1.Analysis of thermal environment and students thermal comfort in primary and secondary school classrooms in winter

Objective:
To evaluate the current situation of thermal environment in primary and secondary school classrooms during winter, and to analyze students thermal comfort needs, so as to provide a basis for improving classroom thermal environment.
Methods:
From December 16 to 26, 2024, a stratified cluster random sampling method was used to select 90 classrooms from 15 primary and secondary schools in centralized/air conditioned heating areas(Liaoning Province, Tianjin City, Shanghai City) and naturally ventilated areas(Anhui Province and Jiangxi Province)for on site environmental measurement. A questionnaire survey was conducted among 743 students. The differences between groups using the χ 2 test were compared. Based on actual measurement data, a predicted mean vote prepared percentage of dissatisfied (PMV-PPD) model for centralized/air conditioned classrooms and an adaptive model for naturally ventilated classrooms were established, and the thermal neutral temperature and comfort interval were calculated.
Results:
The average outdoor temperature during on site measurement was 4.00(0.20,7.00)℃. In classrooms with centralized or air conditioned heating systems, the measured average temperature was (19.33±2.59)℃, with a thermal comfort range of 20.35-25.35 ℃ and a thermal neutral temperature of 22.85 ℃. And 13.92% of students reported feeling cold, while 80.80% felt comfortable. In classrooms with natural ventilation, the measured average temperature was (12.26±1.83)℃, with a thermal neutral temperature of 19.67 ℃ and a thermal comfort range of 16.17-23.17 ℃. About 48.33% of students reported feeling cold, and 49.81 % felt comfortable.The results of univariate analysis showed that there were statistically significant differences in shoe thickness, temperature sensation, relative humidity sensation and wind speed sensation between centralized/air conditioned heating areas ( χ 2= 7.01 , 31.47, 13.57, 13.80,all P <0.05). There were also statistically significant differences in school stage for primary and secondary school students, body mass index, classroom location for seat, temperature sensation, relative humidity sensation and wind speed sensation between naturally ventilated areas ( χ 2=42.13, 11.13, 11.04, 60.39, 29.27, 38.46,all P <0.05).
Conclusions
There are differences in thermal environment and students subjective thermal comfort in primary and secondary schools under different ventilation modes in winter. The temperature standards for heated classrooms should be revised, and differentiated environmental regulation strategies should be adopted based on different ventilation methods to improve students health and comfort levels.
2.Study on the stability and compatibility of butorphanol,nicardipine,urapidil and tirofiban administered via micro-infusion pump

OBJECTIVE To investigate the stability and compatibility of butorphanol, nicardipine, urapidil, and tirofiban. METHODS A micro-infusion pump was used to simulate the infusion process. Samples were collected in real time from the needle tip at 0, 2, 4, 6, and 8 h after mixing of the four drugs. The appearance, pH, and number of insoluble particles were examined. The contents of butorphanol, nicardipine, urapidil, and tirofiban were determined using the ultra-high-performance liquid chromatography method. RESULTS Within 8 hours, all mixed solutions remained clear and transparent, with no visible turbidity, precipitation, color change, or gas generation. The pH fluctuation ranged from 0.02 to 0.09, both within 0.1 pH units. The number of insoluble particles ≥10 μm in size in each compatibility solution was ≤9, and the number of insoluble particles ≥25 μm was ≤2, meeting the requirements of the 2025 edition of the Chinese Pharmacopoeia (Volume Ⅳ). The relative percentage contents of butorphanol, nicardipine, urapidil, and tirofiban at 8 hours after compatibility mixing ranged from 92.38% to 113.40%. CONCLUSIONS Within 8 hours under simulated clinical micro-infusion pump infusion conditions, the stability and compatibility of the mixed solution containing butorphanol, nicardipine, urapidil, and tirofiban were satisfactory.
3.Application of dexmedetomidine in perioperative period of aortic root reconstruction surgery

