1.Association of adverse childhood experiences with psychopathology, sleep, and cognitive function in children: the mediating role of putamen volume

BackgroundAdverse childhood experiences (ACEs) are strong predictors of psychopathology across the lifespan, exerting extensive adverse effects on physical and mental health during early development. While ACEs increase the risk of childhood psychopathology, sleep quality and impair cognitive function, the early neurobiological mechanisms mediating ACE-induced cognitive impairment remain unclear. ObjectiveTo explore the association between ACEs exposure and the risk of psychopathology, sleep disorders, and cognitive function, and to examine the mediating role of the putamen volume in the relationship between ACEs and cognitive function from a neurodevelopment perspective. MethodsData were obtained from the Adolescent Brain Cognitive Development (ABCD) study, comprising 10 572 children aged 9 to 10. Participants were categorized into five groups based on the cumulative score of ACEs reported by their parents and the children themselves: no-exposure group (0 types of ACEs), the one-exposure group, the two-exposure group, the three-exposure group, and the high-risk exposure group (≥ 4 types of ACEs). Logistic regression analysis was employed to evaluate the association between ACEs and psychopathological symptoms [T score of the Child Behavior Checklist (CBCL) ≥ 65]. Multiple linear regression analysis was ultilized to analyze the correlations between ACEs and the scores of the Sleep Disturbance Scale for Children (SDSC), the NIH Toolbox Cognition Battery (NIHTB-CB), as well as the multiple structures in the cerebral cortex and subcortex. The Bootstrap method was applied to test the mediating effect of the putamen volume between ACEs and neurocognitive function. ResultsAfter adjusting for confounders, ACEs cumulative score was correlated with children's psychopathological symptoms in a dose-response manner. Compared with the no-exposed group, the high-risk exposure group had a 2.08-fold (OR=2.080, 95% CI: 1.401–3.088) increased risk of internalizing problems and a 3.13-fold (OR=3.132, 95% CI: 1.703–5.760) increased risk of externalizing problems, and the risk of conduct problems was 4.68 times that of the no-exposed group (OR=4.681, 95% CI: 2.528–8.669). ACEs cumulative score positively predicted the total score of SDSC (β=0.080, 95% CI: 0.059–0.102), with relatively stronger predictive effects on sleep-wake transition disorder (β=0.086, 95% CI: 0.063–0.109) and sleep onset and maintenance disorder (β=0.066, 95% CI: 0.043–0.089). In terms of neurocognition and brain structure, ACEs cumulative score negatively predicted the overall cognitive composite score (β=-0.025, 95% CI: -0.050–-0.001), the crystallized cognitive composite score (β=-0.035, 95% CI: -0.060–-0.010), the volume of the left putamen nucleus (β=-0.025, 95% CI: -0.046– 0.003), and the volume of the right putamen nucleus (β=-0.023, 95% CI: -0.044– 0.003). The total putamen volume played a partial mediating role between ACEs and the overall cognitive composite score, the effect value was -0.002 (95% CI: -0.004–-0.001), accounting for 7.49% of the total effect. ConclusionACEs exposure may increase the risk of psychopathological and sleep disturbances in children and may impair neurocognitive function. Reduced putamen volume may serve a potential neurobiological pathway underlying the impact of ACEs on childhood cognition. 2.A randomized double-blind controlled study on the effects of wearing defocus incorporated multiple segments lenses on binocular visual function and visual quality

