1.Effect of Manchester pain management on pain and recovery after cesarean section in pregnant women with gestational hypertension

Objective To explore the effects of Manchester pain management on postoperative pain and recovery in pregnant women with gestational hypertension.Methods In total,134 pregnant women with gestational hypertension admitted to the Obstetrics Department of Shengjing Hospital between April 2022 and May 2023 were selected.The patients were randomly divided into a control group(group C)and a Manchester group(MPMM group).Pain frequency,pain(VAS),anxiety(SAS),depression(SDS),and sleep quality(PSQI)scores,as well as morning systolic blood pressure(SP),diastolic blood pressure(DP),mean arterial pressure(MAP),first time out of bed,and hospitalization time were recorded and compared between the two groups on days 0,1,and 3 after surgery.Results The pain score and frequency of pain in the MPMM group were lower than those in the C group on days 0 and 1 after surgery(P<0.05);the SAS and PSQI scores were lower in the C group on days 1 and 3 after surgery(P<0.05);and the SDS scores on days 0,1,and 3 after surgery were lower than those in the C group(P<0.05).On days 0,1,and 3 after surgery,the SP in the MPMM group was lower than that in the C group;the DP and MAP were lower than those in the C group on days 1 and 3 after surgery,and the first time of getting out of bed and hospitalization time were significantly shorter than those in group C(P<0.05).Conclusion The MPMM alleviates postoperative pain,reduces anxiety and depression,improves sleep quality,and promotes early recovery in pregnant women with gestational hypertension.
2.Efficacy of traditional Chinese medicine combined with 131I in the treatment of Graves hyperthyroidism

Objective:To explore the value of traditional Chinese medicine combined with 131I in the treatment of Graves hyperthyroidism. Methods:From March 2020 to July 2021, 90 patients (39 males, 51 females, age (33.2±7.0) years) with Graves hyperthyroidism who were diagnosed and treated in Changshu No.2 People′s Hospital were retrospectively analyzed. Patients were randomly divided into 3 groups ( n=30 in each group), including group A who received treatment of antithyroid drugs (ATD), group B who received treatment of traditional Chinese medicine, and group C who received treatment of 131I combined with traditional Chinese medicine. Thyroid function indicators and inflammatory indicators before treatment and 1, 3, and 6 months after treatment were determined, including free triiodothyronine (FT 3), free thyroxine (FT 4), thyroid stimulating hormone (TSH) and TSH receptor antibody (TRAb), and C-reactive protein (CRP), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α). One-way analysis of variance and χ2 test were used to analyze data. Results:The levels of FT 3, FT 4, TSH, TRAb, CRP, IL-6 and TNF-α in group A, B and C before treatment and 1, 3, 6 months after treatment were significantly different ( F values: 193.27-906.11, all P<0.05). The total effective rate in group C (100.0%, 30/30) was significantly higher than that in group A (86.7%, 26/30) or group B (83.3%, 25/30; χ2 values: 8.24, 9.83, P values: 0.006, 0.037), while there was no significant difference between group A and group B ( χ2=3.02, P=0.124). The incidence of adverse reactions in group B (46.7%, 14/30) was significantly higher than that in group A (30.0%, 9/30; χ2=6.59, P=0.042). And the incidence of adverse reaction in group C (13.3%, 4/30) was significantly lower than that in group A or group B ( χ2 values: 12.05, 7.20, P values: 0.004, 0.038). Conclusion:The curative effect of 131I combined with traditional Chinese medicine is effective and reliable, suggesting that clinical researches should be carried out together and perfected.
3.Effects of temperature management based on comfort theory in the prevention of hypothermia in patients undergoing transurethral resection of prostate

Objective:To explore the effect of temperature management based on comfort theory on hypothermia prevention in patients undergoing transurethral resection of prostate (TURP) .Methods:From March to November 2021, 242 patients with benign prostatic hyperplasia who were treated with TURP in Affiliated Jinhua Hospital of Zhejiang University School of Medicine were selected as the study subject by convenience sampling. The patients were divided into control group and observation group according to the method of random number table, 121 patients in each group. The control group carried out routine temperature management, while observation group was given temperature management based on comfort theory on the basis of the control group. We compared the temperature of the two groups when they entered the Operating Room (T 1) , 0.5 h after anesthesia (T 2) , 1 h after anesthesia (T 3) , and at the end of the operation (T 4) . The recovery time of anesthesia was compared between the two groups, and the postoperative complications were counted. Results:The temperature of the observation group at T 2, T 3, T 4 was higher than those of the control group, with statistically significant differences ( P<0.01) . The temperature changes of patients in both groups showed a downward trend from T 1 to T 4. The incidence of shivering and bladder spasm in the observation group were lower than those in the control group, also with statistically significant differences ( P<0.05) . The duration of stay in the Recovery Room, the recovery time of spontaneous respiration and the recovery time of consciousness in the observation group were shorter than those in the control group, with statistical differences ( P<0.01) . Conclusions:Temperature management based on comfort theory can effectively prevent hypothermia in TURP patients, promote postoperative anesthesia recovery, and reduce postoperative complications.
4.Efficacy of clinical immunology intervention for patients with glucocorticoid refractory subacute thy-roiditis