Objective To investigate the effect of 24-hour perioperative application of dexme-detomidine on the occurrence of postoperative atrial fibrillation and delirium in patients undergoing aortic root reconstruction.Methods A total of 600 patients scheduled for elective cardiac surgery under cardiopulmonary bypass were selected as the study subjects.Eventually,584 patients were in-cluded in the randomized grouping,with 292 patients in treatment group and 292 patients in control group.The treatment group received dexmedetomidine treatment,while the control group received sa-line placebo treatment.The occurrence of atrial fibrillation and delirium events,as well as other clini-cal indicators,in the two groups of patients within 5 days after surgery were observed,and the differ-ences in atrial fibrillation and delirium events among different subgroups were analyzed.Results The incidence rates of atrial fibrillation and delirium events within 5 days postoperatively in the treatment group were lower than those in the control group,with statistically significant differences(P<0.05).Subgroup analysis showed that for atrial fibrillation events,dexmedetomidine had a protective effect when the cardiopulmonary bypass time was>180 min,the aortic cross-clamping time was>120 min.For delirium events,dexmedetomidine had a protective effect when the cardiopulmonary bypass time was ≤180 min,the aortic cross-clamping time was ≤120 min,there was no significant increase in creatinine,no significant decrease in left ventricular ejection fraction(LVEF),the European System for Cardiac Operative Risk Evaluation Ⅱ score at low risk,and the allogeneic red blood cell transfu-sion volume was ≤2 U.Conclusion The 24-hour perioperative application of dexmedetomidine in-patients undergoing aortic root reconstruction can reduce the incidence of postoperative atrial fibrilla-tion and delirium.
4.Expression changes of miR-365a-3p and miR-10a-5p in patients with acute pancreatitis and their relationship with disease severity

Objective To investigate the expression changes of serum microRNA(miR)-365a-3p and miR-10a-5p in patients with acute pancreatitis(AP)and their relationship with the severity of the disease.Methods A total of 102 AP patients treated in the hospital from January 2023 to January 2024 were selected as the study group.According to the severity of the disease,the patients were divided into mild acute pancrea-titis(MAP)group and severe acute pancreatitis(SAP)group.A total of 102 healthy people who underwent physical examination in the same hospital during the same period were selected as the control group.Quantita-tive real-time PCR was used to detect the levels of miR-365a-3p and miR-10a-5p.The serum levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and C-reactive protein(CRP)were detected by enzyme-linked immunosorbent assay.Pearson correlation analysis was used to analyze the correlation of serum miR-365a-3p and miR-10a-5p levels with acute physiology and chronic health evaluation(APACHE)Ⅱ score,Ranson score,and biochemical indicators TNF-α,IL-6,and CRP in AP patients.Multivariate Logistic regression was used to analyze the influencing factors of SAP.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of serum miR-365a-3p and miR-10a-5p for AP and SAP.Results The serum levels of miR-365a-3p and miR-10a-5p in the study group were lower than those in the control group(P<0.05).The serum levels of miR-365a-3p and miR-10a-5p in the MAP group were higher than those in the SAP group(P<0.05).The APACHEⅡ score,Ranson score and the levels of TNF-α,IL-6 and CRP in the MAP group were significantly lower than those in the SAP group(P<0.05).Pearson correlation analysis showed that serum levels of miR-365a-3p and miR-10a-5p in AP patients were negatively correlated with APACHEⅡ score and Ranson score(P<0.05).The serum levels of miR-365a-3p and miR-10a-5p in AP patients were negatively correlated with TNF-α,IL-6 and CRP(P<0.05).Multivariate Logistic regression analysis showed that serum TNF-α,IL-6 and CRP were risk factors for SAP(P<0.05),and miR-365a-3p and miR-10a-5p were protective factors for SAP(P<0.05).ROC curve showed that the AUC of miR-365a-3p combined with miR-10a-5p in the diagnosis of AP was 0.832(95%CI 0.777-0.887),which was greater than the AUC of miR-365a-3p alone(0.742,95%CI 0.674-0.810,Z=2.539,P=0.011)and miR-10a-5p alone(0.676,95%CI 0.674-0.810,95%CI 0.602-0.750,Z=4.366,P<0.001).The AUC of the combined diagnosis of SAP was 0.859(95%CI 0.776-0.920),which was greater than the AUC of miR-365a-3p alone(0.733,95%CI 0.637-0.816,Z=2.078,P=0.038),miR-10a-5p alone(0.718,95%CI 0.620-0.803,Z=2.249,P=0.024).Conclusion The expression of serum miR-365a-3p and miR-10a-5p is related to the severity of AP,and the combination of the two is better than single diagnosis of SAP.
5.Investigation and Optimization Strategy on the Compatibility of Drug Mixed Infusions in ICU of A Hospital