Objective:To evaluate the effects of wearing defocus incorporated multiple segments (DIMS) spectacle lenses on binocular visual function and visual quality in myopic children.Methods:A randomized double-blind controlled study was conducted.A total of 176 children (352 eyes) with myopia aged 6 to 15 years were enrolled in the West China Hospital of Sichuan University from January 2021 to March 2022.They were randomly divided into DIMS group of 85 cases (170 eyes) and single-vision group of 91 cases (182 eyes) wearing DIMS and single-vision lenses, respectively, using the random number table method.A total of 151 cases (302 eyes) who completed follow-up visits were included in the analysis, including 72 cases (144 eyes) in the DIMS group and 79 cases (158 eyes) in the single-vision group.Before and after 6 and 12 months of lens wear, accommodation amplitude, binocular accommodative sensitivity, distance/near horizontal eye position, and accommodative convergence/accommodation (AC/A) of subjects were measured.Changes in visual function over the year were compared between the two groups.In addition, subjective contrast sensitivity (CS) was assessed at each follow-up visit to evaluate the effect of the lenses on visual quality.The study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of West China Hospital of Sichuan University (No.2020-06).Parents or legal guardians signed written informed consent before the child's participation.Results:There were significant differences in monocular accommodation amplitude and binocular accommodation sensitivity at different time points between before and after lens wear in both groups (Wald χ2time=84.435, 48.201; both P<0.001).In both groups, monocular accommodation amplitude was decreased and the biocular accommodation sensitivity was increased at 6 and 12 months after wearing glasses compared with baseline (all P<0.001).After 12 months of lens wear, the monocular accommodation amplitude decreased by 2.68 D (95% CI: 1.60-3.75 D) in DIMS group and 2.82 D (95% CI: 1.81-3.84 D) in single-vision group.There were statistically significant differences in distance and near horizontal eye position between the two groups at different time points before and after wearing glasses (Wald χtime2=10.398, 23.947; both P<0.01).In the DIMS group, after 12 months of wearing lenses, the distance horizontal eye position drifted outward by 0.68 △(95% CI: 0.06-1.25 △) compared to baseline, with a significant difference ( P<0.05).There was 1.67 △ (95% CI: 0.15-3.20 △) outward drift at near horizontal eye position in the DIMS group and 1.73 △ (95% CI: 0.49-2.96 △) outward drift in the single-vision group compared with baseline, with significant differences (both P<0.05).There were statistically significant differences in gradient AC/A values and calculated AC/A values between the two groups at different time points before and after wearing glasses (Wald χtime2=22.001, 13.411; both P<0.01).After 12 months of wearing glasses, the gradient AC/A values significantly decreased in both groups compared to respective baseline, and the calculated AC/A values in the single-vision group showed a significant decrease compared to baseline (all P<0.05).There were no significant difference in the monocular accommodation amplitude, binocular accommodation sensitivity, distance and near horizontal eye position, gradient AC/A values and calculated AC/A values between the two groups (Wald χgroup2=2.385, 2.266, 2.070, 0.571, 0.578, 0.053; all P>0.05).There was no significant difference in the CS at 3, 6, 12, and 18 cpd spatial frequencies between the two groups at different time points before and after wearing lenses (Wald χ2group=1.104, 2.263, 1.861, 3.671; all P>0.05.Wald χ2time=2.260, 5.382, 2.573, 1.637; all P>0.05). Conclusions:Visual function performance after wearing DIMS lenses is consistent with that after wearing single-vision lenses.Wearing DIMS lenses has no obvious negative effect on the long-term visual quality in children.

4.Attitudes toward seeking professional psychological help in patients after gastrointestinal tumor surgery and their influencing factors

Objective:To investigate the current status of attitudes toward seeking professional psychological help in patients after gastrointestinal tumor surgery and to analyze their influencing factors.Methods:Convenience sampling was used to select 153 patients who underwent surgery at the Gastrointestinal Tumor Center of Peking University Cancer Hospital as study subjects. General Information Questionnaire, Chinese version of Attitude Seeking Professional Psychological Help Scale-Short Form (ATSPPH-SF) , Simplified Coping Style Questionnaire, and General Self-Efficacy Scale were used to conduct the survey. Spearman correlation was used to analyze the correlation between attitudes toward seeking professional psychological help and coping styles and self-efficacy in patients after gastrointestinal tumor surgery. Multiple linear regression was used to explore the factors influencing attitudes toward seeking professional psychological help in patients after gastrointestinal tumor surgery. A total of 153 questionnaires were distributed and 149 valid questionnaires were recovered, with a valid recovery rate of 97.39% (149/153) .Results:The total ATSPPH-SF score of 149 patients with gastrointestinal tumor surgery was [16.00 (13.00, 19.00) ]. Multiple linear regression analysis showed that work status, seeking professional psychological help, and coping styles were the influencing factors on the attitude toward seeking professional psychological help in patients after gastrointestinal tumor surgery ( P<0.05) . Conclusions:The level of attitudes toward seeking professional psychological help in patients after gastrointestinal tumor surgery is low and is influenced by a number of factors. Healthcare professionals can carry out individualized interventions for different patients, enhance absentee patients' disease knowledge, sense of self-worth, and encourage them to carry out professional psychological counseling and establish a positive coping style, so as to increase the initiative of patients to seek professional psychological help.
5.Visual analysis of the current status and hotspots of nurse-led clinics based on CiteSpace