Objective To assess the efficacy and clinical value of immunoglobulin combined with interferon ( IFN) α-2b in treatment of patients with glucocorticoid refractory subacute thyroiditis. Methods Thirty-five cases of subacute thyroiditis (12 males, 23 females, age (58.1±12.0) years) from January 2016 to May 2017 were retrospectively analyzed. Patients were resistant to the treatment of glucocorticoid and as-pirin, or were not suitable for that treatment because of accompany diseases. Thirty-four healthy controls (16 males,18 females, age (57.0±10.0) years) were included. Thyroid hormone, C-reactive protein (CRP), erythrocyte sedimentation rate ( ESR) and 24 h iodine uptake rate tests, as well as 99 Tcm O-4 thyroid imaging and thyroid ultrasonography were performed before and after co-administration of immunoglobulin and IFNα-2b. Paired t test or Wilcoxon signed rank test, two-sample t test or Wilcoxon rank test were used for data anal-ysis. Results The levels of free triiodothyronine ( FT3 ) , free thyroxine ( FT4 ) and thyroid stimulating hor-mone ( TSH) 4 weeks after immunology intervention were restored compared with the levels before treatment ( t:4.81, 7.76, z=4.60, all P<0.01) and were not significantly different compared with those in the con-trol group ( t:0.02, 0.33, z=0.37, all P>0.05) . CRP and ESR were sharply decreased compared to those before treatment (t:9.59, 13.78, both P<0.01). The 24 h iodine uptake rate was restored 4 weeks after treatment (t=13.92, P<0.01). The positive result was found in 1 patient (2.9%, 1/35) by 24 h iodine uptake rate test, and 5.7%(2/35) of patients were positive in 99TcmO-4 static imaging and 1 patient(2.9%, 1/35) had positive ultrasonography result. The 3-month follow-up showed that 32 cases were cured and the treatment was effective in all 35 patients. Only 1 case recurred after 12-month follow-up, and the effective rate was 96.9%(31/32, 3 cases were lost to follow-up). Conclusion Immunology intervention is an effective way to treat glucocorticoid refractory subacute thyroiditis and should be applied extensively in clinical treatment.
5.Effect of comprehensive intervention on antipsychotic drug induced constipation

Objective To explore the effect of comprehensive intervention on constipation caused by antipsychotic drugs.Methods Ninty patients with antipsychotic drug induced constipation who were admitted to our hospital from June 2012 to October 2013 were enrolled in this investigation and randomly assigned to the comprehensive intervention group(CIG,n=46) and the routine care group(RCG,n=44).The subjects of the RCG received routine diet and medication therapy; and those of CIG received comprehensive intervention of life style,psychological ability,cognition,nursing and traditional chinese medicine(TCM) for 3 months.Constipation symptoms scale and Patient Assessment of Constipation Quality of Life Questionnaire (PAC-QOL) were assessed before and after the intervention.Results CIG showed significant improvement in defecation difficulty,defecation force,defecation duration,frequency,abdominal distension,total symptom score,PAC-QOL,physical discomfort,psycho-social discomfort,anxiety and treatment satisfaction were(2.8±2.1),(2.2±1.6),(2.4±1.3),(2.1±1.7),(1.5±0.9),(14.0±8.1)respectively (t values were 2.629,3.818,2.328,2.593,2.578,2.913,7.499,2.570,5.012,2.249 and 12.259,respectively; all P<0.05).Conclusion Comprehensive intervention may be an effective therapy against psychiatric drug induced constipation.
6.Analysis of the treatment of distal humeral fractures by double plate fixation

Objective To investigate the internal fixation for distal humeral fractures and its outcome .Methods A total of 74 patients with distal humeral fractures were performed surgical operation between Jan 2006 and Jun 2009 .According to AO/ASIF classification ,the factures were classified as type A (n=26) ,B(n=34)and C(n=14) .Internal fixation was applied to the patients , who did functional exercises after operation ,on the basis of classification .The healing of the fractures and the function the joints was examined via follow-up and X-ray films 1 ,3 and 6 months after operation .Results According to the Jupiter elbow score ,40 ca-ses were excellent ,22 cases were good ,12 cases were fair and 0 case was poor ,and the good rate was 83% .Conclusion Internal fix-ation with double plate ,is an effective therapeutic approach to distal humeral fractures in adults .Influencing factors of the efficacy include severity of the initial trauma ,operative scheme ,and post-operation functional recover .
7.Clinical efficacy of single span video-assisted thoracoscope surgery for elderly spontaneous pneumothorax