Objective To evaluate the compatibility of commonly used intravenous drugs in the ICU of Hebei General Hospital and to propose optimization strategies.Methods Data from 35 intravenous drugs used in ICU from January 1 to December 31,2023 were analyzed using the Micromedex database and relevant literature.Results A compatibility chart for 35 drugs was created.Total 595 combinations,of which 279(46.9%)were compatible,22(3.7%)were incompatible,28(4.7%)were uncertain,and 266(44.7%)were unknown.Conclusion Drug co-administration is common in the ICU,but there is a lack of safety evidence and guidelines.There is an urgent to improve compatibility data and develop comprehensive resources to ensure safer intravenous drug administration.
6.Construction and evaluation of hepatocellular carcinoma models in mice with different immune microenvironments

Objective To construct mice hepatocellular carcinoma models with different tumor immune microenvironments(TIME)and explore the differences.Methods H22 and hepa1-6 were used to construct subcutaneous transplantation tumor model of C57 mice as homologous hepatocellular carcinoma cell lines(denoted as H22 group and hepal-6 group,each n=8),and the differences of TIME were evaluated.Immunohistochemistry was used to detect and quantify the infiltration of T cells,CD4+T cells,CD8+T cells,regulatory T cells and B cells in TIME.Flow cytometry was performed to detect the differences of composition of immune cell subpopulations in peripheral blood and tumor parenchyma.Gene expression profile characteristics of tumor tissue were analyzed based on high-throughput transcriptome sequencing technology,and enrichment analyses of immune-related signaling pathways were evaluated combined with gene ontology(GO)and Kyoto encyclopedia of genes and genomes(KEGG).Results H22 group showed cold and hepa1-6 group showed hot TIME characteristics.The number of T cells,CD4+T cells and CD8+T cells in tumor tissue of H22 group were all lower,while the proportion of T cells,CD4+T cells and CD8+T cells in peripheral blood were all higher than those of hepa1-6 group(all P<0.05).Compared with H22 group,up-regulated genes of tumor tissue in hepa1-6 group expressed significantly enriched in tumor immune activation-related signaling pathways.Conclusion H22 and hepa1-6 hepatocellular carcinoma models showed distinct TIME characteristics of cold and hot tumors,respectively,and the amount of immune cells in tumor tissue of the former were significantly lower than those in the latter.
7.Exploring the mechanism of Qiwei Tangmaishu capsules in the treatment of type 2 diabetes based on network pharmacology and animal experiment