Objective:To analyze the status of nurse-led clinics at home and abroad, and to explore the research hotspots and development trends.Methods:China National Knowledge Infrastructure and Web of Science Core Collection database were searched for literature on nurse-led clinics. The search period was from January 1, 2000 to May 26, 2024. Visual analysis was implemented using CiteSpace software on the collected literature.Results:A total of 206 articles in Chinese and 1 990 articles in English were included. The number of English language publications in nurse-led clinics was higher than that of Chinese language publications, and the top three countries in terms of English language publications were the United Kingdom (533), the United States (420), and Australia (355). The number of publications in the Chinese literature grew slowly, and the journal with the highest number of publications was Chinese Nursing Management (69). Keywords such as "nurse specialist", "nursing management" and "health education" in the Chinese literature and "quality of life", "management" and "randomized controlled trial" in the English literature had a high degree of centrality. Rehabilitative care and hospice care for oncology patients was a common research trend in international nurse-led clinics. Conclusions:Nurse-led clinics have gained widespread attention as a model of advanced nursing practice. China's nurse-led clinics in general show a good development trend, but the amount of relevant research publications is relatively small, and the level of research still needs to be further improved.
6.Value of T-SPOT.TB combined with IL-2,IFN-γ in diagnosis of active tuberculosis

OBJECTIVE To explore the value of tuberculosis infection T cell spot test(T-SPOT.TB)combined with cytokines interleukin-2(IL-2),interferon-γ(IFN-γ)in differential diagnosis of active tuberculosis(ATB)and non-ATB.METHODS A total of 186 patients with pulmonary tuberculosis(TB)who were treated in Hebei Provincial Chest Hospital from Feb.2021 to Mar.2024 were recruited as the research subjects and were divided in-to the ATB group with 92 cases and the non-ATB group with 94 cases according to the result of sputum culture for MTB,which was taken as the golden standard.The T-SPOT.TB was carried out for all of the subjects.The base-line data were collected from the two groups of patients.The hierarchical regression analysis was performed by setting the T-SPOT.TB,IL-2 and IFN-γ as the dependent variables,clinical symptoms of cough and fever as the independent variables.The impact of the clinical characteristics on T-SPOT.TB,IL-2 and IFN-γ was observed.The value of T-SPOT.TB combined with cytokines IL-2 and IFN-γ in differential diagnosis and ATB and non-ATB was evaluated by means of principal component analysis(PCA)and partial least squares discrimi-nant analysis(PLS-DA).The efficiencies of T-SPOT.TB,IL-2,IFN-γ and prediction model were examined by receiver operating characteristic(ROC)curves.RESULTS There were significant differences in cough,fever,IL-1β,IL-2,IL-6,IL-8 and IFN-γ between the two groups(P<0.05).There were statistically positive correlations between the clinical symptoms of cough,fever and the T-SPOT.TB,IL-2 and IFN-γ(P<0.05).The result of PCA model and PLS-DA model showed that there were less significant individual differences between the ATB pa-tients and the non-ATB patients,the two groups could be remarkably clustered and identified.The area under the curve(AUC)of the joint model was highest(0.913)(0.875 to 0.950)(P<0.05),with the sensitivity 90.24%,the specificity 74.54%.CONCLUSION The T-SPOT.TB combined with IFN-γ and IL-2 shows high sensitivity and specificity in differential diagnosis of ATB and non-ATB and can be used as method for auxiliary diagnosis of ATB,and it has significant clinical value.
7.Gut microbiota and colorectal cancer liver metastasis: mechanisms and therapeutic interventions

The intestine and liver are closely connected both physiologically and pathologically, forming a so-called gut-liver axis, with the gut microbiota serving as a pivotal link in their bidirectional communication. Gut microbiota dysbiosis and gut-liver axis disruption play a key role in the development and progression of colorectal cancer liver metastasis (CRLM), though the underlying mechanisms have not been clearly elucidated. Certain gut microbiota, such as Escherichia coli and Enterococcus spp., can breach the intestinal barrier and translocate to the liver, promoting the formation of pre-metastatic niche. Fusobacterium nucleatum and Enterococcus faecalis enhance tumor cell invasion/migration, while Parabacteroides spp. suppress anti-tumor immunity in the liver TME. Interventions like fecal microbiota transplantation, dietary modifications, and traditional Chinese medicine have shown potential in clinical and preclinical studies to improve patient outcomes by targeting the gut microbiota, but their long-term efficacy and safety require further investigation. Future research should focus on elucidating the effects of specific bacterial species, metabolites, viruses, and fungi on tumorigenesis. Exploring the potential of gut microbiota-based precision medicine and personalized therapies will improve risk stratification and enable more targeted interventions for CRLM patients.
8.Analysis and Research on the Screening of 23 Deafness Susceptible Gene Mutations Using Microfluidic Chip Method in 10 561 Newborns in Shenzhen Area