Objective To compare the clinical efficacy between single span video-assisted thoracoscope (VATS) surgery and traditional VATS surgery for spontaneous pneumothorax in elderly patients.Methods A total of 88 elderly patients diagnosed as spontaneous pneumothorax by chest X-ray and CT were selected and randomly divided into single span VATS group and traditional VATS group (n=44,each).The bleeding volume,duration of operation,hospitalization time,time of postoperative analgesic use,chest tube indwelling time,postoperative complications and adverse reactions were observed.All patients were follow-up for 1-2 years to record the recurrence rate and death rate.Results All patients were received VATS surgery successfully.Their clinical symptoms disappeared and X-ray showed that pneumothorax disappeared.There were significant differences in bleeding volume,time of postoperative analgesic use,hospitalization time between single span VATS group and traditional VATS group [(46.7±18.8)ml vs.(46.7±18.8)ml,(1.7±0.9)d vs.(2.4± 1.7)d,(6.3±1.7)d vs.(9.1±2.1)d,t=3.04,2.18,5.59,respectively,all P<0.05].The duration of operation and chest tube indwelling were shorter in single span VATS group than in traditional VATS group (both P<0.05).Patients with chest pain and incision numb after operation were less in single span VATS group than in traditional VATS group (11 cases vs.26 cases,7 cases vs.21 cases,t=10.50,10.26,both P<0.05).There were no significant differences in recurrence rate and death rate between the two groups.Conclusions Single span VATS surgery is an safe and effective treatment for spontaneous pneumothorax in elderly patients,which has a better postoperative recovery and fewer adverse reactions.
8.Effect of Silencing LRIG3 Gene on the Proliferation and Apoptosis of Bladder Cancer T24 Cells

This study examined the effect of silencing LRIG3 expression on the proliferation and apoptosis of bladder cancer T24 cells and explored the role of LRIG3 in the tumorigenesis of bladder cancer.Bladder cancer T24 cells were routinely cultured and pSilencer plasmids were employed to construct LRIG3 eukaryotic expression vector of LRIG3-siRNA,i.e.,pSilencer-LRIG3-siRNA.After confirmation,the vector was transfected into HEK293 cells to make a replication-deficient adenovirus,pAd-LRIG3-siRNA,which was then introduced into bladder cancer T24 cells.RT-PCR,Western-blotting were performed to detect the levels of LRIG3 mRNA and proteins.Cells number was determined by using MTT test.Hoechst33258 staining,transmission microscopy,flow cytometery were conducted to examine the cell apoptosis.Three groups included a blank control group,a negative control group (containing non-interfering plasmids) and a pAd-LRIG3-siRNA group.Our results showed that the recombinant pAd-LRIG3-siRNA was successfully transfected into the bladder cancer T24 cells.The siRNA formed by the transcription of the recombinant plasmids resulted in significantly reduced expressions of LRIG3 gene and protein and significantly decreased cell proliferation and growth in the pAd-LRIG3-siRNA group as compared with the control group (P<0.01).The siRNA also caused apoptotic changes of some cells,with the apoptosis rate being (17.69±0.75)%,which was significantly different from that of the control group (P<0.01).It was concluded that recombinant pAd-LRIG3-siRNA plasmids could effectively decrease the expression of LRIG3 mRNA and proteins and,to some extent,inhibit the proliferation and promote the apoptosis of bladder cancer T24 cells.Silencing LRIG3 gene might be a novel alternative for the treatment of bladder cancer.

10.Protective Effect of DADLE on Pulmonary Function in Rats with Sepsis

Objective To study the protective effects of DADLE on pulmonary function in rats with sepsis and the mechanism.Methods Seventy-two SD rats were randomly divided into sham operated group(SO),septic group(SEP),and DADLEtreated group (DADLE).Sepsis model was induced by cecal ligation and puncture(CLP).In SO group,the abdomen was opened without any other treatment.In DADLE-treated group,DADLE(0.5 mg/ml) was administerd at a dose of 5 ml/kg by intravenous injection after CLP.Rats were sacrificed at the 2nd,6th and 10th h after CLP.Arterial blood was collected for blood gas analysis.The wet-to-dry lung weight ratio was measured,and the levels of TNF-a and IL-6 in blood were detected.The levels of MPO,MDA and ATP in lung tissue were determined.The pathologic changes of the lungs were examined under a light microscope.Results As compared with SEP group,PaCO_2,and PaO_2 were significantly increased(P<0.05),while the wet-to-dry ratio and levels of serum TNF-a and IL-6 were significantly reduced(P<0.05),and levels of MPO and MDA were significantly reduced(P<0.05) and content of ATP in lung tissue was significantly increased (P<0.05)in DADLE group.More severe pathological changes were found in SEP group than in DADLE group.Conclusion DADLE has pulmonary protective effects in rats with sepsis.
