Furthermore,these indexes were analyzed based on different pathological features,and the variation in relapse-free survival time was assessed among patients with distinct T/U status as well as Ki-67 and p16 protein expressions.Results The T/U value and the positive expression rate of Ki-67 protein were significantly higher in the relapsed group compared to the non-relapsed group,while the positive expression rate of p16 protein was significantly lower in the relapsed group(P<0.05).Additionally,patients with T/U ≥ 0.18 had a significantly higher proportion of stage Ⅲ patients and patients with low histological differentiation compared to those with T/U<0.18(P<0.05).Furthermore,patients with positive expression of Ki-67 protein exhibited a significantly higher proportion of stage Ⅲ patients,patients with low histological differentiation,and lymph node metastasis compared to those with negative expression of Ki-67 protein(P<0.05).The proportion of stage Ⅲ patients exhibiting positive p16 protein expression,low histological differentiation,and lymph node metastasis was significantly lower compared to those with negative p16 protein expression(P<0.05).Patients with endometrial cancer having a T/U ≥0.18 experienced shorter recurrence-free survival time 2 years post-surgery in comparison to patients with T/U<0.18(x2=6.962,P=0.008).Patients displaying positive Ki-67 expression had a shorter recurrence-free survival time 2 years after surgery than those with negative Ki-67 expression(x2=4.815,P=0.028).The recurrence-free survival time 2 years after surgery for patients expressing p 16 protein positively was longer than that for patients ex-pressing it negatively(x2=4.279,P=0.039).The presence of FIGO stage Ⅲ,lymph node metastasis,depth of myographic invasion ≥1/2,T/U value ≥ 0.18,and positive expression of Ki-67 protein were identified as significant risk factors for postoperative recurrence in endometrial cancer(P<0.05).Conversely,the positive expression of p16 protein was found to be a protective factor against recurrence in endometrial carcinoma following surgery(P<0.05).Conclusion The expression of T/U,Ki-67 protein,and p16 protein in endometrial cancer patients is as-sociated with tumor progression and may augment the risk of postoperative recurrence.