Objective To observe the therapeutic effect of Qiwei Tangmaishu capsules on type 2 diabetes mice,and explore the mechanisms of its treatment of type 2 diabetes based on network pharmacology.Methods TCMSP,ETCM databases were used to query all components and of Qiwei Tangmaishu capsules and their targets.OMIM and DrugBank databases were used to search for targets of type 2 diabetes.The targets of type 2 diabetes and Qiwei Tangmaishu capsules were intersected by Venny 2.1.0.to perform GO and KEGG pathway enrichment analysis on those intersecting targets using the Metascape website.Then,a mouse model of type 2 diabetes was established,and Qiwei Tangmaishu capsules were given to low,medium,and high dose groups(234,468,and 936 mg/kg,respectively),and metformin(MET)group(200 mg/kg)for 2 weeks.The weight of each mouse was measured before and after treatment,and fasting blood glucose was also measured.After the 2 weeks,fasting insulin was measured;ELISA was used to detect levels of inflammatory factors IL-1β,TNF-α,IL-6,TLR4,and NF-κB in serum;Hematoxylin eosin staining was used to observe the morphology of pancreatic islets;and Caspase 3 and INS immunofluorescence were used to detect apoptosis of pancreatic islet cells and the number of pancreatic beta cells.Western Blot assay was used to detect the expression levels of pancreatic tissue proteins such as p-Akt,Akt,p-PI3K,PI3K,Bax,Bcl2.Results 1260 active ingredient targets were identified in Qiwei Tangmaishu capsules;1205 targets of type 2 diabetes were found.Of these,312 targets were intersected by Venny,with core targets involving Akt1,TNF,IL-6,TLR4,among others.Enrichment analysis identified 240 KEGG pathways,among which"insulin resistance""PI3K/Akt signaling pathway"were the key pathways enriched.The animal experiment result showed that compared with the model group,the intervention of Qiwei Tangmaishu capsules and metformin significantly improved blood glucose and insulin resistance;the content of inflammatory factors in serum decreased,and the apoptosis rate of pancreatic islet cells significantly decreased;the number of pancreatic beta cells significantly increased;the expression of pro-apoptotic protein Bax decreased,the expression of anti-apoptotic protein Bcl2 significantly increased,and the expression of p-PI3K and p-Akt was upregulated.Conclusions Qiwei Tangmaishu capsules can significantly reduce blood glucose levels,restore insulin sensitivity,and reduce islet cell apoptosis in type 2 diabetic mice.The mechanism may be related to the activation of the PI3K/Akt signaling pathway.
8.Anti-inflammatory and hepatoprotective triterpenoids from the traditional Mongolian medicine Gentianopsis barbata.

Gentianopsis barbata (G. barbata) represents a significant plant species with considerable ornamental and medicinal value in China. This investigation sought to elucidate the primary constituents within the plant and investigate their pharmacological properties. Fifty triterpenoids (1-50), including nine previously undescribed compounds (1, 2, 7, 10, 20, 28, 29, 37, and 41) were isolated and characterized from the whole plants of G. barbata. Notably, compounds 1 and 2 exhibited the novel 3,4;9,10-diseco-24-homo-cycloartane triterpenoid skeleton. The isolated triterpenoids demonstrated substantial anti-inflammatory activity through inhibition of tumor necrosis factor α (TNF-α) and interleukin-6 (IL-6) cytokine secretion in LPS-induced RAW264.7 macrophages, and hepatoprotective effects by preventing tert-butyl hydroperoxide (t-BHP)-induced oxidative injury in HepG2 cells. These results demonstrate both the presence of diverse triterpenoids in G. barbata and their therapeutic potential for inflammatory and hepatic conditions, providing scientific evidence supporting the clinical application of this traditional Mongolian medicinal plant.

9.Determination of 32 anabolic agents in tablets,capsules and injections by liquid chromatography-high resolution mass spectrometry

Objective To determine 32 anabolic agents in tablets,capsules and injections using liquid chromatography-high resolution mass spectrometry(LC-HRMS),so as to provide technical support in the fields of anti-doping and judicial identification.Methods The tablet grinded into powder,capsule samples extracted using a methanol∶water(7∶3)solution and injection underwent centrifugation and filtration.The separation was performed on a Waters Acquity UPLC HSS T3 column(100 mm×2.1 mm,1.8 μm),followed by gradient elution using ammonium acetate(20 mmol/L)solution containing 0.1%formic acid,5%acetonitrile and acetonitrile system as mobile phases,and detection by heated electrospray ionization in positive mode with a primary full-scan/data-dependent secondary scan mode.The established analytical method was subjected to methodological validation and parameters such as se-lectivity,sensitivity,matrix effect and delay effect were investigated.Meanwhile,the method was ap-plied to the detection of anabolic agents in 20 samples of tablets,capsules and injections provided by the authorities.Results The detection limits for 32 anabolic agents ranged from 1~200 μg/mg in the powder and 1~200 ng/mL in the injection.When this method was applied to the analysis of actual samples,five anabolic agents,including methandrostenolone and stanozolol,were identified among the 20 samples,disconsistent with labels on the package.Conclusion The LC-HRMS method is simple in pretreatment,sensitive and suitable for the detection of anabolic agents in tablet,capsule and injec-tion samples in the fields of anti-doping and judicial identification.
10.Analysis of the application of single-port laparoscopic appendectomy without holder assistance in patients with complicated appendicitis