Objective To screen and analyze 23 mutation sites of deafness susceptibility genes in newborns in Shenzhen using microfluidic chip technology,aiming to enhance the efficiency and accuracy of early diagnosis of congenital deafness.Methods A total of 10 561 newborns delivered in 19 hospitals in Shenzhen between November 2021 and January 2023 were selected.Heel blood samples were collected from them,and 23 key mutation sites within four core deafness susceptible genes(GJB2,SLC26A4,12S rRNA and GJB3)were detected using microfluidic chip technology.Results The results revealed that 21.87%(2 310/10 561)of the newborns carried at least one mutation in deafness susceptibility genes,with the mutation carrier rate of the GJB2 gene reaching as high as 20.42%(2 157/10 561),57 newborns were found to has compound heterozygous mutations.To further verify the accuracy of the screening results,1 203 newborns out of the 2 310 initially screened as deafness gene carriers were recalled for Sanger sequencing,the gold standard,for verification.The verification results were fully consistent with the initial screening results.Conclusion The method combining microfluidic chip technology with Sanger sequence verification significantly improves the detection efficiency and accuracy of neonatal deafness genes,providing scientific evidence and practical guidance for implementing the three-level prevention strategy for congenital deafness.
9.Accuracy of ΔVpeak-CA combined with tidal volume challenge test in predicting fluid responsiveness of patients receiving lung-protective ventilation

Objective:To evaluate the accuracy of respiratory variation in carotid artery blood flow peak velocity (ΔVpeak-CA) combined with tidal volume challenge (TVC) test in predicting fluid responsiveness of patients receiving lung-protective ventilation.Methods:Eighty American Society of Anesthesiologists Physical Status classification Ⅰ-Ⅲ patients of either sex, aged 18-80 yr, with a body mass index of 18-30 kg/m 2, undergoing elective open abdominal surgery with general anesthesia, were selected. Lung-protective strategies were used during surgery: FiO 2 40%, tidal volume (V T) 6 ml/kg (ideal body weight), personalized positive end-expiratory pressure. TVC was conducted at 5 min after hemodynamic stabilization following intubation (T 1): V T was adjusted from 6 ml/kg to 8 ml/kg, lasting for 1 min (T 2), and then V T was decreased to 6 ml/kg. Ultrasound was used to measure ΔVpeak before and after TVC test, and ΔVpeak-CA was calculated. Volume expansion was conducted at 5 min after TVC test, and an increase in cardiac index (△CI≥15%) after volume expansion was considered as positive fluid responsiveness. The patients were divided into fluid responsiveness group (R group) and non-responsiveness group (NR group). The receiver operating characteristic curve was plotted and the area under the receiver operating characteristic curve (AUC) was calculated to evaluate the efficiency of ΔVpeak-CA at T 1 (ΔVpeak-CA T 1), ΔVpeak-CA at T 2 (ΔVpeak-CA T 2) and the difference in ΔVpeak-CA between T 1 and T 2 (ΔVpeak-CA T 2-T 1) in predicting fluid responsiveness. Results:Seventy-three patients were finally included in this study, with 45 in R group and 28 in NR group. The AUC of ΔVpeak-CA T 2 in predicting fluid responsiveness was 0.880, with the sensitivity of 0.778 and the specificity of 0.857, and the optimum cut-off value was 14.5%. The AUC of ΔVpeak-CA T 2-T 1 in predicting fluid responsiveness was 0.876, with the sensitivity of 0.667 and the specificity of 0.964, and the optimum cut-off value was 5.5%, and the AUC of ΔVpeak-CA T 1 in predicting fluid responsiveness was only 0.646. Conclusions:ΔVpeak-CA combined with TVC test can accurately predict fluid responsiveness of patients receiving lung-protective ventilation.