Objective:The aim of this study was to explore the risk factors that affect implementation of the innovative technique of single-incision laparoscopic appendectomy (solo-SLA) without assistance in patients with complicated appendicitis, the goal being improving surgical success rates and reducing the incidence of complications.Methods:This was an observational study. Indications for solo-SLA surgery were as follows: (1) computed tomography or ultrasound findings suggestive of acute appendicitis, accompanied by a high white blood cell count and C-reactive protein concentration; (2) disease course exceeding 72 hours, standard anti-infection treatment ineffective, inflammatory reaction not localized, surgery mainly aimed at abscess drainage, and the appendix removed if indicated intraoperatively; (3) acute onset stabilized for more than 3 months after conservative treatment; and (4) recurrent chronic appendicitis. Relative contraindications comprised: (1) cardiopulmonary insufficiency, extremely high risk for general anesthesia for laparoscopic surgery; (2) severe coagulation dysfunction; and (3) imaging findings suggestive of formation of a peri-appendiceal abscess, stable after anti-infection treatment, and a tendency for the inflammatory reaction to localize. We retrospectively collected clinical data of 106 patients with complicated appendicitis who had undergone solo-SLA in the Department of Emergency Surgery, Peking University People's Hospital from February to October 2023. Preoperative computed tomography showed appendiceal fecaliths, blurring of the tissue surrounding fat, intra- and extra-luminal gas and exudate, peri-appendiceal abscess, ascites, and intestinal obstruction by appendicitis. The study cohort comprised 53 male and 53 female patients aged (41.4±17.4) years. The median body mass index was (24.2±3.6) kg/m 2 and median preoperative body temperature (37.3±0.9)℃ Appendicitis had been present for >3 days in 21 of the patients (19.8%) and the maximum diameter of the appendix was (12.4±3.8) mm. The efficacy of the surgery was assessed and logistic regression analysis used to explore the factors affecting the duration of the procedure. The relationship between the maximum diameter of the appendix and duration of surgery was non-linear and was explored using a logistic regression model with restricted cubic spline (RCS). Results:Only one patient required conversion to open surgery; all the other patients successfully completed solo-SLA with a median intraoperative blood loss of 10 (1-100) ml and a surgical time of (65.4±31.7) minutes. Pain scores on postoperative Day 1 and 7 were (3.4±3.2) points and (1.5±1.7) points, respectively. There were no significant postoperative complications .The postoperative hospital stay was (3.5±1.5) days and the interval to resuming normal activities 14 (2-40) days. According to univariate and multivariate analyses, disease course >3 days (OR=5.19, 95%CI: 1.59-16.98, P=0.006) and C-reactive protein >10 mg/L (OR=1.01,95%CI: 1.00-1.02, P=0.003) were independent risk factors for surgical duration >60 minutes, whereas the maximum diameter of the appendix was not independently associated with duration of surgery (OR=1.10, 95%CI: 0.97-1.25, P=0.119). RCS analysis results showed a "U-shaped" association between the maximum diameter of the appendix and duration of surgery, the inflection point of the RCS curve being at a diameter of 10 mm. When the maximum diameter of the appendix was <10 mm, increases in diameter were not associated with longer duration of surgery (OR=1.15,95%CI: 0.55-2.58, P=0.710); whereas when the diameter was ≥10 mm, the maximum diameter of the appendix was associated with increased duration of surgery (OR=1.20, 95% CI: 1.04-1.42, P=0.022). Conclusion:The solo-SLA procedure can be performed to treat complicated appendicitis. A disease course >3 days, C-reactive protein concentration >10 mg/L, and maximum diameter of the appendix ≥10 mm are all associated with greater difficulty of solo-SLA surgery.
